Abstract: Localised intestinal perforation of newborn was described initially by Siebold in 1825 and was a long time labelled idiopathic and considered as a different entity from necrotising enterocolitis. During these last years, several divergent publications made the distinction between the two entities problematic. We report the case of a newborn baby of 7jours, which presented a focal perforation of the ascending colon; witch required a partial colic resection with colostomy. The histological examination of the specimen shows a focal necrotising enterocolitis with perforation. The establishment of digestive continuity was carried out at the 4 months age with a favourable outcome. From the data of our observation, we try to discuss the existence or not of difference between these two organic entities. [Copyright &y& Elsevier]

Abstract: Background: The use of plasma exchange (PE) constituted an advance in the treatment of myasthenia. The objective of our study was to determine the relevance of PE in the treatment of myasthenia and to study the different complications which can be observed during PE. Patients and methods: We studied retrospectively 11 patients who have generalized myasthenia and underwent PE. We used an intermittent flow cell separator and we performed PE three times a week. Biological assessment was performed before and after PE for all patients. The exchange volume was calculated according to the patient weight, gender and the value of hematocrit. Results: Our series included six women and five men. The mean age at onset of the disease was 41.4±14.1 years (range: 18 to 68). Indication of PE was myasthenia crisis (eight cases), resistance to classic treatment (two cases) and exacerbation after thymectomy (one case). An improvement was observed rapidly in five cases and delayed in three cases. The remaining three patients did not improve. The most frequent side effects of PE were hypotension (four cases), heart arrhythmia (two cases) and hypoglycemia (one case). Three patients dead in the seven days after the first PE. Conclusion: PE represents an interesting tool to treat severe forms of myasthenia and improve prognosis. High incidence of complications in our series can be explained by the initial disease severity, the used method of PE, the existence of associated illness, and a long stay in intensive care unit. Objectif de l’étude Le secteur Ib n’est pas un territoire de drainage habituel pour les carcinomes nasopharyngés avec un taux d’envahissement initial ne dépassant pas les 5 %. De ce fait, ce territoire n’est pas inclus systématiquement dans le volume intéressé par l’irradiation ganglionnaire prophylactique. L’objectif de notre étude était d’évaluer la dose reçue par le secteur Ib au cours de l’irradiation conformationnelle des carcinomes nasopharyngés. Matériel et méthode Il s’agit d’une étude dosimétrique rétrospective ayant repris le plan de traitement de 29 patients ayant reçu une irradiation conformationnelle pour un carcinome nasopharyngé. Le volume cible recevant la dose tumoricide (66–70 Gy) incluait la totalité du cavum avec les extensions tumorales et ganglionnaires. Les aires ganglionnaires (II–V) étaient irradiées à dose prophylactique de 50 Gy. Le secteur Ib n’était pas inclus dans le volume cible ganglionnaire prophylactique. Nous avons délinéé secondairement ce secteur selon le guide de délinéation du Radiation Therapy Oncology Group (RTOG) et nous avons étudié les paramètres de dose reçue par cette aire : la dose minimale (Dmin), la dose maximale (Dmax), la dose moyenne (Dmoy), et le volume qui a reçu 95 % (V47,5 Gy) de la dose totale prescrite. Résultats La médiane des doses moyennes reçues par le secteur Ib était de 53,8 Gy (extrêmes : 19,7 Gy–74,3 Gy). Les médianes des doses minimales et maximales étaient respectivement de 3,1 Gy et 77,7 Gy. La médiane des V47,5 Gy était de 73,7 %. Conclusion Après une radiothérapie conformationnelle, les taux des rechutes ganglionnaires dans le secteur Ib sont inférieurs à 5 %. Cela serait dû, très probablement, à l’inclusion de ce secteur, par nécessité, avec cette technique. Les résultats de notre étude le confirment avec une médiane de dose moyenne supérieure à 50 Gy. Cela nous incite à prêter une attention particulière à ce secteur lors de la définition des volumes cibles lors des irradiations avec modulation d’intensité. [ABSTRACT FROM AUTHOR]

