Stuttering can have a negative impact on the quality of life of people who suffer from it. It can put a brake on the realization of steps requiring oral communication. Health care pathway of these people could then be hindered by these difficulties. It seems important to determine the impact of stuttering on access to medical consultations, because a renunciation of care could have negative consequences on health. The quality of life of adults who stutter would be further altered. The objective of this study is to measure the impact of stuttering on access to health care for adults who stutter and to identify the precise reasons for the difficulties encountered. To answer this issue, a self-administered questionnaire was created for adults who stutter to collect their account about their possible difficulties with the medical world. Descriptive analysis of the answers obtained shows that stuttering doesn’t have an impact on access to health care, but it can cause difficulties for some people. Based on these observations, it would be interesting to modify the questionnaire and to question more precisely the adults who stutter in order to go further in the answer to our issue.; Le bégaiement peut avoir un impact négatif sur la qualité de vie des personnes qui en souffrent. Il peut constituer un frein dans la réalisation de démarches nécessitant une communication orale. Le parcours de soins de santé de ces personnes pourrait alors être entravé par ces difficultés.Il semble important de déterminer l’impact du bégaiement sur l’accès aux consultations médicales car un renoncement aux soins pourrait entraîner des conséquences néfastes sur la santé. La qualité de vie des adultes qui bégaient serait alors davantage altérée.L’objectif de cette étude est de mesurer l’impact du bégaiement sur l’accès aux soins médicaux des adultes qui bégaient et d’identifier les raisons précises des difficultés rencontrées.Pour répondre à la problématique, un questionnaire auto-administré à destination des adultes qui bégaient a été créé afin de recueillir leur témoignage concernant leurs éventuelles difficultés face au monde médical. L’analyse descriptive des réponses obtenues montre que le bégaiement n’a majoritairement pas d’impact sur l’accès aux soins mais qu’il peut mettre en difficulté certaines personnes.À partir de ces observations, il serait intéressant de modifier le questionnaire et d’interroger plus précisément les adultes qui bégaient afin d’aller plus loin dans la réponse à notre problématique.

Developmental stuttering is a fluency disorder which involves negative effects on quality of life. Recent research on genetics, neuroimaging does not give us the keys to understand the precise nature of this trouble yet. Factors which predict chronicity or natural recovery are still unknown so an early speech-language therapy is recommended since stuttering onset. Various approaches, direct or indirect, are available and speech-language pathologists can propose several treatments. In order to choose the best method, Evidence-Based Practice seems to be the most efficient way to do it but does research allow this approach? We realized a systematic review from Pubmed and Scopus data bases. Finally, 6 articles from the scientific literature were reviewed, including a retrospective study, two non-randomized trial, one randomized trial, one randomized controlled trial and one meta-analysis published between 2010 and 2019. Within these articles, only two trials compared different treatments: Lidcombe Program vs RESTART-DCM and Lidcombe Program vs Westmead Program. The research in this field is still very limited worldwide. The creditable research evidence is quite weak for the main part of the clinical trials. Nowadays, there is no evidence that a treatment is more effective than another in early stuttering. The Lidcombe Program is the most trialed therapy but clinical trials with a long-term follow up are lacking and would be conducted.; Le bégaiement développemental est un trouble de la parole affectant la communication et qui a des répercussions importantes sur la qualité de vie des personnes qui en sont atteintes. Les progrès de la recherche en génétique, en neuroimagerie dans ce domaine sont récents et ne nous permettent pas encore de comprendre totalement la nature de ce trouble. Nous ne sommes toujours pas en mesure de savoir quel bégaiement deviendra chronique et lequel aura une récupération spontanée. Dans ces conditions, une prise en charge orthophonique est vivement recommandée dès les premiers signes de bégaiement. Les orthophonistes ont à leur disposition différentes approches (directes ou indirectes) pour traiter ce trouble. Afin de pouvoir choisir la meilleure thérapie chez le jeune enfant, une démarche basée sur la preuve semble être la plus pertinente mais l’état de la recherche permet-il cette démarche ? Nous avons réalisé une revue de la littérature à partir des bases de données Pubmed et Scopus. Nous avons retenu 6 articles dont une étude rétrospective, deux essais cliniques non randomisés, un essai randomisé, un essai randomisé contrôlé et une méta-analyse publiés entre 2010 et 2019. Parmi ces essais deux seulement sont des études comparatives entre plusieurs traitements : Lidcombe Program vs RESTART-DCM et Lidcombe Program vs Westmead Program. La recherche en la matière est encore très limitée au niveau international. La robustesse de la preuve se révèle faible dans une grande majorité des essais. A ce jour, aucune preuve n’a pu être apportée en faveur d’un traitement dont l’efficacité serait meilleure pour éviter la chronicisation du bégaiement précoce. Le Lidcombe Program est le traitement

