An essay by the contemporary Portuguese philosopher José Gil addresses the prominent American choreographer and dancer Merce Cunningham’s conception of dance. Gil emphasizes that Cunningham approaches dance in a way that is fundamentally different from the traditional mimetic and expressive paradigm. With the help of Giles Deleuze’s observations, Gil proves that even in the case of dance shorn of the faculty to represent and express emotional contents, we can talk about units of dance, about their meaning and unity, and thus about the language of dance. Gil spells out this idea by showing a parallel between dance, in Cunningham’s conception of it, and modern painting.

Abstract: Lipomatous hypertrophy of the heart is a benign rare tumor characterized by fatty tissue deposits in the interatrial septum. Its diagnosis during life is mostly seen incidental. We report the case of a 74-year-old female patient, in which a cardiac mass protruding in the right atrium was diagnosed by transthoracic echocardiography. Transoesophageal echocardiography examination detected an echogenic infiltration and hypertrophy of the interatrial septum, sparing the foramen ovale. The diagnosis of lipomatous hypertrophy of the interatrial septum is confirmed by MRI, which shows a well limited and not encapsulated tumor, appearing in increased signal density on T1-weighted images, and that decreases with fat-saturated sequences. This tumor did not enhance with the administration of contrast material. The benign and lipomatous nature of the lesion is confirmed, thus making the use of histology unnecessary. Aims: The diagnosis of acute myocarditis is complex, especially when the clinical presentation mimics an acute coronary syndrome. This condition may promote the progression to dilated cardiomyopathy and the occurrence of severe arrhythmias. A reassessment integrating a cardiac MRI at three months after the acute episode could help identify patients with a poor prognosis., Patients and Results: This prospective series of 43 consecutive patients hospitalised for acute myocarditis included 36 men and seven women, with a mean age of 32 years, with no indication of heart failure. All patients presented elevated levels of troponin I. Echocardiography showed moderate left ventricular dysfunction in six cases and segmental wall motion abnormalities in 22 cases. After gadolinium injection, a subepicardial late enhancement was observed in 39 cases. Three months after the acute episode, all patients were asymptomatic. The echocardiography and laboratory tests were normal. In 23 cases, the MRI showed persistence of the late enhancement without segmental wall motion abnormality. After a mean follow-up of three years, one patient was lost to follow-up and only one suffered a heart failure revealing a dilated cardiomyopathy complicated by ventricular arrhythmias., Conclusion: On admission, the subepicardial localisation of late enhancement in the cardiac MRI is reliable criteria for the diagnosis of acute myocarditis, enabling to rule out an acute coronary syndrome. During follow-up, the persistence of late enhancement has no impact on prognosis. Left bundle branch block (LBBB) during exercise can be associated with chest pain. Though this association is mostly correlated with an underlying coronary artery disease, painful LBBB has been described in patients with normal coronary arteries. We report a case of exercise-induced LBBB with typical angina pectoris related to antimalarial prophylaxis with chloroquine in a 66-year old woman with normal coronary arteries, which was reversed after treatment discontinuation. The effect of chloroquine on the electrophysiological properties of nodal cardiac tissue is responsible for this rate-dependent LBBB. Precautions for future antimalarial prophylaxis are also discussed.

In Africa, hypertension is a major public health problem, and hypertensive heart disease is a leading cause of heart failure. Left ventricular hypertrophy is a common consequence of hypertension. We present a case illustrating an unusual cause of such hypertrophy in a 73-year-old patient with heart failure living in the Comoros Islands, in whom we diagnosed cardiac amyloidosis. This disease has not previously been reported in the Comoros Islands and remains associated with a poor prognosis.

In nursing homes, the presence of a nurse to watch over the residents at night is not required. The residents' sleep is thereby usually in the hands of a nursing assistant and a hospital housekeeper. It is thanks to their skills that they assure the continuity of care and the safety of the elderly people, without overstepping the boundaries of their roles.

