Succinate dehydrogenase (SDH) is a mitochondrial enzyme that participates in both the tricarboxylic acid cycle and the electron transport chain. Mutations in genes encoding SDH are responsible for a predisposition to pheochromocytomas and paragangliomas, and more rarely, to gastrointestinal stromal tumors or renal cell carcinomas. A decrease in SDH activity, not explained by genetics, has also been observed in more common cancers. One of the consequences of the inactivation of SDH is the excessive production of its substrate, succinate, which acts as an oncometabolite by promoting a pseudohypoxic status and an extensive epigenetic rearrangement. Understanding SDH-related oncogenesis now makes it possible to develop innovative diagnostic methods and to consider targeted therapies for the management of affected patients.Rôle de la succinate déshydrogénase dans le cancer.La succinate déshydrogénase (SDH) est une enzyme mitochondriale qui participe au cycle de Krebs et à la chaîne respiratoire. Quand elles sont à l’origine de cancers, les mutations des gènes codant les différentes sous-unités de la SDH sont responsables d’une prédisposition aux phéochromocytomes et aux paragangliomes, et, plus rarement, aux tumeurs stromales gastro-intestinales ou au cancer du rein. Une diminution de l’activité de la SDH, non expliquée par la génétique, s’observe aussi dans certains cancers plus fréquents. Une des conséquences de l’inactivation de la SDH est la production excessive de son substrat, le succinate, qui joue un rôle d’oncométabolite en promouvant un statut pseudohypoxique et d’importants remaniements épigénétiques. La compréhension de l’oncogenèse liée à la succinate déshydrogénase permet aujourd’hui de développer des méthodes diagnostiques innovantes et d’envisager des thérapies ciblées pour la prise en charge des patients atteints.

Considering intracranial tumours, only few indications of protontherapy, such as chordoma, chondrosarcoma or uveal melanoma, are uniformly approved in the world. Other indications, excluding paediatric pathologies, are still debated. The aim of this article is to describe the rationale for the use of protonbeam irradiation for meningioma, pituitary adenoma, craniopharyngioma, paraganglioma, glioma, and schwannoma, and to inform the radiation oncologists if prospective studies or randomized studies are opened for inclusions. This article deals only with indications for adults.

The diagnosis of a pheochromocytoma or paraganglioma secreting during pregnancy is a rare and serious situation, involving maternal-fetal prognosis. The purpose of this case series is to discuss the management of these patients.This is a retrospective study of cases of pheochromocytoma (n=2) or paraganglioma (n=2) managed during pregnancy between 2013 and 2020 in one center (Lille, France).We report four cases of patients with a diagnosis of pheochromocytoma or paraganglioma during pregnancy, at respectively 4, 28, 31 and 34 weeks of amenorrhea (AS). Their pregnancies were affected by a sudden onset of hypertension sometimes associated with headaches, sweating, and palpitations. All patients delivered by Caesarean section after calcium channel blocker impregnation, with a good outcome. Tumor removal took place at a distance from delivery for each patient.The therapeutic strategy includes antihypertensive treatment with calcium channel blockers or alphablockers and surgical curative treatment linked to gestational age. Multidisciplinary management as well as early diagnosis can improve the maternal-fetal prognosis. The preferred way of delivery is Caesarean section, but vaginal delivery can also be considered. Removal should ideally take place at a distance from the birth. The analysis of these cases has led to the development of a protocol for monitoring and management of parturients with diagnosis of pheochromocytoma or paraganglioma during pregnancy.

Hereditary predispositions are responsible for more than 30% of or paraganglioma. Their identification is essential to optimize medical care and to offer an appropriate screening to relatives. To date, there are more than 15 known paraganglioma/pheochromocytoma predisposing genes. The most frequently involved are those encoding the succinate dehydrogenase (SDHx), accounting for half of cases and the VHL gene, causing the Von Hippel Lindau syndrome and representing approximately 20% of genetically determined cases. Patients with SDHB genes mutations have a higher risk of metastatic disease. An oncogenetic counseling is recommended to all patients developing one or several paragangliomas, isolated or associated with other tumors. Apart from the clinical presentation and in particular the syndromic forms characterized by specific tumor spectra, there is no validated morphological criterion allowing to suspect a hereditary form. On the other hand, pathologists have now access to several immunohistochemical tools allowing the identification of some hereditary forms, in particular those linked to the SDHx, VHL and FH genes. Thus, the loss of expression in immunohistochemistry of the SDHB or FH proteins orientates respectively, towards SDHx and FH genes, while the membrane expression of carbonic anhydrase IX (CA-IX) is a sensitive and specific tool pointing towards a VHL anomaly. Other immunohistochemical markers are under evaluation. A systematic SDHB immunohistochemical staining is recommended on all paragangliomas/pheochromocytomas in order to allow an early detection of the most common hereditary forms and to contribute to the interpretation of the genetic results in these patients seen in oncogenetics consultation.

