Obesity is a chronic disease that has become a major public health problem with a prevalence that has doubled in the past two decades in most industrialized and developing countries. Currently, bariatric surgery represents the most effective treatment for extreme or severe overweight (BMI ? 40 kg/m² or ? 35 kg/m2 with weight-related comorbidities). Pre-operative bariatric surgery psychiatric and psychological assessment is essential for various reasons. In addition to the selection of candidates for the operation, its objectives are to prepare patients for future postoperative changes as well as to optimize their psychological and psychiatric care. This article describes a standardized tool, the BIPASS (Bariatric Interprofessional Psychosocial Assessment Suitability Scale), which allows a quality assessment in the field.

Introduction: New techniques have been proposed for the surgical treatment of benign prostatic hypertrophy (BPH): laser enucleation and laparoscopic adenomectomy. The objective of this retrospective study was the comparison of the results of Millin adenomectomy realized under contemporary medical conditions, with the results of laser enucleation and laparoscopic adenomectomy., Method: Two hundred and forty consecutive Millin adenomectomy cases performed between January 2004 and December 2008 were analyzed. Patient evaluation before operation consisted in IPSS score, quality of life score, maximum flow rate (Qmax), post-void residual volume (PRV), total prostate volume (TPV) performed under trans-rectal ultrasound. Operation duration, adenoma weight, blood loss during operation, transfusion rate, duration of Foley catheterization, duration of hospital stay, and immediate postoperative complications according to Clavien classification were reported. Each patient was systematically controlled at 3 months and was invited by mail for a second control in 2012. Each of the two outpatient controls consisted in the realization of an IPSS, quality of life score, Qmax and PRV. Our results were compared to contemporary results published concerning open adenomectomy and new techniques of adenomectomy., Results: Mean age was 69 years, TPV before operation 111 cc, adenomectomy specimen weight 82 gr, blood loss during operation 246 mL, transfusion rate 6 %, operation duration 88 min, Foley catheterization duration 3.8 days, mean hospital stay 5.6 days. Clinical results at 3 months were: IPSS decrease from 25 to 5 points, quality of life score decrease from 5 to 0.7 points, Qmax increase from 6.5 to 22 mL/sec, PRV decrease from 115 to 7.5 mL. According to the use of either the classical classification or of Clavien classification, the rate of early complications varied respectively from 36 to 49 %. No Clavien grade 4 or 5 complication was observed. Long-term results in 137 (57 %) of the patients with a mean age of 80 years, 5 years medial control (4; 8) after adenomectomy showed an IPSS varying from 2.5 to 6.6 points according to follow-up length, quality of life score varying from 0 to 1.2 points, Qmax varying from 13 to 23 mL/sec, PRV varying from 0 to 25 mL., Conclusion: The lack of exhaustivity of the results and the absence of use of Clavien classification rendered difficult any comparison between the different techniques. Urologist motivation in favor of a given technique represented a bias difficult to bypass, even with the realization of a randomized study comparing two techniques. Purpose: To evaluate middle term results of surgical sphincterotomy in neurogenic bladder dysfunction with detrusor-sphincter dyssynergia., Patients and Method: Retrospective study about 44 patients whom underwent urinary sphincterotomy between January 2008 and September 2012. All patients had detrusor-sphincter dyssynergia. Mean age was 49.7 years. Seventy percent (n=30) of patients had spinal cord injury, 80% (n=34) underwent urethral stent. Sphincterotomy was performed in 55% (n=24) with monopolar resection and in 43% (n=19) with Revolix(®) laser, after urethral stent withdrawal., Results: Mean follow-up was 30 months. Mean hospitalization stay was 5 days. Mean preoperative post-voiding residue (PVR) was 149mL and postoperative was 176mL. Twenty-nine percent (n=13) underwent new sphincterotomy with a final mean 116mL PVR, and a mean delay before new sphincterotomy of 9 months. Twenty percent (n=9) underwent other surgical procedure for sphincterotomy fail., Conclusion: In this series, we observed that surgical sphincterotomy is a well tolerated procedure, but with moderate immediate efficiency about 57%. It requires sometimes second procedure and long term follow-up in order not to fail to recognize stenosis, with best surgical success (80%)., (Copyright © 2013 Elsevier Masson SAS. Purpose: To study the feasibility and to present results of laparoscopic nephrectomy with vaginal extraction in overweight or obese patients., Patients and Methods: Three patients having BMI>28 kg/m(2) underwent surgery. Median age was 56 years, median BMI was 44.4. Medical history included for the three patients type 2 diabetes mellitus and arterial hypertension. Two of the