Contexte : Bien que le Canada dispose à la fois d’un système national de surveillance active et de données administratives pour la surveillance passive des infections associées aux soins de santé (IASS), chacun des deux systèmes présente des forces et des faiblesses au niveau de la collecte et de la communication des données. La surveillance active et la surveillance passive fonctionnent de manière indépendante, ce qui donne parfois des résultats divergents. Pour comprendre les divergences qui existent entre les données administratives sur la santé et les données issues de la surveillance active, un examen de la portée a été réalisé. Méthodes : Des recherches ont été menées dans les bases de données Medline, Embase et Cumulative Index to Nursing and Allied Health Literature, ainsi que dans la littérature grise pour trouver des études en anglais et en français évaluant l’utilisation de données administratives, seules ou en comparaison avec les données issues de la surveillance traditionnelle et réalisées au Canada entre 1995 et le 2 novembre 2020. Après avoir extrait les renseignements pertinents des articles sélectionnés, un résumé descriptif des résultats a été présenté, accompagné de suggestions pour améliorer les systèmes de surveillance, dans le but d’optimiser la qualité globale des données. Résultats : Seize articles répondaient aux critères d’inclusion, dont douze études observationnelles et quatre revues systématiques. Ces études ont démontré que l’utilisation d’une seule source de données administratives n’était pas assez précise pour la surveillance des IASS, par rapport à la surveillance active traditionnelle. Cependant, la combinaison de différentes sources de données ou la combinaison de données administratives et de données issues de la surveillance active en améliore la précision. Les systèmes de surveillance électronique peuvent également renforcer la surveillance en augmentant la capacité à détecter les IASS potentielles. Conclusion : Bien que la surveillance active des IASS ait produit les résultats les plus précis et demeure la norme d’excellence en la matière, l’intégration des données issues de la surveillance active et passive peut être optimisée. Les données administratives peuvent être utilisées pour renforcer la surveillance active traditionnelle. De futures études sont nécessaires pour évaluer la faisabilité et les avantages des solutions potentielles proposées pour l’utilisation des données administratives pour la surveillance et la déclaration des IASS au Canada.

3. Divergences between healthcare-associated infection administrative data and active surveillance data in Canada

Background: Although Canada has both a national active surveillance system and administrative data for the passive surveillance of healthcare-associated infections (HAI), both have identified strengths and weaknesses in their data collection and reporting. Active and passive surveillance work independently, resulting in results that diverge at times. To understand the divergences between administrative health data and active surveillance data, a scoping review was performed. Method: Medline, Embase and Cumulative Index to Nursing and Allied Health Literature along with grey literature were searched for studies in English and French that evaluated the use of administrative data, alone or in comparison with traditional surveillance, in Canada between 1995 and November 2, 2020. After extracting relevant information from selected articles, a descriptive summary of findings was provided with suggestions for the improvement of surveillance systems to optimize the overall data quality. Results: Sixteen articles met the inclusion criteria, including twelve observational studies and four systematic reviews. Studies showed that using a single source of administrative data was not accurate for HAI surveillance when compared with traditional active surveillance; however, combining different sources of data or combining administrative with active surveillance data improved accuracy. Electronic surveillance systems can also enhance surveillance by improving the ability to detect potential HAIs. Conclusion: Although active surveillance of HAIs produced the most accurate results and remains the gold-standard, the integration between active and passive surveillance data can be optimized. Administrative data can be used to enhance traditional active surveillance. Future studies are needed to evaluate the feasibility and benefits of potential solutions presented for the use of administrative data for HAI surveillance and reporting in Canada.

4. Untargeted Metabolomics Analysis of Serum Metabolites in Zucker Diabetic Fatty Rats

Zucker Diabtic Fatty rats is a common model used for the studies related to type 2 diabetes. Metabolomics, proven by previous studies, can be used for the prediction of phenotype. This investigation held untargeted metabolomics analysis on the serum metabolites of the Zucker Diabetic Fatty rats, intended to gain insights in the study of the pathology of type 2 diabetes. LCMS method (liquid chromatography coupled with mass spectrometry) is an analytical chemistry technique used to separate and purify complex mixtures. After identifying metabolites using MS2 library spectrum matching, this investigation used Metabo Analyst to compare fatty rats and lean rats. Lastly, by applying over representation analysis, we identified seven metabolic pathways that are important for humans, which assists in discovery of therapeutic targets and the developments of diabetes biomarkers.

