To examine educational practices in type 2 diabetes among health professionals.A cross-sectional study took place from June to September 2012 in Bamako and Ouagadougou. The study population consisted of all health professionals responsible for diabetes care in secondary and tertiary public health facilities.A total of 78 participants were interviewed. Doctors and nurses accounted for 77% and 23% respectively. The median number of patients seen in consultation was 10 per week (range 1-100). The median age of experience in diabetes care for health professionals was 5 years (range: 1-25 years). Sixteen participants reported giving advice for cooking. Of the 62 that did not, the reasons given were mainly: non-competence to give such advice (60/62); lack of time (35/62) and useless counseling (5/62). For out-of-home meals, recommendations included: taking a meal to work (60/77); avoiding meals outside the home (30/77); avoid alcohol and oils at parties (10/77).The recommendations made to patients come down to information and advice. As a result, the data collected lacks an educational strategy that aims to render the patient more independent by facilitating his adherence to counseling and improving his quality of life.

Introduction L’augmentation du risque cardio-métabolique (RCM) dans les pays à faible et à moyen revenu résulte pour large part d’une transition nutritionnelle rapide. Cette étude est réalisée dans deux groupes de population apparentés mais vivant dans des environnements différents. Elle vise à cerner la relation entre le mode de vie et les biomarqueurs de RCM, ainsi qu’entre l’obésité abdominale (OA) et les autres biomarqueurs. Méthodes L’étude porte sur 200 Béninois de Cotonou et 252 Haïtiens de Port-au-Prince (PAP) âgés de 25 à 60 ans et apparemment en bonne santé. L’OA est définie spécifiquement par un tour de taille (TT) ≥ 88cm (hommes) et ≥ 95cm (femmes). Les autres biomarqueurs les plus fréquents sont considérés: un ratio cholestérol total/HDL-cholestérol élevé, une tension artérielle élevée et la résistance à l’insuline d’après HOMA (Homeostasis Model Assessement). Le niveau socio-économique (NSE), l’alimentation, la consommation d’alcool et de tabac sont documentés par questionnaire. Deux schémas alimentaires ressortent de l’analyse typologique, l’un traditionnel et l’autre « transitionnel », avec une fréquence accrue d’aliments occidentaux. Résultats Le NSE, la consommation d’alcool et le tabagisme sont associés au RCM, mais non le schéma alimentaire. L’OA est associée aux autres biomarqueurs de RCM, sans effet marqué du NSE et des variables du mode de vie. Conclusion Les valeurs-seuilsspécifiques de TT se confirment. Aussi, le NSE et le mode de vie influencent le RCM mais non la relation entre l’OA et les autres biomarqueurs de RCM.

Elevated circulating homocysteine (Hcy) is considered as an independent cardiovascular disease risk factor. Hyperhomocysteinaemia (HHcy) is influenced by nutritional, genetic, and environmental factors. The purpose of the study was to assess HHcy prevalence in Benin, its association with intakes of B-vitamins (B2, B6, B9, B12), alcohol intake, and socio-economic status (SES), and its links with other factors of cardio-metabolic risk.The cross-sectional study included 541 apparently healthy subjects, aged 25 to 65 years, from three sites: the main city, a small city and a rural area. Hcy was measured with an ELISA test kit. The HHcy cut-off was 12 μmol/L. Dietary intake was assessed with three 24-hour recalls. We used a structured questionnaire to assess alcohol consumption, demographics, and SES according to education and an amenity score as income proxy. Criteria for obesity, hypertension, dyslipidemia and hyperglycemia were primarily those of World Health Organization (WHO) and the International Diabetes Federation.Mean age was 38.1 ± 10.1 years. The prevalence of HHcy was 52.2% in men and 24.7% in women. In multiple linear regression models, Hcy in men was positively associated with alcohol intake, but only alcohol in beer. In women, Hcy was negatively related to vitamin B12 intake. According to multivariate models of cardio-metabolic risk factors, HHcy was associated in women with more than twice the odds of hypertension and with high TC/HDL-c ratio. In men, Hcy was positively and independently associated with diastolic blood pressure and with LDL-cholesterol and total cholesterol in linear regression models.The prevalence of HHcy is high in Benin, when compared with other studies, and it was as expected higher in men than in women. Elevated Hcy was associated with inadequate intake of vitamin B12 in women, whereas alcohol consumption and its negative correlation with B12 intake was also involved in men. Although HHcy was independently associated with hypertension (in women) and more adverse cholesterol profile, no inference can be made because of the cross-sectional design of the study.

