Abstract

Les crues et les inondations ont un caractère aléatoire qui rend complexe la prise de décision et la communication au citoyen. La description probabiliste du fonctionnement d'un cours d'eau rend le discours difficile, y compris parfois par les spécialistes et engendre souvent des contresens. Or, les enjeux économiques et sociaux des inondations nécessitent des messages clairs et compréhensibles par tous. Nous nous attacherons à identifier quelques freins à la perception objective du risque d'inondation, et nous apporterons quelques pistes de construction d'une représentation des phénomènes. Nous proposerons notamment une formalisation à travers l'image d'une urne colorée en fonction des probabilités d'occurrence des inondations.

Abstract

International audience; During crisis (droughts or floods) hydrometric data are essential for decisions related to real-time water resource and risk management but also afterwards in cases of disagreement. Estimating the uncertainty of discharge measurements is necessary to make consistent decisions. Although discharge measurement techniques have improved in recent decades, uncertainty analysis remains a challenge in hydrometry due to the lack of discharge standards in the field. This study aims at estimating the uncertainty of velocity-area discharge measurements with potential perspective of updating the ISO 748 standard. To address the limitations of existing methods, this study presents a new analytical propagation method, called Flaure, to estimate the uncertainty components. Special attention is paid to the estimation of the dominant u'm component related to cross-sectional interpolation errors of velocities and depths. High-resolution reference gaugings are used to assess the uncertainty component through a statistical analysis. The method then establishes an explicit link between the estimation of cross-sectional interpolation errors and a sampling quality index that can be computed for any given current-meter discharge measurement.

Abstract

Progress leads to increase life duration at the metastatic stage but metastatic disease is most often lethal. Decision-making is necessary for an increasing period of care, beyond evidence-based medicine, dealing with complexity and uncertain benefit/risk ratio. This requires to inform the patient realistically, to discuss prognostication, to develop anticipated written preferences. These changes mean to pass from a medicine based on informed consent to medicine based on respect of the patient wishes even if it can be complex to determine. A new multidisciplinarity is needed, centered on the meaning of the care, the proportionality of the care, the anticipated patient trajectory. The ASCO has published recommendations on early palliative care. The timing and the quality of the discussion between palliative care specialists and oncologists is crucial. We propose 10 steps to organize a multidisciplinary onco-palliative meeting, as it appears the key for the organization of care in non-curable disease.
