LONGITUDINAL method , *EDUCATORS - Abstract
in the following article, starting from a progressive understanding of his own subjectivity, a determining factor in developing his capacity to listen to adolescents, the author puts forward the hypothesis that a professional position he describes as "in-between" requires rigor and flexibility, enabling professionals to remain permeable to the unconscious mobilizations of the adolescent-subjects as well as to his own. The author will draw on the results of a longitudinal, partnership-based study that he conducted with ten adolescents, from 2015 to 2020, involving a team of specialized prevention educators. More specifically, the author will retrospectively analyze some excerpts from his encounter with one of the teenagers to reveal possible ways in which a "professional of the social bond" might adopt a rigorous in-between position. DRUG storage , *RAILROAD trains , *HEALTH products , *PHARMACY technicians , *LONGITUDINAL method - Abstract
The restocking of health products is carried out by supplementing the endowment according to a pre-defined schedule. Apart from these orders, the services make urgent requests, the number of which continues to grow. Are these urgent requests always justified ? A non-interventional monocentric prospective study was carried out in five departments over two periods of 21 days, between which corrective actions were implemented. The number of urgent request lines went from 462 to 300 and their justification rate from 37% to 60%. This is linked to the action plan put in place, articulated around 5 axes: staffing review, harmonized storage of drugs, training of paramedical staff in the proper use of the software, personalized display in each treatment room on the ordering methods and institutional training on good practices in the drug circuit. These measures have had a positive quantitative and qualitative impact on urgent requests. [ABSTRACT FROM AUTHOR]

BCG immunotherapy , *CHI-squared test , *THERAPEUTIC complications , *CATHETERIZATION , *LONGITUDINAL method - Abstract
Introduction. Evaluate the tolerance of patients treated with endovesical BCG immunotherapy and identify risk factors of therapy related complications. Patients and methods. This is a transversal, monocentric and prospective study performed in the urology department of Charles Nicolle Hospital over a period of ten months. We selected patients with at least one adverse event manifestation. We studied factors correlated with the occurrence of 7adverse events. The statistical test used was the Chi-square test. Results. 35 patients were selected. We counted 106 adverse events divided into: class I (45.2%), class II (42.5%), class III (8.5%) and class IV (3.8%). Traumatic catheterization was significant for grade III complications and compliance problems were significant for Grade II complications. Conclusion. The most encountered complications were minor and often self-resolving. Major and severe complications were observed such as disseminated BCGitis. Compliance issues and traumatic catheterization were significant for the occurrence of such complications. [ABSTRACT FROM AUTHOR]

Lateral internal sphincterotomy is increasingly used to treat chronic anal fissure. Since continence disorders are linked to excessive sectionning of the anal sphincter, it is now accepted that better control of the level of sectioning would reduce the occurrence of postoperative incontinence. The aim of this study was to evaluate the midterm results of the lateral internal sphincterotomy at Hopital de la Renaissance of Ndjamena. It was a prospective study of 121 cases of lateral internal sphincterotomy followed up at Hopital de la Renaissance of Ndjamena, during the period from december, 2013 to january, 2019. Lateral internal sphincterotomy was performed in all cases, 79 patients were males (65.3%) and 42 females (44.7%). The average age of our patients was 32 years. Two patients were operated under local anesthesia (1.65%). 113 patients had (93,4%) and 8 (6,6%) anterior anal fissure. 117 patients achieved complete healing at 3 weeks (96.7 %). Anal incontinence was not reported. After a follow-up of 6 months, no recurrence was noticed. Lateral internal sphincterotomy is an effective technique for the treatment of chronic anal fissure. A larger number of cases with a longer follow up could confirm these results. [ABSTRACT FROM AUTHOR]