A syndromic association with PAs and pheochromocytoma/paraganglioma (PPGL), called “3PAs”, was recently described, and is sometimes associated with mutations in PPGL-predisposing genes such as SDHx or MAX. In “3PAs” patients, PAs can occur before PPGL. Our objective was to determine the prevalence of SDHx/MAX mutations in patients with isolated PAs and the characteristics of such patients.Design: Genes involved in PAs (AIP/MEN1/CDKN1B) or PPGLs (SDHx/MAX) were sequenced using next-generation sequencing in patients with isolated PAs. Next, we conducted a review of the published cases of PAs in the setting of “3PAs”.Results: A total of 263 patients were recruited. Seven pathogenic or likely pathogenic variants were found in AIP, 2 in MEN1, 2 in SDHA, and 1 in SDHC. The prevalence of SDHx mutations reached 1.1% (3/263). Among the thirty patients with PAs harbouring SDHx/MAX (27 from the literature and 3 from this study), 6/30 (20%) developed PAs before PPGL, and 8/30 (26.7%) had isolated PAs. Sixteen presented a familial history of PPGL/PA. The PA was a macroprolactinoma in the majority of cases and occurred at an older age than AIP/MEN1-mutated patients.Conclusion: we found for the first time SDHx mutations in patients bearing PAs without any familial or personal PPGL history suggesting a new gateway into SDHx related diseases.Nevertheless, a genetic screening of SDHx remains not justified in case of isolated PAs, except if a familial history of PPGL is present. Further studies are justified to clarify whether SDHx could be new candidate genes, particularly in familial isolated (macro)prolactinomas.; Contexte : les adénomes hypophysaires peuvent survenir de façon sporadique ou moins fréquemment être héréditaires. Une association syndromique entre adénome hypophysaire et phéochromocytome/paragangliome (PPGL), nommée « 3PAs » a récemment été décrite et est parfois associée à des mutations des gènes de prédisposition aux PPGL tels que SDHx ou MAX. Chez les patients « 3PAs », l’adénome hypophysaire peut survenir avant le PPGL. Notre objectif est de déterminer la prévalence des mutations de SDHx/MAX chez les patients porteurs d’un PA isolé et les caractéristiques de ces patients.Objectif : nous avons séquencé par séquençage haut-débit les gènes impliqués dans les adénomes hypophysaires (AIP/MEN1/CDKN1B) ou les PPGLs (SDHx/MAX) chez des patients avec un adénome hypophysaire isolé. Par la suite, nous avons réalisé une revue des cas publiés de d’adénomes hypophysaires rentrant dans le cadre du syndrome « 3PAs ».Résultats : nous avons recruté 263 patients. Sept variants pathogéniques ou probablement pathogéniques ont été retrouvés dans AIP, 2 dans MEN1, 2 dans SDHA et 1 dans SDHC. La prévalence des mutations de SDHx atteignait 1.1% (3/263). Parmi les 30 patients avec un adénome hypophysaire portant une mutation de SDHx/MAX (27 dans la littérature et 3 dans notre étude), 6/30 (20%) ont développé leur adénome hypophysaire avant le PPGL et 8/30 (26.7%) présentaient un adénome hypophysaire isolé. Seize avaient un antécédent familial d’adénome hypophysaire ou de PPGL. L’âge de survenue de l’adénome hypophysaire était plus élevé chez ces patients que chez les patients mutés AIP/MEN1. Ils avaient plus de macroadénomes que les patients contrôlés, en particulier de macroprolactinomes.Conclusion : nous avons trouvé pour la première fois, des mutations de SDHx chez des patients présentant un adénome hypophysaire sans antécédent familial ou personnel de PPGL, suggérant une nouvelle porte d’entrée dans les pathologies liées aux gènes SDHx/MAX. Néanmoins, un dépistage génétique de SDHx reste non justifié dans le cas d’adénomes hypophysaires isolés, à l’exception de la présence d’un antécédent familial de PPGL. D’autres études sont nécessaires pour clarifier si les gènes SDHx peuvent être de nouveaux gènes candidats, en particulier dans les (macro)prolactinomes familiaux.

