Remote monitoring is becoming increasingly popular among healthcare professionals and patients for diagnosing and treating heart disease. Several smart devices connected to smartphones have been developed and validated in recent years, but their clinical use is still limited. Significant advances in the field of artificial intelligence (AI) are also revolutionizing several fields, yet the impact that these innovations could have on routine clinical practice is still unknown. We review the evidence and uses of the main smart devices currently available as well as the latest applications of AI in the field of cardiology, with the aim to ultimately evaluate the potential of this technology to transform modern clinical practice., Competing Interests: N. - Abstract
Angor with non-obstructive coronary arteries or ischemia with non-obstructive coronary arteries (ANOCA/INOCA) is a condition where a patient experiences symptoms and/or signs of myocardial ischemia, without significant coronary artery stenoses. This syndrome is often caused by a direct imbalance between supply and demand, leading to inadequate myocardial perfusion due to microvascular limitations or coronary arteries' spasms. Although previously considered benign, there is increasing evidence that ANOCA/INOCA is associated with a poor quality of life, significant burden on the healthcare system, and major adverse cardiac events. - Abstract
The diagnosis of acute coronary syndrome with ST segment elevation (STEMI) is based on clinical symptoms suggestive of acute myocardial ischemia and precise ECG criteria. STEMI is due to an acute occlusion or subocclusion of a coronary artery, generating a transmural ischaemia, requiring coronary angiography with urgent coronary revascularization. However, some authors consider the current STEMI ECG criteria to be too restrictive: up to 30 % of patients who do not meet these criteria present with critical coronary artery stenosis or acute coronary occlusion. Atypical electrocardiographic patterns, known as "STEMI equivalents", can be found in some cases and justify a rapid cardiological evaluation. The last years, various large randomized studies have demonstrated the benefit of TAVI (transcatheter aortic valve implantation) over medical therapy or over conventional surgical treatment in inoperable or very high risk patients with symptomatic severe aortic stenosis. More recently, new data has also shown that among patients at intermediate risk, TAVI is at least as beneficial as surgical valve replacement. 13. [Influence of circadian rhythms on myocardial infarction].

- Abstract
Myocardial infarction is one of the most important causes of mortality and its incidence exhibits a significant circadian pattern with a peak of maximum frequency between 10 am and 11 am. Furthermore, myocardial infarction size and related mortality rate also undergo a variation over 24 hours. Recent publications have shown greatest myocardial injury when symptoms onsets are around midnight and this was independent of ischemic time and quality of care. These data were corroborated by studies using experimental models that unravel correlation between myocardial infarction's size and genes involved in circadian rhythm the link between circadian biology and pathophysiology of ischemia provides a new era of cardiovascular research and in addition new potential therapeutic targets to prevent myocardial ischemic burden.