Stuttering, a disorder affecting close to 1% of the population, is a public health issue in which speech and language pathologists have an important role to play. However, SLPs surveyed for this study were unanimous in reporting the lack of training material in this specific therapeutic area. The aim of this study is to answer this need and create working material easy to handle, offering transversal leads in the care of these patients with multifactorial displays of their disorder. To do so, the most frequent impairments were identified in the literature and organized according to the CALMS model. This resulted in a mapping of the 5 factorial areas implicated in stuttering : cognitive, affective, linguistic, motor and social. For each identified lacking skill, therapeutical aims were set that resulted in the conception of specific training material. The surveyed SLPs stated that its design was very attractive and its structure based on the 5 skill areas was complete and didactic. After a more thorough assessment, the SLPs confirmed that this work material met their expectations, finding both its analytical and functional contents relevant. Their enthusiasm in regard to this new therapeutic tool will justify its submission to publication so that other SLPs and patients can benefit from it., Le bégaiement, trouble touchant près d’1% de la population, se révèle être un enjeu important de santé publique, au coeur duquel l’orthophoniste a tout son rôle à jouer. Pourtant, les thérapeutes sondées dans le cadre de cette étude se sont montrées unanimes sur l’insuffisante proposition de matériels de rééducation spécifiques au bégaiement à leur disposition. L’objectif de cette étude a donc été de construire un support de prise en soins pratique à prendre en main, qui fournirait des pistes rééducatives transversales répondant à la multifactorialité des manifestations du bégaiement. Pour cela, les altérations fréquemment retrouvées chez les personnes qui bégaient ont été relevées dans la littérature, et structurées autour du modèle CALMS. Cette représentation a offert une schématisation des 5 domaines de facteurs impliqués dans le bégaiement : cognitif, affectif, linguistique, moteur, et social. Pour chacune des compétences déficitaires identifiées, des objectifs thérapeutiques ont été formulés, afin d’aboutir à la conception d’un support de travail pour chaque axe. Les professionnelles interrogées ont jugé son design très attrayant, et son articulation autour de 5 pôles complète et didactique. Après une évaluation plus approfondie, ces thérapeutes ont affirmé que le matériel répondait à leurs attentes, jugeant son contenu pertinent, tant sur le plan analytique que fonctionnel. Leur enthousiasme face à l’apport de ce nouvel outil de prise en soins justifiera sa soumission à publication, afin que d’autres orthophonistes et patients en tirent bénéfice.