The paraganglioma of the cauda equina is a rare tumor, the diagnosis is morphological and the immunohistochemistry provides a definite diagnosis. The objective of our study is to specify the clinical signs, radiological and associated pathological criteria and to compare our data with those of the literature.This is a retrospective study of nine cases of paragangliomas of the cauda equina diagnosed in our department from 2003 to 2018. The median age of the patients was 50 years-old with a male predominance (sex ratio: 3,5/1). All patients had preoperative magnetic resonance imaging (MRI) and surgery to remove the tumor. The diagnosis was performed after HES (Hematoxylin Eosin Saffron) and immunohistochemical sections examination.Radiculalgia was the chief symptom of these tumors. MRI showed an oval lesion uniformly enhanced by Gadolinium in the eight patients whose records were available. Histologically, the tumors had a lobular and trabecular pattern with neuroendocrine-like cells and a rich vascularization. By immunohistochemistry, the cells expressed chromogranin, synaptophysin and CD56.Paragangliomas of the cauda equina are rare, benign tumors. Except for cases of secreting tumors, the preoperative diagnosis is difficult. MRI is useful and may reveal radiological features suggestive of these tumors. However, it is rare for the diagnosis to be made before surgery. The diagnosis is established by histological examination and immunohistochemical techniques must be used to confirm the diagnosis. The paragangliomas of the cauda equina are well encapsulated tumors whose complete excision is curative. When the excision is incomplete, treatment with radiotherapy is recommended. Long-term clinical and radiological monitoring is recommended because of the slow evolution of the tumor and the potential for recurrence.

Phaeochromocytomas and paragangliomas (PPGL) are rare neuroendocrine tumors that arise from the adrenal medulla or sympathetic and parasympathetic ganglia. These tumors produce most often catecholamines in excess, causing hypertension and sometimes severe acute cardiovascular complications. The diagnosis is based on plasma or urines metanephrines measurements and on conventional and nuclear medicine imaging. Catecholamines-producing PPGL is very unlikely if levels are normal. The diagnosis of PPGL cannot be made without visualization of a tumor. Therapeutic management consists mostly of surgical excision, after drug preparation, and should be done in referral centers. About 40% of pheochromocytomas and paragangliomas occur in the context of an autosomal inherited syndrome, making genetic testing essential. The follow-up must be prolonged because a metastatic evolution or a recurrence can be observed in about 15% of the cases.

