Abstract

L’analyse des emplois prépositionnels et des emplois conjonctifs d’ “excepté”, de “sauf” et d’ “hormis” permet d’envisager les trois prépositions/conjonctions comme le pivot d’un binôme, comme la plaque tournante d’une structure bipolaire. Placées au milieu du binôme, ces prépositions sont forcées par leur sémantisme originaire dûment métaphorisé de jouer le rôle de marqueurs d’inconséquence systématique entre l’élément se trouvant à leur gauche et celui qui se trouve à leur droite. L’opposition qui surgit entre les deux éléments n’est donc pas une incompatibilité naturelle, intrinsèque, mais extrinsèque, induite. Dans la plupart des cas (emplois limitatifs), cette opposition prend la forme d’un rapport entre une « classe » et le « membre (soustrait) de la classe », ou bien entre un « tout » et une « partie » ; dans d’autres (emplois exceptifs), cette opposition se manifeste au contraire comme une attaque de front portée par un « tout » à un autre « tout ». De plus, l’inconséquence induite mise en place par la préposition/conjonction paraît, en principe, tout à fait insurmontable. Dans l’assertion « les écureuils vivent partout, sauf en Australie » (que l’on peut expliciter par « Les écureuils vivent partout, sauf [qu’ils ne vivent pas] en Australie »), la préposition semble en effet capable d’impliquer le prédicat principal avec signe inverti, et de bâtir sur une telle implication une sorte de sous énoncé qui, à la rigueur, est totalement inconséquent avec celui qui le précède (si « les écureuils ne vivent pas en Australie », le fait qu’ils « vivent partout » est faux). Néanmoins, l’analyse montre qu’alors que certaines de ces oppositions peuvent enfin être dépassées, d’autres ne le peuvent pas. C’est, respectivement, le cas des relations limitatives et des relations exceptives. La relation limitative, impliquant le rapport « tout » - « partie », permet de résoudre le conflit dans les termes d’une somme algébrique entre deux sous énoncés pourvus de différent poids informatif et de signe contraire. Les valeurs numériques des termes de la somme étant déséquilibrées, le résultat est toujours autre que zéro. La relation exceptive, au contraire, qui n’implique pas le rapport « tout » - « partie », n’est pas capable de résoudre le conflit entre deux sous énoncés pourvus du même poids informatif et en même temps de signe contraire : les valeurs numériques des termes de la somme étant symétriques et égales, le résultat sera toujours équivalent à zéro.

23. SFP-P133 – Néphrologie – Histoire d’un Nail Patella syndrome

Abstract

T. est né à terme normotrophe avec des anomalies articulaires évoquant une arthrogripose. Le diagnostic de Nail Patella Syndrome a été suspecté par le généticien dans les premiers mois de vie en raison d’une hypoplasie des ongles associée à des troubles rhumatologiques : limitation des amplitudes articulaires sur dysplasie des coudes et genoux. Ce diagnostic a été secondairement confirmé par mutation génique positive : mutation faux sens dans l’homéodomaine W243 C. Le développement psycho-moteur est émaillé d’importants troubles du comportement. Lors d’un dépistage systématique dans le cadre du syndrome, on découvre une protéinurie à l’âge de 3 ans qui devient rapidement néphrotique. De ce fait, on réalise en mars 2003 une ponction biopsie rénale retrouvant un aspect de sclérose mésangiale diffuse caractéristique d’un Nail Patella Syndrome. L’évolution se fait ensuite rapidement vers un syndrome néphrotique mal toléré avec décompensation œdémato-ascitique, anorexie et péritonites en rapport avec des surinfections d’ascite. Après discussion avec les parents, on décide de réaliser une néphrectomie chimique par indométacine et IEC pour limiter la diurèse et la protéinurie. Devant l’échec de cette technique, on réalise une néphrectomie chirurgicale bilatérale en janvier 2006 avec mise en dialyse péritonéale (DP). En février 2006, la DP est remplacée par une hémodialyse sur cathéter central après diminution des risques thrombotiques. La dialyse est alors quasi quotidienne permettant à T. de bénéficier d’une prise en charge multi-disciplinaire de ces troubles du comportement (pédo-psychiatre, institutrice, psychomotricienne) et d’une bonne reprise staturo-pondérale. Ceci a pu amener T. à l’inscription sur liste d’attente de greffe rénale en décembre 2006. Une transplantation de rein de donneur décédé est effectuée le 19/07/07. Les suites opératoires sont simples et le traitement immunosuppresseur comprend tacrolimus et azathioprine. Les corticoïdes ont pu être interrompus au bout de 8 semaines. T. a actuellement 7 ans 2 mois et sa taille est à – 1.7 DS pour une taille-cible à la moyenne. Il va bien et a une fonction rénale normale sans protéinurie. [Copyright &y& Elsevier]