The objective of this thesis is to study the phonetic characteristics of sounds on which there are speech dysfluencies of bilingual people with stuttering.To achieve this research, several collections and analyzes of data were carried out. A total of 11 bilingual Mina-French speakers, who stuttered, were recorded in Lomé (Togo). These recordings allowed us to study i-the speech dysfluencies rates for the phonetic features of the dysfluent sound and ii-the phonetic environment of these same dysfluent sound. Nasofibroscopic data were also used to observe events occurring at the laryngeal level during some dysfluencies. An EMA study has been done to observe the movements of the articulatory during typical dysfluencies of stuttering.In general, our results show higher rates of the speech dysfluency in French than in mina. The study of the phonetic features that give rise to the most accidents in speech, shows that unvoiced consonants, nasal vowels and anterior vowels are significantly more stammered. Observation of the phonetic environment of the speech dysfluencies shows that unvoiced dysfluency sounds are generally present in a voiced environment. In addition, data on the larynx notably reveal the presence of inappropriate movements of openings and closings of the glottis during the speech dysfluent sequences including an unvoiced consonant. Finally, the EMA data acquired on a bilingual speaker show that in articulatory terms, the speech dysfluencies are comparable in the two languages.Keywords: stuttering, speech dysfluence, mina, french, bilingualism, L’objectif de cette thèse est d’étudier les caractéristiques phonétiques des phones sur lesquels interviennent des disfluences dans la parole de personnes bilingues présentant un bégaiement.Pour mener à bien cette recherche, plusieurs recueils et analyses de données ont été effectués. 11 locuteurs bilingues mina-français, qui étaient porteuses d’un bégaiement, ont été enregistrées à Lomé (Togo). L’étude des traits phonétiques donnant lieu au plus d’accidents de parole, montre que les consonnes non-voisées, les voyelles nasales et les voyelles antérieures sont significativement plus bégayées. L’observation de l’environnement phonétique des disfluences démontre que les sons non-voisés disfluents sont généralement présentes dans un environnement voisé. Par ailleurs, les données sur le larynx révèlent notamment la présence de mouvements inappropriés d’ouvertures et de fermetures de la glotte lors de séquences disfluentes incluant une consonne non-voisée. Enfin, les données EMA acquises sur une locutrice bilingue montrent qu’en termes articulatoires, les disfluences sont comparables dans les deux langues.Mots clé : bégaiement, disfluence, mina, français, bilinguisme

Mindfulness is a practice increasingly used as a complement to traditional Western medicine therapies. By acting on body awareness, attention control, emotional regulation and metacognitive abilities, full awareness can be beneficial to patients concerned with speech therapy. The objective of this study was to analyze the relevance of the practice of mindfulness in the context of therapies offered to people who stutter. A daily mindfulness program (60 sessions over 3 months) was offered to four stuttering adults. The analysis of their experience showed significant benefits but does not have a direct impact on the fluence of their speech. Based on these conclusions, it would be interesting to consider a complementary study including a larger cohort and aiming to analyze the benefits specifically related to speech of a meditative practice over the long term (6 months minimum).; La pleine conscience est une pratique de plus en plus utilisée en complément des thérapies traditionnellement proposées dans le champs de la médecine occidentale. En agissant sur la prise de conscience corporelle, le contrôle attentionnel, la régulation émotionnelle et les capacités métacognitives, la pleine conscience peut être bénéfique aux patients concernés par une prise en charge orthophonique. L’objectif de cette étude est d’analyser la pertinence de la pratique de la pleine conscience dans le cadre des thérapies proposées aux personnes qui bégaient. Pour répondre à la problématique, un programme quotidien de pleine conscience (60 séances sur 3 mois) est proposé à quatre adultes qui bégaient. L’analyse de leur expérience met en évidence des bénéfices notables mais n’ayant pas une répercussion directe sur la fluence de leur parole. À partir de ces conclusions, il est intéressant d’envisager une étude complémentaire comprenant une cohorte plus importante et visant à analyser les bénéfices spécifiquement liés à la parole d’une pratique méditative sur le long terme (6 mois minimum).

Stuttering is a communication disorder affecting speech which is then considered disfluent. What happens when a person who stutters speaks several languages? There will be constant interactions between multilinguism and stuttering. The disorder will generally appear differently according to the spoken language. Linguistic factors have an impact on stutte­ring, with disfluencies appearing differently according to the speaker’s proficiency in using each language. Emotional factors related to the choice of the spoken language can exag­ge­rate the disorder, hence influencing the speaker’s choice of a language, even choices in life. These factors are to be taken into consideration when assessing stuttering, with a particular attention to cultural aspects. Finally, stuttering variability according to the spoken language will also have an impact on therapeutic decisions.

