Introduction Les hémopathies malignes sont fréquemment associées à un déficit immunitaire secondaire, facteur majeur d’infections. Chez les patients présentant un déficit immunitaire secondaire avec hypogammaglobulinémie, une immunothérapie substitutive par immunoglobulines polyvalentes humaines normales (IgG) diminue le risque d’infections graves. La mise en route et la poursuite de l’immunothérapie de substitution dépendent d’une analyse individuelle du risque infectieux. Le traitement vise à assurer un taux résiduel d’Ig d’au moins 5 g/L chez l’adulte. Matériels et méthodes Étude observationnelle multicentrique, prospective, longitudinale, en conditions de vie réelle, incluant des patients adultes atteints d’un déficit immunitaire secondaire dans le cadre d’une hémopathie maligne et débutant une immunothérapie de substitution quelles que soient la voie ou le lieu d’administration. L’objectif de l’étude Épicure était de décrire les indications et les modalités de l’immunothérapie de substitution à l’inclusion puis, pour les patients recevant l’Octagam ® ou le Gammanorm ® , au cours d’un suivi de 12 mois ou jusqu’à l’arrêt de l’immunothérapie de substitution. Résultats Trente centres ont recruté 238 patients entre mars 2012 et mars 2014. 7 patients ne respectaient pas les critères d’inclusion et l’analyse à l’inclusion a porté sur 231 patients. Parmi eux, 160 patients (67 ± 11 ans, 62 % d’hommes) ont reçu Octagam ® ou Gammanorm ® . Ces patients étaient atteints d’un myélome ( n = 54), d’une leucémie lymphoïde chronique ( n = 44), d’un lymphome B non hodgkinien agressif ( n = 19), d’un lymphome B non hodgkinien indolent ( n = 29, 51 % traités), ou une maladie de Hodgkin ( n = 4). Les patients ont été suivis pendant 8,7 ± 4,0 mois. 17 (10,6 %) sont décédés au cours du suivi et 5 décès ont été considérés par le médecin comme étant d’origine infectieuse. Chez les patients survivants, l’hémopathie maligne est restée stable dans 65 % des cas, s’est améliorée dans 31 % des cas et s’est aggravée dans 4 % des cas. 95 % des patients n’ont bénéficié d’aucune antibioprophylaxie en dehors du sulfaméthoxazole-triméthoprime et du valaciclovir. La fréquence et la sévérité des infections ont diminué sous immunothérapie de substitution ( Tableau 1 ), tandis que le taux d’Ig augmentait en moyenne de 2,1 ± 2,6 g/L par rapport au taux basal à l’entrée dans l’étude. 32 % des patients avaient au moins un dosage d’Ig inférieur à 5 g/L au cours du suivi, sans différence en fonction de la voie d’administration. Les changements de voie d’administration au cours du suivi ont été rares : 4 patients sont passés de la voie intraveineuse à la voie sous-cutanée et 4 patients de la voie sous-cutanée à la voie intraveineuse. L’immunothérapie de substitution a été abandonnée par 9 % des patients, arrêtée pour futilité par 31 % des patients, suspendue puis reprise par 8 % des patients, suspendue pendant l’été par 14 % des patients et poursuivie sans interruption par 38 % des patients. Les arrêts estivaux étaient inégalement répartis entre les centres, soulignant l’hétérogénéité des pratiques. La compliance s’est avérée excellente : seuls 7 patients (4 %) ont manqué une injection ou plus au cours du suivi. L’immunothérapie de substitution a été bien tolérée : 5 patients ont arrêté le traitement en raison d’effets indésirables. La clairance de la créatinine est restée stable quelle que soit la voie d’administration. Conclusion L’immunothérapie de substitution a permis de diminuer le risque infectieux chez les patients présentant un déficit immunitaire secondaire dans le cadre d’une hémopathie maligne et s’est avérée bien tolérée. L’étude a permis de mettre en évidence l’hétérogénéité des pratiques pour les conditions de poursuite de l’immunothérapie de substitution. [ABSTRACT FROM AUTHOR]

Le déficit immunitaire secondaire peut être causé par l’hémopathie maligne, ou être induit ou aggravé par ses traitements ou ses complications (immunosuppresseurs, anticorps monoclonaux anti-CD20, immunomodulateurs, corticothérapie). Les déficits immunitaires secondaires avec hypogammaglobulinémie peuvent nécessiter une immunothérapie de substitution par administration d’immunoglobulines polyvalentes humaines normales (IgG). Les données concernant cette utilisation sont anciennes et ne prennent pas compte des traitements actuels [1,2] . L’étude Épicure est une étude de « vrai vie » visant à étudier la pratique actuelle en France. Matériels et méthodes Étude observationnelle multicentrique, prospective incluant des patients adultes présentant un déficit immunitaire secondaire dans le cadre d’une hémopathie maligne et débutant une immunothérapie de substitution quels que soient la voie (intraveineuse ou sous-cutanée) ou le lieu d’administration (hôpital ou domicile). L’objectif de l’étude Épicure était de décrire les indications et les modalités de l’ immunothérapie de substitution ainsi que les attentes des médecins. Résultats Trente centres ont recruté 238 patients entre 2012 et 2014. 