Psoriasis is a chronic inflammatory skin disease affecting almost 1% of children, starting the first months of life. In some journals, this prevalence is reported to be between 0.4 and 0.7%. We are reporting a case of childhood psoriasis in a 2-year-old patient. The case was observed in the Dermatology unit at the Hubert Koutoukou Maga National University Hospital of Cotonou (CNHU-HKM). The case clinical description will allow us to show the difficulties in establishing a diagnosis and how little are the treatment choices available in our clinical settings. [ABSTRACT FROM AUTHOR]

The article summarizes the results of scientific activities of the Dermatovenereology Department of Bukovinian State Medical University for 75 years of its existence. The authors consider the main scientific achievements of the department in the historical aspect and and over the past 10 years. Research methods: the chronological, historical and and system analysis method. Scientific novelty. For the first time, the directions of the scientific work of the department staff in the historical aspect and the main scientific achievements over the past 10 years are comprehensively highlighted. Conclusions. It is shown that the priority directions of the department scientific work at different historical stages of its existence were scientific research devoted to solving the actual problems of the dermatovenereological morbidity in the region and the country overall - studying aspects of etiopathogenesis and improving the diagnostics and treatment of dermatomycoses, tuberculosis, psoriasis, allergic diseases, lupus erythematosus, acne, rosacea, syphilitic infection. The development of the new improved schemes of diagnostics and treatment of the chronic dermatoses and their implementation into the medical practice, the defence of PhD and doctoral dissertations, helding the scientific conferences, participation with reports at the international forums, the issued monographs, numerous scientific publications, patents, etc. were the results of scientific work, as well as the implemention of the research results into the pedagogical process of the department, which assist in increasing the efficiency of the future medical professionals training at different levels.

Psoriasis is a chronic inflammatory dermatosis, which in addition to the skin, can also and rarely affect the mucous membranes, which may include the oral, genital and ocular mucosa and whose particular surface anatomy may influence the clinical presentation. In this article, we will present the different clinical aspects of psoriasis of the ocular, genital, and especially oral mucosa. Each compartment of the eye may be affected, but the eyelid and conjunctiva are the main sites of involvement, whereas uveitis in the context of psoriasis is a therapeutic emergency. The genitals are very often affected during reverse psoriasis, however, this location is rarely examined systematically. Oral involvement is underestimated, although its prevalence would be much higher if psoriatic patients underwent a thorough oral examination. Indeed, in oral psoriasis, the clinical aspects are divided into two major categories: specific clinical aspects whose lesions have a parallel evolution and a common histology with skin lesions, and nonspecific ones that include the majority of oral involvement in psoriasis such as fissured tongue and geographic tongue. Psoriasis is a chronic inflammatory disease that affects the patients’ quality of life. In psoriatic patients, the quality of life is generally low due to multiple co-morbidities, treatments, stigma and misperceptions about this condition. Sexual dysfunction, in both women and men, is a comorbidity that may impact the quality of life of these patients, in addition to those listed above. Because the recognition of sexual dysfunction by the patient can be difficult and considered shameful, it is necessary for the family doctor to intervene to improve the sexual health of these patients.

12. Treatment of endothelial dysfunction with Rosuvastatin in patients with psoriasis

Introduction. Psoriasis is an immune-mediated inflammatory skin disease. Since the use of therapeutic agents is often hampered by low efficacy and/or adverse effect profile, the presence of comorbid pathology, it is necessary to develop new therapeutic approaches for psoriasis. The objective of the study was to evaluate the clinical and laboratory efficacy of rosuvastatin in the treatment of patients with chronic plaque psoriasis. Material and methods. The study involved 52 patients with mild and moderate psoriasis, aged 45-64 years. The blood serum level of interleukin-6 (IL-6), high-sensitivity C-reactive protein (hs-CRP) and vascular endothelial growth factor (VEGF) were determined by immunoassay analysis. The serum lipid profile was also determined. Patients were divided into two groups, depending on the method of therapy. Patients of the 1st group received a combination therapy – rosuvastatin, topical corticosteroids (TCS) or phototherapy, patients of the 2nd group were prescribed only TCS or phototherapy. Results. The clinical improvement of psoriasis is noted under the influence of combination therapy. The PASI (Psoriatic Area and Severity Index) score decreased in 3 months by 68.2%. A marked decrease in the level of IL-6, hs-CRP after 1 month was noted, less significant VЕGF relative to the results before therapy. Lipid metabolism indices changed reliably only at the end of the 3rd month – the levels of TC and LDL-C were reduced. Conclusions. The use of rosuvastatin in the combination therapy of patients with psoriasis increases the effectiveness of treatment, helps to reduce the level of inflammatory markers, proangiogenic growth factor and normalize blood lipid spectrum.

