Résumé Contexte : À partir de la mi-mars 2020, la plupart des dentistes en pratique libérale se sont retrouvés sans emploi étant donné le confinement imposé par le département de la santé publique du Québec en raison de la COVID-19. La pandémie a induit de multiples changements ayant pu affecter la santé psychologique des dentistes québécois. Objectif: Évaluer l’état de la santé psychologique des dentistes québécois dans le contexte de la pandémie de la COVID-19 lors du déconfinement à l’été 2020. Méthodes: Le 10 juin 2020, suite à la réouverture des cliniques dentaires après l’état d’urgence sanitaire, un sondage en ligne a été envoyé par courriel aux 5001 dentistes possédant un permis de pratique au Québec. L’Association des chirurgiens-dentistes du Québec (ACDQ), l’Ordre des dentistes du Québec (ODQ) et la Fédération des dentistes spécialistes du Québec (FDSQ) ont participé au recrutement des participants. Le statut du travail et les données sociodémographiques ont été recueillis et la santé psychologique évaluée. L’évaluation de la santé psychologique comprenait trois outils de mesures validés : l’échelle de symptômes dépressifs Patient Health Questionnaire (PHQ-9), l’échelle d’épuisement personnel, professionnel et relié au patient Copenhagen Burnout Inventory (CBI) et l’échelle de détresse psychologique Kessler-10 (K-10). La prévalence d’idéations suicidaires et de tentative de suicide a également été documentée. Les données ont été analysées en utilisant des tests statistiques bivariés et des analyses multivariées de régression logistique. Résultats: Le taux de réponse était de 18% (n=902) avec 835 répondants qui ont fourni complètement les données psychologiques inclues dans les analyses. Une prévalence élevée a été observée pour l’épuisement d’intensité modérée à sévère sur le plan personnel (49.7%, 95%CI=46.3-53.1), sur le plan professionnel (48.4%, 95%CI=44.9-51.8) et lié au patient (37.5%, 95%CI=34.2-40.9). Des symptômes dépressifs d’un niveau moyen à sévère ont été répertoriés chez 23.4% (95%CI=20.5-26.4) des participants et 39.5% (95%CI=36.2-42.9) des participants présentaient un niveau modéré à sévère de détresse psychologique. The psychological health of Quebec dentists was challenged because of several sudden changes in their daily lives. Objectives: To assess the state of the psychological health of Quebec dentists in the context of the COVID-19 pandemic during the summer of 2020. Methods: An online questionnaire was sent to the 5001 licensed dentists in the province of Quebec, Canada on June 10, 2020. Working status, psychological health assessment, and sociodemographic data were evaluated. The validated psychological health measurement tools included the Patient Health Questionnaire (PHQ-9) to evaluate depression, Copenhagen Burnout Inventory (CBI) to measure burnout, and Kessler-10 scale (K-10) to evaluate psychological distress. Suicidal ideation and past suicidal attempts were also assessed. Data were analyzed using bivariate statistical tests and multivariate logistic regression analysis. Results: The response rate was 18% (n=902) with 835 participants providing complete data sets for psychological outcomes included in the analysis. Prevalence of moderate to severe level of personal (49.7%, 95%CI=46.3-53.1), professional (48.4%, 95%CI=44.9-51.8), and patient-related (37.5%, 95%CI=34.2-40.9) burnout was high. Objectif: évaluer les connaissances, attitudes et pratiques des médecins dentistes exerçant en Suisse, en matière de soins bucco-dentaires pendant la grossesse. Matériel et méthodes: Une enquête transversale a été menée auprès de 200 médecins-dentistes de Suisse alémanique et romande. Les données comprenaient 16 questions qui évaluaient les connaissances, les attitudes et les obstacles rencontrés par les dentistes durant la prise en charge des femmes enceintes au cabinet. Résultats: Selon la majorité des praticiens, les soins dentaires devraient faire partie des soins prénataux. Dans l'ensemble, un bon accord entre les dentistes francophones et germanophones a été trouvé concernant le timing pour chaque acte, l'administration d'anesthésiques locaux, ainsi que la prescription des différents médicaments pendant la grossesse. L'incertitude a été observée sur le lien entre la maladie parodontale et les conséquences sur la grossesse. Conclusion: L'enquête a révélé que les dentistes suisses exerçant en pratique privée ont les connaissances nécessaires pour prodiguer des soins dentaires aux femmes enceintes. Cependant, ils ont tous exprimé le besoin d'obtenir des directives et des orientations claires sur ce sujet important de la santé publique.