Paragangliomas (PGL) are rare neuroendocrine tumors, genetically determined in around 40% of cases. Despite recent developments in PGL genetics, there is still about 15% of patients for whom no genetic cause can be identified, while their clinical presentation is suspicious of a genetic form. Moreover, it has never been demonstrated that the identification of a germline mutation in any of the PPGL predisposition genes has a positive impact on the patients’ management and clinical outcome. My PhD research project has been organized around two main objectives: 1) to evaluate the benefits for the patients of the identification of a germline mutation on one susceptibility gene at the time of PGL diagnosis; 2) to search for new PGL genes and / or new mechanisms of inactivation of the previously known genes, which could explain PGL suspected to be a genetic form but without detected mutation. Thanks to a retrospective multicenter study involving 221 patients with a PGL due to a mutation on the SDHB, SDHD, SDHC or VHL genes, I observed that the patients who benefited from the genetic analysis at the time of the diagnosis of PGL had a better follow-up than those who had the data of their genetic test later on after the initial PGL diagnosis. Knowledge of a positive genetic status in the PGL diagnosis period favored the detection of smaller recurrent PGLs and less extensive metastatic disease and improved the median of survival. These results validate the international recommendations of offering a PPGL genetic testing to all affected patients at the time of initial diagnosis. I identified germline mutations in a novel PGL gene, SLC25A11 with a whole-exome sequencing strategy, which encodes for the mitochondrial 2-oxoglutarate/malate carrier and showed that SLC25A11 germline mutations predispose to malignant PGL. I demonstrated that human tumors as well as the knockout of slc25a11 gene in a murine experimental model induces a pseudo-hypoxia and a global hypermethylation of the DNA, which explains the tumourigenesis secondary to the inactivation of this gene. The identification of this new PGL susceptibility gene expands the role of mitochondrial dysfunction in paraganglioma tumorigenesis and reveals a new pathway linking metabolic defects and cancer.; Les paragangliomes (PGL) sont des tumeurs neuroendocrines rares, génétiquement déterminées dans 40 % des cas. Malgré les évolutions récentes des connaissances sur la génétique des PGL, il reste toujours environ 15 % des patients pour lesquels aucune cause génétique ne peut être identifiée alors que leur présentation clinique évoque une forme génétiquement déterminée. De plus, il n’a jamais été démontré que l’identification d’une mutation constitutionnelle dans l’un de ces gènes de prédisposition avait un impact positif sur la prise en charge et l’évolution des patients. Mon travail de thèse a été organisé autour de deux grands objectifs : 1) évaluer le bénéfice pour les patients de l’identification d’une mutation constitutionnelle sur un gène de prédisposition au moment du diagnostic du PGL ; 2) mettre en évidence de nouveaux gènes de prédisposition aux PGL et/ou de nouveaux mécanismes d’inactivation des gènes de prédisposition connus pouvant expliciter les formes suspectes d’être génétiques sans mutation identifiée. Grâce à une étude rétrospective multicentrique rassemblant 221 patients ayant un PGL secondaire à une mutation sur les gènes SDHB, SDHD, SDHC ou VHL, j’ai pu observer que les patients ayant bénéficié de l’analyse génétique au moment du diagnostic de leur PGL avaient été mieux suivis que ceux ayant bénéficié du diagnostic génétique à distance de leur prise en charge initiale. La connaissance d’un statut génétique positif dans la période du diagnostic du PGL a permis de dépister des PGL plus petits en cas de récidive et une maladie métastatique moins étendue en cas de malignité et d’améliorer la survie. Ces résultats valident les recommandations internationales qui proposent un test génétique à tous patients avec PGL au moment du diagnostic initial. Sur le plan fondamental, j’ai identifié par une stratégie de séquençage d’exome des mutations dans un nouveau gène de prédisposition au PGL, SLC25A11, qui code pour le transporteur mitochondrial du 2-oxoglutarate/malate et montré que les mutations constitutionnelles de SLC25A11 prédisposent à des formes malignes de PGL. J’ai de plus démontré que les tumeurs humaines ainsi que le modèle expérimental cellulaire inactivé pour ce gène développent une pseudo-hypoxie et une hyperméthylation globale de l’ADN expliquant la tumorigenèse secondaire à l’inactivation de ce gène. L’identification de ce nouveau gène de prédisposition étend le champ des dysfonctions mitochondriales dans la tumorigenèse et dans la cancérogénèse des PGL et révèle un nouveau lien entre mitochondrie et cancer.

The management of pheochromocytoma and paraganglioma has deeply evolved over the last years due to the discovery of novel genes of susceptibility, especially SDHx, MAX and TMEM127. While the modalities of diagnosis and management of patients presenting with hereditary pheochromocytoma and paraganglioma are now well defined, screening and follow-up strategies for asymptomatic mutation carriers remain a matter of debate. This raises major questions as these asymptomatic patients will require a lifelong follow-up. The aim of this review is an attempt to give insights on the optimal screening and follow-up strategies of asymptomatic carriers of SDHx, MAX and TMEM127 mutations, with additional thoughts on the forensic and psychological aspects of the management of such patients with rare diseases.

The European Society of Endocrinology has published this year a series of guidelines for hypoparathyroidism, the management of adrenal incidentalomas as well as for the long-term follow-up of patients operated on for a phaeochromocytoma/paraganglioma (PPGL). For hypoparathyroidism, guidelines insist on screening for chronic complications and monitoring treatment with calcium and vitamin D; the use of recombinant PTH may provide new opportunities for the future. Concerning adrenal incidentalomas, the panel of the guidelines primarily recommends non contrast CT for the evaluation of the risk of malignancy. Patients operated on for a PPGL, should be offered an individualized follow-up plan based on assessment of their risk of tumor recurrence.Des nouvelles recommandations concernant l’hypoparathyroïdie, l’évaluation des incidentalomes surrénaliens ainsi que le suivi à long terme des patients opérés d’un phéochromocytome/paragangliome (PPGL), ont été publiées en 2016 par la Société européenne d’endocrinologie. Pour l’hypoparathyroïdie, l’accent est mis sur l’évaluation des complications chroniques et la titration du traitement par calcium et vitamine D; la supplémentation par PTH-recombinante (rhPTH) est un traitement prometteur. Concernant l’évaluation du risque de malignité des incidentalomes surrénaliens, les études montrent une supériorité de la densité spontanée (DS) de ces tumeurs au CT-scan non injecté, en tant que critère diagnostique. Enfin, un suivi personnalisé est indiqué pour les patients opérés d’un PPGL, après évaluation du risque de récidive à long terme.