13. Neutrophil-lymphocyte ratio and platelet-lymphocyte ratio as an alternative to C-reactive protein in diagnostics of inflammatory state in patients with psoriasis

Background: The ratio of neutrophils to lymphocytes (NLR), the ratio of platelets to lymphocytes (PLR), the ratio of monocytes to lymphocytes (MLR), and mean platelet volume (MPV) are considered novel inflammatory markers. We stated a hypothesis that apart from C-reactive protein (CRP) level, also NLR, PLR, MLR, and MPV are higher in patients with psoriatic arthritis (PsA, where both skin and joints are involved) than in patients with psoriasis vulgaris (PsV) Material and Methods: Our study is based on a retrospective analysis. We collected laboratory data, namely CRP levels as well as a total blood count, and calculated the following additional parameters: NLR, PLR, MLR, and MPVLR (mean platelet volume/lymphocyte ratio). For all data, two groups of patients were compared: PsV with N=80 and PsA with N=80. Results: CRP levels were significantly higher in PsA patients compared with those in PsV patients (median PsV – 19.43 nmol/L vs median PsA – 37.90 nmol/L, p=0.001). Similarly, both NLR and PLR were higher in PsA patients than in PsV patients (NLR, median PsV – 2.0 vs median PsA – 2.28, p=0.030; PLR, median PsV – 111.61 vs median PsA – 121.85, p=0.027). Moreover, for patients with psoriatic arthritis a weak to moderate positive correlation between C-reactive protein levels and WBC, neutrophils count, monocytes count, platelets count, NLR, PLR, and MLR was observed. CONCLUSIONS: CRP levels are higher in patients with PsA than in patients with PsV, which can be helpful in predicting arthritis in patients with psoriasis. Furthermore, similar information can be obtained from a blood count. In particular, NLR and PLR are simple predictors which can indicate ongoing joint inflammation in patients with psoriasis, hence they can be used as an alternative to the CRP level.

15. Ulcérations cutanées secondaires à la thérapie par méthotrexate chez un patient suivi pour psoriasis

Patient de 26 ans, suivi pour un psoriasis érythrodermique confirmé histologiquement. Il a bénéficié d´un traitement local et systémique par des injections de méthotrexate (MTX) hebdomadaire à la dose de 0,3mg/Kg/sem en intramusculaire (IM) et a reçu 4 injections au total soit une dose cumulée de 60 mg. Après la 3e injection, il a présenté des ulcérations cutanées douloureuses bien taillées à surface propre intéressant les membres, le tronc et au niveau scrotale. Le bilan biologique a objectivé une hypo-albuminémie à 25g/l. Le reste du bilan était correct. Une pharmacovigilance fut signalée incriminant le méthotrexate. L´évolution fut marquée par une cicatrisation des ulcérations après arrêt du méthotrexate et sous traitement local cicatrisant. Le méthotrexate (MTX) est un antimétabolite qui inhibe la synthèse de l'ADN par inhibition compétitive de la dihydrofolate réductase dans les cellules immunologiquement actives. Sa toxicité cutanée est liée à la dose et survient lorsque les recommandations sont ignorées ou lorsqu'il y a diminution de l'excrétion rénale. Le méthotrexate est partiellement lié à l'albumine sérique, et sa toxicité risque de s'accroître en cas d´hypoalbuminémie responsable ainsi d´un surdosage qui se manifeste par une hépatotoxicité et une toxicité mucocutanée par des érosions douloureuses des plaques psoriasiques signalées comme signe précoce de toxicité au MTX. Notre cas a présenté une toxicité cutanée au MTX malgré le respect des recommandations et des doses cela suggère que le risque de surdosage rencontré chez notre malade était secondaire au taux d´albumine.

Objective: The aim of this study was to describe the epidemiological and clinical profile of erythroderma in hospital setting in Lomé (Togo). Methods: We reviewed the epidemiological and clinical data of adults patients diagnosed with erythroderma in dermatology departments over a 20-year period (1997 through 2016). Results: In total, one hundred and forty seven patients were diagnosed with erythroderma during the study period (0.4% of all consulted patients). The average age at the onset of the disease was 41.1 years with a male: female ratio of 1.5:1. The most frequent cause of erythroderma was exacerbation of preexisting dermatoses (80.8%), including eczema (84.3%) and psoriasis (11.7%). No cause could be identified in forty one cases (27.9%). Apart from erythema and scaling, that were present in all patients, clinical findings were dominated by pruritus (62.33%), fever (8.2%), and altered general condition (8.8%). Treatment was symptomatic and etiologic in all patients. The evolution was favorable in 47.62% of the cases, and one death occurred in a patient with cutaneous lymphoma. Conclusion: This study outlines that erythroderma is rare in Lomé (Togo). Our series had a high percentage of erythroderma secondary to preexisting dermatoses and idiopathic cases. Rarely pemphigus foliaceus could be associated with generalized pustular psoriasis. A 66-year-old woman with diffuse flaccid bullae and erosions of pemphigus foliaceus underwent two sessions of pulse therapy of corticosteroid and oral prednisolone during the interval time. Two weeks after the second session of pulse therapy with improvement of the lesions, during tapered dosage of oral prednisolone, facial annular erythematous lesions appeared and were superimposed by generalized pustular eruptions. Skin biopsy of pustular lesions showed pustular psoriasis so intramuscular methotrexate was added. Two years later, during decreasing dosage of methotrexate and prednisolone, she had eruptive recurrence of pustules and flaccid bullae associated with erythroderma and fever which were controlled with increasing dosage of corticosteroid and starting oral retinoid. These hypotheses may explain co-occurring pemphigus foliaceus and pustular psoriasis: decreasing dosage of corticosteroid, interleukin-8 overproduction in keratinocytes and increased activity of plasminogen activator in skin lesions.