7 patients ne respectaient pas les critères d’inclusion et l’analyse a porté sur 231 patients (66 ± 12 ans, 65 % d’hommes). Les patients étaient atteints d’un myélome ( n = 64, immunothérapie ou chimiothérapie en cours pour 73 %), d’une leucémie lymphoïde chronique ( n = 84, 30 % traités), d’un lymphome B non hodgkinien agressif ( n = 32, 25 % traités), d’un lymphome B non hodgkinien indolent ( n = 39, 51 % traités), d’une leucémie aiguë ( n = 6, 33 % traités), une maladie de Hodgkin ( n = 6, 17 % traités). 41 % des hémopathies malignes étaient des rechutes ou réfractaires, 23 % des patients avaient reçu une autogreffe et 5 % une allogreffe. Les patients présentaient d’autres facteurs de risque infectieux : cytopénie auto-immune (8 %), autre maladie auto-immune (3 %), insuffisance rénale (16 %), diabète (12 %). Au cours des 12 derniers mois, les patients avaient été atteints pour 1,9 ± 1,5 d’épisodes infectieux ayant requis une antibiothérapie, pour 0,5 ± 0,9 d’épisodes avec antibiothérapie intraveineuse, pour 0,7 ± 1,0 d’épisodes avec hospitalisation. 42 % avaient été hospitalisés au moins une fois pour une infection durant cette période. 68 % des patients avaient un taux sérique de gammaglobulines inférieur à 5 g/L. Les IgG ont été prescrites par voie intraveineuse (50 %, 412 ± 233 mg/kg/mois) ou sous-cutanée (50 %, 95 ± 32 mg/kg/semaine) ; la première injection a eu lieu à l’hôpital dans 98 % des cas ; les futures administrations étaient prévues à l’hôpital pour 99 % des patients recevant le traitement par injection intraveineuse et à domicile pour 97 % des patients recevant le traitement par voie sous-cutanée. Le choix de la voie d’administration était indépendant de la fonction rénale. 7 % des patients ont également reçu une antibioprophylaxie (autre que sulfaméthoxazole-triméthoprime et valaciclovir). Les médecins attendaient de l’immunothérapie de substitution une prévention des infections modérées et sévères (attente importante ou très importante pour 82 % et 86 % des patients), une amélioration de la qualité de vie des patients (74 %), une diminution du nombre d’hospitalisations (73 %) et une diminution de la consommation d’antibiotiques (81 %). Les médecins espéraient également une amélioration de la survie chez 56 % des patients. Conclusion L’étude Épicure représente à ce jour la plus importante collecte de données de patients souffrant d’un déficit immunitaire secondaire associé à une hémopathie maligne et traités par IgG polyvalentes. Ces patients ont un risque infectieux avéré et une hypogammaglobulinémie marquée, alors que dans la moitié des cas, l’hémopathie maligne est en rémission ou fait l’objet d’une surveillance simple. Les IgG sont pour 50 % administrées par voie sous-cutanée. Dans ce contexte, les attentes des médecins portent sur la prévention du risque infectieux sévère mais également sur les infections modérées dans un objectif d’amélioration de la qualité de vie de ces patients de plus en plus substitués à domicile. Résumé Introduction La sarcoïdose est une affection granulomateuse multisystémique d’étiologie inconnue. L’apparition ou la majoration d’une granulomatose après transplantation de cellules souches hématopoïétiques (TCSH) a rarement été rapportée dans la littérature. Observation Nous rapportons le cas d’une patiente de 62 ans, ayant présenté une poussée de sarcoïdose dans les suites d’une autogreffe de cellules souches hématopoïétiques (CSH) réalisée dans le cadre d’un POEMS syndrome. La poussée de sarcoïdose a été mise en évidence par les examens d’imagerie et confirmée par l’obtention d’une histologie ganglionnaire ayant révélé la présence de granulomes épithélioïdes et gigantocellulaires non nécrotiques. La patiente était asymptomatique et ne nécessitait pas de traitement. Conclusion Le diagnostic de granulomatose doit être envisagé dans les suites d’une TCSH devant une présentation clinicoradiologique compatible. La confirmation reste histologique. Les diagnostics différentiels de granulomatose doivent être éliminés. Chez un malade ayant une sarcoïdose devant bénéficier d’une TCSH, le risque d’aggravation de la sarcoïdose devra être considéré. The French hospital system crises are constantly forcing the heads of departments to adapt and find solutions for maintaining optimal patient care in