Context: Build decision support tools for clinicians first requires to select the relevant variables to model the clinical decision. For this, two sources of information can be used: the observed data collected in electronic health records, and the expertise of physicians. Few methods can combine these two types of information. The originality of my thesis is to propose strategies to take into account these two sources of data. Our use case focuses on modeling the medical decision to modify Irinotecan doses based on patient characteristics and adverse effects in the treatment of metastatic colorectal cancer. Methods: The methods developed are based on Bayesian variable selection methods in which the expertise is integrated in the form of weights associated with each variable and elicited by the experts. The first model is an adaptation of a Bayesian variable selection method, the Stochastic Search Variable Selection (SSVS) method, for which expert information is introduced into the prior distribution of the variable selection indicator. The model was called Weights-Based SSVS (WBS). The second model is based on the power prior method (PP) and allows combining simulated data from expert parameters and observations. Variable selection is also provided by an SSVS model. We performed simulations to analyze the performances of the first model compared to the LASSO and SSVS models' ones. For the use case, the care data of the patients who received a protocol including Irinotecan at a theoretical dose of 180 mg/m2 over a cycle of a theoretical duration of 14 days were extracted from the electronic health record of the Georges Pompidou European Hospital in Paris. To take into account the clinical expertise, we built a questionnaire in collaboration with the oncologists in which they quantify the importance of each patient characteristic and each grade of each toxicity type on dose reduction through weights taking their values between 0 and 100. Results: The WBS model performs better than models based only on observed data. However, these performances depend on the weights elicited by the clinicians as well as their sum, which must therefore be carefully calibrated according to the number of variables that one wishes to select. The model based on the PP gives similar performances to the SSVS model including only patient data actually observed. However, it does not select the same variables and allows to exclude negligible variables from the expert point of view while revealing those which, important but rare in the data, could be omitted. Regarding the use case, the weights elicited by the experts are very variable and our approaches select clinically relevant variables. Conclusion: This thesis introduces two Bayesian methods of variable selection combining elicited expert information and observed data. The first method improves predictive performance on small samples. The second method allows us to manage larger samples, and automatically select both important variables for experts and variables used in real life. The differences in the choice of weights elicited by oncologists show a certain variability of professional practices. The modeling of medical decision-making is a first step in modeling the complex relationship that links patient characteristics, chemotherapy doses, toxicities, and survival., Contexte : La construction d’outils d’aide à la décision à destination des cliniciens nécessite de sélectionner, au préalable, les variables pertinentes pour modéliser la décision clinique. Pour cela, deux sources d’information peuvent être utilisées : les données observées recueillies dans les dossiers informatisés des patients, et l'expertise des médecins. Peu de méthodes permettent de combiner ces deux types d’information. L'originalité de ma thèse est donc de proposer des stratégies pour prendre en compte ces deux sources de données. Notre application porte sur la modélisation de la décision médicale de modification des doses d’Irinotecan en fonction des caractéristiques des patients et des effets indésirables dans le traitement du cancer colorectal métastatique. Méthodes : Les méthodes développées reposent sur des méthodes de sélection de variables bayésiennes dans lesquelles l’expertise est intégrée sous forme de poids associées à chaque variable et élicités par les experts. Le premier modèle est une adaptation d'une méthode de sélection de variables bayésienne, la méthode de Stochastic Search Variable Selection (SSVS), pour laquelle l'information experte est introduite dans la distribution a priori de l’indicatrice de sélection de variable. Le modèle a été appelé Weights-Based SSVS (WBS). Le deuxième modèle est fondé sur la méthode du power prior (PP) et permet de combiner des données simulées à partir des paramètres experts et les observations. La sélection de variables est elle aussi assurée par un modèle SSVS. Nous avons réalisé des simulations afin d’analyser les performances du premier modèle par rapport à celles des modèles LASSO et SSVS. Pour le cas d’usage, les données de soin des patients ayant reçu un protocole incluant de l’Irinotecan à une dose théorique de 180 mg/m2 sur un cycle d’une durée théorique de 14 jours ont été extraites des dossiers patients informatisés de l’Hôpital Européen Georges Pompidou à Paris. Pour prendre en compte l'expertise clinique, nous avons construit un questionnaire en collaboration avec les oncologues dans lequel ils quantifient l'importance de chaque caractéristique des patients et de chaque grade de chaque type de toxicités sur la réduction de dose par l’intermédiaire de poids élicités prenant leurs valeurs entre 0 et 100. Résultats : Le modèle WBS présente de meilleures performances que les modèles uniquement basés sur les données observées. Cependant, ces performances dépendent des poids élicités par les cliniciens ainsi que de leur somme, qui doit donc être minutieusement calibrée en fonction du nombre de variables que l’on souhaite sélectionner. Le modèle basé sur le PP donne lui des performances similaires au modèle SSVS n’incluant que des données patient réellement observées. Toutefois, il ne sélectionne pas les mêmes variables et permet d’écarter des variables négligeables du point de vue expert tout en révélant celles qui, importantes mais peu fluctuantes dans les données, pourraient être omises. Concernant le cas d’usage, les poids élicités par les experts sont très variables et nos approches sélectionnent des variables pertinentes sur le plan clinique. Conclusion : Cette thèse introduit deux méthodes Bayésiennes de sélection de variables combinant information experte élicitée et données observées. La première méthode permet d'améliorer les performances prédictives sur des petits échantillons. La deuxième méthode permet de gérer des échantillons de plus grande taille, et de sélectionner automatiquement à la fois les variables importantes pour les experts et les variables utilisées en vie réelle. Les différences dans le choix des poids élicités par les oncologues montrent une certaine variabilité des pratiques professionnelles. La modélisation de la prise de décision médicale constitue une première étape dans la modélisation de la relation complexe qui lie caractéristiques du patients, doses de chimiothérapie, toxicités et survie.

