La douleur capture l’attention et interfère avec des tâches demandant un effort cognitif. Les courtes interventions de pleine conscience produisent des effets bénéfiques sur l’attention et la sensibilité à la douleur et pourraient réduire l’interférence causée par la douleur sur les processus cognitifs. Cependant, la pleine conscience demeure un concept flou étant donné qu’il n’y a pas de consensus sur sa définition dans la littérature scientifique. Ce mémoire explore d’abord différentes conceptualisations de la pleine conscience, puis définit ce construit comme une capacité psychologique universelle. Ensuite, une revue de littérature systématique sur les courtes interventions est présentée. Finalement, une étude expérimentale a été menée pour évaluer l’effet d’un entraînement de cinq jours de méditation pleine conscience (20 minutes par jour) sur l’interférence d’une douleur thermique sur la performance de la mémoire de travail dans une tâche de 2-back. Trois groupes de participants en bonne santé ont été aléatoirement répartis dans un groupe de méditation pleine conscience (n=15), un groupe d’éducation conceptuelle à la pleine conscience sans pratique de méditation (n=15) ou dans un groupe ne recevant aucune intervention (n=15, contrôle). Des sessions d’évaluations ont été menées avant et après l’évaluation, dans le but de mesurer l’effet des interventions. À chaque séance, les stimuli douloureux et la tâche de 2-back étaient calibrés individuellement avant la tâche d’interférence douloureuse, pour ainsi contrôler de potentiels changements du niveau de base de sensibilité à la douleur ou de fonctionnement cognitif. Des mesures secondaires de sensibilité à la douleur produite par la chaleur, de la tolérance à la douleur produite par le froid, d’inhibition cognitive, de flexibilité cognitive et d’attention divisée ont aussi été récoltées. Comparées à celles du groupe contrôle, l’interférence douloureuse et la douleur rapportée durant le test de tolérance étaient significativement réduites après l’intervention conceptuelle, alors que l’intervention de méditation n’a produit aucun changement. Aucun autre effet n’a été trouvé sur la douleur ou les mesures cognitives. Ces résultats mettent en lumière les limitations des courtes interventions de pleine conscience et suggèrent qu’une compréhension conceptuelle de la pleine conscience pourrait contribuer à diminuer l’interférence douloureuse. Brief mindfulness interventions have been shown to improve attention and reduce pain sensitivity and could potentially reduce pain interference. However, mindfulness is a construct that has been defined in multiples ways in an abundant and heterogeneous scientific literature. This thesis first explores different conceptualisations of mindfulness and defined mindfulnes as a universal psychological function. Next, a systematic review of the literature on brief mindfulness intervention was conducted. Finally, an experimental study was conducted to assess the effect of a 5-days mindfulness intervention (20 min/day) on the interference produced by thermal pain on working memory performance, using a 2-back task. Three groups of healthy participants were randomized into mindfulness meditation practice training (n=15), mindfulness education comprising only conceptual information (n = 15), or no intervention (n = 15 control). Evaluation sessions were conducted before and after the interventions to assess the effect of pain on 2-back performance (pain interference). Importantly, both pain stimuli and the 2-back task were calibrated individually and in each session before assessing potential changes in pain interference, thereby controlling for possible changes in baseline pain sensitivity and cognitive performance. Secondary measures of heat pain sensitivity, cold pain tolerance, cognitive inhibition, cognitive flexibility, and divided attention were also collected. Compared to the control group, pain interference and pain reported during the tolerance test were significantly reduced following the conceptual intervention but not the meditation intervention. No other effects were found in pain or cognitive measures. These results highlight limitations of brief attention regulation training and suggest that conceptual understanding of mindfulness might iv contribute to reducing pain and pain interference. Furthermore, a theoritical model of pain interference reduction by mindfulness is proposed.