Dans le monde romain, d’après les textes de la fin de la République et du début de l’Empire, le fait de boiter, ou de vaciller en marchant, est un défaut du corps, comme le bégaiement, qui se dit avec les mêmes termes. Le lexique latin de la claudication sert aussi à décrire les défauts d’un raisonnement ou d’un discours. De façon générale, la boiterie, des jambes ou de la langue, témoigne d’un manque de contrôle de soi. Même lorsqu’elle est la conséquence d’une blessure de guerre, la boiterie est le plus souvent dissimulée par ceux qui en sont atteints, car elle est considérée comme contraire à la beauté digne du citoyen honorable et donc sujette aux railleries. Cependant, elle peut aussi être traitée comme une marque de courage et valorisée. Dans les deux cas, c’est la parole qui lui donne, en quelque sorte, « droit de cité ». In the Roman world, at the end of the Republic and the beginning of Empire, the fact of limping or wobbling while walking is described as a physical defect, like stuttering, which is referred to with the same words. Latin terms for lameness are used also for a defect of reasoning or speech. By and large, lameness of the legs or of the tongue reveals a lack of self-control. Even if lameness is due to a war wound, it is most of the time concealed by those who suffer from it, because it is considered contrary to the beauty of honourable citizens and therefore subject to mockery. However, lameness can also be treated as a sign of courage and thus be valorized. In both cases, speech allows it, in a way, to be part of the city.

The aim of this research is to analyse the timing of articulatory gestures in perceptually fluent speech in people who stutter (PWS) and in control subjects. 4 speakers were recruited to carry out this study: 2 PWS and 2 persons who do not stutter. They were matched up in sex and age. Movements used for the labial closure for the consonant [p], i.e. those of the mandible and the lower and upper lips were tracked by means of an EMA. The starting order for the 3 articulators and the way they reach their velocity peak were analysed during the repetitions of logatomes. Our results show differences between the 2 groups in articulatory sequencing and in the order in which articulators reach their peak velocity., Notre recherche avait pour objectif d’étudier le déroulé temporel des gestes articulatoires chez des personnes qui bégaient (PQB) en parole perceptuellement fluente, tout en les comparant à celui de sujets de contrôle. Quatre personnes ont pris part à cette étude, deux locuteurs qui bégaient et deux locuteurs normo-fluents, appariés en sexe et en âge. Les mouvements nécessaires à la fermeture labiale du [p], à savoir ceux de la mandibule, de la lèvre inférieure et de la lèvre supérieure, ont été suivis grâce à un articulographe électromagnétique. L’ordre de la mise en route des trois articulateurs cités ainsi que l’ordre dans lequel ils atteignent leur pic de vélocité précédant l’occlusion ont été analysés dans les productions orales comprenant des répétitions de logatomes. Nos résultats montrent des différences dans les séquençages articulatoires ainsi que dans l’ordre avec lequel les articulateurs atteignent leur vélocité maximale entre les deux groupes de locuteurs.