21. The characteristics of severe forms of psoriasis on pigmented skins: A retrospective study of 102 cases in Dakar, Senagal

Background: Severe forms of psoriasis can be life-threatening for patients. Our objective was to determine the epidemiological, clinical, therapeutic aspects and outcome of severe forms of psoriasis. Methodology: A 18 years retrospective study conducted in the department of Dermatology at Le Dantec hospital listed all the severe forms of psoriasis. Results: We have collected 102 cases of severe forms. The sex ratio was 1.55 and the average age of 47 years. The family histories of psoriasis were reported in 5 patients. Arterial hypertension was noted in 10 patients, diabetes in 4 patients and obesity in 6 patients. Smoking and alcoholism were found respectively in 10 cases and 7 cases. The average duration of the lesions was 2 years. Phytotherapy was reported in 62 patients (62%). Pruritus was present in 86 patients (84%). The clinical forms of psoriasis were: erythroderma in 76 cases (74.5%), psoriatic arthritis in 14 cases (13.7%) and a pustular form in 12 cases (11%). HIV serology was positive in 12 patients. All patients had received a treatment with topical corticosteroids and emollients. Methotrexate was initiated in 57 patients and retinoids in 27 cases. The outcome was favorable in 67 patients (65%). We recorded 6 deaths (6%). Conclusion: The large use of oral phytotherapy medicines and inappropriate treatment precipitated or speeded up the occurrence of severe forms of psoriasis among our patients.

Psoriasis is a chronic cutaneous disorder, which is protean in its manifestation. It is a significant source of morbidity and mortality. It is associated with arthropathy as well as disorders such as diabetes mellitus and coronary artery disease. Therapeutic guidelines offer a wide range of topical and systemic treatments, yet a number of patients are often dissatisfied with the treatment of their disease and, in consequence, seek other alternatives. Homeopathy is a gentle, safe method of treatment that would appear to have positive effects in the treatment of psoriasis. Three cases of palmar and plantar psoriasis treated with homeopathy are presented.

Psoriasis is an inflammatory skin disease. Five classic types of psoriasis have been defined: plaque, inverse, pustular, guttate, and erythrodermic. The early psoriasis immunologic skin immune response is not well understood. Here we aim to show an immune and cell signaling response in a case of early psoriasis. A 56 year old female presented with a desquamative lesion on her right leg. A skin biopsy for hematoxylin and eosin (H&E) and immunohistochemistry (IHC) staining was taken. The diagnosis indicated early psoriasis, and IHC showed positive IgD staining in the epidermal corneal layer, as well as positive staining with ribosomal protein S6-pS240 (RIBO) in the hyperproliferative epidermis. Cyclooxygenase-2 (COX-2) was also very positive in the granular layer in spots, at the basement membrane zone of the skin and in the inflammatory infiltrate in the dermis subjacent to hyperproliferative psoriatic areas. In an early case of psoriasis, we confirmed the presence of IgD, RIBO and COX-2. Each molecule seems to be playing a role in inflammation and intracellular signaling pathways in early psoriasis. The role of IgD is unknown, and this case brings to light the complexity of the pathologic changes occurring in early psoriatic lesions.

Plaque-type psoriasis is a major dermatosis with significant effects on quality of life. Case complexity is often high in low-resourced settings such as in Africa where the incidence has been on the rise. Despite major advancements and newer therapeutic modalities over the last decade, an insight into the real-life, day to day challenges in low resourced settings reveal an interplay between the difficulty in obtaining these drugs and use of alternative traditional indigenous agents. We report the case of a 50 year old immunocompetent male who presented with chronic and extensive well demarcated plaques covered with silver-white scales occupying about 61% of his body surface area. Patient was however lost to follow up for about 8 months during which time, the lesions responded to some unknown homemade indigenous medications which was preferred to a systemic medication. Paramount importance on proper counselling and the need to retain patients in care is warranted by physicians and allied health personnel. Also, incentives aimed at subsidizing the newer systemic agents for patients in low resourced cohorts will go a long way to combat this multi-faceted disorder which is often unrecognized and under diagnosed.