Pheochromocytomas and paragangliomas are catecholamine-secreting tumors usually associated with arterial hypertension. They can contribute to acute cardiovascular events. Ten to 15 percent of tumors are metastatic. Autosomal dominant gene alterations are present in more than a third of cases. The secretory phenotype and the risk of malignancy are driven by the presence of gene mutations, specifically in the subunits of succinate dehydrogenase. Recent advances in genomics have clinical implications for family screening, biological follow-up, prediction of the risk of recurrence, and therapeutic options in cases with malignant recurrence.

Les paragangliomes malins présentent un réel challenge pour les praticiens. Il s'agit de tumeurs rares, de caractérisation complexe, et très hétérogènes dans leur évolution et pronostic. Devant la rareté de ce groupe tumoral, il n'existe pas de consensus sur la prise en charge thérapeutique. A travers ce cas illustratif, nous rapportons l'observation d'une patiente de 29 ans suivie pour un paragangliome malin traitée initialement par chirurgie. Après six mois d'intervalle, récidive multifocale fixant à l'Octreo PET-CT (Positron Emission Tomography- Computer Tomography), la patiente a été mise sous injection mensuelle de Somatuline pendant un an puis arrêté suite à une progression. Une 2ème chirurgie de cytoréduction a été réalisée suivie d'une radiothérapie. Après un an, la patiente a présenté une progression massive. Une chimiothérapie à base de Dacarbazine a été initiée et dont l'évaluation a montré une réponse métabolique quasi complète après huit cures. Le but de ce travail est d'exposer les différentes options thérapeutiques possibles dans la prise en charge des paragangliomes malins.