Stuttering is a speech fluency disorder. It can be mainly characterized by an increased presence of disfluencies that affect the speech intelligibility. The aim of this thesis is to study stuttering-like disfluencies (SLDs) produced by persons who stutter (PWS) during reading tasks and during spontaneous speech. More specifically, we propose, as our first objective, to verify if any morphological or phonetic elements can explain the presence of these disfluencies. Our second objective is to observe articulatory events before and during SLDs. For the studies dealing with the linguistic and phonetic elements that can be problematic to PWS, 10 French-speaking and 10 Slovak-speaking PWS were recorded while reading a text and while having a conversation in their mother tongue. The studies on speech motor events taking place before and during SLDs were realized by means of an EMA. 4 French-speaking subjects participated in this part of the study (2 PWS and 2 control subjects). Our results show that non-voiced consonants and stops were part of the most problematic elements to produce for PWS. The morphological study reveals that the risk of a SLD appearance was higher when the word contained more morphemes. This result should be correlated to the number of syllables that constitute the word. As for the second couple of studies, they focus on the speech motor events in stuttered speech. Our data show that similar articulatory events can take place in the supraglottic cavity during disfluencies perceived as blocks or prolongations. Furthermore, a disruption of coarticulatory gestures was observed in certain disfluencies., Le bégaiement est un trouble de la fluence de la parole qui se caractérise, entre autres, par une présence accrue d’accidents de parole venant entraver l’intelligibilité de l’énoncé. Ce travail de doctorat a pour objectif d’étudier les disfluences catégorisées comme pathologiques produites par des locuteurs qui bégaient et ce, en tâche de lecture et en situation de parole spontanée. Plus précisément, il s’agit, d’une part, de vérifier si des éléments morphologiques et phonétiques peuvent expliquer l’apparition d’un bégayage et, d’autre part, d’observer les événements articulatoires présents avant et pendant les disfluences. Pour mener à bien les études ayant trait aux éléments linguistiques posant le plus de difficultés aux personnes qui bégaient, 10 locuteurs francophones et 10 locuteurs slovacophones, tous atteints de ce trouble, ont été enregistrés en train de lire un texte et de parler spontanément dans leur langue maternelle. Quant aux travaux portant sur les événements moteurs se déroulant avant et durant les disfluences, ils ont été réalisés grâce à des données EMA acquises auprès de 4 locuteurs francophones (2 locuteurs qui bégaient et 2 sujets normo-fluents) en tâche de lecture. Nos résultats ont montré que les consonnes non-voisées et les occlusives faisaient partie des éléments les plus problématiques à prononcer pour les personnes bègues. L’étude morphologique a révélé que plus un mot contient de morphèmes et plus le risque de voir apparaître une disfluence est accru. Ce résultat doit notamment être mis en corrélation avec le nombre de syllabes présentes dans le mot. En ce qui concerne le second couple d’études, portant sur le niveau moteur de la parole bègue, nos données montrent, en particulier, des similitudes dans les événements articulatoires se déroulant au niveau supra-glottique entre les disfluences perçues acoustiquement comme des blocages et des prolongations. Enfin, une perturbation des gestes coarticulatoires a pu être relevée lors de la production de certaines disfluences.

Normal disfluences vs. stuttering-like disfluences: an acoustic study The aim of this research is to study acoustic and perceptive characteristics of stuttering-like disfluencies and other disfluencies. An acoustic analysis has been conducted on stuttering-like disfluencies produced by four persons who stutter and on other disfluencies produced by four non-stutterers that were annotated by all members of a jury of expert listeners. Different parameters, including duration and number of disfluencies or their prosodic particularities, were studied. Our results show that, in addition to the duration and number of disfluencies, other parameters can be used to differentiate other and stuttering-like disfluencies, such as presence or not of audible tensions within the disfluency, type of repeated elements (e.g. word vs. sound) or location of the disfluency., RESUME L'objectif de cette recherche est d'étudier les caractéristiques acoustiques et perceptives des disfluences normales et sévères. Pour ce faire, un jury d'auditeurs experts a relevé les disfluences sévères et normales de 4 locuteurs qui bégaient ainsi que les accidents de parole de 4 sujets normo-fluents. Une analyse acoustique portant sur des paramètres tels que la durée de la disfluence, le nombre de disfluences ou encore sur la présence d'éléments prosodiques particuliers a été menée sur les seules disfluences ayant été relevés par l'ensemble du jury. Nos résultats montrent que si les prolongations et les répétitions sont bien évidemment catégorisées comme sévères respectivement en fonction de leur durée et du nombre d'éléments réitérés, d'autres paramètres sont également significatifs, tels que la présence ou non d'une tension audible, le type d'éléments répétés ou encore le fait que la syllabe soit ou non clivée.