The coexistence of child malnutrition and maternal overweight in the same households typifies rapid nutrition transition in developing countries (DCs). It is reportedly less common in Africa than in Latin America or Asia, but the phenomenon is still little documented. The purpose of our study in poor urban neighbourhoods of the capital city of Benin (West Africa) was to assess the magnitude of the overlap of child protein-energy malnutrition (PEM) and maternal overweight within households, and to compare these households with other nutritional phenotypes in terms of socio-economic circumstances and diet quality. Our hypothesis was that both child PEM and maternal overweight could stem from poor socio-economic conditions, including lack of sanitation, and poor diet quality. Food diversity was used as an index of diet quality, or the qualitative dimension of food security. A random sample of 148 households that included a least the biological mother, one child between 6 and 59 months of age and a second one between 5 and 11 years was selected in two poor neighbourhoods of the capital city of Cotonou to assess the prevalence of "double burden" households and of other nutritional phenotypes of households: with PEM only in at least one child; with maternal overweight only; and without PEM or overweight. Body weights and heights of mothers and of the two targeted children were measured. As long as one child had low weight-for-height or height-for-age (z-score-2.0), the household came under the "PEM" type. In mothers, overweight was defined as body mass index (BMI)or = 25, obesityor = 30 and chronic energy deficiency18.5. We retained 126 households for interviews with mothers on socio-economic circumstances and food diversity. A socio-economic status (SES) score was constructed on the basis of household amenities and maternal education. Food insecurity was based on reported shortage of food in the last year in the household. Based on the frequency of consumption of 13 different food groups in the previous week, a food diversity score was computed. Overall 35.5% of children were malnourished, and school-age children had a worse nutritional status than under-5 children: 41% and 30% PEM (chronic or acute or both), respectively. The rate of maternal overweight was 39.1% including 15.5% of obesity. Child PEM coexisted with maternal overweight or obesity in 16.2% of the households; 27.7% of households had PEM only, 23% overweight only, 20.3% showed no malnutrition or overweight, and 12.8% had an underweight mother. Maternal BMI status was significantly associated with both children's weight-for-height z-score, particularly the elder one. The rate of child malnutrition, particularly wasting, was significantly higher among underweight mothers and lower in overweight mothers . Underweight mothers were merged with mothers with BMIor = 25 for the remaining analyses. Households with overweight mothers tended to enjoy relatively better socio-economic conditions--higher SES, higher maternal education, less food insecurity, better household sanitation; they also tended to have a more diversified diet. This is in contrast with PEM households. Dual burden households shared several socio-economic features with the PEM households, except for a higher (not significant) SES score. Dual burden households also had the lowest food diversity score of all household types. Logistical regression models revealed that a relatively higher SES level was associated with a higher likelihood of maternal overweight in PEM households, whereas poor household sanitation increased the odds of PEM among maternal overweight households. Food diversity appeared significantly associated with a lower likelihood of dual burden in all types of households. The study highlights the importance of addressing the double burden of malnutrition and overweight even in poor areas of low income countries of West Africa. It suggests that prevention efforts should be aimed at improving diet quality and sanitation in poor urban households.

Anemia is a widespread public health problem in developed and developing nations, and its prevalence is highest among pregnant women. In developing countries, schoolchildren constitute the population with the next-highest prevalence. Because there are few studies of anemia in schoolchildren in Morocco, this study aimed to determine its prevalence and its risk factors among preadolescents. We recruited 306 pupils from seven primary schools; blood samples were taken with their parents' or guardians' consent. We also collected anthropometric data, information about social and demographic characteristics (parent questionnaire) and school attendance and performance.More than 30% of these children had anemia: prevalence did not differ by sex, but was higher among those living in urban environments. Factors related to food behavior, especially diet diversity, appeared to be important. Our results found no relation between anemia and school performance. In the future more detailed cognitive tests should be used for this type of study.The prevalence of anemia among schoolchildren is high in the province of Kénitra, and the school health system is weak. Decision-makers have recently become aware of the need for an integrated approach to this age group: schools offer an opportunity for prevention and cure.