Abstract

Background: PENS is a rare neuro-cutaneous syndrome that has been recently described. It involves one or more congenital epidermal hamartomas of the papular epidermal nevus with "skyline" basal cell layer type (PENS) as well as non-specific neurological anomalies. Herein, we describe an original case in which the epidermal hamartomas are associated with autism spectrum disorder (ASD)., Patients and Methods: A 6-year-old boy with a previous history of severe ASD was referred to us for asymptomatic pigmented congenital plaques on the forehead and occipital region. Clinical examination revealed a light brown verrucous mediofrontal plaque in the form of an inverted comma with a flat striated surface comprising coalescent polygonal papules, and a clinically similar round occipital plaque. Repeated biopsies revealed the presence of acanthotic epidermis covered with orthokeratotic hyperkeratosis with occasionally broadened epidermal crests and basal hyperpigmentation, pointing towards an anatomoclinical diagnosis of PENS., Discussion: A diagnosis of PENS hamartoma was made on the basis of the clinical characteristics and histopathological analysis of the skin lesions. This condition is defined clinically as coalescent polygonal papules with a flat or rough surface, a round or comma-like shape and light brown coloring. Histopathological examination showed the presence of a regular palisade "skyline" arrangement of basal cell epidermal nuclei which, while apparently pathognomonic, is neither a constant feature nor essential for diagnosis. Association of a PENS hamartoma and neurological disorders allows classification of PENS as a new keratinocytic epidermal hamartoma syndrome. The early neurological signs, of varying severity, are non-specific and include psychomotor retardation, learning difficulties, dyslexia, hyperactivity, attention deficit disorder and epilepsy. There have been no reports hitherto of the presence of ASD as observed in the case we present., Conclusion: This new case report of PENS confirms the autonomous nature of this neuro-cutaneous disorder associated with keratinocytic epidermal hamartoma syndromes., (Copyright © 2014 Elsevier Masson SAS. Abstract

Background: loxosceles spiders are found throughout the world and are responsible for numerous cases of envenomation in America and Southern Europe. We describe, to our knowledge for the first time in France, two clinical cases of cutaneous loxoscelism., Case Report: two cases of skin necrosis arising after supposed spider bites were grouped together because of their similar clinical presentation: an initial painless bite and rapid development of an inflammatory and painful cutaneous lesion with a central hemorrhagic bulla surrounded by a perimeter of blanched skin (the "red, white, and blue" sign). The outcome in both cases was deep skin necrosis and chronic ulceration requiring surgical treatment., Discussion: loxoscelism can result in dermonecrosis. Although our cases were not documented by capture of the spider, the diagnosis of cutaneous loxoscelism was supported by the characteristic appearance of the lesion, a typical clinical course, elimination of differential diagnoses, and the confirmed presence of Loxosceles rufescens in the region., Conclusion: loxoscelism can occur in the south of France and although rare, must be considered in this region as a possible cause of skin necrosis., (2010 Elsevier Masson SAS.