Stuttering concerns about 1% of the worldwide adult population, but is a lot more common amongst children. About 8% of children seem to be facing stuttering at the age of 3 to 5 years old. 80% of them will recover with or without the help of a speech therapist. The Lidcombe Program is a method of treatment used for preschoolers which requires parents to evaluate their child’s stuttering on a daily basis. The elaboration of a website thanks to which it will be possible to collect the severity rating scale of several children and a corpus of videos taken during treatment will enable to evaluate the reliability of this subjective score by linking it to objective indicators (percentage of stuttered syllables and speech rate). This corpus will also be used to deal in depth with the results found concerning coarticulation and vowel centralization in children during the elaboration of a scientific poster, and compare them to the results found for adults by Marine Verdurand-Pendeliau in her thesis.; Le bégaiement touche environ 1% de la population adulte mondiale, mais est beaucoup plus répandu chez les enfants. En effet, on estime que 8% des enfants sont touchés par le bégaiement vers 3 à 5 ans. 80% d’entre eux retrouverons une parole fluente avec ou sans suivi par un orthophoniste. Le Programme Lidcombe est une méthode de suivi des enfants d’âge préscolaire ou scolaire qui requiert l’évaluation quotidienne du bégaiement de l’enfant par ses parents. L’élaboration d’un site web sur lequel il sera possible de recueillir le score des enfants sur l’échelle de sévérité à grande échelle ainsi qu’un corpus de vidéos prises durant le traitement des enfants va permettre d’évaluer la fiabilité du score subjectif donné par les parents en le corrélant avec des indicateurs objectifs tels que le pourcentage de syllabes bégayées et le débit de parole. Ce corpus permettra également d’approfondir les résultats trouvés lors de l’élaboration d’un poster scientifique pour la coarticulation et la centralisation des voyelles chez les enfants en comparaison avec les données obtenues pour les adultes dans la thèse de Marine Verdurand-Pendeliau.

