Pre-extensively drug resistant tuberculosis (pre-XDR-TB) has been an area of growing concern, and posing a threat to global efforts of TB control. We report a case of PreXDR-TB spondylodiscitis with resistance to a Fluoroquinolone, in an immunocompetent patient under antibacillary treatment for pleural tuberculosis, managed with drug sensitivity-based second-line antituberculous drug regimen. Our case shows the challenges of the diagnostic and management of Drug-resistant TB spondylodiscitis.

INTRODUCTION: We aimed to analyze the prevalence of fall, fear of falling and risk of falls in the patients with rheumatoid arthritis and the associated factors. METHODS: one hundred and twenty-three participants with rheumatoid arthritis (RA) aged over 18 years were enrolled based on the 1987 ACR/EULAR classification criteria. Patients were asked to complete an interviewer-assisted questionnaire about the number of falls and fear of falling during the last 12 months. Comorbidities and complications related to the disease were also obtained. The disease activity was evaluated by the patient and evaluator global assessment (PGA, EGA), the tender and swollen joint counts (TJC28, SJC28), the disease activity Score 28 ESR (DAS28 ESR) and the clinical and simple disease activity indexes (CDAI, SDAI). The functional disability was measured by the health assessment questionnaire (HAQ). The risk of falls was estimated by five standardized performance tests: the Tinetti test (TIT), the timed get up and go test (TUG), the chair-rising test (CRT), the tandem and the tandem stand tests. The relation between disease parameters and the test of fall were analyzed by the r of Pearson, Spearman and Chi-square. The factors associated with falls and fear of falls were evaluated using multiple linear regression analysis and t independent test. RESULTS: twenty-three (18.7%) participants reported a fall and forty-five (36.6%) of them the fear of falls. Risk of falls was strongly correlated with age (CRT: r = 0.345, TIT: r = -0.314), TJC 28 (TIT: r = -0.482, TUG: r = 0.260), PGA (TIT: r = -0.481, TUG: r = 0.375) EGA (CRT: r = 0.27, TIT: r = -0.48), DAS28 ESR (CRT: r = 0.32, TUG: r= 0.25, TIT: r = -0.51), CDAI (TIT: r= -0.421, TUG: r= 0.292), HAQ (CRT: r= 0.411, TIT: r= -0.648, TUG: r= 0.537) and disease duration (CRT: r= 0.343, TIT: r= -0.400, TUG: r= 0.363). No relation was found between corticosteroid use and the risk of falls. Fear of falls was related in simple analysis to the presence of a comorbidity (p = 0.002 OR: 0.252 CI [0.104-0.611]), the walk with aid (p = 0.010 OR: 0.1225 CI[0.025-0.602]), the body mass index (BMI) (p = 0,001 OR:1.143 CI[1.064-1.228]), the disease duration (p = 0.001 OR:3.69 CI[1.97-6.92]), the VAS pain (p = 0.001 OR:1.03 CI[1.011.05]) and the HAQ (p =0.001 OR: 3.69 CI[3.69-6.92]). In multiple regression analysis, fear of falls remained related to BMI, HAQ and disease duration. CONCLUSION: the falls in RA seems to be frequent. Several factors were associated to a high risk of falls and fear of falling: disease duration, HAQ, and especially the high disease activity, which is the case of many patients in African countries. These results suggest that the physician has to be aware of the high risk of falling in RA patients and the importance of identifying them with a simple interrogation and physical examination.

La dysplasie fibreuse des os est une affection osseuse bénigne congénitale mais non héréditaire, où l'os normal est remplacé par un tissu fibreux renfermant une ostéogenèse immature. Elle est due à une mutation du gène GNAS 1sur le chromosome 20q13, une mutation activatrice de la sous-unité de la protéine G. C'est une pathologie qui est le plus souvent silencieuse, de découverte fortuite sur une radiographie standard ou révélée par une douleur osseuse ou une fracture pathologique. L'imagerie et l'histologie, quand elle est nécessaire, permettent d'établir le diagnostic. Bien qu'il ne s'agisse pas d'une tumeur, elle est souvent classée dans la catégorie des tumeurs osseuses bénignes pour des raisons de diagnostic différentiel radiographique et anatomopathologique. Elle peut être monostotique ou polyostotique. L'approche thérapeutique est essentiellement symptomatique. Quelques publications récentes ont suggéré l'intérêt majeur d'un bisphosphonate, en particulier le pamidronate, qui diminuerait les douleurs et stimulerait une reminéralisation progressive des zones ostéolytiques chez les patients traités. D'autres traitements tels que la thérapie ciblée sont en cours d'évaluation.

