If patients consider that their orthodontic treatment is a failure, they can claim compensation from their practitioner. Most often, discontented patients call on their third-party liability insurance when they are covered for "legal expenses protection". The patient's insurance company will then get in touch with the practitioner's insurance firm. Three-quarters of all claims are dealt with by the insurance companies. However, if an agreement cannot be reached or if the practitioner's insurance company manages to establish that he/she is not at fault, the patient can apply to the Tribunal de Grande Instance, the French Regional Court. The judge appoints a forensic expert who will examine the patient and follow a very precise procedure involving a dozen different questions. The advice of the forensic expert is therefore very valuable to orthodontists, in particular regarding their duty to inform patients and the importance of the medical file in order to avoid being held accountable following a course of treatment.

Summary In the context of this study, a clinical audit of cases treated by a single orthodontist was carried out to illustrate one practical application of the PAR index. Fifty pairs of dental casts taken from the patient group before and at the end of orthodontic treatment were evaluated by an orthodontist trained in the use of the PAR index. This evaluation shows that the average overall PAR score for the subjects included in the study fell from an initial value of 25.64 ± 11.73 points to 1.78 ± 2.79 points at the end of orthodontic treatment. The average reduction attributable to orthodontic treatment was 23.86 ± 0.95 points, for an average percentage reduction of 93.36 ± 9.02%. When cases were classified according to the degree of improvement suggested by the nomogram of the PAR index, 23 (46%) were in the “Improved” category after treatment, and 27 cases (54%) in the “Greatly improved” category. This adds up to a total of 100% in these two categories, with none in the “No better” or “Worse” categories. It should be recalled that a high standard of orthodontic treatment is considered to be reached when the average percentage reduction of the PAR score exceeds 70% and when the number of cases in the “Worse or no better” category is below 5%.

In a retrospective multicentre study, the success rate and efficiency of activator treatment were analysed. All patients from two University clinics (Giessen, Germany and Berne, Switzerland) that fulfilled the selection criteria (Class II division 1 malocclusion, activator treatment, no aplasia, no extraction of permanent teeth, no syndromes, no previous orthodontic treatment except transverse maxillary expansion, full available records) were included in the study. The subject material amounted to 222 patients with a mean age of 10.6 years. Patient records, lateral head films, and dental casts were evaluated. Treatment was classified as successful if the molar relationship improved by at least half to three-fourths cusp width depending on whether or not the leeway space was used during treatment. Group comparisons were carried out using Wilcoxon two-sample and Kruskal-Wallis tests. For discrete data, chi-square analysis was used and Fisher's exact test when the sample size was small. Stepwise logistic regression was also employed. The success rate was 64 per cent in Giessen and 66 per cent in Berne. The only factor that significantly (P < 0.001) influenced treatment success was the level of co-operation. In approximately 27 per cent of the patients at both centres, the post-treatment occlusion was an 'ideal' Class I. In an additional 38 per cent of the patients, marked improvements in occlusal relationships were found. In subjects with Class II division 1 malocclusions, in which orthodontic treatment is performed by means of activators, a marked improvement of the Class II dental arch relationships can be expected in approximately 65 per cent of subjects. Activator treatment is more efficient in the late than in the early mixed dentition.

The Herald of Free Enterprise disaster on the 6th of March 1987 just outside Zeebrugge was the first major disaster that happened in Belgium after the federal authorities put together a DVI team. It was also the first time dental identification by forensic odontologists was used in a Belgian mass disaster. Although international cooperation and coordination through Interpol in cases of disasters with multinational victims hadn't reached today's level, the handling of the disaster and the cooperation between Belgian and British specialists within DVI teams was a prime example of today's Interpol DVI philosophy. The lessons learned from this disaster were reported at the Interpol DVI working group annual meeting and subsequently new guidelines were implemented in the DVI Guide and Forms. The experience gained from this major event definitely helped in the development and proficiency of the Belgian DVI team, which became very professional in the years to follow through its involvement in different mass disasters and major incidents within Belgium and abroad and contributed a whole lot to the development of forensic odontology in Belgium.

The patient's file is often mismanaged because of the crowd and the frequency of emergencies caused by pain. The practitioner is worrying about handling it in first place. The objective of this work is to recall the importance of a good management of the dental records and to suggest a charting system. The interest of the dental chart, fundamental part of the patient dental file, is described before developing the implications related to an adequate management. The charting system of the service of Operative Dentistry and Endodontics of Abidjan Dental school is described. It translates the target of a good dental records management, which presents appreciable assets for practitioner, patient and administration.

Riskmanagement means the prevention of all accidental damages, prevention of claims in general and prevention of paying excessive insurance premiums. The main objective of riskmanagement is promotion of quality care. The riskmanagement rules are presented and discussed.

A model for the initial medical certificate is presented. It is a key document in the child's record and will constitute a reference during an expertise.