Stuttering is a complex (complicated-intricate) disorder, and it is difficult to give a satisfactory and complete definition of it. As well as secondary symptoms and entailed comportments, language factors can interfere in this disorder. Difficulties in phonological competence might be concomitant if not interactive in stuttering, with, mainly in adult language, an inconsiderable impact of the phonological complexity Furthermore, the coarticulatory difficulties might be at the core of stuttering. But the results of different studies diverge on the subject when both children and adults' stuttering are considered. And let us note that the influence of the auditory-feedback is surprising as every change in this feedback seems to have an improving power in an appreciable number of patients. So the definition of stuttering, being a production disorder, can be questioned. Stuttering rather appears as a perceptual motor disorder. The aim of this thesis is to study the fluent and disfluent language in French and Italian stutterers knowing that the differences between stutterers and fluent persons mainly appear when the motor system is conditional to a destabilizing factor. We chose to study how the coarticulatory behavior adapts when the phonological complexity increases. We also tried to analyze how the modification of the auditory-feedback influences the coarticulatory behavior. And we studied the part played by the phonological complexity on disfluencies mainly in a situation of dialogue over a picture. Italian and French adults and children stutterers and their fluent counterparts were registered placed in four different situations of language: reading, spontaneous speech, repetitive task and picture description and in two perceptive conditions: a normal one and one with modified auditory-feedback. When placed in normal perceptive condition, the stutterers' coarticulation is weaker than that of their fluent counterparts. Besides, the language seems to play an important role since Italian and French stutterers have not the same response towards their fluent counterparts. Let us note that this stutterers' coarticulatory comportment is also sensitive to the increase of the phonological complexity. The results we have when the auditory-feedback is altered are contradictory. Disfluency could also be influenced by phonological complexity but this influence seems to be dependent on the acuteness of the stuttering as well as on the stutterer's age. A significant clinical breakthrough is evoked, Le bégaiement est un trouble complexe, qu'il est encore difficile de définir de manière satisfaisante et complète. Outre les symptômes secondaires, et comportements accompagnateurs, des facteurs langagiers peuvent interférer avec ce trouble. Des difficultés dans les compétence phonologiques pourraient être concomitantes, voire en interaction avec le bégaiement, avec notamment dans la parole adulte, un impact non négligeable de la complexité phonologique. Par ailleurs, des difficultés coarticulatoires seraient au cœur du bégaiement. Mais les résultats des différentes études sont très disparates, tant dans la parole des enfants que dans celle des adultes qui bégaient. Enfin, l'influence du feedback auditif est surprenante puisque toute modification de ce type de feedback a un pouvoir améliorant chez un certain nombre de personnes. Ce constat questionne la définition du bégaiement en tant que trouble de la production et l'oriente plutôt vers un trouble perceptivo-moteur. Cette thèse se propose d'étudier la parole fluente et disfluente de personnes bègues françaises et italiennes. Etant donné que les différences entre personnes qui bégaient et personnes fluentes, apparaissent essentiellement quand le système moteur est soumis à un facteur déstabilisant, nous avons choisi d'étudier l'adaptation du comportement coarticulatoire lorsque la complexité phonologique augmente. Nous avons également voulu analyser l'impact de la modification du feedback auditif sur le comportement coarticulatoire. Enfin, nous avons étudié le rôle de la complexité phonologique sur les disfluences notamment, dans une situation de dialogue autour d'une image. Des adultes et des enfants, italiens, et français, bègues et fluents ont été enregistrés dans 4 situations de parole : lectures, discours spontané, tâche de répétition, et tâche de description d'image. Toutes ces tâches ont été réalisées dans deux conditions perceptives : une condition normale, et une condition avec feedback auditif modifié. En condition perceptive normale, la coarticulation des personnes qui bégaient est plus faible que celle des personnes fluentes. La langue semble jouer également un rôle important puisque les personnes qui bégaient des deux langues ne se comportent pas toujours de la même manière vis-à-vis de leurs homologues fluents. Le comportement coarticulatoire des personnes bègues semble également sensible à l'augmentation de la complexité phonologique. Les résultats sous feedback auditifs modifiés apparaissent contradictoires. Par ailleurs, les disfluences pourraient être influencées par la complexité phonologique, mais cette influence semble dépendre de la sévérité du bégaiement et de l'âge du sujet. Des perspectives cliniques sont évoquées.

The aim of this study is to analyse the steady-state portion of the first two formants (F1 and F2) in the production of [CVp] sequences, containing vowels [i, a, u] pronounced in two speech rates (normal and fast), by groups of untreated and treated stutterers, and control subjects.We recorded 15 speakers (5 control speakers, 5 non-treated stutterers and 5 treated stutterers) who had to repeat sentences containing [CV] sequences ten times, in normal and fast speech rates, where [C] was [p], [t] or [k] and [V], [i], [a] or [u].Comparing data between the three groups of speakers, a vowel space reduction is observed for stutterers at a normal speaking rate, compared with control speakers and treated stutterers. When speech rate increases, no reduction of vowel space is noticeable for non-treated stutterers, contrary to treated stutterers and controls.Results for stutterers could be interpreted in terms of lack of articulatory flexibility.

These disfluencies are characterized by an interruption at the border of a word and its resumption (the repetition) or an interruption in the middle of a word followed or not by a resumption (the word-fragment). These two types of disfluencies prove to be distinct and quite different from what can be observed for example in the stuttering. The detailed analysis, in a one hour corpus, of readjustments after the speaker has produced a word-fragment shows that the speaker always resumes at least the interrupted word (what sometimes the stammerer does not do). He goes back in 70% of the cases till the beginning of the group (nominal or verbal) but he can also exceed this limit or content himself with resuming only the affected word.; Une étude a été entreprise sur des achoppements à l'oral, en français parlé. The studies on some characteristics of the stammering show that the differences observed between stutterering subjects and non stutterers are much more quantitative than qualitative. Except for the dislocation of the syllables (which seems to be pathognomonic), the disfluences (repetitions, truncations, discursive particles) are more frequent, affect more certain types of words but that remains within the structure observed in standard speakers.; Les performances langagières (d'un point de vue morphosyntaxique et syntaxique) d'un sujet bègue ont été comparées à celles observées dans des énoncés standard provenant d'un important corpus de français parlé (CRFP). The author presents a short tutorial of Gilles de la Tourette's syndrome followed by a state of the art of review of the clinical links between stuttering and the different kinds of Tourette and tics disorders. The author relates her own experience of working for four consecutive years at the new stuttering diagnostic center at the "Hôpital Européen Georges Pompidou ". The article ends with a practical review of the actual therapies available when such a diagnosis is made.