Electronic health records in hospitals contribute to improving the quality of care by enabling better management of clinical information. The databases thus constituted facilitate the exchange of health information with healthcare providers and optimize multidisciplinary coordination for better therapeutic results. The EHR4CR (Electronic Health Records for Clinical Research) European project has developed an innovative pilot platform enabling the reuse of this digital information for clinical research. By enhancing and speeding up clinical research procedures, this innovative approach makes it possible to conduct clinical trials more efficiently, faster, and more economically.

Often described as a tool to build trust among stakeholders with divergent interests, blockchain technology has been of interest to many sectors since it was first used in 2008. Initially designed to record financial transactions between individuals, its applications have largely evolved with technological advances and the growing interest of international companies. In the healthcare sector, blockchain is interesting for many of its features: its immutability which makes it an excellent support for authenticating sensitive data such as clinical trials consents, the possibility of publishing smart contracts that automate and facilitate many processes or the constitution of a network that agrees on the state of the information. Much acclaimed, blockchain technology is still to be tested in real-life conditions and adapted to a particularly complex regulatory and economic context in the healthcare sector.

Over the past few years, numerous medical digital initiatives have blossomed, displaying tangible signs of efficacy in improving, for example, medication adherence or lifestyle. Such patient-centered solutions free themselves, at least conceptually, from the silos between the major players in healthcare (pharmaceutical industry, health authorities, hospitals, payers). The lack of a global rethinking of patient care has resulted in structural fragility. This could provide fertile ground for the arrival of players from the digital world, called "pure players", who could radically rethink and disrupt business models by proposing personalized digital solutions based on patients' needs. Thus, in the management of chronic disease, such as cardiovascular disease or type 2 diabetes, "pure players" could bring about a paradigm shift via a commitment to achieve results which are driven by real-world outcome assessment rather than being means-driven.

A paradigm shift seems to emerge, not only in industrial engineering ("Industry 4.0") but also in medicine: we are on the threshold to "Medicine 4.0". For many years, molecular biology had a leading position in life sciences, but today scientists start realizing that microelectronic systems, due to an increasing miniaturization, are reaching the scale of human cells and consequently can be used for therapeutic approaches. This article shows how microelectronics can play a major role in modern medicine, through the example of customized chemotherapy. This consists in determining, before the beginning of the treatment, what kind of chemotherapy or drug combination will be most effective for a given patient, and at which dose. This of course allows the lessening of a patient burden during treatment, but also to be more efficient and, in the long run, to save money. In order to do this, we have developed the Intelligent Microplate Reader (IMR), which allows us to accurately test different drugs on living cells by mimicking part of their usual environment.

Healthcare is considered as one of the most promising areas where big data can be applied to foster innovation for the benefit of patients and of the whole system. Healthcare analytics have the potential to accelerate RD, increase knowledge on diseases and risk factors, improve treatments, develop personalised medicine, help physicians with decision support systems… The access to data is also a driving force for patients' empowerment and for the democratic debate. However, there are also concerns about the societal, economic and ethic impacts of this wave of digitization and of the growing use of data, algorithms and artificial intelligence. Given the issues at stake, collecting and analysing data generated by health care systems is a strategic challenge in all countries; in that respect the French National System of Health Data (a national data warehouse linking data from several sources and giving a vision of the care pathways for the entire population, with a ten-year history) is an asset, but it has to be completed and enriched with data from electronic health records.

