Since the description of cortical deafness, it has been known that the superior temporal cortex is bilaterally involved in the initial stages of language auditory perception but the precise anatomical limits and the function of this area remain debated. Here we reviewed more than 40 recent papers of positron emission tomography and functional magnetic resonance imaging related to language auditory perception, and we performed a meta-analysis of the localization of the peaks of activation in the Talairach's space. We found 8 studies reporting word versus non-word listening contrasts with 54 activation peaks in the temporal lobes. These peaks clustered in a bilateral and well-limited area of the temporal superior cortex, which is here operationally defined as the speech sensitive auditory cortex. This area is more than 4cm long, located in the superior temporal gyrus and the superior temporal sulcus, both anterior and posterior to Heschl's gyrus. It do not include the primary auditory cortex nor the ascending part of the planum temporale. The speech sensitive auditory cortex is not activated by pure tones, environmental sounds, or attention directed toward elementary components of a sound such as intensity, pitch, or duration, and thus has some specificity for speech signals. The specificity is not perfect, since we found a number of non-speech auditory stimuli activating the speech sensitive auditory cortex. Yet the latter studies always involve auditory perception mechanisms which are also relevant to speech perception either at the level of primitive auditory scene analysis processes, or at the level of specific schema-based recognition processes. The dorsal part of the speech sensitive auditory cortex may be involved in primitive scene analysis processes, whereas distributed activation of this area may contribute to the emergence of a broad class of "voice" schemas and of more specific "speech schemas/phonetic modules" related to different languages. In addition, this area is activated by language-related lip movement, suggesting that a multimodal integration of the auditory and the visual information relevant in speech perception occurs at this level. Finally, there is a task-related top-down modulation of the pattern of activation of the speech sensitive auditory cortex which may reflect the fact that the different parts of this structure are connected to different down-stream cortical regions involved in the neural processing of different types of tasks.

- Abstract
Neuroethics is a field of bioethics on the ethical challenges of advances in neuroscience. Born in the early 2000s, neuroethics is considering a number of issues raised by the opportunities created by advances in knowledge and techniques in the field of neurology and psychiatry. In fact, what we learn about brain functions allows us to potentially influence our behavior and our actions, and questions human nature, freedom and individual responsibility, and even the place of morality in our society.

- Abstract
Stroke can produce irreversible brain damage of massive proportion leading to severe disability and poor quality of life. When prognosis is very poor, "do-not-resuscitate (DNR) orders" and "withhold- or withdrawal-of-treatment (WHDT)" may be discussed. Precise estimates of withhold and withdrawal of treatment are not available, but studies have shown that DNR orders are relatively frequent in acute stroke (up to 30% of all patients). DNR orders are closely associated with age, comorbidities, and severity of neurological deficit. The main explicit criteria for WHDT is the risk of "unacceptable" disability, which raises many ethical questions. Evaluation of neurological outcome is highly uncertain and difficult and does not always reflect quality of life. Decisions for life sustaining therapies in severe stroke are always difficult and often based on subjective and uncertain criteria. We have to improve prognosis estimation and our understanding of patient's preferences to promote patient centered care. Ethical approach may guide these complex decisions.

- Abstract
Stroke prognosis remains difficult to determine. It depends on stroke severity, patient's age and his rehabilitation ability. In essence, individual prediction is uncertain. Neurological severity is related to many factors such as low blood pressure or hyperthermia, which are important to consider before prognosticate. Do-not-resuscitate orders are given in up to 30% of acute stroke. More than 50% of them are given upon admission (in the first 24h of stroke). Withdrawing and withholding treatments are usually decided when neurological prognosis is supposed to be "catastrophic". Other factors, such as physician's disability representation or hospital use of aggressive procedures, certain daily routines or organizational constraints, are also probably implied. In France, end-of-life decisions are guided by the law: "loi relative aux droits des malades et à la fin de vie du 22 avril 2005". Palliative approach in severe stroke remains unknown and rarely developed, but is necessary to implement in such situations with high mortality rate. End-of-life decisions in severe stroke have several ethical issues such as the definition of what could be an "unacceptable" disability for the individual and/or the society., (Copyright © 2012. - Abstract
Although recommendations for diagnosis and treatment of stroke are available, the aim of this study was to identify indicators of quality and risk management for acute ischemic stroke hospital patients. We conducted a descriptive study of stroke patients who were diagnosed less than 12 hours before admission to the Pitié-Salpêtrière hospital's neurology and stroke unit. Data were collected using a literature review and from existing recommendation. During the study period (August 2003 through April 2005) 310 eligible patients were identified. In 87.5% of the cases, patients suffered from a cerebral infarction and in 10.3% from an intracranial haemorrhage. The initial deficit was mild to severe. The average time between the first symptoms and admission in the stroke unit was 212 +/- 130 minutes. Forty percent of patients who underwent a thrombolysis did so within the first 3 hours. The average length of stay in the stroke unit was 17.5 days. Thirty-one percent of the patients were discharged to go home, 47% to a rehabilitation unit and 8% died. Ten indicators of quality and risk management are proposed, taking in account the events before admission, hospital care, side effects, duration of stay, discharge location and the handicap.