- Abstract
Interleukin (IL)-17A and then IL-17F have been discovered through their roles in chronic inflammatory diseases. These cytokines share 50% of sequence homology and bind to the same receptor made of the IL-17RA et IL-17RC chains. While they have rather similar pro-inflammatory effects, slight differences exist depending on the cell type considered or whether there is TNF or not. Indeed, there is a synergistic effect of TNF and IL-17A or IL-17F on many cell types. In addition, the interactions between immune and stromal cells also modulate their effects which vary according to stromal cell subtype. The identification of IL-17A and IL-17F roles in inflammatory diseases, as psoriasis, has led to the development of inhibitors of those cytokines. Anti-IL-17A, then anti-IL-17A/F and now anti-IL-17RA have been approved for different diseases and are particularly efficient in psoriasis. Their use is expending to other diseases like psoriatic arthritis and spondyloarthritis. Introduction: Psoriasis is a chronic systemic disease which often significantly reduces the quality of life in extreme situations can provide to severe depression and even suicide. Indirect self-destructiveness is a generalized trend of behavior consisting of taking steps to increase the likelihood of negative and reduce the likelihood of positive consequences for the entity within a general manifestations such as transgression of norms and risk, addictions, poor health maintenance, personal and social neglect, lack of planfulness, helplessness and passiveness. Polish and world literature has no publications on indirect self-destructiveness in psoriasis nor in any skin diseases. The main aim of this study was to investigate the intensity and symptoms of indirect self-destructiveness in population of patients with psoriasis vulgaris Material and methods: The material consisted of 82 patients with psoriasis vulgaris hospitalized in the Department of Dermatology, Pediatric Dermatology and Oncology in 2013-2014. For the achievement of the objectives of the research socio-demographic questionnaire (own authorship) and Indirect Chronic Self-Destructiveness Scale by Kelley in Polish adaptation of Suchańska (version for men and women – each containing 52 issues) was used. Results: The results showed that in a population of people with psoriasis overall severity of indirect self-destructiveness was in the lower range of the average results. The dominant class of indirect self-destructiveness was helplessness and passivity that preceded the poor health maintenance and lack of planning. Conclusions: The results will enrich the existing knowledge about the harmful conduct of psoriasis and a better approach to the patient.

30. Serum levels of homocystiene, vitamin B12 and folic acid in Indian patients with psoriasis: results of a pilot study

Introduction: Hyperhomocystienemia has emerged an independent risk factor for cardiovascular diseases with an implication for cardiovascular morbidity in psoriasis patients. Both vitamin B12 and folic acid influence homocystiene metabolism as cofactors. Aim: To study the serum levels of homocystiene, vitamin B12 and folic acid in patients with chronic plaque psoriasis and age matched controls. Methods: 55 males aged 22-66 years with chronic plaque psoriasis of 6 months to 20 years and 55 healthy male controls aged 20-65 years were studied. Results: Body surface area involvement was 20% in 7 (12.8%) patients, respectively. The PASI was 12 in 4 (7.3%) patients, respectively. The serum homocystiene levels of >12 µmol/L were higher than normal (5-12 µmol/L) in all patients and 11 (22%) controls and the difference was statistically significant. The serum vitamin B12 levels of 24ng/ml and elevated levels of 17.83 to >24ng/ml (normal 3-17 ng/ml) were noted in 17 (30.9%) patients. Except for elevated serum homocystiene in 11 (22%) controls, other biochemical parameters were within normal range. Conclusions: Implications of hyperhomocystienemia for cardiovascular comorbidities in psoriasis patients and whether supplementing vitamin B12 and folic acid will prevent comorbidities by normalizing homocystiene metabolism needs evaluation by large well designed studies.

Introduction: The alteration of the life quality in psoriasis is currently proved. Aim: To evaluate the particularity of this impact in Moroccan psoriatic population. Methods: It was a prospective cohort of 140 psoriatic patients who filled the 16-Skindex questionnary to evaluate this alteration of the life quality. Results: The life quality was significantly affected in patients having severe or old psoriasis and in young females, also it was related to the low Socioeconomic level and the living in rural areas, to the presence of psoriatic arthritis, to the scalp, the nails and mucosal involvement and to the use of systemic treatments. However, the emotional life, theimpact on daily activities and the sleep quality were not affected in our population. Conclusions: we had a low negative impact on the sleep quality and the emotional life which may be explained by the role of the family support in our society.

Le psoriasis est une pathologie complexe, avec un fort retentissement psychologique et social. La prise en charge se base sur une thérapie médicamenteuse. Du fait de ses différents aspects, le retentissement et la réponse aux traitements peut être diffèrent, et une mauvaise observance, voir un décrochage thérapeutique est souvent constaté. Une réponse globale et personnalisée doit être proposée. Le pharmacien a un rôle à jouer dans le suivi des patients et l’expertise médico-pharmaceutique est de plus en plus sollicitée. Il se doit de maîtriser tous les aspects de cette pathologie afin de conseiller et de diriger au mieux les patients. C’est pourquoi nous avons voulu étudier les thérapies dites complémentaires non médicamenteuses, afin de permettre au pharmacien de tenir au mieux son rôle de proximité et de coordinateur. Nous avons fait l’état des connaissances physiopathologiques et sur la prise en charge médicamenteuse. Nous avons aussi voulu comprendre les raisons pour lesquelles les patients se tournent vers d’autres thérapies et décrochent de leur traitement. Ce qui ressort des études sont : l’échec, l’insatisfaction, les effets secondaires, le manque de suivi ou encore la corticophobie. Les thérapies complémentaires non médicamenteuses étudiées dans cette thèse sont : la photothérapie, le thermalisme, la prise en charge psychologique ainsi que les thérapies naturelles telles que la : phytothérapie, aromathérapie, homéopathie, vitamines et minéraux. En conclusion, les thérapies complémentaires non médicamenteuses n’entendent pas remplacer les thérapies conventionnelles. Elles sont néanmoins un élément incontournable au succès thérapeutique de façon globale.