This thesis presents a pharmacoepidemiology approach to describe drug utilization in lymphomas during their whole care pathway. The use of the French claims database (Système National d'Informations Inter-Régimes de l'Assurance Maladie (SNIIRAM)) allows to exhaustively quantify this drug utilization in real life conditions. This thesis consists of three mains objectives. First, we aimed to develop validated algorithms for the identification of incident cases of lymphoma. For the validation, we used data from a regional Cancer Registry as the gold standard. The purpose of this validation study was to enhance validity of future studies conducted on lymphomas in the SNIIRAM database. The results of this study associated to strengths of this database demonstrate that this approach is of great interest to conduct pharmacoepidemiological or medico-economic studies in lymphomas. Second, we aimed to estimate the incidence of use of psychotropic drugs during the active treatment phase of lymphoma in comparison with control groups. Indeed, the increased probability of developing anxio-depressive disorders after diagnosis could lead to an increased initiation of psychotropic drugs and a potential inappropriate chronic use of these drugs after initiation. Such inappropriate chronic use can unnecessarily expose patients to adverse event. For this aim, we conducted a new-user cohort study using data from the SNIIRAM database. The results of this study revealed that the initiation rate of these drugs is higher than in the general population or for life-threatening diseases such as myocardial infarction. Moreover, we observed an inappropriate prolonged use for a significant fraction of patients (20% to more than 50% according to therapeutic class). On the basis of these findings, the third objective was to identify factors associated with prolonged use of these drugs during survivorship. [...], Ces travaux de thèse présentent une approche de pharmaco-épidémiologie explorant l'exposition médicamenteuse chez des patients atteints de lymphome tout au long de leur parcours de soin. L'utilisation des données du Système National d'Information Inter-régimes de l'Assurance Maladie (SNIIRAM) nous a permis de quantifier cette exposition médicamenteuse et de déterminer les facteurs associés dans le contexte de la vie réelle. Les travaux de recherche réalisés au cours de cette thèse répondaient à 3 objectifs. Dans un premier objectif, nous avons souhaité valider l'utilisation du SNIIRAM à des fins de recherche pour améliorer la robustesse des futures études menées sur le lymphome. Après avoir défini plusieurs algorithmes d'identification des cas incidents de lymphome, nous avons testé leur validité par confrontation aux données cliniques exhaustives du Registre des cancers du Tarn. Les paramètres de performance obtenus permettent de considérer les données disponibles dans le SNIIRAM comme un outil puissant pour mener des études pharmaco-épidémiologiques ou médico-économiques sur le Lymphome. Le second objectif était d'explorer l'existence d'une surconsommation de médicaments psychotropes au cours de la phase active de traitement par rapport à une population témoin, sous l'hypothèse d'une initiation accrue de ces médicaments pour pallier les complications psychologiques associées à la prise en charge du Lymphome. Par ailleurs, la chronicisation fréquemment observée de ce type de consommation peut conduire à terme à des complications potentiellement évitables. Pour répondre à cet objectif, nous avons réalisé une étude selon une approche " new-user design ", à partir d'une cohorte de patients incidents de lymphome identifiés dans les données du SNIIRAM à l'échelon régional. Nous avons observé un taux d'initiation supérieur au taux observé en population générale ou chez des patients atteints d'autres pathologies mettant en jeu le pronostic vital (infarctus du myocarde). En fonction de la classe thérapeutique étudiée, 20 à plus de 50% des patients restaient exposés de façon inappropriée (au-delà des recommandations) à ces médicaments. Le troisième objectif a donc été d'explorer les déterminants associés à une utilisation prolongée de médicaments psychotropes dans la phase de l'après-cancer à partir des données de l'Echantillon généraliste des bénéficiaires (EGB). The aim of this thesis is part of the broad issue of information retrieval in Electronic Health Records (EHRs). The aspects tackled in this topic are numerous : on the one hand clinomics information retrieval within EHRs and secondly information retrieval within unstructured data from EHRs. As a first step, one of the objectives is to integrate in EHRs