Chronic pain is a highly prevalent condition that is associated with distressing somatic and emotional experiences. Consequently, an individual's distress tolerance, the perceived capacity to tolerate negative psychological and physical states, may influence their pain experience. This effect could be explained in part by a reduction in the catastrophic interpretation of pain which is associated with increased pain intensity and interference in everyday activities.The first aim of this study was to explore the association between the components of the 5-factor model of distress tolerance and (1) pain intensity and (2) pain interference in everyday activities. The secondary aim was to assess the potential mediating effect of pain catastrophizing in the eventual association between components of distress tolerance and (1) pain intensity or (2) pain interference in everyday activities.This is a cross-sectional study of adult (18 years or older) university students and staff with chronic pain (3 months). They were invited to complete the online questionnaire through an email invitation. Pain intensity and interference in everyday functioning were assessed with the corresponding subscales of the Brief Pain Inventory. The following instruments were used to assess the components of the 5-factor model of distress tolerance: Ambiguity Tolerance Scale (tolerance to ambiguity), Intolerance to Uncertainty Scale (reversed score: tolerance to uncertainty), Discomfort Intolerance Scale (reversed score: discomfort tolerance), Distress Tolerance Scale (tolerance to negative emotions), Frustration Discomfort Scale (tolerance to frustration). Participants also completed the Pain Catastrophizing Scale.Eighty participants were recruited (57 % women, mean age=33.09; standard deviation=12,87). Tolerance to negative emotions was the only component of distress tolerance that was associated with pain (ß=-0.04; 95% CI): -0.07--0.01; t (78)=-3.06, p0.01) or pain interference in everyday functioning (ß=-0.07; 95% CI: -0.10--0.03; t (78)=-3.97, p0.01), independently of the others. Combined with age, these factors explained 16.2 % of the variance in pain intensity and 19.4 % of the variance in pain interference. Pain catastrophizing partially mediated the association between tolerance to negative emotions and pain interference in everyday functioning, but it was not involved in the association between tolerance to negative emotions and pain intensity.Tolerance to negative emotions appears to be the most relevant aspect of distress tolerance in the context of chronic pain and is a potential clinical target that is independent and complementary from pain catastrophizing.

La douleur chronique non cancéreuse (DCNC) est un phénomène complexe et des interventions multimodales qui abordent à la fois ses dimensions biologiques et psychosociales sont considérées comme l’approche optimale pour traiter ce type de désordre. La prescription d'opioïdes pour la DCNC a augmenté d’une façon fulgurante au cours des deux dernières décennies, mais les preuves supportant l'efficacité à long terme de ce type de médicament en termes de réduction de la sévérité de la douleur et d’amélioration de la qualité de vie des patients souffrant de DCNC sont manquantes. L'objectif de cette étude était d'investiguer dans un contexte de vraie vie l'efficacité à long terme des opioïdes pour réduire l’intensité et l’impact de la douleur et améliorer la qualité de vie reliée à la santé des patients souffrant de DCNC sur une période d’une année. Méthodes: Les participants à cette étude étaient 1490 patients (âge moyen = 52,37 (écart-type = 13,9); femmes = 60,9%) enrôlés dans le Registre Québec Douleur entre octobre 2008 et Avril 2011 et qui ont complété une série de questionnaires avant d'initier un traitement dans un centre multidisciplinaire tertiaire de gestion de la douleur ainsi qu’à 6 et 12 mois plus tard. Selon leur profil d'utilisation d'opioïdes (PUO), les patients ont été classés en 1) non-utilisateurs, 2) utilisateurs non persistants, et 3) utilisateurs persistants. Les données ont été analysées à l'aide du modèle d'équation d'estimation généralisée. Résultats: Chez les utilisateurs d’opioïdes, 52% en ont cessé la prise à un moment ou à un autre pendant la période de suivi. Après ajustement pour l'âge et le sexe, le PUO a prédit d’une manière significative l’intensité de la douleur ressentie en moyenne sur des périodes de 7 jours (p, Chronic non-cancer pain (CNCP) is a complex phenomenon and multimodal interventions that address both its biological and psychosocial dimensions are considered as the optimal approach for treating this type of disorder. Prescription of opioids for CNCP has dramatically increased over the past two decades but little is known about the long-term effectiveness of this type of medication for reducing pain severity and improving quality of life of CNCP patients. The objective of the present study was to investigate in a real life context the long-term effectiveness of opioids for reducing pain intensity and interference, and improving health-related quality of life in patients with CNCP over a one-year period. Methods: Participants were 1,490 patients (mean age = 52.37 (SD = 13.9); female = 60.9%) enrolled in the Quebec Pain Registry between October 2008 and April 2011 who completed a series of questionnaires before initiating treatment at a tertiary multidisciplinary pain management facility as well as 6 and 12 months thereafter. Based on their opioid use profile (OUP), patients were categorized into 1) non users, 2) non-persistent users, and 3) persistent users. Data were analyzed using generalized estimating equation models. Results: Among the users of opioids, 52% stopped taking them during the follow-up period. After adjusting for age and sex, patients’ OUP significantly predicted pain intensity felt during 7-day periods (p < 0.001) and physical quality of life (pQOL) over time p < 0.001). Compared to non-users, persistent users had significantly higher levels of pain intensity and poorer pQOL. A significant interaction was found between patients’ OUP and time in the prediction of worst pain intensity (p = 0.001) but the persistent users reported the highest pain scores across time. A significant interaction was also observed between patients’ OUP and type of pain in the prediction of pain interference (p = 0.048) and mental quality of life (mQOL) (p = 0.042). Irrespective of the type of pain they suffered, persistent users reported higher pain interference scores and lower mQOL compared to non users. However, all effect sizes were small (Cohen’s d < 0.5), thus questioning the power and clinical significance of these group differences. Conclusion: These results contribute to the debate by adding evidences on the effectiveness of long term opioid therapy and question the role this type of medication should play in the therapeutic arsenal for managing CNCP.