5. Expanding Free School-based Human Papilloma Virus (HPV) Vaccination Programs to Include School-aged Males in Nova Scotia, Canada

Bill 70 (HPV Vaccine Act) was presented to the Nova Scotia House of Assembly with the aim of expanding the current Nova Scotia school-based HPV vaccination program to include males. In recent years, increased awareness of HPV and HPV-caused cancers has led to the implementation of school-based female HPV vaccination programs across Canada. Changing guidelines, based on recent evidence, suggest that males should also be included in these programs. Program expansion to include males aims to reduce the prevalence of HPV-causing cancers and their ensuing costs, to promote equal access to healthcare services, and to make Nova Scotia a leader in HPV prevention. Support from the Canadian public and high profile political actors along with pressure from other provinces and interest groups, including the Society of Obstetricians and Gynaecologists of Canada, influenced the passing of the HPV Vaccine Act. In order to implement this reform, the provincial financial commitment to the previous HPV program was expanded to cover the cost of male vaccination. Le projet de loi 70 (Loi sur les vaccins contre le papillomavirus), dont l’objet est d’étendre aux garçons le programme de vaccination contre le papillomavirus humain (PVH) en vigueur actuellement dans les écoles de Nouvelle Écosse, a été présenté devant l’Assemblée parlementaire de la province. La prise de conscience récente du rôle du PVH comme facteur de risqué dans de nombreux cancers a conduit à la mise en place de programmes de vaccination scolaires pour les filles partout au Canada. Des données empiriques récentes ont conduit à modifier les recommandations de pratiques pour ouvrir ces programmes aux garçons. L’objectif d’une telle ouverture des programmes est de diminuer la prévalence (et les coûts) des cancers provoqués par le PVH, ainsi que de renforcer l’égalité d’accès aux soins et de placer la Nouvelle Écosse à la pointe de la prévention du PVH. Une opinion publique canadienne favorable, ainsi que le renfort d’acteurs politiques majeurs et la pression venue d’autres provinces et de groupes d’opinion, comme la Société des Obstétriciens et Gynécologues du Canada, ont contribué au vote de la loi sur la vaccination PVH. La province a augmenté son engagement financier dans le programme PVH afin de couvrir le coût des vaccins pour les garçons.

6. Simple innovative comparison of costs between tied-arch bridge and cable-stayed bridge

The proposed paper compares tied-arch bridge alternatives and cable-stayed bridge alternatives based on needed load-bearing construction material amounts in the superstructure. The comparisons are prepared between four tied arch bridge solutions and four cable-stayed bridge solutions of the same span lengths. The sum of the span lengths is 300 m. The rise of arch as well as the height of pylon and cable arrangements follow optimal dimensions. The theoretic optimum rise of tied-arch for minimum material amount is higher than traditionally used for aesthetic reason. The optimum rise for minimum material amount parabolic arch is shown in the paper. The mathematical solution uses axial force index method presented in the paper. For the tied-arches the span-rise-ration of 3 is used. The hangers of the tied-arches are vertical-The tied-arches are calculated by numeric iteration method in order to get moment-less arch. The arches are designed as constant stress arch. The area and the weight of the cross section follow the compression force in the arch. In addition the self-weight of the suspender cables are included in the calculation. The influence of traffic loads are calculated by using a separate FEM program. It is concluded that tied-arch is a competitive alternative to cable-stayed bridge especially when asymmetric bridge spans are considered.

7. Impact of Single-sex Instruction on Student Motivation to Learn Spanish

AbstractTo increase male motivation to learn additional languages studies have suggested teaching males in single-sex second and foreign language classes (Carr & Pauwels, 2006; Chambers, 2005). Despite the reported benefits of this unique arrangement, a review of literature found no related research conducted in Canada or the United States. To address this lack of research, a study was conducted in the spring of 2008 to investigate the impact of single-sex instruction on student motivation to learn Spanish. Using Gardner's model of second language motivation (1985), 57 high-school students studying Spanish in either single-sex or coeducational classes completed a pre and post questionnaire to gauge their motivation to learn the language. Follow-up interviews were also conducted with both students and teachers. Results indicated that while both sexes enjoyed some educational advantages from the single-sex environment, the benefits appeared to be greater for the males than the females.RésuméAfin d'accroître la motivation des garçons pour l'apprentissage des langues, certaines études ont suggéré d'enseigner les langues secondes ou étrangères à des classes de garçons exclusivement (Carr & Pauwels, 2006; Chambers, 2005). Malgré le bénéfice confirmé d'un tel contexte, il n'existe aucune recherche similaire connue sur ce sujet au Canada ou aux États Unis. Pour remédier à cette lacune, on a conduit au printemps 2008 une étude portant sur l'impact de l'enseignement non mixte sur la motivation des élèves à apprendre l'espagnol. Selon le modèle de Gardner quant à la motivation dans l'apprentissage d'une langue seconde (1985), 57 élèves au niveau secondaire apprenant l'espagnol dans des classes mixtes et non mixtes ont rempli avant et après le cours, des questionnaires destinés à mesurer leur motivation. Le suivi a été assuré par le biais d' entrevues avec élèves et professeurs. Les résultats montrent que si les élèves des deux sexes bénéficiaient d' un environnement éducatif non-mixte, les garçons semblaient en tirer plus d'avantages.