Whilst it is forbidden to charge patients with CMU‑C health cover fees in excess of the reimbursable regulated fee (or extra‑fees), so as to make their access to care easier, field experiment studies report discrimination against the latter by physicians. This issue is approached here from the angle of healthcare supply, using four waves of longitudinal administrative data on physicians in private practice between 2005 and 2014. We examine whether this ban on excess fees for CMU‑C beneficiaries, i. e. charging them fees in excess of the standard social security‑negotiated fees agreed under the public health insurance scheme, generates a real financial constraint for Sector 2 physicians (those who charge extra‑fees) and dentists in private practice. Estimates show a significant drop in the average extra‑fees per procedure when physicians accept more CMU‑C patients in their practice. Even if costs are transferred (cost‑shifting), with other patients being charged higher extra‑fees, this is not enough to offset the financial impact. However, this restriction does not have a negative impact on total fees for Sector 2 specialists, general practitioners and dentists, as they increase their volume of activity at the same time., Alors que la facturation de dépassements d’honoraires aux bénéficiaires de la CMU‑C est interdite pour favoriser leur accès aux soins, des études de testing font état de discrimination à leur encontre de la part des médecins. Cette question est ici abordée du côté de l’offre de soins, à partir de quatre vagues de données administratives longitudinales concernant les médecins libéraux entre 2005 et 2014. Nous examinons si cette interdiction des dépassements pour les patients CMU‑C crée une réelle contrainte financière pour les médecins du secteur 2 (pratiquant des dépassements) et les dentistes libéraux. Les estimations montrent une baisse significative du dépassement moyen par acte quand les médecins accueillent plus de patients CMU‑C. S’ils existent, des reports avec des dépassements plus élevés sur les autres patients ne suffisent donc pas à annuler le choc financier. Introduction l´exposition au mercure est un problème de santé publique mondialement reconnu. Si les intoxications aiguës ou subaiguës par ce métal font l´objet d´un consensus diagnostique, l´impact d´une exposition chronique à de faibles doses est encore sujet à débat. En dentisterie, le mercure est utilisé pour la réalisation des amalgames dentaires. De ce fait, les professionnels de la médecine dentaire et spécialement des médecins dentistes représentent une population exposée à ce facteur de risque. Dans ce contexte, une enquête épidémiologique descriptive, transversale et exhaustive a été réalisée afin d´évaluer cette exposition chez les médecins dentistes dans deux régions du centre du Maroc. Parallèlement, leur état de santé globale a été relevé afin de juger de la pertinence d´une enquête étiologique ultérieure pour étudier l´impact probable de l´exposition à ce facteur de risque. Méthodes la présente étude a été réalisée par le biais d´un questionnaire auto-renseigné par les participants. L´exposition professionnelle des médecins dentistes a été évaluée en déterminant la fréquence d´utilisation de l´amalgame dentaire. Les différentes sources de mercure qui modulent la pollution de fond ou qui provoquent des pics d´exposition et qui sont en rapport avec l´environnement ou les habitudes du travail ont aussi été étudiés. Les sources d´exposition non professionnelle prises en compte sont le tabagisme, la vaccination, la consommation de poisson et la présence des amalgames dentaire en bouche. Enfin, l´état de santé des praticiens recrutés dans l´étude a été sondé globalement en se basant sur leurs déclarations. Résultats cent quatre-vingt douze (192) médecins dentistes ont été recrutés dans cette étude. Parmi lesquels, 76,04% déclarent utiliser l´amalgame dans leur pratique avec une fréquence variable. De plus, plusieurs sources de surexposition en rapport avec les conditions ou les habitudes du travail ont été mises en évidence. La principale source d´exposition non professionnelle dans notre population est en rapport avec la présence des amalgames dentaires en bouche (63,45%). Les plaintes sanitaires exprimées par les participants concernaient en chef de file les problèmes neuropsychologiques et ce chez 46,35% de la population. En prenant en compte la neurotoxicité reconnue du mercure, une étude étiologique s´avère parfaitement justifiable dans notre contexte. Conclusion l´exposition au mercure chez les médecins dentistes des régions étudiées est réelle. Des mesures de prévention sont à promouvoir. La réalisation d´une recherche étiologique pour juger l´impact de cette exposition est parfaitement indiquée.

Upper Normandy has several disadvantages in terms of access to dental health care for disabled people: an insufficient medical population, a lack of dental schools and dental care networks.A cross-sectional epidemiological investigation, using a questionnaire form, was carried out between November 2014 and February 2015 with a cohort of dentists in this region.The topics on this form focused firstly on accessibility and compliance with standards of dental offices and the influencing factors which are hampering their accessibility implementation, and secondly on the oral care of disabled patients by practitioners of this region : the impact of incentives, influencing factors which are complicating the dental care and the usage of private and public structures.If the care of disability concerns most practitioners, an apprehension is observed in the adaptation of these doctors' treatment protocols for these specific patients. The expectations of dentists concerning disability are multiple : the creation of specific care networks, better coordination of medical and social actors, and specific training programs concerning the dental care of disabled patients.