Psoriasis verrucosa is a rare clinical variant of psoriasis with peculiar histologic features. Only few cases have been reported in the literature. We herein report a rare case of psoriasis with verrucous appearance occurring in a 63 year-old woman who presented with verrucous and scaly erythematous plaque of the legs which was developed thirty years ago. The biopsy specimen showed regular psoriasiform epidermal hyperplasia with acanthosis, hyperkeratosis, and focal spongiosis with a superficial perivascular infiltrate. The patient was diagnosed with verrucous psoriasis. Recognition of this entity should preempt confusion with verruca vulgaris or other entities capable of producing wart-like epidermal changes.

Le psoriasis est une dermatose chronique évoluant par poussées, faite de plaques érythématosquameuses généralement bien limitées. La forme typique, dite psoriasis vulgaire, est habituellement bénigne. Il existe cependant des formes graves, notamment le psoriasis pustuleux, le rhumatisme psoriasique et l'érythrodermie psoriasique. L'érythrodermie est la forme la plus grave et constitue une urgence diagnostique et thérapeutique, pouvant entraîner le décès du patient. Plusieurs facteurs peuvent déclencher l'érythrodermie, en particulier le stress, l'infection, une prise médicamenteuse ou encore l'arrêt d'une corticothérapie générale, par effet rebond. Nous rapportons ici le cas d'un patient de 56 ans présentant une érythrodermie d'origine psoriasique compliquant un psoriasis cutané après prise d'une corticothérapie générale prescrite par un médecin généraliste dans le but de soulager des gonalgies. La découverte des dermocorticoïdes a révolutionné la thérapeutique en dermatologie. Toutefois, leur utilisation sur des durées prolongées ou à des doses fortes est responsable des manifestations secondaires locales et systémiques. Nous rapportons deux observations illustrant ces effets secondaires graves. Observation 1 : une patiente âgée de 43 ans présentait, depuis cinq mois, un érythème facial et des bouffées de chaleur, que la patiente traitait par application quotidienne de dermocorticoïde fort sur le visage. Lors de la décroissance, elle a présenté un tableau grave de rosacée fulminante à double localisation cutanée et oculaire. Observation 2 : un nourrisson de 4 mois, ayant comme antécédents une dermite de siège évoluant depuis deux mois, que la maman traitait quotidiennement par un dermocorticoïde très fort, et qui a présenté des signes d'hypercorticisme avec insuffisance surrénale secondaire au dermocorticoïde. Discussion : plusieurs facteurs ont, chez ces deux patientes, favorisé l'absorption des dermocorticoïdes et, partant, la survenue des effets secondaires. Ceux-là peuvent être locaux ou généraux. Localement, notre première observation a présenté une aggravation sous une forme fulminante, d'une rosacée. Les effets systémiques peuvent survenir lors de l'utilisation prolongée de corticoïdes puissants sur des surfaces importantes ; notre seconde patiente a ainsi développé une insuffisance surrénalienne. Conclusion : les dermocorticoïdes sont une arme à double tranchant. La prévention de leurs effets secondaires passe par le respect des règles de prescription par le médecin et la réglementation de leur délivrance par les pharmaciens. [ABSTRACT FROM AUTHOR]

Introduction: Psoriasis is a chronic immune disorder that multiple factors are involved in its creation. Psoriasis is not curable but a large number of local and systemic treatments for psoriasis are available that can reduce symptoms and recurrence of the disease. Treatment is selected based on severity of illness, associated comorbidities, patient preference[including costs and convenience], and evaluation of the effectiveness of patient response to treatment. Methods: 94 cases included from patients admitted to the Sina dermatology clinic with diagnosis of psoriasis. They were randomly divided in two groups. First group treated with Calcipotriol alone while the second group treated with calcipotriol and NBUVB phototherapy. Results: According to the results of our study the subjects in the two groups in terms of sex, age, weight, height, history of systemic disease, duration of psoriasis, site of involvement and the extent of the lesions were not significantly differ. An average improvement in lesion thickness, erythema and scaling and investment, and improve overall recovery of waste per week in the fourth, eighth and twelfth week after treatment, In the second group were significantly higher than the first group. Conclusion: Daivonex cream in combination with NBUVB phototherapy has a faster and more recovery in a given time. Daivonex with NBUVB phototherapy can be used to provide better results.

