CHILDREN with cerebral palsy , *PARENTS of children with disabilities , *MOVEMENT disorders in children , *LONGITUDINAL method , *PSYCHOLOGICAL well-being , *PARENT-child relationships - Abstract
Summary: Cerebral palsy is the commonest cause of motor impairment in childhood. Parents of children with this particular neurodevelopmental disorder face many problems encountered by disabled children''s parents. The aim of the present paper is to report the current knowledge on this parental impact, highlighting consensus and disagreement. A literature search was conducted using the key words “Cerebral palsy” and “Parents/Father/Mother” and “Adapt/Adjust/Cost/Economic/Impact/Well-being” in the Medline and PsycInfo databases searching for articles published between 1989 and 2009. Seven parental impact dimensions were distinguished: time spent, occupational restrictions, social relationships, family relationships, psychological well-being, physical health, and financial burden. Of 40 selected references, the studies were mostly cross-sectional, although longitudinal surveys highlighted the causal relationship between factors. Despite various methodologies, this review confirms that parents of CP children have greater risk of experiencing a sense of burden than parents of typically normally developing children. Time spent caring for the child appears to be an important factor that depends on the child''s autonomy. The 7 impact dimensions seem to be related to each other and to child''s and caregiver''s characteristics. The severity of motor impairment is not unanimously viewed as a worsening factor: however, the child''s behavioral problems influence the impact experienced by the parents. The level of intellectual impairment also has a negative influence on family relationships and on the parent''s psychological well-being. The child''s developmental stage seems to be related to the level of parental impact, but there is no agreement on the dimensions involved. We also observed that the mother and father do not experience this situation in the same way, probably because of the role played by each one in the family. The current literature lacks data on caregiver characteristics, identifying families at risk of burden, and the environmental context that would allow for a less negative impact on parents. In addition, the tools measuring the impact lack standardization. No questionnaire covering all 7 dimensions exists, but useful validated questionnaires for different dimensions were identified. We consider that the caregiver''s occupation and physical health needs further research. The current knowledge is insufficient for proposing an overall model taking all the dimensions into account. Research is needed before a complete model of the CP child''s impact on parents can be tested in view of providing guidelines to professionals for identifying families with a risk of maladaptation and suggesting solutions to decrease the negative impact. Abstract: Background: The aim of this paper is to describe the major results of two prospective studies on auditory hallucinations in prépubertal children and the case reports of two children followed three years after inclusion. Methods: Samples of out patients aged five to twelve years old were consecutively recruited over two differents period and followed subsequently twelve and six months. K-SADS interview was conducted to determine DSM-IV diagnoses and all children were interviewed using a questionnaire investigating auditory hallucinations. Results: 151 patients were recruited. Twenty-eight out of the 151 reported auditory hallucinations. For 13 children supportive therapy facilited the children’s full recovery from hallucinations within three months and all of these suffered from separation anxiety. For six children hallucinations persisted over the study period and five of these showed conduct disorders. Out of these six children, two presented a diagnostic of schizophrenia and/or schizophrenic affective disorder at age 14 and 15. Two cases followed three years after inclusions and from the two followed groups are presented and discuss. Conclusions: Our study provides further evidence of the high prevalence of auditory hallucinations in pre-pubertal children. In the majority of cases hallucinations seem non specific and may be a manifestation of anxiety. Although for four children herald the development of a diagnosis of the spectrum of schizophrenic disorders. [Copyright &y& Elsevier]

The increase in available student placements at colleges and universities, the implementation of provincial and federal postsecondary education policies, and the rise of the educational aspirations of families and individuals have all led to the massification of Canadian higher education. Based on Merle's typology of the forms of democratization, this article attempts to revisit the theory of equality of opportunities by critically analyzing the link between massification of higher education and social equity. The results of an analysis of longitudinal data from the (YIT) Youth in Transition Survey show that at the age of 24 in 2008, approximately 77% of young Canadians have pursued studies in a college or university. If access to postsecondary education is now higher, to what extent has it improved social equity? The article shows, in light of the Merle's typology, that mass university education is achieved in part under the seal of a segregative democratization, while college education tends to be egalitarian.
