The thesis examines the correlation between land rights and biological diversity in Burkina Faso using the example of Sissili province, where various actors coexist. The issue is that of legal pluralism following the consecration of State law in legal relations with the land in a local context where social cohesion is confronted with the survival of endogenous rights that have been excluded in the process of building national law since the country's colonial conquest. Starting from the complexity and evolution of social links, in particular land conflicts between actors with different legal references, the study, in a legal anthropology approach, aims to initiate a reflection on the development of a normative model for access to land that can promote the sustainable management of social relations and biodiversity. To this end, it addresses the dual dimension, legislative and customary (endogenous), of the norms governing the relationship between land rights and biological diversity. The review of the legislative and regulatory framework as well as targeted field surveys make it possible to identify the endogenous rights exercised by local populations. Like the problem of land management in the province on a national scale that it highlights, the reading of this plural normative system – national and local, written and customary, exogenous and endogenous – makes it possible to formulate, in a co-construction approach of a postmodern law, legal proposals reconciling social justice and environmental sustainability.; La thèse examine la corrélation entre droits fonciers et diversité biologique au Burkina Faso à travers l’exemple de la province de la Sissili où cohabitent divers acteurs. L’enjeu est celui d’un pluralisme juridique consécutif à la consécration du droit étatique dans les rapports juridiques à la terre, dans un contexte local où la cohésion sociale est confrontée à la survivance de droits endogènes écartés dans le processus de construction du droit national entamé dès la conquête coloniale du pays. Partant de la complexité et de l’évolution des liens sociaux, notamment les conflits fonciers entre acteurs aux référents juridiques différents, l’étude, dans une démarche anthropo-juridique, vise à engager une réflexion sur l’élaboration d’un modèle normatif d’accès au foncier à même de favoriser une gestion viable des rapports sociaux et de la biodiversité. Pour ce faire, elle appréhende la double dimension, légiférée et coutumière (endogène), des normes régissant les relations entre droits fonciers et diversité biologique. L’examen du cadre législatif et réglementaire, ainsi que des enquêtes de terrain ciblées, permettent de cerner les droits endogènes exercés par les populations locales. À l’instar de la problématique gestion du foncier dans la province à une échelle nationale qu’elle met en évidence, la lecture de ce système normatif pluriel – national et local, écrit et coutumier, exogène et endogène – permet de formuler, dans une approche de co-construction d’un droit postmoderne, des propositions juridiques conciliant justice sociale et durabilité environnementale.

À l’instar des autres collectivités territoriales burkinabé, la commune de Koudougou regorge de riches patrimoines naturels, archéologiques et historiques. Chaque secteur et village de la commune possède ses sites patrimoniaux. Ces biens culturels constituent des repères historiques des communautés locales. Chargés d’histoire, ils révèlent aussi bien les grandes étapes de la formation des peuples que leur évolution dans le temps. Ils passent pour être des âtres de la promotion touristique pour peu qu’ils soient pris en compte dans les politiques nationales de développement. Ils sont cependant, dans leur grande majorité, menacés de destruction face aux travaux d’aménagement intensifs et d’autres facteurs anthropiques. Dans cette étude, nous verrons comment s’opère le processus de protection et de valorisation malgré la multitude de défis qui menacent ce patrimoine. Nous présenterons d’abord les bois sacrés et les sites associés, puis nous décrirons les mesures de protection et de valorisation envisagées dans un contexte marqué par l’abandon des cultures locales au profit d’une « modernité » très peu soucieuse de respecter les valeurs culturelles ancestrales. Like other territorial collectivities of Burkina Faso, the municipality of Koudougou abounds in rich natural, archaeological and historical heritages. Each sector and village have its heritage sites. These cultural goods constitute important historical references to local communities. Filled with history, these cultural goods reveal the main steps of the formation of local populations and their evolution. They could also contribute to local tourism promotion if taken into account in national development policies. However, most of these heritages are threatened of destruction with the progress of intensive planning works and other anthropogenic factors. In this study, we analyse the issue of the protection and the valorization of sacred forests and their associated sites, in a context of local cultures neglected at the advantage of the so-called “modernity”, that is, for the most, irrespective of ancestral cultural values.

Cet article cherche a analyser l’influence des caracteristiques socio-economiques de l’individu sur sa probabilite d’adoption du mobile-banking. A partir d’un echantillon de 4 141 individus, nous estimons un modele logistique afin de determiner les facteurs d’adoption. Les resultats montrent que le sexe, le niveau d’etude, l’emploi, le fait de savoir lire et ecrire et le fait d’etre bancarise influencent positivement la probabilite d’adoption. En revanche, le fait d’etre encore a l’ecole et le revenu par tete ont une influence negative. Classification JEL : D11, O33, O12

Dans le domaine de la résolution mentale des additions simples, la conception classique depuis 40 ans suggère que les enfants utilisent des stratégies de comptage jusqu'à l'âge de 10 ans. A partir de cet âge, s'effectuerait un réel changement du comptage vers la récupération en mémoire. Récemment, de nouvelles hypothèses suggèrent plutôt une accélération par automatisation des procédures lentes remettant en question les théories classiques. La présente étude longitudinale de deux ans vise à tester l'hypothèse que les enfants de 8, 9 et 10 ans utilisent des procédures pour résoudre les additions non doubles. Ces procédures s'accélèrent au cours du développement à travers un processus d'automatisation progressif. Les enfants ont été soumis à une tâche d'additions impliquant les opérandes de 1 à 9 et à une tâche de mémoire de travail...