The "Miror" (1974) of Andrei Tarkovski and "Passion" (1982) of Jean-Luc Godard are two movies where the stuttering takes an important place. This trouble is a main subject of these two movies.

Echo-reading is a practical therapeutic tool which has been proposed for use in stuttering, most especially when the functions of rhythm and association are disrupted in stuttered speech. After a period of preparation in which the mechanical work involved in speech is integrated, it is proposed that there should follow a period of listening and association, which serves to re-establish the spoken word with its meaning and expressivity. The subject undergoes rehabilitation in a living language which is correlated with the imagination, and with that Other quality which always forms a part of speech.

For some years the dystonias have been the subject of major studies and, as far as the generalised dystonias are concerned, of major therapeutic advances. The opposite is true of the so-called focal or functional dystonias, which include conditions such as Meige's syndrome, spasmodic torticollis, writer's cramp, dystonias using instruments especially in musicians, and spasmodic dysphonia. For the last group, the term functional dysphonia would seems to us to be more appropriate. It would appear that what is involved is a disorder not of a muscle group, but rather of a function. Consequently stuttering can, in our opinion, be legitimately considered as a dystonia affecting the articulation of speech, within the global context of a new neurological grouping which could be called 'dysfunctional neurology'.

Fluency disorders other than stuttering, which is the major and above all most known form, can be observed and require rehabilitation. Fluency is the quality which enables speech to flow effortlessly with a comfortable delivery, pace and simple transitions. Acquired over a period of several years during childhood, fluency can be altered by numerous neurological syndromes, such as Parkinson's disease, spasmodic dysphonia, or substitution phonations such as the esophageal or tracheoesophageal voice. Their severity is variable and judged above all by the efficacy of communication. Management of the duration and spacing of language segments and their homogeneity with respect to meaning are just as important as the stability of the phonatory organs.

The language can not be broken away from the oral: the actual psychoanalytic conceptions are called again. That allows to approach the stammering in its clinical theoretic aspects. One can consider the relation to the mother, the accession to the spoken word, the voice's wealth and possibilities, the necessity of the listening and of silence, with in all its aspects the emergence of the differences in stammerer subject or not. The links with the body and/or with the corporeal bring the notions on the psyche or psychic reality and permit to understand better the importance of a therapeutic activity (sufficient or with other treatments) the relaxation: its effects are multiples and I can give to the spoken word more spoken word.

Stuttering implements communication with interference through challenges other than sharing information, messages and emotion. This dramatic behaviour implies a phobic origin. The therapist must reproduce the symptom in its overall economy. Attentive listening, reassurance and non-submission of the esteem to the performance of expression should lead to a veritable reassessment of the challenges of expression.

The purpose of this article is to summarize those main indications where neuroleptics have been proven efficient in children. In some types of pathology (hyperactivity, conduct disorders, mental retardation with agressiveness...), their impact is purely symptomatic, generally sedative. In other cases (Tourette's syndrome, stuttering) they seem to have a more specific effect. Whichever may be the case, neuroleptics seem to be irremplaceable, whenever the symptoms become incapacitating such as when they hinder the person's social functioning and learning abilities. On a practical level, one has to avoid "poorly justified" prescriptions, including low-dose prescriptions. In children and adolescents, therapy cannot be considered for one angle only: the administration of neuroleptics has to be integrated into a general treatment plan which involves other therapeutic approaches which are not mutually exclusive.
