Abstract

to summarize the knowledge regarding the maladaptive beliefs of patients with non-specific low back pain.a narrative literature review on these beliefs was conducted by an international and multidisciplinary team of experts in the field.these beliefs, which can result in negative consequences on functioning and on patient prognosis, have various origins: family and friends, media, previous experience and/or health care professionals' messages. The latter, who have a considerable and enduring influence, have the potential to change and correct the patients' misbeliefs; however, they can also reinforce them in case of inappropriate messages and attitudes. Informing and educating the patient (by means of reassurance, explanations of the non-systematic association pain-injury, encouragement to get and stay physically active) are the basis of treatment. Taking into account the consequences of some words which may be misinterpreted, the results of imaging should be wisely discussed with the patient. Pain neurophysiology education and cognitive behavioral therapy (i.a., in vivo graded exposure techniques) are effective additional treatments.Misbeliefs are frequent in patient with low back pain. They do need to be looked for and corrected.

15. Les croyances délétères des patients lombalgiques : revue narrative de la littérature

Abstract

Objectif : synthétiser les connaissances relatives aux croyances délétères des patients souffrant de lombalgie commune. Matériel et méthode : une revue narrative de la littérature relative à ces croyances a été réalisée par un groupe international multidisciplinaire de 8 experts dans le domaine. Résultats : les croyances délétères, qui peuvent avoir de nombreuses conséquences sur le fonctionnement et le pronostic du patient, ont diverses origines : entourage, médias, expériences préalables, attitude et/ou discours du thérapeute. Ces derniers, dont linfluence est très forte et très durable, ont la possibilité de les modifier et de les corriger, mais ils peuvent également les renforcer en cas de discours et/ou de comportements inadéquats. Linformation et léducation du patient constituent les bases de la prise en charge. Compte tenu des répercussions que peuvent avoir la mauvaise interprétation de certains mots, les résultats de limagerie médicale devront être commentés judicieusement. Léducation à la neurophysiologie de la douleur et les thérapies cognitivo-comportementales (comportant notamment les techniques d«exposition graduelle in vivo») constituent des prises en charge complémentaires efficaces. Conclusion : les croyances délétères sont fréquentes chez les patients lombalgiques. Elles doivent impérativement être recherchées et corrigées.

Abstract

Abstract: Advances in bioengineering have lead to the possibility to conduct large scale production of monoclonal antibodies (MoAB) and to reduce progressively the murine component from 30% (chimeric MoAB) to 5% (humanized MoAB) to 0% (human MoAB). Three types of extracellular components are targeted in solid tumours : (1) Growth factors with transmembrane tyrosine kinase receptors either of tumour cells (IGF1) or endothelial cells (Bevacizumab). Bevacizumab has activity additive to that of chemotherapy in advanced colorectal, non squamous lung, ovarian, metastatic breast cancers and glioblastomas; (2) Extracellular domain of those transmembrane receptors : EGFR in colorectal cancer if no activating of KRAS with cetuximab and panitumumab, head and neck carcinomas with radiotherapy, and probably squamous lung cancers. Anti-ERBB2 MoAb are now a constitutive part of therapy of ERBB2 positive breast cancers at any stage; (3) Differenciation cluster regulating relationship ot tumour and stromal cells and in particular immunologic effectors. This is the case of anti-CTLA4 MoAB ipilimumab which activates and amplifies immunological cytotoxic response against melanoma with improved survival. These activities are achieved to the expense of class, target related toxicity conditioned by expression of the target on normal cells and or mechanism of action (immunological toxicity with ipilimumab). Of note a synergistic or additive activity with valid treatment regimens of targeted cancers and now targeted small molecules. [Copyright &y& Elsevier]

49. Hormone replacement therapy in breast cancer patients: a study of 230 patients, with a case-control study

Abstract

Objectives. – After recalling the classical contra-indication of hormone replacement therapy (HRT) concerning patients with a personal history of breast cancer (BC), and arguments that may be opposed, the authors report the present results of a prospective study undertaken in the Center of Breast Diseases in Saint-Louis hospital in Paris since February 1992.Patients and methods. – By April 2001, 230 patients had been included. A free interval of 2 years at least since the treatment of the primary BC has been observed. The reasons for prescribing HRT were vasomotor troubles (flushes, nightly sweats) or a dyspareunia, which were severe and not controlled by non-hormonal treatments. There was also an indication of a major osteoporotic or cardiovascular danger. In fact, many of these patients had a premature, artificial, chemo-induced menopause. The HRT most often used was an estro-progestin association (estradiol + a progestin compound) given either continuously or with a 5-d interruption each month. The mean duration of treatment was 2.5 years.Results. – Results, concerning the improvement of menopause troubles, were remarkable in the great majority of troubles. HRT had to be stopped in 39 cases, reading as follows: 17 cases for relapses (seven local, six in the contro-lateral breast and four metastases (7%)). Also, 22 patients (9%) interrupted their HRT for serious side-effects. A case-control study did not show any significant difference between with and without HRT patients concerning the overall survival without relapse.Discussion and conclusions. – Quality of life of patients was often substantially improved, and a deleterious effect on the cancer disease was not found. Our results are in agreement with the literature from other countries. However, one must be cautious. In such circumstances, HRT must be prescribed with the informed consent of the patients and delivered in appropriate hospital and university centers. It is wished that large randomised prospective studies may be undertaken. [Copyright &y& Elsevier]