Pheochromocytomas and paragangliomas (PCC/PGL) are rare neuroendocrine tumours that arise from chromaffin cells of the adrenal medulla, sympathetic and parasympathetic paraganglia respectively. Around 15% of PCC are malignant. SDHB mutations are associated with malignancy and poor prognosis. SDH deficiency leads to succinate accumulation that induces a cellular pseudohypoxic phenotype, promoting in particular VEGF and GLUT-1 expression and increasing angiogenesis and glucose metabolism. The high malignancy hazard associated with SDHB and the absence of curative treatment of metastatic forms of the disease make it essential to develop a mouse model for preclinical trials launching. The quest for a predisposed mouse model of Sdhb-deficient tumors being unsuccessful, Sdhb-/- and wild-type (WT) immortalized mouse chromaffin cells previously generated in the laboratory were propagated in the fat pad of NMRI nude mice, thereby providing the first pattern of Sdhb- deficient tumors. These mice were compared to a control group receiving non-mutated imCC (WT) and characterization was performed in vivo by multimodality imaging. Optical imaging assessing the tumor angiogenesis with Angiostamp®, an RGD fluorescent peptide, found an increased expression of integrins αvβ3 in the Sdhb-/- group 12 h after tracer injection. Dynamic contrast enhanced MRI (DCE-MRI) showed an overall tumor enhancement significantly higher in the Sdhb-/- model secondary to an increase of the tumor blood flow (F) and of the intratumoral capillary volume fraction (Vb) (compartmental analysis using PhysioD3D software). Metabolic imaging assessed by 18FDG-PET confirmed the expected high glucose consumption by Sdhb-/- tumors. Finally, magnetic resonance spectroscopy (1H-MRS) detected succinate accumulation in Sdhb-/- tumors and not in WT tumors. This result was confirmed by mass spectrometry and this innovative procedure for in vivo detection of succinate was translated into patients suffering from PCC/PGL. A succinate peak was specifically observed in SDHx-related PCC/PGL patients. In conclusion, these results show strong differences between Sdhb-/- and WT allografts and suggest that preclinical therapeutic studies could be implemented in this unique model of Sdhb-deficient tumour. Our noninvasive, highly sensitive and specific method allowing in vivo detection of succinate, the major biomarker of SDHx-mutated tumors was translated into clinical imaging.; Les phéochromocytomes et les paragangliomes (PCC/PGL) sont des tumeurs neuroendocrines rares qui se développent aux dépens de la médullosurrénale et des paraganglions sympathiques et parasympathiques. Près de 40% des patients ont une prédisposition génétique, et, à ce jour 13 gènes de prédisposition ont été identifiés. Les mutations du gène SDHB sont un facteur de risque de malignité et de mauvais pronostic qui conduisent à une perte de fonction de la succinate déshydrogénase (SDH), enzyme du complexe mitochondrial II. Il en résulte une accumulation intracellulaire de succinate responsable du phénomène dit de « pseudohypoxie » qui stimule la production de VEGF et donc l’angiogenèse, ainsi que l’expression du transporteur membranaire du glucose GLUT-1. Le surrisque de malignité liée à SDHB et l’absence de traitement curatif des formes métastatiques de PCC/PGL justifient l’élaboration d’un modèle murin permettant de mettre en place des essais précliniques. L’obtention d’un modèle murin prédisposé au PCC/PGL par mutation du gène Sdhb s’étant révélée infructueuse, notre équipe a finalement généré des cellules chromaffines Sdhb-/- immortalisées (imCC Sdhb-/-) dont l’implantation dans le coussinet adipeux sous-cutané (Fat-pad) de souris nudes NMRI a permis l’établissement du premier modèle de tumeurs porteuses d’une inactivation complète du gène Sdhb. La caractérisation phénotypique du modèle a été réalisée par imagerie multimodalité in vivo, comparativement à un groupe contrôle de souris ayant reçu des imCC non mutées (WT). L’angiogenèse tumorale évaluée par imagerie optique retrouve une expression des intégrines αvβ3 plus marquée dans le groupe Sdhb-/- avec une rétention du traceur prolongée 12 h après injection. L’IRM dynamique de contraste (IRM-DCE), montre un rehaussement tumoral global nettement plus important dans le modèle Sdhb-/-, que le post-traitement par le logiciel PhysioD3D permet d’attribuer à une augmentation du volume capillaire intratumoral. Sur le plan métabolique, la consommation globale de glucose mesurée par TEP au 18FDG est également plus marquée dans les tumeurs Sdhb-/-. Enfin, la spectroscopie par résonance magnétique (1H-MRS) a mis en évidence une accumulation de succinate dans les tumeurs Sdhb-/-, non retrouvée dans les tumeurs WT. Ce résultat a été confirmé par spectrométrie de masse et cette technique innovante de détection du succinate in vivo a été transférée avec succès en clinique pour l’exploration des patients porteurs de PCC/PGL. En conclusion, l’imagerie in vivo a permis de distinguer le phénotype des tumeurs Sdhb-/- de celui des WT, avec une néoangiogenèse, une microcirculation et un métabolisme glucidique augmentés. Ces résultats permettent de proposer des études précliniques de réponse précoce aux traitements. La détection de l’accumulation de succinate par 1H-MRS ouvre la possibilité d’un diagnostic « métabolique » préopératoire pour détecter les patients de mauvais pronostic.

Pheochromocytoma and functional paraganglioma (PH/PGL) are classical causes of secondary hypertension. The clinical practice guidelines for PH/PGL have been changed by the recent identification of a dozen of susceptibility genes. PH/PGL is the neuroendocrine tumor most affected by genetics. Total metanephrin should be measured in every young patient suffering of a resistant hypertension. The diagnosis is based on conventional imaging assciated with nuclear imaging. Genetic testing should be offered to every patient diagnosed for PH/PGL because the identification of a germline mutation, which is found in over 30% of the cases, will change his work-up and follow-up as well as offer the opportunity of a familial genetic testing in relatives. A specialized management is indicated, especially for patients with hereditary or metastatic PH/PGL, and should be performed in an expert center with a multidisciplinary team.

Nous rapportons l'observation d'un patient âgé de 62 ans qui présente des douleurs abdominales avec une masse du flanc droit. Les explorations morphologiques ainsi que les données biologiques ont permis de faire le diagnostic d'un paragangliome secrétant. Les particularités diagnostiques ainsi que la gestion périopératoire sont envisagées.