This paper proposes a method to estimate the appropriate minimum output of diesel generator in the microgrid system that composed of renewable energy, diesel generator and energy storage systems. While maximizing the output of the renewable energy in the microgrid, to maintain the power system stability is very important. To analyze the effectiveness of the proposed method, simulation is carried out by using the PSCAD/EMTDC program with past weather and power load data in Chuja Island located in the south area of Korean peninsula, respectively. In the simulation result, power quality of Chuja Island power system under variable renewable energy generation is stable. The proposed method is useful for operating the isolated microgrid system.

Introduction: De nombreuses études ont démontré les effets bénéfiques de l’éducation thérapeutique du patient (ETP) en terme d’obtention d’objectifs biomédicaux, d’acquisition de compétences d’auto-soins et d’« empowerment » chez les patients souffrant de maladies chroniques. Le but de cette enquête est d’explorer de manière détaillée l’impact de l’ETP sur les capacités d’adaptation à la maladie (connaissance et confiance en soi, gestion des émotions, capacité à se fixer des buts, à communiquer et à développer des relations) et l’acceptation de la maladie chez des patients diabétiques de type 2 en situation de précarité sociale, ainsi que les facteurs facilitant ou freinant l’obtention de ces effets. Matériels et méthodes: Une enquête qualitative a été menée par le biais d’entretiens individuels semi-directifs enregistrés auprès de 18 participants ou usagers du réseau PREVALENS (Lens). Les patients de plus de 18 ans, diabétiques de type 2, ayant un score EPICES supérieur à 30 et ayant réalisés ou suivis au moins un cycle d’ETP dans les 6 mois précédents étaient inclus dans l’étude après consentement, délivré par des professionnels de santé libéraux Résultats: Les entretiens ont pu mettre en évidence des effets perçus par les personnes interrogées : Amélioration de l’acceptation de la maladie, de l’image de soi, de la capacité à communiquer et à se socialiser. Les principaux facteurs ayant aidé à maintenir les objectifs de l’ETP étaient la motivation personnelle, l’influence des proches et des autres participants. Ceux ayant pu le freiner étaient l’influence familiale, plus rarement les contraintes financières, d’emploi du temps, ou d’éloignement géographique. Conclusion: Bien qu’elles soient en situation de vulnérabilité sociale, les personnes interrogées décrivent des effets en terme d’amélioration des compétences d’adaptation à la maladie suite aux séances d’ETP. Il pourrait être utile d’explorer l’influence des facteurs favorisants ou freinants les plus fréquemment évoqués par ces patients afin d’optimiser l’ETP dans cette population. - Abstract
Purpose: To determine if this educational care meets the needs of the patients., Objective: To show that to know the representations made by the patient on his disease, his health and on his educational needs allows to define with him a programme of adapted accompaniment and to reduce the risk of ineffective care of his therapeutic programme., Method: 7 interviews of patients were carried out before their integration in an educational process, in three different medico-surgical specialities, with the condition that the latter offer to the patients a formalized educational process. An interview of each of the 3 people in charge or referent of the educational unit was also carried out to understand the educational care proposed to the patients. The grid of interview of the patient used original techniques of expression, and aimed at investigating the representations of the patient on his disease, his health and his educational needs., Results: The 3 services in which the interviews of the patients took place present 3 different educational structures of care: the first one has a profile of "psycho-pedagogical" educational care, the second, a profile of "psychological" type and the third, a profile of "educational" type. They thus propose different tools of educational care. The exploration of the representations of the patients shows that the latter have a groundless vision of the disease and of health within our hospitals which are "health promoters" today. Their educational needs are finally very far from their pathology, and thus from the medical field. They both take root in their personality and in society. Consequently, the 3 aforesaid educational structures are only very partially adapted to the needs of the patients. The educational optimal care of the patient can only be realized "tailor-made" to every patient, in a relation of collaboration between the health professionals and the patients. Finally, it must come largely outside the hospital., Conclusion: The educational care of the patient turns out much wider than the simple sanitary framework, consequently, besides a necessarily multiprofessional and interdisciplinary teamwork it requires to show creativity and open-mindedness in comparison of various points of view, to modify as health professional our representations of the disease, of health and of the educational needs of the patients.