information beyond the scope of medicine to integrate data, information and knowledge from molecular biology ; omic data from genomics, proteomics or metabolomics. The integration of this type of data improves health information systems, their interoperability and the processing and exploitation of data for clinical purposes. An important challenge is to ensure the integration of heterogeneous data, through research on conceptual models of data, ontology and terminology servers, and semantic data warehouses. The integration of this data and their interpretation into a conceptual data model is an important challenge. Finally, it is important to integrate clinical research and fundamental research in order to ensure continuity of knowledge between research and clinical practice and to understand personalized medicine challenges. This thesis thus leads to the design and development of a generic model of omics data exploited in a prototype application for information retrieval and visualization in omic and clinical data within a sample of 2,000 patients. The second objective of this thesis is the multi-terminological indexing of medical documents through the development of the Extracting Concepts with Multiple Terminologies tool (ECMT). It uses terminologies embedded in the Health Terminology/Ontology Portal (HeTOP) to identify concepts in unstructured documents. From a document written by a human, and therefore potentially showing typing errors, spelling or grammar mistakes, the challenge is to identify concepts and thus structure the information contained in the text. In health information retrieval, indexing is of great interest for information retrieval in unstructured documents, such as reports and medical notes. This thesis proposes several methods and their evaluation along two axes : the indexing of medical texts using several terminologies and the processing of natural language in narrative medical notes., Les objectifs de cette thèse s’inscrivent dans la large problématique de recherche d’information dans les données issues du Dossier Patient Informatisé (DPI). Les aspects abordés dans cette problématique sont multiples : d’une part la mise en oeuvre d’une recherche d’information clinomique au sein du DPI et d’autre part la recherche d’information au sein de données non structurées issues du DPI. Dans un premier temps, l’un des objectifs de cette thèse est d’intégrer au sein du DPI des informations dépassant le cadre de la médecine pour intégrer des données, informations et connaissances provenant de la biologie moléculaire ; les données omiques, issues de la génomique, protéomique ou encore métabolomique. L’intégration de ce type de données permet d’améliorer les systèmes d’information en santé, leur interopérabilité ainsi que le traitement et l’exploitation des données à des fins cliniques. Un enjeu important est d’assurer l’intégration de données hétérogènes, grâce à des recherches sur les modèles conceptuels de données, sur les ontologies et serveurs terminologiques et sur les entrepôts sémantiques. L’intégration de ces données et leur interprétation selon un même modèle de données conceptuel sont un verrou important. Enfin, il est important d’intégrer recherche clinique et recherche fondamentale afin d’assurer une continuité des connaissances entre recherche et pratique clinique et afin d’appréhender la problématique de personnalisation des soins. Cette thèse aboutit ainsi à la conception et au développement d’un modèle générique des données omiques exploité dans une application prototype de recherche et visualisation dans les données omiques et cliniques d’un échantillon de 2 000 patients. Le second objectif de ma thèse est l’indexation multi terminologique de documents médicaux à travers le développement de l’outil Extracteur de Concepts Multi-Terminologique (ECMT). Il exploite les terminologies intégrées au portail terminologique Health Terminology/Ontology Portal (HeTOP) pour identifier des concepts dans des documents non structurés. Ainsi, à partir d’un document rédigé par un humain, et donc porteur potentiellement d’erreurs de frappe, d’orthographe ou de grammaire,l’enjeu est d’identifier des concepts et ainsi structurer l’information contenue dans le document. Pour la recherche d’information médicale, l’indexation présente un intérêt incontournable pour la recherche dans les documents non structurés, comme lescomptes-rendus de séjour ou d’examens. Cette thèse propose plusieurs méthodes et leur évaluation suivant deux axes : l’indexation de textes médicaux à l’aide de plusieurs terminologies et le traitement du langage naturel dans les textes médicaux narratifs.