L’expérience de douleur et la réponse à son traitement sont teintées par les facteurs psychologiques, parmi lesquels figurent les attentes. Cette thèse propose de raffiner la compréhension de l’effet des attentes sur la régulation de la douleur par l’entremise de deux études quantitatives. D'abord, une étude expérimentale explore l'impact des attentes sur l'analgésie induite par contre-stimulation. Des volontaires sains ont été assignés à une condition contrôle ou à l’un des trois groupes au sein desquels les attentes étaient évaluées (attentes a priori) ou manipulées par l’entremise de suggestions (hyperalgésie ou analgésie). La douleur, l’anxiété et le réflexe nociceptif de flexion en réponse à des stimulations électriques ont été mesurés avant, pendant et après l’application d’un sac de glace. Les résultats suggèrent que les attentes a priori prédisent l’amplitude de l’analgésie, tandis que les attentes suggérées parviennent à potentialiser ou à bloquer les mécanismes endogènes d'inhibition de la douleur déclenchés par la contre-stimulation, indépendamment du niveau d’anxiété. Ensuite, une étude clinique se penche sur l'apport des attentes aux issues thérapeutiques de patients traités en centres multidisciplinaires de gestion de la douleur chronique. Préalablement au traitement, les attentes des patients quant aux résultats anticipés après six mois de traitement ont été évaluées. Les changements dans l’intensité douloureuse, les symptômes dépressifs, l’interférence de la douleur et la dramatisation face à la douleur, ainsi que l’impression de changement et la satisfaction face au traitement ont été évalués au suivi de six mois. Les résultats d'analyses de modélisation par équations structurelles appuient la relation prédictive entre les attentes et les issues thérapeutiques et soulignent l’impression de changement du patient comme variable médiatrice de cette association. En plus de se montrer supérieur à un modèle alternatif, ce modèle s'est avéré pertinent à travers différents sous-groupes. Dans l'ensemble, cette thèse contribue à démontrer le rôle déterminant des attentes dans le contexte de la douleur, tant au niveau physiologique que thérapeutique. Les conclusions qui se dégagent de cet ouvrage entraînent des retombées non négligeables et réitèrent la nécessité de considérer la perspective du patient afin d'optimiser les soins destinés au soulagement de la douleur., The experience of pain and response to pain treatments are influenced by psychological factors, such as expectations. This doctoral dissertation aims at refining the comprehension of the impact of expectations on pain regulation through two quantitative studies. First, an experimental study examined the impact of expectations on analgesia induced by heterotopic noxious counter-stimulation. Healthy volunteers were assigned to a control group or one of three experimental groups in which expectations were either assessed (a priori expectations) or manipulated using suggestions (hyperalgesia or analgesia). Acute shock-pain, shock-related anxiety and the nociceptive flexion reflex were measured in response to electrical stimulations before, during and after the application of a tonic cold pain. Results suggest that a priori expectations predict the magnitude of analgesia, while manipulated expectations either enhance or block counter-stimulation analgesia, independently of anxiety processes. Subsequently, a clinical study examined the association between expectations and clinical outcomes of chronic pain patients treated in multidisciplinary pain centers. Prior to their initial visit, patients' expected treatment outcomes were assessed. Six months after their initial visit, changes in pain intensity, depressive symptoms, pain interference and pain catastrophizing, along with satisfaction with treatment and impression of change were measured. Structural equations modeling analyses support the predictive association between expectations and all clinical outcomes as well as the mediating effect of patient’s impression of change. In addition to being superior to an alternative model, the proposed model was shown to be relevant across subgroups. Overall, this thesis contributes to the demonstration of the determinant role of expectations on physiological and therapeutic processes in the context of pain regulation. These findings reiterate the need to consider the patient's perspective and offer a valuable avenue towards the optimization of treatments intended at relieving pain.