Left ventricular noncompaction (LVNC) is a recently identified and probably rare congenital cardiomyopathy characterized by changes in the structure of the myocardium secondary to incomplete embryogenesis. The purpose of this report is to describe three cases of LVNC involving African patients. To our knowledge these are the first cases described in Africa. All three patients in this series were men from sub-Saharan Africa ranging in age from 23 to 45 years. The first patient in whom cardiomegaly was recognized on a routine chest x-ray was asymptomatic. The second who presented with exertional dyspnea developed left bundle branch block. The third was admitted to the hospital for acute pulmonary edema. In all three cases transthoracic echocardiography suggested diagnosis. The left ventricle was dilated and hypokinetic and the myocardium exhibited a spongy aspect in association with the presence of prominent trabeculations separated by crypts located at the apex and lateral wall. Color Doppler demonstrated that intratrabecular recesses were filled by intraventricular blood flow. Magnetic resonance imaging (MRI) specifically confirmed this morphological feature. Sudden death due to arrhythmia, cardiac insufficiency, and systemic emboli are the main complications of LVNC. The incidence of LVNC, which is certainly underestimated, is highest in young adults but it can be diagnosed at any age. Echocardiography and MRI are effective tools for detection of the morphologic diagnostic criteria. Recent evidence suggests that LVNC is of genetic origin and the data reported here shows that the underlying mutations are present in sub-Saharan populations. Family screening in African populations is still difficult. Therapeutic management is currently based on symptomatic treatment of cardiac insufficiency and can require techniques not readily available in tropical settings.

Cardiovascular disease is a major worldwide health problem with a growing impact in developing countries. Heart failure is the clinical manifestation of many advanced cardiac disorders. It can have numerous etiologies and the incidence of non-infectious causes is increasing with socio-economic development, thus illustrating the global nature of this epidemiologic transition. Several of the numerous non-infectious causes of heart failure involve cardiac diseases specific to tropical areas including dilated cardiomyopathy, endomyocardial fibrosis, and peripartum cardiomyopathy. Other widespread disorders are becoming more common as a result of the epidemiologic transition. Cardiovascular risk factors are changing particularly with regard to the incidence of coronary artery disease, ischemic cardiomyopathy, and hypertension-related complications. The purpose of this article is to provide an overview of non-infectious causes of heart failure in terms of frequency, onset, and therapeutic requirements. Symptomatic treatment of heart failure is same as in developing countries but is often delayed due to shortcomings in the care system.

The latest reviews of the literature devoted to the epidemiology of ALS all agree that exogenic risk factors play a role in sporadic ALS. Nevertheless, there is no convincing evidence demonstrating in a reproducible manner an association between an environmental risk factor and ALS. This discordance is mainly explained by methodological skews. Over the last ten years, exogenic factors have been analyzed within the framework of specific lifestyle factors such as place of residence, smoking or not, or certain eating practices. The most recent work suggests that interactions between genetic and environmental factors depend on the age at exposure and the duration of exposure. The objectives of this general review is: to analyze the principal case-control studies, historical cohort studies or mortality studies which looked at the associations between an environmental factor and ALS, to present main results of studies having analyzed lifestyles in relation to one or more exogenic factors, and to discuss the limitations of epidemiologic studies on ALS.

The authors studied the responses of the main systems of sympathetic and hormonal regulation in valvular aortic stenosis, a special model of dissociation between arterial pressure and left ventricular function. The series comprised 14 patients with an average age of 70 +/- 9 years without diuretic therapy presenting with pure calcific aortic stenosis without other valvular or coronary disease. All were in sinus rhythm; 5 were taking an angiotensin converting enzyme inhibitor. Plasma concentrations of endothelin 1, atrial natriuretic factor (ANF), arginine vasopressin (AVP), catecholamines, plasma renin activity (PRA), angiotensin II and aldosterone were measured in resting, fasting patients, by blood samplings from a peripheral vein immediately before cardiac catheterisation. The results were compared with the severity of the aortic stenosis (aortic valve area greater or less than 0.7 cm2), the ratio of left ventricular work/myocardial mass (greater or less than 0.6) and treatment (with or without ACE inhibitors). Catecholamine levels were much higher in severe aortic stenosis (noradrenaline: 579 +/- 66 pg/ml when valve surface area > 0.7 cm2 versus 900 +/- 92 pg/ml when valve surface area < 0.7 cm2; p < 0.01). Endothelin -1 and AVP concentrations were normal. Whereas PRA was normal, aldosterone levels were increased in patients without treatment by ACE inhibitors. This treatment did not, however, normalise the noradrenaline levels. The increase in ANF concentration was large when left ventricular work decreased with respect to myocardial mass (190.8 +/- 42.3 pg/ml if W/M was decreased versus 82.7 +/- 15.4 pg/ml when W/M was normal): this could be related to the degree of left ventricular hypertrophy.

We present four cases of lip carcinomas which have been extended to the mandible in between of one to four years through the mental nerve with peripheric neuropathy which was multiple in one of the cases.