- Abstract
The incidence of cerebral hemorrhage in France is 25000 new cases per year. Their prognosis is especially grim: six months afterward, 30-50% of these patients have died and only 20% are able to live independently. Management of acute forms requires, above all, hospitalization in a specialized neurovascular unit of proven quality. Initial treatment includes general measures to avoid possible early aggravation and the prevention and treatment of some complications. Specific treatment by recombinant factor VII can be effective in the first 4 hours after symptoms begin. A phase III study is under way to confirm its efficacy against hematoma growth in the first 24 hours and prognosis at 3 months. According to the recent STICH study, surgery is indicated in the case of hydrocephaly, for large cerebellar hematomas and for cortical supratentorial hematomas located less than 1 cm from the cortical surface. Beyond these situations, surgery must be considered on a case-by-case basis, until other studies more clearly specify indications and techniques. The pathophysiology of cerebral hemorrhage remains poorly understood but numerous research pathways are being explored and should make it possible to identify new treatment targets to combat, in particular, cerebral edema and neuronal lesions.

- Abstract
Surgical treatment of symptomatic atherosclerotic stenosis of vertebral arteries has been proposed for many years but this technique remains quite confidential due to technical difficulties and relatively high risks. Transluminal angioplasty has been proposed and we developed a simplified technique using coronary stent placement. The aim of this study was to evaluate the feasibility and efficacy of transluminal angiography with primary stenting for proximal stenosis for vertebral arteries. Eleven patients with symptomatic atherosclerotic stenosis of vertebral arteries were treated by trans luminal angioplasty with primary stent placement. Two patients, one with stenosis of the proximal vertebral artery and one with distal stenosis of the vertebral artery where only treated by transluminal angioplasty. In all cases transluminal angioplasty and stenting were feasible with restitution ad integrum of the diameter of the artery in 98 cases and with residual moderate stenose (<20%) in 5 cases. All patients were followed for more than one year, only one patient had recurrence of symptoms, but he stopped spontaneously the anti platteless drugs. Vertebro basilar symptoms disappeared completely in 12/13 cases and were improved in 1/13 cases. No restenose of the artery was observed on control (echodoppler) excepted in one case, where a tight stenose of pre vertebral sub clavian artery developed. Transluminal angioplasty for symptomatic stenosis of vertebral artery appears as a very successful technique with a low complication rate. It should be proposed in many cases of vertebrobasilar insufficiency related with tight vertebral artery stenosis.

- Abstract
A 40 year old woman presented successively an arterial and a venous cerebral thrombosis, revealing an inflammatory bowel disease. Digestive manifestations were very poor until then. Stroke is a rare complication of inflammatory bowel disease, and is usually correlated with and active phase of the disease. The mechanism by which the thrombogenic process occurs is unclear. The possible prothrombotic role of hemostasis disorders and the role of an angiitic process are discussed.

- Abstract
Objectives: Cervical spondylotic myelopathy is usually a chronic and progressive disease. In a few cases, however, dramatic tetraplegia sometimes occurs after even minor injury., Methods: We report seven patients (6 males, 1 female, aged from 41 to 63), who suffered from acute myelopathy after an injury., Results: In 5 cases, the injury revealed the cervical spondylotic myelopathy. A hyperextension of the cervical spine was found in 5 cases. The injury was severe in only one case, but there was no bony abnormalities, except arthrosis. The most common cause was a fall. In contrast, severe tetraplegia was found in 4 cases. Spontaneous, but incomplete, recuperation occurred in 4 patients. In all 7 cases, CT scan and MRI showed congenital cervical stenosis associated with cervical spondylosis. The level of disco-osteophytic changes was mainly in C4, C5, C6. On T2-weighted spin-echo image, an increased signal intensity was present in the cord of 3 patients, but was not correlated with the severity of the symptoms, nor with improvement. Surgical treatment was performed in 6 cases: 3 laminectomies, 3 anterior or antero-lateral approaches. Improvement after operative decompression was observed in all but one case, even when the motor or sensory deficit persists for more than one year. Motricity of the inferior limbs improved better than the other deficits, perhaps because of the location of non-reversible lesions in the spinal gray matter., Conclusion: The acute medullary syndrome of cervical spondylotic myelopathy is serious and can cause major handicaps. This complication justifies a preventive surgical attitude when medullar signs are moderate, and cervical imagery shows a spondylotic compression of the cord with congenital stenosis.