This article proposes a brief inquiry into the field of scientific and medical practices currently referred to as "personalized medicine". Our inquiry identifies four recurring themes in the literature: health care that is tailored to the individual patient, that is enabled by emerging technologies, in which genetics and genomics occupy a prominent place, and which requires the collection of a massive amount of data. Personalized medicine, thus characterized, turns out to be less interested in the uniqueness of each patient's case than in the differences among patients within the same category. The aim of personalized medicine, thus described, is to obtain, with the help of cutting edge technology, more objective biological data on patients, in an attempt to improve the tools it has at its disposal to establish diagnoses, make therapeutic decisions, and provide more effective preventive measures.

Electronic health records generate a large amount of data that can be used for non-clinical purposes. We propose two innovative ways of reusing these clinical data. An update of the computerized medical observation was performed after the analysis of its previous use during routine clinical practice. The deletion of rarely used items was discussed. Terminological analysis of guidelines and free-text answers from the clinical record identified clinically important concepts to be incorporated in the structured part of the record. The evaluation of practice individualization requires a representation of cases with a formal conceptual model. The construction of this ontology was based on the same terminological analysis as above. It underscored concepts pertaining to standardized management and others related to patient centered care. The data have also been reused to perform retrospective observational studies covering the main types of clinical or epidemiological research questions., Le dossier patient informatisé génère une grande quantité de données susceptibles d'être réutilisées à d'autres fins que les soins. Nous proposons deux modalités innovantes de réutilisation de ces données cliniques. Une actualisation de l'observation médicale informatisée s'est appuyée sur l'analyse de son utilisation en pratique courante. La suppression des items très rarement utilisés a été discutée. L'analyse terminologique de guides pour la pratique clinique et des réponses en texte libre de l'observation a identifié des notions cliniquement pertinentes susceptibles d'être incorporées dans la partie structurée de l'observation. L'évaluation de l'individualisation des pratiques requiert une représentation des cas à l'aide d'un modèle conceptuel formel. La construction de cette ontologie a reposé sur la même analyse terminologique que précédemment. Elle a mis en évidence des concepts relatifs au versant standardisé de la prise en charge et d'autres relatifs à la prise en charge individualisée. Les données ont par ailleurs été mises à profit pour réaliser des études observationnelle rétrospectives couvrant les principaux types de questions en épidémiologie et recherche clinique.

To determine if using a combination of hospital administrative data and ambulatory care physician billings can accurately identify patients with congestive heart failure (CHF), we tested 9 algorithms for identifying individuals with CHF from administrative data.The validation cohort against which the 9 algorithms were tested combined data from a random sample of adult patients from EMRALD, an electronic medical record database of primary care physicians in Ontario, Canada, and data collected in 2004/05 from a random sample of primary care patients for a study of hypertension. Algorithms were evaluated on sensitivity, specificity, positive predictive value, area under the curve on the ROC graph and the combination of likelihood ratio positive and negative.We found that that one hospital record or one physician billing followed by a second record from either source within one year had the best result, with a sensitivity of 84.8% and a specificity of 97.0%.Population prevalence of CHF can be accurately measured using combined administrative data from hospitalization and ambulatory care.Repérage des cas d’insuffisance cardiaque congestive à partir de données administratives : étude de validation utilisant des dossiers de patients en soins primaires.En vue de déterminer si l’utilisation conjuguée des données administratives hospitalières et des factures présentées par les médecins au titre des soins ambulatoires pouvait permettre de repérer avec exactitude les patients souffrant d’insuffisance cardiaque congestive (ICC), nous avons mis à l’essai neuf algorithmes pour repérer à partir de données administratives les personnes souffrant d’ICC.La cohorte de validation par rapport à laquelle les essais ont été effectués combinait des données provenant d’un échantillon aléatoire de patients adultes inscrits dans la base de données EMRALD de dossiers médicaux électroniques des médecins de premier recours en Ontario, au Canada, et des données recueillies en 2004-2005 à partir d’un échantillon aléatoire de patients en soins primaires pour une étude sur l’hypertension. On a évalué la sensibilité, la spécificité, la valeur prédictive positive, l’aire sous la courbe ROC et la combinaison des rapports de vraisemblance positif et négatif des algorithmes.Nous avons constaté qu’un dossier d’hospitalisation ou de facturation de médecin suivi d’un second dossier provenant de l’une ou l’autre de ces sources dans la même année produisait les meilleurs résultats, avec une sensibilité de 84,8 % et une spécificité de 97,0 %.Nous concluons que la prévalence de l’ICC dans la population peut être mesurée avec exactitude à partir de données administratives issues de l’hospitalisation et des soins ambulatoires.

The development of digital technologies in all activities sectors of our society leads to a growing number of digital documents. A significant part of these documents needs to be durably preserved. This long term preservation has to face the short life expectancy of the technologies and the digital storage media. Large national organizations have already take this problem into account and set up teams, skills and means to face this challenge. At the opposite, the small structures, doctor's offices, individuals, students, etc. are not generally aware of the problem or are stripped to face there. A certain number of simple actions, not requiring specific skills in data processing can nevertheless be undertaken. Without important expenditure, they increase to a significant degree, the security level of the documents over the long term.