This paper focuses on changes in vitamin A (VA) intakes as part of the evaluation of a pilot project on social marketing of red palm oil (RPO) as a VA supplement for mothers and children in central-north Burkina Faso. The objectives of the 30-month project are to demonstrate the feasibility and effectiveness of introducing RPO in non-consuming areas. RPO is collected from women in the South-West region and it is sold in project sites by village volunteers. RPO is promoted by community workers trained in persuasive communication and social marketing. The target population is free to buy and consume RPO. Evaluation design includes data collected at onset, then 12 and 24 months later, from the same sample of 210 mothers and their children randomly selected in seven project sites. Children were 1 to 3 years old at onset. Blood samples were collected at baseline from mothers and children for serum retinol determination by HPLC. VA intakes are estimated by a semi-quantitative food frequency questionnaire, using the conventional beta-carotene to retinol conversion factors and the newly revised lower factors. VA deficiency is a major public health problem in the area: 64% of mothers and 85% of children had serum retinol concentrations0,70 mumol/l at baseline. VA came mainly from plant foods, particularly fruits and dark green vegetables which provided more than 90% of the dietary VA at onset of the project. Mean vitamin A intakes are low. We found 138 106 mug ER for the children and 302 +/- 235 microg ER for the mothers with conventional factors and 64 +/- 58 microg ER and 133 +/- 162 microg ER, respectively, with the revised factors. One year later, one third of respondents had consumed RPO in the previous week, and it supplied around 56% of the VA intake of children and 67% of mothers (36% and 46% respectively for the whole group). VA intakes were significantly increased at 510 +/- 493 microg ER and 801 +/- 913 microg ER for the children and their mothers respectively (347 +/- 443 microg ER and 568 +/- 803 microg ER respectively, with the revised factors). Analyzing serum retinol and dietary data collected at baseline, it was found that VA intakes62,5% of safe level of intake were highly sensitive to low serum retinol (0,70 micromol/l) and using revised conversion factors to assess total VA intake slightly enhanced sensitivity. The proportion of mothers and children at risk of inadequate VA intake changed from nearly 100% at baseline to 60% one year later. The results show that promoting RPO (and other VA rich foods) was effective in improving VA intakes. This improvement will hopefully be sustained and even further enhanced during the remaining 12 months of the project, after which repeated measurement of serum retinol and VA intakes will allow the actual impact of the project to be truly assessed.

Intrauterine growth retardation, which reflects in large part maternal malnutrition in poorer communities, contributes to chronic disease risk through foetal programming, according to the early origins hypothesis of Barker. Foetal programming implies that during critical periods of prenatal growth, permanent changes in metabolism or structures result from adverse intrauterine conditions. Observational studies first showed an association between lower birth weights and higher rates of coronary disease in the 80s, in England and Scandinavia. The link between low birth weights, or other indicators of small birth size, and cardiovascular disease was later confirmed in many epidemiological studies, including in the USA and in India. Similarly, a reverse relationship of birth weight and systolic blood pressure was shown in men and women, in developed as well as developing countries, and in all age groups, although it was less consistent in adolescents. Insulin resistance and type-2 diabetes have also been found to be independently related to small size at birth in several studies around the world. Insulin resistance associated with small size at birth was frequently shown to be present at a young age. The association of small birth size with chronic disease tends to increase with catch-up growth and obesity, and usually persists after adjusting for confounding factors such as age, family history, and socio-economic status. Several, but not all, twin studies lend support to the hypothesis. There is a tendency for lighter members of twin pairs to have a higher blood pressure, and more diabetes. Observations in people exposed to the Dutch famine while in utero also tend to corroborate the hypothesis. Those who were exposed early in their intrauterine life did not have lower birth weights, but they were prone to becoming obese later on. In contrast, those exposed towards the end of gestation had lower birth weights, and showed a higher rate of impaired glucose tolerance, while having a lower risk of obesity. Dietary manipulations in animal models provide further support and mechanistic explanations, in particular protein deficiency in pregnant rats, which elevates blood pressure, impairs glucose tolerance, and increases the likelihood of obesity in the progeny. Although there are still controversial areas, there is at present sufficient scientific evidence for foetal programming to be regarded as an additional risk factor for chronic disease, in interaction with genetic and lifestyle risk factors. The fact that intrauterine growth retardation may predispose to nutrition-related chronic disease has serious implications for developing countries, particularly those undergoing rapid nutritional transition, as it may further increase the rates of obesity, cardiovascular disease and diabetes when diets and lifestyles are in themselves "atherogenic". The challenge is for programmes to simultaneously combat apparently opposite nutrition problems, malnutrition and "over-nutrition". Improving the nutrition of women is even more imperative when considering that it may contribute to preventing chronic diseases in the next generation, in addition to enhancing health and survival of mothers and children.