International audience; OBJECTIVE:the aim of this study is to evaluate the performance of stool microscopy and culture in the diagnosis of pulmonary tuberculosis (PTB).MATERIAL AND METHOD:In resource-limited settings, PTB is mainly diagnosed in sputum by microscopy methods. In patients unable to provide sputum, invasive procedures are necessary to obtain alternative respiratory tract specimens. Because Mycobacterium tuberculosis complex (MTC) organisms are known to survive in gastric fluid, we hypothesized that swallowed MTC organisms would be detectable in stool samples. We compared the presence of MTC organisms in sputum and stool specimens collected at the same time from the same patients.RESULTS:We included samples routinely submitted to our laboratory of microbiological diagnosis from patients with suspected PTB. In addition, a stool specimen was collected within 24 h of the sputum collection or gastric aspirate. In the 57 patients included, sputum microcopy and culture confirmed respectively 9 (21%) and 15 (26%) cases. Stool samples made it possible to confirm 9 cases by microscopy and 5 cases by culture. The sensitivity of microscopy was 60% and that of culture 33%. Sputum analysis was negative for one HIV-infected patient, but the stool sample was positive and permitted the diagnosis of PTB.CONCLUSION:This study proves that despite the low sensitivity of stool cultures it can be an alternative or additional interesting sample for the diagnosis of PTB in patients who have difficulty expectorating.

Les ganglions de la tête et du cou sont parmi les localisations les plus fréquentes de la tuberculose, un problème de santé publique dans le monde. Une étude rétrospective conduite entre 2001 et 2010 rapporte les caractéristiques épidémiologiques et cliniques de l'adénite tuberculeuse de la tête et du cou, au CHU Sanou Souro, au Burkina Faso. Au total, 115 patients ont été observés dont l'âge était compris entre 2 ans et 64 ans (moyenne 31,46 ans). Il y avait 53 patients de sexe masculin (46,1%) et 62 de sexe féminin (53,9%). Un pic de fréquence de 39,8% était observé entre 30 et 39 ans. Les adénopathies cervicales étaient multiples chez 96,5% des patients et abcédées chez 30%. Elles étaient associées à des adénopathies extra cervicales chez 16,6% des patients. Chez 83,4% des patients, il a été noté un ou plusieurs signes à type d'asthénie et ou d'amaigrissement (70,8%), de fièvre 25% ou de toux (20,8%). L'infection associée la plus fréquente était celle par le VIH, observée chez 43,3% des patients. Les résultats de cette étude commandent la recherche systématique de l'infection par le VIH chez tout patient porteur d'adénite cervicale tuberculeuse dans un contexte de double endémicité de la tuberculose et de l'infection à VIH.

Introduction La tuberculose constitue toujours un problème de santé publique. Sa localisation péricardique reste fréquente. L’objectif de cette étude rétrospective descriptive était de décrire les caractéristiques cliniques et évolutives des cas de péricardites tuberculeuses dans le service de cardiologie du centre hospitalier universitaire de Bobo-Dioulasso. Méthodes Nous avons mené une étude rétrospective descriptive des cas de péricardite tuberculeuse colligés en deux ans à partir des dossiers et registres dans le service de cardiologie du CHU de Bobo-Dioulasso de janvier 2009 à décembre 2010. Résultats De janvier 2009 à décembre 2010, parmi 945 hospitalisations dans le service de cardiologie, une péricardite tuberculeuse a été diagnostiquée chez dix patients âgés de 18 à 82 ans. L’âge moyen était de 46,8±25 ans avec un sexe ratio de un. Soixante pour cent des patients avaient moins de 40 ans. Tous les patients avaient un niveau socio-économique bas. Une notion de contage tuberculeux a été retrouvée chez six patients. Trois patients présentaient une tuberculose pulmonaire à microscopie positive. L’insuffisance cardiaque était constante chez tous les patients avec deux cas de tamponnade à l’admission ayant nécessité une ponction péricardique d’urgence. Tous les patients avaient une sérologie VIH négative. L’échocardiographie a été importante pour le diagnostic positif et dans la prise en charge. L’évolution sous traitement antituberculeux et de l’IC a été bonne chez neuf patients à la fin de la première phase du traitement antituberculeux. Un cas de décès a cependant été enregistré chez un patient avec une HTA déjà compliquée d’accident vasculaire cérébrale ischémique. Conclusion Les péricardiques tuberculeuses sont fréquentes au Burkina Faso. Elles touchent surtout les sujets jeunes et un intérêt particulier devrait être accordé au dépistage et au traitement précoce des cas.