a context of staff shortage. Facing these challenges, we had the desire to create a community of department heads capable of helping each other, sharing their experiences, relying on collective intelligence and, ultimately, contributing to rebuilding their hospitals from the bottom up. In this respect, we arranged a two-day seminar, which brought together fourteen heads of hematology departments who share the same desire to challenge their organizations with a collaborative approach and make them evolve. The seminar was animated by an external speaker and included many fruitful sessions, both formal and informal. Following this seminar, participants are now interested in sharing this experience with other department heads throughout the organization of "collaborative seminars of heads of department." Such seminars would serve to create a real community of department heads capable of supporting each other to improve our organizations and to generate new ideas to participate in the reconstruction of our health system from the bottom. This approach is in line with the current strategy of public services to restore a prominent role to hospital departments. Pyoderma gangrenosum (PG) belongs to neutrophilic dermatoses. PG can have different clinical presentations (ulcerated, bullous, pustular), is often painful, and preferentially affects the lower limbs. The diagnosis can be challenging, and a cutaneous biopsy is often necessary, which shows an aseptic cutaneous infiltrate of neutrophils. The association with inflammatory or hematologic conditions is frequent, especially in older patients. The hematologic diseases the most frequently associated with PG are myelodysplastic syndrome, followed by monoclonal gammopathy of undetermined significance. Because of the strong impact of its treatment, recognition of PG is crucial. The treatment is based on first-line corticosteroids and topical or systemic immunosuppressive drugs and most often leads to a favourable outcome. The management of an acute hematologic disease would further improve the prognosis of PG. The singularity of geriatric patients encourages to thoroughly balance the risks and benefits of the recommended drugs and to consider associated non-drug measures. Here, we propose a review of the scientific literature about the association between PG and hematologic diseases, with a special focus on older patients, accompanied by the report of two cases in geriatric ward.

HIV testing is recommended at time of cancer diagnosis, HBV and HCV screening because of the risk of reactivation with certain anticancer drugs.This is a cross-sectional study. The objectives were to assess the screening practices in cancer patients and the satisfaction of professionals in the event of use of the CancerHIV network. A questionnaire drafted by the CancerHIV expert and the OncoPaca-Corse Regional Cancer Network (RCN) was distributed in the region at the end of 2018 (part 1: V1) before being extended to the national level via the CancerHIV network (part 2: V2). Participation reached 160 and 130 respondents (V1 and V2, respectively). At the initial cancer assessment, 23% of respondents declared that they systematically screened for HIV at V1 (V2: 17%), 25% for HBV (V2: 20%) and 24% for HCV (V2: 19%). Before immunotherapy, the rates were 54% for HIV in V1 (V2: 52%), 57% for HBV (V2: 60%) and 55% for HCV (V2: 57%). Among the respondents, satisfaction when requesting a regional or national remedy was high (almost 100%). Screening for HIV, HBV and HCV allows supervised prescription of immunosuppressive or cytotoxic treatment to a potentially immunosuppressed patient. This study, resulting of an original collaboration between a RCN and a national expert network, underlines the lack of screening at the 2 examined stages of patient care, and the need for raising practitioners' awareness to recommendations., (Copyright © 2021 Société Française du Cancer. We report the case of a man with a primary diagnosis of Waldenström macroglobulinemia. He secondarily presented a diffuse large B cell lymphoma (DLBCL) located in the nasal fossae, which relapsed later in the eye. The diagnosis of these two malignancies is based on a multidisciplinary biological approach using new sensitive and specific techniques. These techniques revealed that the two diseases harbor different B cell clones, indicating a distinct origin. This observation highlights the importance of targeted biological techniques for the diagnosis of these two rare hemopathies. It also shows that it is possible to prove the independent nature of the two tumor clones, thus allowing optimized therapeutic management.