three patients were multiparous, one was nulliparous. Patients underwent standard laparoscopic nephrectomy with vaginal extraction of the specimen through a short incision of the posterior vaginal fornix. Feeding was normal on the first operating day. No morphine was prescribed during hospitalization stay., Results: No intraoperative complication occurred. Mean procedure time was 224 min. Mean blood loss was 140 mL. Postoperative period was uneventful and walking was normal on the first operating day. Hospitalization stays were 2, 3 and 5 days. One month follow up found no complication including normal sexual activity., Conclusion: Laparoscopic nephrectomy with vaginal extraction is a feasible technique in obese or overweight patients. Purpose: To assess both economical and organizational impact as well as bacteriologic safety of a flexible cystoscope with sterile disposable sheath (FCSDS) compared to standard flexible cystoscopy (SFC) in two French urologic academic units., Patients: Two-center prospective study, comparing the use of the FCSDS to the SFC on two consecutive periods of time. Two hundred and five patients were included and divided into each group. Duration procedures and costs were analysed in the two techniques. The urinary tract infection rate was also described. A dedicated sheaths leaks test after use was performed systematically., Results: The preparation time of the fibroscope was longer for the sheathed cystoscopy group: 16.2 minutes versus 10.9 minutes for the standard group. The mean duration of disinfection was significantly shorter for the sheathed cystoscopy group: 53.8 minutes saved compared to the standard group; 99.01% of the tested sheaths, after their use, had no breaches. Urinary tract infections rate were similar in the two groups. The average cost of a sheathed cystoscopy compared to the standard was significantly cheaper in Lyon and almost equivalent in Marseille., Conclusion: The FCSDS allows significant saving over the disinfection duration, consumable costs and staff costs, while ensuring patient bacteriologic safety similar to SFC., (Copyright © 2013 Elsevier Masson SAS. Introduction: Determinate if the adjunction of PCA3 score and/or prostatic MRI can improve the selection of the patients who have an indication of first prostate biopsy., Patients and Methods: Multiparametric prostatic MRI and PCA3 score were made before biopsy to men scheduled for initial prostate biopsy for abnormal digital rectal examination and/or PSA superior to 4 ng/mL. T2-weighted imaging, diffusion-weighted imaging and dynamic contrast-enhanced imaging looked for suspect target classified on a scale of four. It was a prospective, single centre study. The diagnostic accuracy of PCA3 score and MRI was to evaluate in comparison with biopsy results., Results: Sixty-eight patients were included, median PSA was 5.2 ng/mL (3.2-28). Negative predictive value (NPV) of MRI score 0, 1 and 2 were respectively 80%, 43% and 69%. Positive predictive value (PPV) of MRI score 3 and 4 were 50% and 81%. The PCA3 cutoff with best accuracy was 21 (Se: 0.91; Sp: 0.50). Only one patient with positive biopsy (0.5mm of Gleason score 3+3) had negative MRI and PCA3 inferior to 21., Conclusion: MRI and PCA3 score in association allowed, in this study, to consider reduction of unnecessary initial biopsy without ignoring potential aggressive tumor., (Copyright © 2012 Elsevier Masson SAS. Objectives: The aim of this work is to measure the inter-judge concordance in thirteen psychiatric expertises for invalidity insurance., Method: We analyzed thirteen interdisciplinary (somatic and psychiatric) expertise files, blinded for the patient identity. The patients had a first opinion of a "classic" psychiatric expertise and then a second opinion of other experts, from a center specialized for interdisciplinary expertises in Switzerland. The inter-experts' judgments were compared according to the complexity of every file, due to the intervention of four foreign independent experts with regard to the expertise process (France, Belgium, United States, Switzerland, Romania)., Results: A satisfactory inter-judge concordance in terms of diagnoses and to incapacities was observed in nine files out of thirteen, with a better consensus for the male patients (chi2 = 4.95; p = 0.026). Divergent opinions seem to be in relation with the lack of explicit expertise guidelines used by the first psychiatric experts. We highlight some general principles issued from guidelines of the Swiss Insurance Psychiatric Society for medical expertise of psychiatric disorders. In our opinion these principles would have been able to improve the inter-judge concordance in the thirteen analyzed expertises., Discussion: In spite of his methodological limits, especially the reduced number of analyzed expertises, this work underlines the interest to use of certain guidelines for the realization of psychiatric expertises in terms of invalidity insurance., Conclusion: The use of guidelines could improve the inter-judge concordance for the realization of psychiatric expertises about invalidity insurance.