Introduction: Le syndrome des 3 A ou syndrome d’Allgrove est une maladie autosomique récessive associant maladie d’addison, alacrymie et achalasie du cardia. Le gène a été identifié et localisé au niveau du chromosome 12. Objectif: Etudier les aspects épidémiologiques, cliniques, biologiques, génétiques, thérapeutiques et évolutifs du syndrome d’Allgrove chez nos patients. Patients et Méthodes: étude rétrospective des cas du syndrome de 3 A hospitalisés au service de pédiatrie du CHU Hédi CHAKER de Sfax sur une période de 18 ans (1990– 2007). Résultats: Durant la période d’étude, nous avons colligé 7 cas de syndrome des 3 A, issus de 5 familles. Il s’agit de 3 filles et 4 garçons. L’âge moyen était de 5 ans (2 ans 11mois – 9 ans). Tous nos patients avaient une alacrymie constatée dés la naissance. L’insuffisance surrénalienne a été inaugurée par une crise convulsive en rapport avec une hypoglycémie dans 2 cas et une mélanodermie chez tous les patients. L’achalasie de l’œsophage a été notée dans 7 cas. La cortisolémie était basse chez 5 enfants, elle était en moyenne 20 ng/ml, le test au synacthéne a été réalisé chez 2 enfants il était négatif dans les 2 cas. L’ACTH était élevée ans tous les cas. L’axe minéralocorticotrope était normal dans tous les cas. L’étude génétique a trouvé la mutation du gène AAAS dans 6 cas, elle n’a pas été réalisée chez un patient. Nos patients ont été mis sous hydrocortisone, 5 patients ont eu des dilatations de l’œsophage et un a eu une intervention de Heller. Après un recul moyen de 3 ans 10 mois, l’évolution de nos patients était favorable chez 5 patients, une patiente a été perdue de vue et un patient est décédé dans un tableau de dénutrition. Conclusion: Le syndrome d’Allgrove est une affection grave, malgré les thérapeutiques préconisées le traitement chirurgical de l’achalasie associé au traitement médical de l’insuffisance surrénalienne et de l’alacrymie semble être, du moins actuellement, la meilleure alternative thérapeutique chez l’enfant. Le syndrome de Griscelli (SG) est une maladie héréditaire rare caractérisée par un albinisme partiel, une sensibilité accrue aux infections et l’apparition au cours de l’évolution d’un syndrome d’activation macrophagique (SAM). Nous rapportons un cas de SG chez un nourrisson de 2 ans. Observation: A…, âgé de 2 ans,a été hospitalisé à plusieurs reprises depuis l’âge de un mois pour fièvre d’origine infectieuse mal tolérée. À l’âge de 2 ans, A… a présenté une dyspnée aigue associée à une fièvre, pâleur extrême et une énorme hépatomégalie. L’hémogramme a révélé une anémie régénérative à 4g/dl et une thrombopénie. La ferritinémie a été à 1 500ng/l et les triglycérides à 4,1mmol/l. Le diagnostic retenu était un SAM. Devant le retard psychomoteur, l’albinisme partiel et le SAM, un SG a été suspecté. L’examen des cheveux en lumière polarisée a montré des secteurs pigmentés sous forme de tâches brunâtres mélaniques localisées dans l’axe de la tige en faveur d’un SG. La biologie moléculaire est en cours. Conclusion: Le SG est une affection rare dont le diagnostic est suspecté sur l’aspect de cheveux à la lumière polarisée mais confirmé par la biologie moléculaire. La tuberculose rénale est une pathologie rare de l’enfant. La fréquence des formes urogénitales ne dépasse pas 5% chez l’enfant, vu que le temps d’incubation est long (allant de 3 à 20ans).Nous rapportons dans ce travail 3 cas à travers lesquels nous mettrons le point sur les difficultés diagnostiques et thérapeutiques de la tuberculose rénale. Il s’agit de 2 filles et un garçon âgés respectivement de 4 ans, 11ans et 15 ans. Deux d’entre eux ont été admis pour prise en charge de calculs coralliformes; alors que la troisième a présenté une infection urinaire compliquée de pyonéphrose qui a été drainée par une néphrostomie d’urgence. Opérée un mois plutard : elle a eu une pyélotomie, extraction des calculs et drainage par sonde JJ, les suites ont été marquées par un suintement chronique par la plaie, infection urinaire et dégradation de la fonction rénale justifiant la néphrectomie à six mois post opératoire. Une néphrectomie a été réalisée de première intention chez la fille de 4 ans. Dans ces 2 cas c’est l’anapath qui confirme le diagnostic de tuberculose. Deux de nos patients ont reçue une antibiothérapie anti-tuberculeuse triple pendant 2 mois puis double à base d’INH et Rifadine pendant 6mois. [ABSTRACT FROM AUTHOR]

Introduction: Décrit par Pfeiffer en 1964 le syndrome de Pfeiffer est un syndrome héréditaire rare, à transmission autosomique dominante associant une craniosténose, des pouces et des orteils élargis et déviés et une syndactylie partielle des mains et des pieds. Le syndrome de Pfeiffer atteint un individu sur 100000 naissances. Il est causé par une mutation au niveau des gènes FGFR-1 ou FGFR-2. Objectif: préciser les particularités cliniques, radiologiques thérapeutiques et évolutive du syndrome de Pfeiffer. Observation: Nouveau né Douaa est hospitalisée à la première heure de vie pour détresse respiratoire néonatale et syndrome polymalformatif. Elle est issue d’une 3e grossesse normalement déroulée, bien suivie, Le poids à la naissance était de 3000 grammes. L’examen à son admission a montré un syndrome dysmorphique associant une brachycéphalie, un occiput plat, une asymétrie temporale et une hypoplasie centro-faciale avec une hypoplasie du maxillaire et un prognathisme relatif, la racine du nez est déprimée. Elle présentait une exophtalmie importante avec ouverture permanente des yeux. Elle a une limitation des mouvements des coudes et des 2 genoux, une syndactylie partielle du 3e – 4e doigts droits et du 23e – 3eorteils gauches. L’hallux et le pouce sont larges et déviés aux niveaux des 2 mains et des 2 pieds. Le fond d’œil a montré un œdeme papillaire stade2. La radiographie du pied a montré un aspect trapézoïdal de la première phalange de l’orteil, le même aspect a été retrouvé au niveau de la première phalange du pouce. Le scanner cérébral a montré une craniosténose complexe avec synostose des parties moyenne des sutures coronales droite et gauche responsable d’une brachycéphalie. Les sutures sagittales sont d’aspect normal. Sur le plan thérapeutique la patiente a été mise sous gel larme pour son exophtalmie. l’enfant est décédée à un age de 1 mois. Conclusion: Bien que rare, le syndrome de Pfeiffer est une affection grave au vu des complications oculaires et neurologiques qu’il occasionne. Le diagnostic prénatal peut être fait lorsque l’échographie montre une craniosténose, une proptoseou un pouce élargie ou bien par la biologie moléculaire. [Copyright &y& Elsevier]