Primary central nervous system lymphomas (PCNSL) in immunocompetent patients have long suffered from their rarity. Biological studies as well as initiation of comparative prospective therapeutic studies have been delayed in this population. A significant improvement in survival was observed when radiotherapy was preceded by chemotherapy with high dose methotrexate (MTX). Median survival improved from 16 months, with radiotherapy alone, to 35-45 months with combined treatments, at the cost of a risk of neurological toxicity especially common among people over the age of 60. From then, many studies have attempted to improve outcomes while reducing the toxicity on the central nervous system. If the optimal treatment is not yet established, these studies, mostly retrospective, provide a better understanding of the therapeutic issues to be addressed. Controversies regarding the impact of monoclonal anti-CD 20 antibodies as well as the role and the best modalities of radiotherapy remain and intensive chemotherapy. The latest imaging techniques provide useful elements to rule out differential diagnosis, but histological diagnosis remains mandatory. The evaluation of therapeutic response needs to be improved. Recent biological studies initiated the biological characterization of PCNSL and potential therapeutic targets are identified. Target therapies used in systemic non-Hodgkin's lymphomas have to be tested in PCNSL. PCNSL occurring in the context of immunosuppression are increasingly rare, especially in human immunodeficiency virus (HIV) infected population. They are well characterized in terms of clinical and biological aspects. Epstein-Barr virus (EBV) plays a major role in their lymphomagenesis. Treatment depends mainly on the underlying cause and degree of immunodepression. Whenever possible, similar treatments to those used in immunocompetent PCNSL patients will be offered to immunocompromised patients.

Introduction: The most frequent neurological complication of Waldenström's macroglobulinemia is IgM-mediated polyneuropathy. Direct tumor cell infiltration of the nervous system is very rare and better known as the "Bing and Neel syndrome". This syndrome relates usually to a meningeal or meningo-myelo-cerebral tumor infiltration., Observation: A 54-year-old man developed a terminal cauda equina syndrome over several years. MRI disclosed lumbar roots infiltration and lumbar puncture the presence of lymphocytic meningitis with intrathecal synthesis of monoclonal IgM identical to that found in the blood. The bone biopsy revealed a lymphoplasmocytic infiltration consistent with the diagnosis of Waldenström's macroglobulinemia. The final diagnosis was meningeal and lumbar roots infiltration revealing Waldenström's macroglobulinemia. Partial remission was obtained after polychemotherapy with CHOP, rituximab and methotrexate. At the end of the treatment, the patient improved his bladder's control and was able to walk with a stick., Discussion: We reviewed the 35 cases of "Bing and Neel syndrome" we have identified by a PubMed research. acroglobulinemia, AL amyloidosis and the POEMS syndrome. The hematologist must characterize the hematological disorder, evaluate the need for treatment, and choose the best treatment option. The neurologist must define the type of neuropathy and evaluate the relationship between the abnormal immunoglobulin and the peripheral neuropathy. Thus, close co-operation between the hematologist and neurologist is necessary when a patient presents with both monoclonal immunoglobulin and peripheral neuropathy.

Chronic lymphoproliferative disorders represent a group of malignant lymphoid diseases of whom chronic lymphoid leukaemia (CLL) is the more frequent. They are incurable diseases of indolent evolution. Diagnosis is usually made on blood puncture when circulating malignant lymphocytes are present. Explorations are limited to CT-scan, and, for CLL, karyotype. Bone marrow biopsy must be avoided, as possible. Treatment indications will be limited to really evolved cases. The aim of treatment must be the quality of life. Adapted treatment are available for the majority of patients, with limited toxicity and prolonged efficacy.

Post-transplantation lymphoproliferative disorders (PTLD) represent a severe complication of organ transplantation, so the determination of prognostic factors for the response and the survival is of major interest. Studies on PTLD are hindered by the small number of cases, explaining the conflicting published results. In order to identify, prognostic factors in this kind of disease, 61 patients have been analyzed in a retrospective multicenter study. A new index is proposed to assess the survival probability, it is composed of two factors: the performance status (PS < or = 1 versus 2-3) and the number of involved sites (1 versus > 1). This index seems to be more accurate in PTLD than the IPI, the reference in immunocompetent patients with non Hodgkin lymphomas. However, a similar analysis in a more homogeneous population remains necessary.