Despite the high prevalence of panic disorders in patients in primary-care settings, this condition is frequently under-recognised and under-treated. After the description of DSM-IV diagnosis criteria of panic disorders, this paper underline the importance of an adequate somatic and psychiatric differential diagnosis. Even if cognitive-behavioural therapy is the best studied psychotherapeutical approach, several efficacious psychodynamic psychotherapies were also described. High-potency benzodiazepines provide a rapid efficacy with beneficial effects during the first days of treatment. However, benzodiazepines should be avoided in the long term, because of the development of tolerance and dependence. Antidepressants (SSRI, venlafaxine) are effective in preventing panic attacks, especially in improving anticipatory anxiety and avoidance behaviour.

Background: Management of suicide attempters accounts for 10% of the psychiatric activity in the emergency room. In this population, the prevalence of borderline personality disorder (BPD) is high (10 - 55%). These patients present poorer psychosocial outcome and more frequent suicide attempts repetitions. However, the utility of the assessment of BPD in the referral to a specific treatment plan has not been yet studied., Objective: To examine the association between the assessment of a diagnosis of BPD after a suicide attempt and the referral from the emergency room to a specific treatment plan., Hypothesis: Suicide attempters with BPD, according to clinicians diagnosis, differ in terms of severity from those without more risk factors of suicide attempt repetitions and poorer psychosocial functioning, and in psychiatric referral from the emergency room., Method: Our case-control study took place during 10 months in the Geneva general hospital. We continuously enrolled patients admitted to the emergency room for deliberate self poisoning and separated them into two groups (BPD and control) according to the clinician's diagnosis. Data from medical records were systemically and anonymously gathered. We compared BPD patients' socio-demographic and clinical characteristics, as well as psychiatric referral, with the control group., Results: Of the 478 subjects admitted to the emergency room for deliberate self-poisoning, 99 (22.6%) were diagnosed BPD by clinicians. Compared to controls, they were more frequently female (OR=3.9) and living alone (OR=3.8) and more often resorted to psychiatric care (OR=2.9), notably to emergency care (OR=3.8). Past history of suicide attempt was also more frequent (OR=1.9) as was the use of neuroleptics in the attempt (OR=2.7). No difference was detected in terms of psychiatric referral after emergency room care., Conclusion: Even if borderline personality disorder in suicide attempters is associated with more severity criteria, it is not associated with a referral to a specific treatment plan.

This paper discusses the importance of psychiatric guidelines and the position of the psychiatrist in the management of agitation in the intensive care unit. The use of psychiatric validated scales to assess agitation seems to ameliorate the quality of care in psychiatry, but also in intensive care. Psychiatric experts' recommendations for managing agitation are given, which is useful to create an open discussion with the intensivists. The use of sedative medication to protect the patient, staff and to prevent an escalation of violence remains a personal choice for each practitioner, depending on individual patient needs and context. In the treatment of agitated patients, an equilibrium needs to be found between the subjective dimension and the available data from evidence based medicine.

In order to evaluate the feasibility of the autologous transfusion in an alloimmunized sickle cell patient, changes in the hematologic and biochemical characteristics of erythrocytes stored for 42 days from two patients with sickle cell SC anemia were compared with control subjects' (Hb A) red blood cells. Erythrocytes were stored in Saline Adenosine Dextrose Mannitol at +4 degrees C. The cryopreservation storage was made and 51Cr red cell survival was measured in one patient. No significant difference in the hematologic and biochemical parameters of the SC red blood cells and the control subjects was observed during the storage at +4 degrees C. Red cell survivals determined in fresh cells, cells stored for 42 days at +4 degrees C and thawed cells from one patient demonstrate much shorter half-life values than those of normal red blood cells. Before application, our results need to be confirmed by the same protocol with another patient with sickle cell SC.