Introduction: The correct patient – doctor relationship is important in shaping the whole process of treatment. The scientific studies highlight the various irregularities in this relationship and its negative impact on the effectiveness of medical treatment. The purpose of this study was to assess the relationship between levels of doctors’ support and attitude to certain aspects of the treatment process and quality of life among patients with psoriasis. Material and Methods: The study was conducted on 50 patients with psoriasis aged from 21 to 78 who are treated in dermatological clinics. The Psoriasis Area and Severity Index (PASI) was used to assess the severity of psoriatic skin changes. The patients completed a questionnaire for the assessment of receive doctors’ support, and its relationship with the attitude towards the disease. The research tool was developed based on literature review. Results: The level of doctors’ support had a direct impact on the patients’ attitude the disease, including attitudes towards the treatment and medical personnel, as well as adherence to medical recommendations; and indirectly on satisfaction with the treatment and the quality of life. Conclusions: Results of this study have shown clear evidence the importance of the level of doctors’ support in psoriasis which could help to improve the overall functioning of these patients. The level of doctors’ support indirectly affects the quality of life in patients with psoriasis.

Introduction: Psoriasis is a chronic, inflammatory papulosquamous disease clinically characterized by erythematous, sharply demarcated, indurated papules and rounded plaques covered by silvery, micaceous scales. Psoriasis is characterized by hyperproliferation and abnormal differentiation of epidermal keratinocytes, infiltration of mostly T lymphocytes and various endothelial vascular changes in the dermal layer, such as angiogenesis, dilatation and high endothelial venule (HEV) formation. Material and Methods: The study was conducted on 70 patients of psoriasis attending the outpatient department. Patients who fulfilled the selection criterion were alternately assigned into two groups, 35 patients in each group. The assessment of effectiveness was done with the help of PASI. Follow up of patients were done after 1st, 2nd, 4th and 6th week of initiation of treatment. Results: We observed 70 patients and both the group showed statistically significant reduction in the disease severity. Conclusion: The calcipotriol and mometasone furoate ointment were found to be equally effective in clearance of the disease.

Introduction: Psoriasis is a multi-system chronic inflammatory skin disease targeting 2% to 3% of the general population. It is a prototype of immune dysregulation mediated by TH1 proinflammatory cytokines such as TNF-α, IFN-gamma, IL-6, and IL-12 with far reaching systemic effects. There is growing and emerging evidence that psoriasis patients have a higher prevalence of associated comorbid diseases, with severe skin disease portends a serious risk for development of these comorbidities and are found to have a higher association of extracutaneous disease manifestations. Aim: To look for eye involvement in psoriasis patients and to evaluate the risk and prognostic factors of disease association. Material and Methods: 200 Patients with psoriasis were screened for any eye involvement after few unusual case presentations with eye complications during the period from September 2013 - August 2014. Results: First case was a post cataract sudden loss of vision secondary to development of uveitis in a female patient aged 52 years, with past history of psoriasis with minimal skin lesions and no arthritis. Another 5 cases of psoriasis with eye involvement were detected during the screening employed in a series of 200 psoriasis cases. Conclusion: The present report highlights the importance of psoriasis and eye involvement, need for collaboration between dermatologists and ophthalmologists for thorough examination and evaluation prior to any surgical intervention and also further long term follow-up studies are warranted for confirmation of this causal relationship.

41. Détection échographique infraclinique d’enthésopathies et d'atteintes unguéales chez des patients à risque de rhumatisme psoriasique.

Résumé Objectifs Détecter des anomalies infracliniques des enthèses et des ongles par échographie en échelle de gris ( mode B) et en mode Doppler puissance (DP) en comparant des patients présentant soit un psoriasis unguéal, soit un psoriasis inversé et du cuir chevelu. Méthodes Cette étude prospective monocentrique a inclus des patients atteints de psoriasis unguéal, inversé ou du cuir chevelu ne présentant pas de rhumatisme psoriasique ou ne recevant pas de traitement systémique. Une évaluation clinique et échographique de 14 enthèses et de 12 ongles a été réalisée par un dermatologue (évaluation clinique) et un rhumatologue (échographie). Résultats Cinq cent dix-huit enthèses ont été analysées sans qu’aucune différence statistique ne soit observée entre les deux groupes pour l’enthésopathie en échelle de gris ( p = 0,66). Le signal DP des enthèses était faible (< 1 %) dans les deux groupes. La matrice était significativement plus épaisse en cas d’atteinte unguéale (1,94 mm vs 1,77 mm ; p = 0,007). Le signal DP mesuré dans l’épaisseur de la peau au niveau de l’articulation distale et la perte de la structure trilaminaire ont également été associés de manière significative au psoriasis unguéal ( p = 0,037 et p < 0,0001 respectivement). Conclusion La détection d’enthésopathie infraclinique par échographie est relativement courante dans les deux groupes, contrairement à la détection d’enthésites en mode DP, mais sans que la différence soit statistiquement significative. L’échographie est un outil efficace pour évaluer les composantes de l’ongle psoriasique (perte de la structure trilaminaire, épaississement de l’ongle et inflammation) significativement associées au psoriasis unguéal. Cutaneous adverse events are frequent among patients with inflammatory bowel disease (IBD) treated by anti-Tumor Necrosis Factor (TNF)-α agents, concerning 20% of patients. Infections represent half of skin complications, immune-mediated lesions 40% and, neoplastic lesions and local reactions the remaining percentage. Infections that are mostly caused by bacteria or herpes virus reactivation can usually be treated without anti-TNFa withdrawal. Psoriasis and eczema lesions have typical locations, with a palmo-plantar involvement, folds and scalp eruptions. They could be treated in most of cases with topical steroids and vitamin D. Few patients have to stop anti-TNFα and switch to drug with another mode of action. Dermatologic surveillance is now yearly recommended in IBD patients. [ABSTRACT FROM AUTHOR]