Background: Therapeutic alliance represents a rarely studied object when it relates to nurses and care provided by a nursing team in acute care hospitalization. Objective: The objective was to study how factors might influence the therapeutic alliance built between nurses and aides and adult inpatients in an acute care unit of sectorial general psychiatry.Method: This is a prospective, observational and cross-sectional study using a therapeutic alliance measurement scale. Therapeutic alliance (TA) score was measured with a STAR-P scale in a sample of 240 patients. Results: The median score found is 33.4 (±7.8) out of a maximum theoretical score of 48. The global score of TA in patients aged 60 years old or more is significantly higher than the score of patients between 18 and 29 years old (p=0.021). The lack of external follow-up in the three months after hospital release is not associated with TA global score (p=0.73). If inpatients, no matter what their diseases or types of care are, under legal obligation or not, consider their TA is rather good after their hospital stay. Only sociodemographic factors like age, housing conditions (insecure or sustainable), having a job or not, living alone or with a partner affect TA and follow-up.Conclusion: Results evoke concepts of anomie and attachment, that seem to play an important role in the lack of follow-up after hospital stay, and indicate the mandatory global approach to care and an involvement of health professionals as well as social beings, where empathy must find its place. LINEAR accelerators , *TECHNOLOGISTS , *RADIOTHERAPY , *LONGITUDINAL method - Abstract
The main goal of our research is to better understand the appropriation of a new linear accelerator especially, a workstation dedicated to imaging by radiotherapy technologist in radiotherapy. We realized a longitudinal study, over 9 months, to understand the evolution of work before, during and after the introduction of a new linear accelerator in a radiotherapy department in Il de France. This research is based on instrumentation theory (Bationo-Tillon & Rabardel, 2015; Folcher & Rabardel, 2004; Rabardel, 1995) and the appropriation is addressed through the «instrumental genesis». The results match the beginnings of these geneses: the first uses of the workstation dedicated to imaging by radiotherapy technologist. The analysis highlights 1) that, appropriation may be made easier or more complex by the current radiotherapy technologist' instruments, 2) that, the appropriation is differentiated according to the treatment location, 3) that, the projections in the future activity are inherent in the process of appropriation. The results obtained serve to provide recommandations about technology-oriented training to promote the appropriation of a new therapy device for radiotherapy. [ABSTRACT FROM AUTHOR]

In Translation Studies, most of the empirical studies on translators in society rely on a cross-sectional methodology, which takes a snapshot of a population at one specific time. However, giving the evolving nature of translation work, translators would experience changes from time to time. Therefore, follow-up studies of translators are paramount because this approach allows Translation Studies researchers to explain the changes that translators may experience over time. This article presents findings from a longitudinal study which examines translators’ possible changes in their occupation, visibility between clients and end-users, capital received and job-related happiness over a three-year period. The analysis is based on 92 Chinese translators in China, Hong Kong and Taiwan between 2009–2010 survey data (Wave 1) and 2012–14 survey data (Wave 2). It is found that freelance translators are less happy than other current translators. Some social variables such as age, level of education, region in which thetranslator lives, the translator’s major field of study and the appearance of the translator’s name on translations are found not related to their decision to leave the field. What are found to be significantly related include sex, years of translation experience, and time spent working on translation assignments. It is also found that the turnover rate for female translators is higher than that for male translators. In addition, those who abandon the work of translation are mostly inexperienced translators.

Objective : We aim to evaluate how well do patients treated by prolonged corticosteroid therapy know their treatment, as well as how they comply with the required hygiene rules and the factors influencing their behaviour. Methodology : Observational and prospective study, carried in our hospital's department of internal medicine, over a 4m period and including patients who have been undergoing treatment for over 3m. A questionnaire has been put together for data collection. Factors for good compliance predictability have been determined through a bivariate and a multi variate analysis. Results : In total, the study sample counted 97 patients of average age 51.1y+/- 13y. Sex ratio M/F was 0.18. A good observance has been found with 45% of the sample. Good compliance predicting factors, after a multivariate analysis, were: age< 40y (OR: 0,076, IC95% [0,015-0,378], p=0,002) and the knowledge about the treatment (OR: 0,033, IC95% [0,003-0,318]: p=0,003). The knowledge about dietary rules were related to a good compliance (p=0,044). Conclusions : The low level of compliance found suggests a need for additional efforts from all the healthcare actors are worthwhile. [ABSTRACT FROM AUTHOR]