Le Phéochromocytome extra-surrénalien ou paragangliome est une tumeur rare. Il dérive de la crête neurale du système neuroendocrinien et il est souvent sécrétant. La plupart de ces tumeurs sont situées dans les ganglions sympathiques abdominaux, y compris l'organe de Zuckerkandl adjacent à la bifurcation de l'aorte abdominale. Les Formes non fonctionnelles sont rares: moins de 50 cas sont rapportés dans la littérature. Nous rapportons le cas d'un paragangliome non sécrétant de l'organe de Zuckerkandl opéré au sein de notre service d'urologie ainsi qu'une revue de la littérature afin de discuter les différents aspects diagnostiques et thérapeutiques de cette entité tumorale rare.

Pheochromocytomas and paragangliomas (PHEO/PGL) are neuroendocrine tumors that arise from sympathetic and parasympathetic paraganglia. Although well described in the adult population, diagnosis and treatment of these exceptionally rare neoplasms remains poorly characterized in children. This article reviews recent advances in clinical presentation, genetics, biochemistry, imaging and treatment of children with benign or malignant PHEO/PGL. Compared to adults, pediatric PHEO/PGL are more frequently familial, bilateral, multifocal and malignant. Approximately 50% of pediatric PHEO/PGL are associated with a mutation of one of the 12 known susceptibility genes. Von Hippel-Lindau disease, type 1 neurofibromatosis, type 2 multiple endocrine neoplasia and familial PGL syndrome are hereditary tumor syndromes associated with an increased risk of developing such diseases. Clinical presentation includes symptoms related to catecholamine hypersecretion and/or tumor mass effect. Plasma and/or urine metanephrine dosages are recommended as first-line diagnostic biochemical tests. Magnetic resonance imaging is useful as initial radiological approach. Most pediatric PHEO/PGLs are benign. Surgical resection, with appropriate perioperative management of catecholamine-related symptoms, remains the treatment of choice. In case of metastatic disease, surgical removal of metastases when possible and I-131-MIBG radiotherapy provide limited results whereas chemotherapy is reserved for more advanced stages.

Pheochromocytomas (PHEO) and paragangliomas (PGL) are rare neuroendocrine tumors secreting catecholamines in most cases. The clinic can be very variable. Morbidity and mortality PHEO and PGL are primarily cardiovascular and haves catecholamine-mediated origin. The PHEO and PGL can sign in with a syndromic association with multiple tumors and genetic counseling is necessary in search of a germline mutation. The first step includes a diagnostic assay of metanephrine and normetanephrines. In case of positive biology a CT or MRI imaging will be needed to locate the tumor. Treatment with alpha-blocker before surgery reduces the perioperative risk. A long term follow up is recommended to detect recurrence.

Up to 30% of phaeochromocytomas and paragangliomas occur in the context of inherited tumor syndromes. Familial history and clinical presentation have to be strongly detailed to guide genetic testing. The identification of a genetic predisposition in a patient with phaeochromocytoma or paraganglioma has a positive impact in terms of medical care and follow-up for the proband and allows genetic testing in apparently asymptomatic family members. Two clusters of genes are described depending on their implication in the pathogenesis of inherited tumors. An algorithm for the genetic diagnosis of phaeochromocytomas and paragangliomas is proposed by The French network of oncogenetic laboratories. These recommendations will probably change with the identification of new predisposition genes and the development of new sequencing technologies.Genetic testing is prescribed by a specialist, as part of a cancer genetics specialist consultation in endocrine tumors. The psychological support is essential throughout the family survey.

Paragangliomas can exist anywhere in the distribution of neural crest derived cells. In the spermatic cord, they are exceedingly rare. We report a case of histologically discovery, occurring in a man of 50 years who had a para-testicular mass, painless, firm without other signs associated. In view of this observation and a review of the literature, we propose to establish the epidemiological profile and study the prognostic and the clinicopathologic features of this tumor.

The frequency of the diagnosed causes of secondary hypertension is only known from hospital-based records, which probably overestimate the true prevalence. Excluding oral contraceptive users and cases with renal failure, their overall frequency was estimated at 1 percent in the eighties, 5 percent in the nineties, and 9 percent in recent years. This increase in frequency was mostly due to an increased number of diagnosed cases of endocrine hypertension. The diagnosis of endocrine hypertension is not synonymous with the diagnosis of a surgically correctable form of hypertension. Indeed, hypertension is surgically curable in a minority of patients, mostly in patients with aldosterone-secreting adenomas or with pheochromocytomas or functional paragangliomas. The presentation, screening, diagnosis and therapeutic management of endocrine hypertension are discussed in the present issue.