- Abstract
Résumé L’inflammation chronique est un phénomène présent dans de nombreuses pathologies telles que la polyarthrite rhumatoïde, le psoriasis ou les maladies inflammatoires chroniques de l’intestin. Les cytokines pro-inflammatoires sont largement impliquées dans ces pathologies et agissent à différents niveaux : local, amplifiant la réaction inflammatoire par une influence sur la néo-vascularisation, l’hyperprolifération cellulaire et la dégradation tissulaire, et systémique, augmentant les risques cardiovasculaires ou même le risque de dépression. Cette revue s’intéresse principalement à quatre cytokines majeures, le TNF, l’IL-6, l’IL-1 et l’IL-17, décrivant leur fonctionnement, leur rôle et leur ciblage thérapeutique. En effet, ces cytokines sont devenues des cibles importantes pour de nouvelles thérapies. Si certaines biothérapies comme les anti-TNF sont couramment utilisées dans le traitement des maladies inflammatoires chroniques, d’autres sont encore en cours d’étude ou de développement. Le ciblage de ces cytokines permet une amélioration de la prise en charge des patients même si une grande hétérogénéité reste présente entre les patients, influant la réponse aux traitements. Il est donc crucial de continuer à développer de nouvelles stratégies, telles que les biomarqueurs, afin de pouvoir fournir une médication plus personnalisée et accompagner au mieux les patients souffrant de maladies inflammatoires chroniques. Chronic inflammation is a phenomenon present in several pathologies such as rheumatoid arthritis, psoriasis or inflammatory bowel diseases. Pro-inflammatory cytokines are largely involved in these pathologies and act at different levels: local, amplifying the inflammatory response by influencing neo-vascularization, cell hyperproliferation or tissue degradation, and systemic, increasing cardiovascular or depression risks. This review focuses on four major cytokines, TNF, IL-6, IL-1 and IL-17, describing their properties, their key role and their therapeutic targeting. Indeed, these cytokines became critical targets for new therapies. If some biotherapies as anti-TNF are currently used in chronic inflammatory diseases, others are still in study or under development. The cytokine targeting allows an improvement of the patient management even if a great heterogeneity remains present between patients, influencing the response to the treatments. Therefore, it is crucial to continue to develop new strategies, such as biomarkers, in order to be able to provide more personalized medication and better support patients with chronic inflammatory diseases. 44. Hidradenite suppurative et psoriasis: anguille sous roche

L'hidradénite suppurative et le psoriasis sont des maladies inflammatoires chroniques. La première siège préférentiellement au niveau des zones riches en glande apocrine, la deuxième est ubiquitaire. L'association des deux pathologies n'a jamais été décrite dans la littérature. Cette association cachait une infection à HIV évolutive. Mr MB, âgé de 45 ans, tabagique chronique depuis 10ans, était hospitalisé pour la prise en charge de nodules durs douloureux érythémateux fistulisés des régions axillaires et inguinales apparus 6 mois avant avec des lésions érythémato-squameuses des plantes (A). Les nodules ont augmenté de taille et ont conflué en placard avec apparition secondaire de fistules laissant sourdre de pus jaune nauséabond en grande abondance (B). L'état général était conservé et l'examen somatique ne relevait pas d'anomalie. Le patient a bénéficié de deux biopsies une au niveau axillaire et l'autre au niveau de la plante confirmant les diagnostics de maladie de Verneuil sur la première et de psoriasis sur la deuxième. Une sérologie HIV a été demandée, fortement positive ce qui a été confirmé par le western blot avec taux CD4=500/mm3. Les présentations atypiques des dermatoses et les associations inhabituelles des dermatoses ayant des mécanismes physiopathologiques très différents doivent faire suspecter une infection rétrovirale. L'association de psoriasis et de l'hidradenite suppurative n'a jamais été décrite même au cours de l'infection HIV. L'infection VIH peut occasionner une multitude de dermatoses de présentation atypique. Le dermatologue doit être vigilant pour permettre d'établir un diagnostic précoce de l'infection.