Introduction: One of the consequences of the global pandemic linked to COVID-19 was the confinement of the general population in metropolitan France and the overseas departments. This situation changed the organization of care, emphasizing the urgent respond against the epidemic. Chronically ill patients, who are more at risk of developing a serious form of virus infection, have seen their care and education disrupted. Consequently, health professionals have had to adapt their TPE practices. Objectives: The aim of this study is on the one hand, to describe the changes in TPE programs that occurred during this containment period and on the other hand to specify the extent to which TPE professionals have developed specific interventions on the prevention of COVID-19. Methods: This is a questionnaire survey, conducted during the confinement period, from March 13 to May 11, among the coordinators of authorised TPE programs in France and overseas departments, accessible on the SETE website. The questionnaire included 14 questions, closed type with choice of answers, mixed and open questions. The responses to the closed questions were processed using Excel © software, the others were subject to a content analysis, this is a mixed method integrating quantitative and qualitative. Results: 410 programs/Transversal Patient Education Units (UTEP) coordinators responded to the survey, covering a total of 714 programs (16% of the 4500 programs authorized in France). 70% were completely interrupted, 29% saw their activities continued with modifications. Specific COVID-19 prevention activities were carried out for 11% of the programs. The vast majority of collective TPE activities have been abandoned in favor of individual sessions integrated into care, carried out remotely using the telephone and various digital solutions. Discussion: TPE seems to be a low priority in healthcare structures. The customization of educational pathways and the insurance of an educational continuity appear to be a necessity to overcome the difficulties imposed by the emergency situation. Digital adaptations were often used, questioning patient's illectronism, material resources for the teams and related skills in driving digital education. Conclusion: This survey, testifies the fragility of TPE and the low level of importance given to it during the health crisis turned to conventional care. While the TPE program approach is not questioned, the possibilities of adaptation are considered by giving more place to more relevant TPE activities to ensure personalized educational follow-up. These results should be completed by surveys on the experience of patients' experiences during this crisis in terms of their education or lack of education. [ABSTRACT FROM AUTHOR]

Chronic kidney disease (CKD) is a major concern of public health. The pharmacist is known as a health practitioner involved in prevention and therapeutic education. Our study aimed at defining the impact of community pharmacists' interventions for preventing and screening CKD. In our observational prospective study of 5 months conducted in 109 community pharmacy, we included 2 groups of patients: A (therapeutic optimization): CKD patients and B (CKD screening): population at risk. In group A, we included 354 patients, mainly women (51.2%), in stage 3 of CKD, mean age 73 years old, with hypertension alone (40.6%) or associated with diabetes (44%). About 70% of the patients had a follow up by a nephrologist and 45% of them were good adherent according to the Morisky Green self-report. However, approximately 20% of patients did not have nephroprotective treatments in their regimen although they were on stage 3 or 4 CKD patients, and about half of them were not aware of medical situations at risk. Concerning group B, 532 patients were included. The pharmaceutical interventions screened 10 % of patients with a GFR < 60 mL/min/1.73m2. The community pharmacists' interventions helped to optimize the therapeutic management of CKD patients and in the early screening of patients at risk. More studies are needed to extrapolate our observations to a larger population. [ABSTRACT FROM AUTHOR]

This study focuses on 22 former students looking for a job or newly entered into the teaching profession. Based on their autobiographical writings, this longitudinal research identifies nine variables influencing their integration. Their weights differ as well as the polarity of their influences. For example, while the steps to get a stable job are often seen as an obstacle, relational skills are an asset for integration into the profession. Moreover, this research suggests, in order to understand these integration processes, that variables beyond classroom work should also be included. [ABSTRACT FROM AUTHOR]

