Abstract

Les lésions traumatiques du squelette en pratique pédiatrique sont fréquentes, faites de macrotraumatismes et de microtraumatismes répétés. Le diagnostic et le bilan de cette pathologie sont, dans l'immense majorité des cas, réalisés grâce aux clichés standards, surtout pour le squelette appendiculaire. Les fractures de l'enfant sont plus souvent diaphysométaphysaires incomplètes qu'épiphysométaphysaires. Elles touchent principalement le membre supérieur par ordre de fréquence. Il existe de nombreuses images pièges, variantes de la normale ; la consultation d'ouvrages de référence est fondamentale. Les clichés standards en incidences orthogonales sont la règle et permettent de diagnostiquer et de faire le bilan précis de la majorité de ces fractures. Les clichés comparatifs ne doivent pas être systématiques mais réservés à quelques rares situations. L'échographie a peu de place dans cette indication chez l'enfant. Le scanner et l'imagerie par résonance magnétique (IRM) sont à réserver à la compréhension de fractures complexes ou de leurs complications évolutives potentielles (épiphysiodèse). En revanche, l'IRM présente un intérêt évident dans certaines indications communes à l'enfant et à l'adulte, comme les lésions méniscoligamentaires, les fractures occultes, les fractures de fatigue, voire les fractures apophysaires. D'autres indications sont propres à la pathologie pédiatrique, en particulier les lésions intéressant la physe ou cartilage de croissance. Le développement de la pratique du sport chez l'enfant et l'adolescent est à l'origine d'une pathologie traumatique particulière.L'imagerie par résonance magnétique nucléaire (IRM) est la méthode d'imagerie appropriée pour l'étude de la maturation cérébrale, en particulier de la myélinisation. L'IRM apporte aussi une approche fonctionnelle du développement cérébral par l'utilisation de séquences de diffusion et de la spectrométrie du proton. Dans cet article, les techniques IRM sont détaillées pour les périodes à la fois pré- et postnatales. L'effet de la maturation sur le signal de résonance magnétique nucléaire doit être également pris en compte afin de comprendre la maturation normale et par conséquent son influence sur les différentes pathologies rencontrées avant et après la naissance. L'IRM étant aveugle en termes de signal sur les séquences conventionnelles après 2ans, une attention particulière est donc donnée aux images de diffusion et à la spectrométrie de protons.

Abstract

Diffuse ganglioneuromatosis of the digestive tract is a rare condition, especially in children. It is frequently associated with multiple endocrine neoplasia type 2b and less commonly with neurofibromatosis type 1 (NF1). We report the case of an 8-month-old baby presenting with vasoactive intestinal polypeptide (VIP)-secreting diffuse ganglioneuromatosis affecting the small intestine and the colon and responsible for severe hydric diarrhea. Postoperatively the infant's symptoms resolved and the serum VIP level was normal. NF1 was clinically suspected and then confirmed through genetic testing. Abstract

The pseudopapillary tumor of the pancreas (or Frantz's tumor) is a rare exocrine pancreatic tumor with a low degree of malignancy. It occurs more frequently among women between 20 and 40 years of age and in the Asian population. This tumor is rarer in the pediatric population. The symptoms are subtle, the most striking being pain and an abdominal mass. Pathologically, the tumor is usually well circumscribed with regions of necrosis, hemorrhage and cystic degeneration. A thick, fibrous capsule is often present. The low grade of malignancy of this tumor with a fibrous capsule led to perform a surgical resection. The localization and local invasion determine the surgical technique. Despite its potential for local infiltration and metastatic disease (up to 15% confined often to the liver), the prognosis is favorable after a surgical resection with correct margins. Long follow-up is necessary to detect a possible recurrence, even late.

Abstract

Preeclampsia, a pregnancy-specific syndrome characterized by hypertension, edema and proteinuria, resolves spontaneously on placental delivery. Its pathogenesis is thought to involve placental hypoxia, which leads to maternal vascular dysfunction through increased placental release of anti-angiogenic factors such as the soluble form of VEGF receptor-1 (VEGFR1). VEGFR1 binds VEGF and PIGF, which are also produced by villous trophoblastic cells. In the absence of VEGF and PIGF in the maternal circulation, endothelial dysfunction occurs in several vascular territories (liver, kidneys, brain, heart, lungs, etc.). In experimental models, sVEGFR1 not only has an anti-angiogenic action but also augments endothelial expression of NO synthase through intracellular transduction. When NO production is increased, pericytes and perivascular smooth muscle cells are recruited and their adhesion to endothelial cells is strongly stimulated. This can hinder both trophoblast invasion and increase uteroplacental flow during preeclampsia.