Celiac disease (CD), a malabsorption syndrome caused by hypersensitivity to gliadin fraction of gluten. CD can manifest with classic symptoms; however, significant myopathy and multiple fractures are rarely the predominant presentation of untreated celiac disease. Osteomalacia complicating celiac disease had become more and more rare. We describe here a case of osteomalacia secondary to a longstanding untreated celiac disease. This patient complained about progressive bone and muscular pain, weakness, fractures and skeletal deformities. Radiological and laboratory findings were all in favor of severe osteomalacia. Improvement of patient's weakness and laboratory abnormalities was obvious after treatment with gluten free diet, vitamin D, calcium and iron. This case affirms that chronic untreated celiac disease, can lead to an important bone loss and irreversible complications like skeletal deformities.

Pregnancy complicated by Non Hodgkin lymphoma (NHL) is rare, about 100 cases have been reported. We will describe the case of a multifocal bone lymphoma revealed by a left hyperalgetic and deficient cruralgia in a female in the second trimester of pregnancy.

Pregnancy is one of conditions that increase the risk of gonococcal arthritis which result from blood dissemination of neisseria gonorrhoeae. We describe a case presentation. A 20-year-old africain female patient (in the second trimester of pregnancy), was admitted to hospital because of fever, asymmetric joint swollen affecting the hands, wrists, left ankle, and right sternoclavicular joint. Laboratory findings (erythrocyte sedimentation rate was 117 mm in first hour, The serum C-reactive protein level was 152 mg/L) the gram stain of genital sample was positif of neisseria gonorrhoeae and trichomonas vaginalis. With antibiotics, outcome of pregnancy was timely and uneventful. Conclusion: Patients should be educated about the mode of transmission of gonorrhea. Sexual partners should also be treated to prevent dissemination and gonococcal re- infection.

Les complications infectieuses sont redoutables au cours de la polyarthrite rhumatoïde (PR). Le but de notre étude est d'estimer leur fréquence et de déterminer les facteurs associés à l'infection chez ces patients. Il s'agit d'une étude rétrospective incluant les cas de PR établis recensés entre 2007 et 2011 au service de rhumatologie au CHU Hassan II de Fès au Maroc. Nous avons inclu 164 patients atteint de PR, l'âge moyen des patients était de 47,9 ans, avec une prédominance féminine (137 F/27H). La fréquence des infections dans notre série était de 26,2 %, dominées par les infections urogénitales (22 cas), pleuro pulmonaires (11 cas) dont 2 cas de tuberculose pulmonaire et un cas d'infection H1N1, 3 cas d'infections cutanées et 4 cas d'arthrite septiques. Dans notre série 127 patients étaient sous corticothérapie orale, 147 patients étaient sous méthotrexate, 25 patients étaient sous rituximab et 8 patients étaient sous tocilizumab. Dans notre étude, les facteurs associés à la survenue d'infection étaient l'âge avancé (p= 0,02), une CRP élevée (p= 0,04) et une dose de corticothérapie - 7.5 mg/j (p= 0 ,03). Notre étude a mis en évidence certains facteurs associés à la survenue d'une infection au cours de la PR. En connaissant ces facteurs, il faut instaurer une surveillance particulière pour améliorer la qualité de prise en charge.

Calcification of the alar ligament is rare. It usually develops as a result of traumatic injury and is especially prominent in the elderly. CT scanning is the gold standard of the diagnosis. We report a case of a calcification of the transverse and alar ligament in a patient with rheumatoid arthritis.

La tuberculose ostéoarticulaire représente 2 à 5% de l'ensemble des tuberculoses, la localisation au médio-pieds est rare. Nous en rapportons le cas d'une patiente âgée de 18 ans, qui présente depuis 2 ans des douleurs du médio-pied droit inflammatoires avec tuméfactio, La radiographie a montré des géodes et des érosions des os naviculaire et cunéiformes. L'IRM a orienté vers le dignostic de tuberculose confirm par biospie et étude anathomopathologique. C'est pourquoi devant tout tableau clinique traînant ou devant toute lésion osseuse suspecte ou de présentation atypique le diagnostic de tuberculose doit être évoqué afin d'éviter un retard diagnostique.

L�acromegalie est une maladie endocrinienne rare, en rapport avec une hypersecretion d�hormone de croissance. Elle a des consequences rhumatologiques: l�arthropathie peripherique, l�atteinte rachidienne et les syndromes canalaires. L�atteinte articulaire accompagne une acromegalie active, sa survenue apres un traitement radical et une remission complete est rare. Nous presentons le cas d�une patiente de 70 ans ayant un antecedent d�acromegalie sur adenome hypophysaire il y a 25 ans, traitee chirurgicalement et declaree en remission complete, a developpe une arthropathie destructrice des deux epaules. Le but de notre observation est de mettre le point sur la possibilite d�une atteinte articulaire au cours de l�acromegalie et de son retentissement fonctionnelle.

L’association spondylarthrite ankylosante et sarcoïdose est rare, seize cas ont été publiés dans la littérature, nous en rapportons un nouveau cas. Cette association qui pose des problèmes diagnostiques est probablement fortuite du fait de l’absence de facteurs génétiques prédisposant partagés et du petit nombre des cas publiés, mais elle suggère l’hypothèse d’une liaison physiopathologique entre eux.