Introduction: dermatoscopy is an innovative non-invasive technique for clinical morphology of pigmented lesions and skin tumor. The purpose of our study was to highlight the morphological and dermoscopic features of basal cell carcinoma (CBC) in our study population. Methods: we conducted a prospective unicentric study over a period of 2 years. We used dermoscopy in all patients with BCC. Statistical analysis was performed using SPSS software version 17. Results: the study included 100 patients with BCC, the average age of patients was 51.87 years, with a sex ratio F/M = 0.6. BCC most commonly occurred in the face and most patients were prototype III and IV. We established conventional and non-conventional dermatoscopic criteria. Our study showed that there is a significant relationship between prototype and the degree of BCC pigmentation. Conclusion: this study highlighted the key role of dermoscopy in detecting small-sized BCC and in facilitating the diagnosis of pigmented BCC. [ABSTRACT FROM AUTHOR]

PROFESSIONAL practice , *CAREGIVERS , *TEACHING methods , *QUALITATIVE research , *CONCEPTUAL structures , *QUESTIONNAIRES , *PATIENT education , *LONGITUDINAL method - Abstract
Introduction: If the professionals trained in therapeutic patient education (TPE) have been the subject of only few studies, a regular report suggests that this training process would lead to a real transformation of professionals, in the way they consider their daily practice. However, a blur surrounds this result. Description: Based upon a longitudinal qualitative survey of professionals enrolled in training courses, it appears that the curiosity or the attraction for TPE goes hand in hand with prior arrangements that are congruent with the ethico-practical framework transmitted during training. The appropriation of a posture explicitly oriented toward the respect and development of the patient's autonomy constitutes a logical continuity of the initial posture of professionals, less than a rupture. This is expressed by the passage from a practice lived as spontaneous, intuitive, to a practice nurtured by specific tools and advices. Conclusion: Less than a process of identity transformation, it is an identity confirmation linked to a process of reassurance of caregivers that is set up during their training in TPE. Undoubtedly answering to real needs, it is perhaps in the end the regular lack of TPE in the initial training of caregivers that is questioned here. [ABSTRACT FROM AUTHOR]

NON-nucleoside reverse transcriptase inhibitors , *NATURAL immunity , *VIRAL load , *LONGITUDINAL method ,DEVELOPING countries - Abstract
Introduction: HIV-2, endemic in West Africa, has a natural resistance to non-nucleoside reverse transcriptase inhibitors (NNRTIs) which makes it difficult to treat it in developing countries. Methods: we conducted a descriptive, longitudinal, prospective study over the period November 2005-June 2017. Virologic failure has been defined as any viral load greater than 50 copies/ml after 6 months of ARV treatment administered twice. Assays for detecting drug-resistance mutations was performed in the protease-coding region and in the reverse transcriptasecoding region. Results: data from a total of 110 patients were collected. The patients had a median age of 46 years (ranging from 18 to 67) with a sex-ratio F/M of 2.54. At inclusion, viral load could be assessed in 44% of cases with a median of 935cp/ml (ranging from 17 to 144038). Antiretroviral regimen consisted of a combination of 2 NRTIs and 1IP in 94% of cases. The median follow-up was 1200 days (ranging from 1 to 3840); 94 then 76 patients completed their 12-month and 24-month assessments respectively. At 24-month follow-up, 39 patients had virologic failure, reflecting a prevalence of 39% estimated at 33% at 12-month follow-up and at 11% at 24-month follow-up; NRTIs resistance was observed in 45% of patients, IP resistance in 41% of patients while multi-NRTIs resistance and multi-IP resistance in 30% of patients. Conclusion: currently, there is an urgent need to make available the new therapeutic classes of ARV for second line ART for patients living with HIV-2 with therapeutic failure in resourcelimited settings. ANTI-inflammatory agents , *LONGITUDINAL method , *AMITRIPTYLINE , *HIGHER education , *HEADACHE - Abstract
Introduction: medication-overuse headaches (MOHs) are the least studied of all headaches in Africa. Methods: we conducted a longitudinal study in order to describe our experience with patients placed in Neurology Outpatient observation in Brazzaville over a period of 4 years, from September 2010 to August 2014. All patients with chronic primary headaches, according to the International Classification of Headache Disorders (ICHD) (2nd edition), were included in the study. All patient with secondary headaches or who hadn't given consent were excluded. The patients were divided into two groups: those who had progressed to medication-overuse and those who didn't meet the criteria for medication-overuse (without-MOH). The variables used were the sociodemographic characteristics of patients, the clinical features of primary headaches and MOH treatment. Results: the study population was constituted by 193 patients out of 212. The average age of patients was 42 ± 14 years, of whom 66.32% were women. MOH rate was 35.75%. The associated factors were: early age (p=0.003), concomitant use of nonsteroidal anti-inflammatory drugs (NSAIDs) and paracetamol (p=0.0001) as well as self-medication (p<0.0001). By contrast, higher education (p<0.0001) and the use of NSAID alone (0.002) were protective factors against the onset of MOH. Ambulatory withdrawal was the most practiced treatment and amitriptyline was the most commonly used medication. Conclusion: medication-overuse headaches are frequent in patients placed in Neurology Outpatient observation in Africa and deserve identification for a better management. [ABSTRACT FROM AUTHOR]