There are three hypotheses to explain the relation between allergens and other factors and the onset of atopy: the hygiene hypothesis, the TH2 like hypothesis and the allergenic hypothesis. None of them have been confirmed. It seems necessary to wait for the results of prospective studies before giving recommendations for primary prevention of respiratory allergic diseases. In contrast, tertiary prevention appears as an essential method for treatment of allergic diseases.

We report a fortuitous discovery of primary pulmonary myxoid liposarcoma in an HIV-positive patient. Primary pulmonary localizations are uncommon. Generally, pulmonary localizations are metastatic. There is a male predominance and diagnosis is generally made around 40 years of age. The two main features of liposarcoma are the large tumor size and the complex histology that evolves over time. Pathology findings are rarely reproducible and vary from one pathologist to another. Macroscopically, liposarcomas can mimic benign tumors. The risk of recurrence is high after simple enucleation due to microscopic extracapsular extensions. Surgery remains the predominant treatment. Wide complete excision, if possible, provides long-term survival.

Bronchial fibroscopy is a recent investigation method that requires equipment and facilities difficult to implement in respiratory diseases units in developing countries. In Burkina Faso, this technique was introduced for the first time in February 1997. The purpose of this study was to determine the contribution of bronchial fibroscopy for the diagnosis of respiratory disease in countries with limited resources. This study was conducted between February 1997 and October 1998 at the respiratory diseases unit of the Yalgado Ouedraogo National Hospital Center in Ouagadougou, Burkina Faso. Thirty-five cases of tuberculosis were diagnosed, including 29 cases with bronchial node involvement, where bronchial fibroscopy is an essential diagnostic examination, and 6 cases of bacteriologically proven pulmonary tuberculosis. Ten cases of lung cancer were diagnosed (40% squamous cell carcinoma). Malignant disease is a reality in developing countries despite low rates of diagnosis due to insufficient diagnostic facilities. For tuberculosis, the importance of specific treatment is certainly well established and should always be initiated, even if fibroscopy cannot be performed. This contrasts with the situation for malignant disease, where the high prevalence of lung cancer (9.9% of the bronchial fibroscopies performed) is associated with total lack of treatment due to the absence of a thoracic surgery unit or a radiotherapy unit, and the impossibility of providing satisfactory surveillance of anti-cancer chemotherapy.

The purpose of this study was to determine the clinical, radiographic, and ultrasonographic aspects of mediastinal nodal tuberculosis and ascertain its clinical course in the era of HIV infection.We reviewed retrospectively 39 patients referred to the Ouédraogo Yalgado National Hospital Center and the National Anticancer Institute between February 1996 and December 1999 for mediastinal nodal tuberculosis. Endoscopic proof of tuberculosis was obtained in 30 cases (81.8%). HIV serology was positive in 26 of the 30 patients tested (86.6%).Nodal mediastinal tuberculosis accounted for 1.7% of the cases of tuberculosis recorded over the same period at the Anticancer Institute. Mean age of the patients was 32.8 years and the sex ratio was 1.05 in favor of men. Clinically, a past medical history was found in 18 cases (46%) including a herpes zoster in 6 (15.4%), cough in 38 (97.5%). Weight loss (95%), fever (100%) and peripheral node enlargement (20%) were found frequently, probably related to HIV infection more than tuberculosis. Radiographically, standard x-rays evidenced associated lesions in 22 cases, with 59% having predominant parenchymatous lesions. Other localizations of tuberculosis were very frequent (42.5%).Bronchial fibroscopy is most contributive to diagnosis of mediastinal nodal tuberculosis with an 81.8% yield in our series. HIV infection had a determining effect on the disease course since among the 16 patients who died, 14 were HIV-positive (52%).

The prevalence of iodine deficiency disorders (IDD) was studied in the health district of Zitenga village (Oubritenga province, Burkina Faso), as part of an assisted development program. The study was designed to assess the prevalence of IDD in a population of more than 2,000 people. A multistage cluster sample of 210 people was randomly selected. The women in the sample were aged 0 to 45 years and the men, 0 to 25 years. Clinical and laboratory results (circulating TSH concentration, T4 and iodine excretion in urine), and dietary intake were recorded. The prevalence of goiter (all grades) was 55.2%. The values recorded for iodine excretion in urine showed that there was moderate iodine deficiency in this area. Blood thyroxine levels were below the laboratory reference range for 63% of the subjects, whereas TSH levels were above the laboratory reference range for 69.3% of the subjects. There was no correlation between hormone status, iodine excretion and goiter. The entire population of this area was deficient in iodine; some individuals had clinical signs of iodine deficiency, such as goiter. Further studies are required to investigate the factors responsible for goiter in this area and iodine supplementation should be provided for the population.