Unlabelled: Tuberculosis prevention is classically based on early detection of potential contagious cases and their effective treatment. But due to new parameters such as migration flows around the world, the emergence of Mycobacterium tuberculosis resistant strains and the increase of the population at risk, screening should be more active and target those who are more vulnerable to developing the disease. Traditional screening methods such as chest X-ray and tuberculin skin test, due to their high sensitivity and low cost, remain valid especially in populations with a high prevalence of the disease. The interferon-gamma release assays (IGRAs) seem to be very useful in immunodeficient patients with prior BCG vaccination. The treatment of subjects at high risk of developing active tuberculosis with a daily isoniazid self-administrated dosage for a period of 9 months is a prevention measure not only at the individual level but also at the collective one. All prevention interventions should be part of a national program concordant with the guidelines of the WHO Stop TB program that recommend a universal access to quality diagnosis and treatment focused on the patient., Outlook: New methods of detection based on gene amplification would better suit to detect tuberculosis in immunodeficient patients and identify treatment-resistant strains. Acquired ptosis is a common ophthalmologic problem. It is in the large majority of cases aponeurotic and occurs in the elderly as an involutional disorder or after ophthalmic surgery. In younger patients, it may occur after ocular trauma, periocular infection, contact lens wear, or palpebral edema. Allergic blepharoconjunctivitis is an unsuspected cause of acquired ptosis. Our study investigated patients (5 to 15 years old) with a history of allergic blepharoconjunctivitis and having a unilateral or bilateral ptosis. Physiopathological hypotheses are discussed. Background: A better understanding of angle-closure glaucoma mechanisms is necessary to indicate the appropriate treatment., Aim: the aim of our study is to discuss the laser indication in various type of angle-closure glaucoma., Methods: The study investigate 25 eyes of 22 patients who had an angle-closure glaucoma. The mechanisms are an anterior chamber angle blockage in 19 eyes, a plateau iris structure in 4 eyes and a nanophthalmos in 2 eyes., Results: Laser peripheral iridotomy is the most used technic; it was practised in 19 eyes. Argon laser iridoplasty was practised in 4 cases of plateau iris structure and in 2 patients who had a residual angle closure with a functional iridotomy., Conclusion: Laser is an effective alternative in the treatment of angle-closure glaucoma and can alleviate the need for high-risk filtering surgery.

Renal tubular dysgenesis (RTD) is a rare and severe nephropathy characterized by persistent fetal anuria leading to oligohydramnios with the Potter sequence, and perinatal death. The diagnosis is based on the histological finding of absence or paucity of proximal tubules. A consanguineous family is described in which 2 siblings, born after pregnancies complicated by oligohydramnios were affected with RTD. Patients were small for gestational age at birth. The first patient died after a few hours, the second after a few days of life, with persistent anuria unresponsive to treatment. Histologically, there was marked reduction in the number of proximal tubule sections and no renin was detected by immunohistochemistry. An homozygous mutation of the gene encoding renin was identified in both patients. This study underlines the interest of the histological examination of the kidney for the diagnostic of RTD in anuric fetuses and newborns, and the possibility of mutation analysis of RAS genes for genetic counselling and early prenatal diagnosis.

We retrospectively analyzed 31 cases of dermoid cysts surgically treated between January 1992 and October 2000. Mean patient age was 18.9 years. The preseptal localization predominated, with 29 cases. Two cases of intraorbital localization required orbitotomy. The surgical result was excellent.

The Common Food Toxin Infections has been subject of constant and exhaustive supervision watch over since 1994 in Bizerta. 77 centers in total has been recorded during the six last years (1994 to 1999) and have shown 738 patients. The evolution of incidental rate declared has been considered by annual fluctuations with extreme rates as 11.2 for 10,000 inhabitants in 1995 and 41.5 for 10,000 inhabitants in 1999. The classic season peak each year, and so 58.4% of centers has been notified during the months of July, August and September. The average size of centers is 9.5 persons with an extension of 02 to 40 persons. One death has been deplored a weak spreading rate of 0.14%. The hospitalization recommended for 106 patients (14.4%). About three quarters of the centers has been concerned by the familial. The cooked food is being at the top of contaminating food (49.3%).