LARYNGEAL nerves , *ANATOMICAL variation , *THYROID gland , *LONGITUDINAL method , *NERVES , *THYROIDECTOMY - Abstract
Thyroid surgery requires a thorough knowledge of cervical anatomy and anatomical variations, in particular of the lower laryngeal nerve, in order to avoid iatrogenic lesions. The objective of our study was to analyze the relationships of the lower laryngeal nerve, the existence of branches of nerve division and a subjective appreciation of the size of the nerve. This is a prospective study of 1 year including 60 patients who underwent thyroidectomy. Sixty patients underwent surgery on the thyroid gland between February 2014 and January 2015 by the same principal operator (10 men and 50 women). The average age of our patients was 51 years. For men were performed 6 total thyroidectomies, 2 left lobo-isthmectomies and 2 right lobo-isthmectomies. For women were performed 33 total thyroidectomies, 8 left lobo-isthmectomies and 9 right lobo-isthmectomies. On the right, the nerve was superficial relative to the artery in 71.6% of cases; it was divided in 33.3% of cases and was abnormally thin in 16.6% of cases. On the left, the nerve was deep in relation to the artery in 83.3% of cases; it was divided in 15% of cases and was abnormally thin in 11.6% of cases. Knowledge of the anatomical variations of the lower laryngeal nerve is essential in thyroid surgery, the risk is particularly important on the right side given the sometimes very small caliber and the existence of branches of division more frequent than on the left side. This article tackles the migration project of Ukrainian immigrants in Spain in a context of economic and geopolitical uncertainty as a dynamic process, articulating the temporal, spatial and relational dimensions. More specifically, the residential intentions (to stay, return or re-emigrate) are studied in relation to the evolution of relational anchoring both in the home country, in the host country and in other countries. The analyses are based on a longitudinal study conducted between 2009 and 2016, using a mixed methodology design. A quali-quantitative and visual analysis of the change and / or stability of the composition and structure of the personal networks has shown that the migration project is very much related to the sense of belonging, depending on both the local and transnational configuration of personal networks. Moreover, this network configuration determines the impact of economic crises and armed conflict on the planning of future place residence.

This cross-sectional study examines the relationship between internalizing and externalizing behaviours and overweight in preschool children from Quebec City aged between 3 and 5 years. A total of 330 parents completed the French-Canadian version of the Child Behavior Checklist for Ages 1.5-5. Based on their body mass index, gender, and age (all self-reported data from the parent), children were classified as normal weight (3rd to 85th percentile) or overweight (> 85th percentile). Normal weight girls showed higher total internalizing scores than their overweight counterparts. Normal weight girls also had higher scores for the anxious/depressed and withdrawn subscales than overweight girls. No significant difference was observed between normal weight and overweight children for externalizing behaviours. Longitudinal studies are needed to better understand the emergence and direction of the relationship between behavioural problems and overweight in preschool children and the mechanisms involved. Multiple myeloma is a cancer that is considered to be a chronic disease today, due to novel therapies prolonging survival. It is therefore necessary to formally define the content of therapeutic education programs. The French Association of Multiple Myeloma Patients produced a competency framework for multiple myeloma patients and created tools helping the educational diagnosis referring to the patient's knowledge about its treatment, and the associated beliefs. This material enabled a prospective observational survey during five months in two hospitals, through semi-structured interviews to assess patient's knowledge about its treatment, and the importance given for each drug using a score. Twenty-six interviews were conducted. Patients could generally not mention all their treatments and resorted to self-medication. Strong representations were recorded, with less importance given to anti-thrombotic and anti-infectives drugs even though the risks against which they are fighting are vital. The highest scores were given to anti-cancer drugs and supportive care such as painkillers. Other tools have been built to meet patient training needs. [ABSTRACT FROM AUTHOR]

