INTRODUCTION : décrire les aspects épidémio-cliniques et échographiques des môles hydatiformes (MH) à Abidjan.Méthodes: étude transversale de 6 ans (janvier 2011 à décembre 2016) réalisée au CHU de Yopougon au Service de Radiologie. Elle a consisté en la description du profil épidémio-clinique et échographique des patientes porteuses de MH. Les examens échographiques ont été réalisés par voie mixte (endovaginale et sus pubienne) en modes B et Doppler Couleur par des radiologues seniors. Une étude anatomo-pathologique du contenu utérin a été effectuée.Résultats: vingt-cinq cas de MH ont été diagnostiquées sur 12,190 échographies obstétricales réalisées soit 0,2% d'incidence radiologique. L’âge moyen des patientes était de 33,4 ans avec des extrêmes de 22 et 50 ans. Il n'y avait pas de classe d'âge dominante. La notion de masse abdominale 36% et de métrorragies 28% étaient les signes cliniques prédominants qui accompagnaient l'aménorrhée (100%). Sur le plan échographique, l'utérus était hypertrophique dans 100% des cas, homogène dans 96% et myomateux dans 4% des cas. La MH avait une épaisseur moyenne de 42,7mm. Son aspect était décrit comme vésiculaire dans 68%, en nid d'abeille dans 16%, multikystique dans 12% et en tempête de neige dans 4%. Les MH étaient classées partielles dans 4% des cas, complètes dans 92% des cas et invasives dans 4% des cas. Les ovaires étaient hypertrophiques dans 44% des cas avec une notion de macrofollicules dans 32% et des kystes dans 8% des cas. Le diagnostic échographique de MH a été confirmée à l'anatomopathologie dans 100% des cas. Conclusion: les MH demeurent rares à Abidjan et sont dominées par la forme complète. La notion de survenue à des âges extrêmes n'a pas été retrouvée.

Introduction: According to European and American series, up to 20% of colorectal cancers are characterised by instability at microsatellites sites. MMR deficient colorectal cancers are predominantly found in the right colon. Although an increasing rate of colorectal cancer has been observed in many low-income countries including in West-Africa, data on epidemiology and biology of colorectal cancer in native Africans from this region are scarce., Materials and Methods: We aimed to study the incidence of MMR deficiency in Côte d'Ivoire and to compare the data with those from a tertiary center in Belgium. Immunohistochemistry for MLH1, MSH2, MSH6 and PMS2 was performed on paraffin-embedded tissue samples from 83 colorectal cancers (46% males) operated in Abidjan and from 343 colorectal cancers (53% males) from Brussels., Results: Colorectal cancer was occuring at a younger age in Côte d'Ivoire compared to Belgium (median age: 53 versus 66). MMR deficiency was detected in 11,7% of Belgian cases and in 13,3% of Ivorian cases. Whereas MMR deficient cancers in Brussels were mainly found in women (24/40 i.e. 60%), in Abidjan only 3/11 (27%) were female. Moreover, the predominant location of MMR deficient tumours was different between both series: in Brussels, mainly located in the right colon (24/40 i.e. 60%) whereas in Abidjan predominantly (10/11 i.e. 91%) in the left colon. In Brussels we observed in the majority of cases (67,5%) loss of expression of MLH1 and PMS2, in Abidjan loss of expression of MSH2 and MSH6 (54,5%)., Conclusions: Our pilot study reveals differences in presentation of MMR deficient colorectal cancer between the two geographic regions suggesting differences in epidemiology and biology of colorectal cancer in native Africans., (Copyright © 2019 Elsevier Masson SAS. Objectives: In Africa, lymphomas are widely represented by pediatric Burkitt lymphomas. In Abidjan, cytology performed after needle biopsy may be an examination of choice for pediatric lymphomas because of its low cost. We evaluated the value of this cytological examination in comparison with MYC rearrangement assessment., Patients and Methods: A cytological examination was performed after needle biopsy of masses suspected for lymphoma. The reliability of this cytological examination was assessed versus a cytogenetic technique of fluorescence in situ hybridization with a probe targeting locus 8q24 (MYC) which is recurrently rearranged in Burkitt lymphomas., Results: Thirty-four patients were enrolled in this study. The median age was 8 years old. The main locations of the suspicious masses were maxillofacial and abdominal. Thirty cytological examinations identified cytological aspects of Burkitt lymphoma. Among these cases, the cytogenetic technique identified 24 cytogenetic rearrangements compatible with a translocation involving MYC as described in Burkitt lymphoma. Six cytological examinations identified cytological aspects of Burkitt lymphoma without MYC translocation. Two cytological examinations were not compatible with Burkitt lymphoma and a normal MYC status was observed. Two cytological examinations were technically not contributive., Conclusions: The cytological examination showed good performance, notably with excellent sensitivity. The cytological examinations compatible with a Burkitt lymphoma without MYC translocation (6/30=20.0%) could be explained by the absence of translocation involving locus 8q24 (MYC) in some endemic Burkitt lymphomas., (Copyright © 2011 Elsevier Masson SAS. Duodenal infection by Mycobacterium avium-intracellulare is a common opportunistic disease in HIV-infected patients. Individuals with CD4 counts <50 cells/mm3 are at highest risk. The main symptoms are diarrhea, abdominal pain, weight loss, and fever. Endoscopic examination shows various abnormalities including disseminated nodules that may be yellowish, whitish, or pinkish in color. Other mucosal lesions may be found such as erosion, erythema, or friable edematous aspect. Since these findings are non-specific, it is important to obtain biopsy specimens for histological and microbiological examination. The most common histologic features are atrophic mucosa resembling Whipple's disease with strongly positive PAS staining. The presence of BARR in macrophages is typical of MAI. Diagnosis is based on identification of the bacteria using either conventional culture techniques or polymerase chain reaction (PCR). Differential diagnosis includes other gastrointestinal infections associated with AIDS, i.e., microsporidiosis, cryptosporidiosis, giardiosis, anguillulosis, CMV, and isoporosis. The course of the disease is usually unfavorable even with antibiotic treatment. The purpose of this report is to describe a case of duodenitis due to atypical mycobacterial infection in a 30-year-old woman who was seropositive for human immunodeficiency virus. She was hospitalized due to fever with deterioration of her general condition (more than 10% of body weight loss) and chronic abdominal pain with inflammation. Diagnosis of MAI was confirmed by biopsy and Ziehl-Neelsen coloration. The patient was treated with rifampicine, isoniazide, ethambutol, and pyrazinamide in association with stavudine, lamuvidine and efavirenz. Despite improvement of general condition, fever persisted and the patient died after 40 days of treatment.