45. Unique psoriatic lesion versus multiple lesions

Aim: To evaluate the number of lesions of psoriasis and to find risk factors for multiple lesions. Material and Methods: 1,236 patients (male 54.13%, female 45.87%) with psoriasis were seen over a period of 8 years in an Outpatient Clinic. Patients filled out questionnaires containing age at onset, number of lesions and location at the beginning of the disease, gender, type and localization of psoriasis at the time of clinical examination, psoriasis family history, previous treatment, comorbidities, and social status. Results: The number of psoriasis lesions correlates with: onset age of psoriasis (F=8.902, p=0.0029); age at the moment of clinical examination (F=8.902, p=0.0029); residence in rural area (χ2=8.589, p=0.00338, 95%CI); alcohol intake (χ2=16.47, p=0.00005, 95%CI); smoking (χ2=8.408, p=0.00373, 95%CI); occupation: workers/pupils/students (χ2=14.11, p=0.0069, 95%CI). Conclusions: There is a correlation between number of psoriatic lesions and some factors. Multiple lesions were observed in older patients, smokers and drinkers, coming from rural area and social active (workers and pupils/students). No correlation was statistically proved between number of lesions and gender, comorbidities and family history of psoriasis.

Female patient, 35 years old who came to the private office due to discoloration of her left thumbnail and little pain since 1 month ago. Clinical examination shows nail disease on her left thumbnail with onycholysis and dyschromia, dermatoscopy showed white-yellowish discoloration (Fig. 1A, B). The rest of the clinical examination was normal. Patient use to using acrylic nails since 2 years ago and denied some trauma at the nail. The diagnosis of onycholysis due to trauma was done and recommended her not to use acrylic nail, maintain the nail short and avoid wetness.

Introduction: Peroxisome proliferators- activated receptors (PPARs) represent a major research target for the understanding and treatment of many skin diseases, such as benign epidermal tumors, psoriasis and atopic dermatitis. 48. A study of clinical and biochemical correlation in patients of psoriasis in acute exacerbation

Introduction: Psoriasis is a genetically determined chronic disease of skin in which a number of biochemical alterations take place during its course. The main objectives of our study were to calculate various biochemical parameters in psoriasis correlate them clinically. Materials and Methods: An attempt was made to detect the biochemical changes during its exacerbations. Fifty biopsy proved psoriasis patients were selected. Clinical examination and the various biochemical tests were performed at the time of enrollment and at the end of eight weeks of treatment. Results: Hyperurecemia was seen in 16% patients and hypocalcemia was seen in 10 % patients and hypoalbuminaemia was seen in 56% patients. Conclusion: With improvement of clinical picture, a shift of biochemical values towards normal was recorded.

Introduction: Psoriasis is a common chronic, non-contagious, immune mediated disease. Plaque psoriasis is characterized by thick, white, silvery or red patches on the skin. Plaque psoriasis mainly affects scalp, and palms of hands and soles of feet. T-cells are a type of immune system cells which protect the body from foreign substances. But during psoriasis T-cells activated at a great rate than normal, in turn release of cytokines increases as a result skin cells growing too quickly, moving to the surface to the skin and appear on the skin as dead cells. Materials and Methods: Plaque psoriasis is common form of psoriasis, which mainly affects palms of hands and soles of feet and scalp. Out of 76 patients with plaque psoriasis we selected 50 patients based on PASI scores. Within the 50 patients, 32 patients with Palmoplantar psoriasis and 18 patients with scalp psoriasis. All 50 patients were subjected to randomized control trail, so each type of psoriasis is divided into two groups i.e. A and B. All A group patients were treated with MTX, whereas all B group patients were treated with combination of MTX and Calcipotriol. All necessary investigations like complete blood count, liver enzyme,creatinine, urea, alkaline phosphate, PASI were measured at 0, 2,4 6,8 weeks interval. Results: We observed that, a significant reduction in PASI is observed with combination of MTX and Calcipotriol as compared to Methotrexate alone. Conclusion: From the above study we concluded that, combination of MTX and Calcipotriolis more effective than MTX alone in plaque psoriasis.

Introduction: Psoriasis is a common chronic inflammatory, recurrent, immune mediated disease of the skin and joints. Toll-like receptors are pattern recognition receptors for conserved molecular patterns of pathogenic microorganisms.Under certain circumstances, self nucleic acids can trigger TLR 7 and TLR 9, which can lead to autoimmune diseases such as psoriasis. Materials and Methods: The study included 15 psoriatic patients (plaque type) and 15 controls, patients received 36 sessions of phototherapy (NB-UVB). Skin biopsies were taken from all the patients (before & after NB-UVB) and controls and were assessed for TLR 7 and TLR 9 by PCR. Results: Showed significant difference between patients and controls as regards TLR 7 and TLR 9. In addition a significant decrease in thier levels in patients after phototherapy with NB-UVB. Conclusion: TLR 7 and TLR 9 may play a role in the pathogenesis of poriasis. Decrease in their levels after NB-UVB may be one of the therapeutic mechanisms of NB-UVB in psoriasis.
