Abstract

Medication errors including inappropriate prescriptions and drug omissions are one of the causes of adverse drug events in children. Our aim was to develop a preliminary screening tool to detect omissions and inappropriate prescriptions in pediatrics based on French and international guidelines.Disease classification was based on the prevalence rate of pathology and hospital statistics. The criteria were obtained by reviewing many French and international references. The Delphi consensus technique was used to establish the content validity of POPI. The level of agreement and the proposals of healthcare professionals was noted on a nine-point Likert scale.The criteria were categorized according to the main physiological systems (gastroenterology, respiratory infections, pain, neurology, dermatology, and miscellaneous). They were distributed to 16 French pediatric panelists (eight pharmacists, eight pediatricians who were hospital-based [50%] or working in the community [50%]). After two rounds of the Delphi process, 101 of 108 criteria were chosen with strong consensus (76 inappropriate prescriptions and 25 omissions).POPI is the first screening tool to detect inappropriate prescriptions and omissions in pediatrics. It is now necessary to conduct a prospective study to determine inter-rater reliability and the tool's detection capacity.

Abstract

Résumé Objectif L’erreur médicamenteuse est une des causes d’iatrogénie médicamenteuse chez les enfants. Notre objectif était de développer un premier outil de détection des prescriptions médicamenteuses inappropriées ou des omissions de prescription en pédiatrie à partir des recommandations françaises et internationales. Matériel et méthodes Les thèmes abordés ont été choisis selon la fréquence des pathologies dans la population. Les critères ont été développés à partir des recommandations des sociétés savantes et des instances nationales. La méthode Delphi a été utilisée afin de valider les propositions de l’outil. Le niveau d’accord et les propositions des professionnels de santé a été noté sur une échelle de Likert à 9 points. Résultats L’outil POPI comprend cinq catégories (pathologie divers, digestif, oto-rhino-laryngologie/pneumologie, dermatologie et neuropsychiatrie). Tous les critères ont été argumentés par des références bibliographiques. Ils ont été soumis à 16 professionnels de santé français : 8 pharmaciens et 8 médecins (8 hospitaliers et 8 libéraux). Après deux tours de Delphi, 101/108 critères ont été retenus (76 prescriptions inappropriées et 25 omissions). Conclusion POPI est le premier outil pédiatrique pour la détection des prescriptions inappropriées et des omissions de prescription. Cet outil permettra d’estimer la proportion des prescriptions inappropriées et son impact devra être évalué. Abstract

Résumé: La gale est une maladie en constante augmentation en Île-de-France. Le traitement de référence, l’Ascabiol® (benzoate de benzyle 10 %/sulfiram), est en rupture depuis plusieurs mois et son retour est incertain. Pour faire face à cette pénurie, l’Agence nationale de sécurité du médicament (ANSM) a importé un médicament d’Allemagne, l’Antiscabiosum 10 % (benzoate de benzyle), afin de pallier les contre-indications des scabicides restant sur le marché en France (ivermectine, esdépalléthrine). Toutefois, les nourrissons de moins de 1an (<15kg) et asthmatiques restent en impasse thérapeutique. Un groupe de travail multidisciplinaire s’est réuni afin de réfléchir aux différentes alternatives thérapeutiques existantes en France et à l’étranger. À partir des préconisations de l’ANSM et des réflexions du groupe, une proposition de prise en charge des patients a été réalisée sous la forme d’un logigramme. Toutefois, le contexte pédiatrique de la prise en charge nécessite l’utilisation de certains médicaments en dehors de leurs autorisations de mise sur le marché. Les traitements proposés étant pour certains méconnus des praticiens, des ordonnances-types, précisant la posologie, modalités d’utilisation et de dispensation, et des fiches à destination des patients ont été réalisées afin de favoriser l’efficacité des traitements.