Plusieurs études ont démontré que prés de deux tiers des patients subissant une chirurgie cardiaque souffrent de douleur d’intensité modérée à sévère. Ces niveaux de douleur semblent être partiellement expliqués par la présence de barrières individuelles face au soulagement de la douleur. Le savoir acquis sur les cognitions associées à la douleur ne semble pas avoir été traduit dans de nouvelles approches éducatives face à la douleur aiguë. Le but du projet doctoral était 1) de développer une intervention visant à faciliter l’autogestion de la douleur postopératoire après une chirurgie cardiaque et, 2) d’en évaluer l’acceptabilité et la faisabilité, ainsi que le potentiel d’efficacité sur le soulagement de la douleur de patients subissant une chirurgie cardiaque. Le développement de l’iintervention SOULAGE-TAVIE a inclus quatre étapes fondées sur les savoirs clinique, empirique et théorique et inspirées de plusieurs écrits méthodologiques. SOULAGE-TAVIE consiste en une session préopératoire éducative sur la gestion de la douleur post-chirurgie cardiaque d’approximativement 30 minutes sur un ordinateur, dispensée sur mesure en fonction d’un algorithme. Deux renforcements cliniques en personne sont aussi offerts en phase postopératoire. L’évaluation de l’intervention consistait en un essai clinique pilote à répartition aléatoire incluant 60 participants répartis entre le groupe expérimental (GE : SOULAGE-TAVIE) et le groupe contrôle (GC : soins usuels incluant un feuillet d’information). Les données étaient recueillies au moment de l’admission et dans les sept jours postopératoires. SOULAGE-TAVIE a été jugée comme acceptable et faisable. De plus, les participants du GE n’ont pas expérimenté une douleur de plus faible intensité mais ils ont rapporté significativement moins d’interférence de la douleur avec la toux et la respiration profonde, ont démontré moins de barrières face à la gestion de la douleur et ont consommé plus d’opiacés. Cette étude pilote procure des résultats prometteurs sur les bénéfices potentiels de cette nouvelle approche sur mesure utilisant les technologies de l’information et de la communication (TIC). L’autonomisation des personnes est cruciale et complémentaire pour soutenir le soulagement de la douleur dans le contexte actuel de soins., Several studies have shown that many patients undergoing a cardiac surgery suffer from moderate to severe pain. These levels of pain may be explained by individual barriers toward pain relief. Knowledge acquired on pain-related cognitions has not been translated into new approaches. The aim of the doctoral project was 1) to develop an intervention to facilitate the self-management of postoperative pain after cardiac surgery and, 2) to assess its the acceptability and feasibility and to investigate its efficacy potential on pain relief in patients undergoing cardiac surgery. The development of SOULAGE-TAVIE included four steps based on clinical, empirical and theoretical knowledge and inspired by diverse methodological considerations. SOULAGE-TAVIE consists in a 30-minute computer-tailored preoperative educational session about postoperative pain management, tailored according to an algorithm. Two short reinforcements are provided in person. The evaluation of SOULAGE-TAVIE consisted in a pilot randomized controlled trial including 60 participants assigned to the experimental group (EG: SOULAGE-TAVIE) and the control group (CG: usual care including educational pamphlet). Data were collected at the time of admission and across Day 1 to 7 after surgery. The results provide preliminary support for the acceptability and feasibility of a tailored and virtual intervention. The results revealed that patients of the EG did not experience less intense pain but they reported significantly less pain interference when breathing/coughing, exhibited fewer pain-related barriers, and consumed more opioid medication. This pilot study provides promising results to support the potential benefits of this new web-tailored approach. Patient empowerment is complementary yet crucial in the current context of care and may contribute to improve pain relief.