Introduction: Patients with diabetes mellitus have chronic neuropathic pain. The aim of our study was to 1) evaluate the feasibility of providing analgesia with a long-term sciatic perineural catheter in a medical unit for diabetic patients and 2) evaluate its effectiveness. Methods: A prospective, monocentric, non-randomized study was conducted over two years. All diabetic patients with an ineffective optimal systemic treatment were included. Written consent was obtained. Popliteal-sciatic nerve catheters were inserted under ultrasound guidance; ropivacaine was started. The primary endpoint was pain at Day 2. Pain intensity was measured using a numeric rating scale (NRS). Secondary endpoints were patient's pain relief, the impact on quality of life, and morphine consumption, evaluated at Day 0, Day 2, at the time of catheter removal, and one month after catheter removal. Results: Feasibility was evaluated over one year. Fifty-five perineural catheters were placed in 32 patients. The median duration of catheter placement across patients was 13 [4-23] days. No toxic or infectious complications were seen. Effectiveness was evaluated during the second year of the study. Seventeen catheters were placed in 12 patients; 83% of patients had a NRS score ≤3 at Day 2. More than 70% of patients experienced pain relief while the catheter was in place and at one month after its removal. The impact on quality of life was negligible. Morphine consumption was less on Day 2, at the time of catheter removal and one month after removal. Conclusion: The use of sciatic perineural catheters as an alternative analgesia technique in a non-surgical environment for diabetic patients with chronic pain was possible and effective. Aggressive natural killer cell leukemia (ANKL) is a disease entity within the spectrum of lymphoproliferative syndromes of NK cells. It is rare, preferentially affecting Asiatic people. It has been very rarely reported in the African population; hence the interest of our case. Our study involved a 19-year old female patient of Moroccan origin presenting with acute-onset bone marrow failure syndrome associated with tumor syndrome. The diagnosis of natural killer cell leukemia was retained based on microscopic and immunophenotypic study of the bone marrow. Patient's outcome was fatal; she died within 2 months of diagnosis due to septic shock. Patients with ANKL have a worse prognosis and the median survival time following diagnosis is, on average, two months. In the absence of a prospective study, no therapeutic guidelines have been developed. [ABSTRACT FROM AUTHOR]

Western powers have created a wide range of military networks whose operating modes remain largely unknown to us. While the subfield of International Relations debates the ability of networks to generate convergence, this article addresses this debate with a longitudinal study of one such military network: the Multinational Interoperability Council (MIC). Our analysis reveals that, despite limited resources and influence, members of the MIC produce an identity, norms and capabilities for collective action that are specific to the network. They are able to mobilise them strategically in order to produce soft convergence in the area of military coalition building. [ABSTRACT FROM AUTHOR]

The increase in available student placements at colleges and universities, the implementation of provincial and federal postsecondary education policies, and the rise of the educational aspirations of families and individuals have all led to the massification of Canadian higher education. Based on Merle's typology of the forms of democratization, this article attempts to revisit the theory of equality of opportunities by critically analyzing the link between massification of higher education and social equity. The results of an analysis of longitudinal data from the (YIT) Youth in Transition Survey show that at the age of 24 in 2008, approximately 77% of young Canadians have pursued studies in a college or university. If access to postsecondary education is now higher, to what extent has it improved social equity? The article shows, in light of the Merle's typology, that mass university education is achieved in part under the seal of a segregative democratization, while college education tends to be egalitarian.