The objective of this study is to assess the level of knowledge in a cohort of oral health professionals and patients about preventive and therapeutic actions of sugar-free chewing gums.A forward-looking monocentric study of perception regarding the level of information about the effects of sugar-free chewing gums consumption was conducted on 135 young patients, from 11 to 17 years old, carriers of fixed orthodontic appliances and treated in the Department of Orthodontics in the Oral Medicine and Surgery Center of the University Hospitals of Strasbourg. Besides, 34 practitioners in the Department of Orthodontics and Pediatric Dentistry were also included. Data were collected between May 2016 and July 2016. A specific questionnaire, using the adapted terminology and dealing with the same items was developed for each studied population.The majority of the individuals of both studied populations believe that the consumption of sugar-free chewing gum leads to a greater risk of orthodontic device unsticking or fracture and that it is not associated to a decrease of the orthodontic pain.Our results confirm the fact that the knowledge, mainly acquired in an empirical way, is against the data of the current literature. The evaluation of the level of knowledge demonstrated that there is a real lack of information about the preventive interests of the consumption of sugar-free chewing gums during orthodontic treatment.This study highlights the need for information campaigns and oral prevention in general population, as well as in healthcare professionals, concerning the preventive and analgesic interests of sugar-free chewing gums during orthodontic treatments.

The aim of this study is to assess the attractiveness in the smile arch of different vertical positions of the incisal edges of the maxillary lateral incisors.A frontal photograph of a Caucasian woman's smile was digitally modified to obtain an image with the incisal edges of the maxillary lateral incisors symmetrically tangent to the smile arch. These incisal edges were then moved vertically by 0.5 mm increments. The value was considered positive for an occlusal shift and negative for a gingival shift. Four other images were obtained by moving the incisal edges from +0.5 mm to -1.5 mm. The five images were rated on a visual analog scale by three groups of evaluators: orthodontists, dentists and laypersons. A statistical analysis was performed.Laypersons preferred first lateral incisors tangent to the smile arch, second a +0.5 mm shift, and third a -0.5 mm shift. Orthodontists and dentists preferred first a -0.5 mm shift and second a null shift.The aesthetic perception of the smile, as regard to the vertical relationships between the incisal edges of the anterior teeth, differs between laypersons and professionals but remains homogenous between orthodontists and dentists. The most consensual aesthetic situation, from both professionals' and laypersons' points of view, is the geometry in which the incisal edges of the maxillary lateral incisors are tangent to the smile arch.

Introduction L’halitose, état morbide caractérisé par une mauvaise haleine, présente à la fois un aspect pathologique et social. Dans notre contexte, l’halitose pose de nombreux problèmes de prise en charge aux plans diagnostique et thérapeutique en pratique clinique. Le but de notre travail était d’étudier les aspects diagnostiques et thérapeutiques de l’halitose. Méthodes Il s’agit d’une étude transversale sur une année. Ont été inclus, les patients âgés de plus de 15 ans qui ont consulté pour halitose au centre hospitalier universitaire Yalgado Ouédraogo. Ont été exclus les patients avec mauvaise haleine mais consultant pour un autre motif. L’haleine a été évaluée par un praticien selon le test organoleptique de Rosenberg. Résultats Au total 35 patients ont été inclus pour un sex-ratio de 1,2. L’âge moyen était de 31,9 ans. Dans 57,1% des cas, la plainte venait du patient lui-même. La durée moyenne de l’halitose était de 4,3 ans. Dix-neuf patients avaient un score de Mel Rosenberg ≥ 2. La carie dentaire (07 cas), la sinusite (07 cas), l’infection à Helicobacter pylori (09 cas) et l’ulcère gastro-intestinal (10 cas) étaient associés à l’halitose. Le traitement a été étiologique dans 82.9% des cas avec une amélioration satisfaisante à deux semaines de l’ordre de 71,8%. Conclusion L’halitose reste une pathologie peu étudiée et pose un problème de diagnostic positif, mais aussi étiologique dans notre contexte. Le rôle de l’odontologiste est crucial dans la recherche de la cause de l’halitose. Cependant, une prise en soins pluridisciplinaire de l’halitose permettra d’y apporter une réponse plus efficace.

OBJECTIVES: Conduct an inventory of the dentists’ practices and knowledge about dental care and delivery of medicines regarding pregnant women. MATERIALS AND METHODS: The study has been conducted by a semistructured and anonymous questionnaire, distributed electronically from February 6th to March 15th 2016. The study is based on a transversal analysis, prospective, mono centrique and determined by appraisal. RESULTS: 100% of the dentists surveyed managed pregnant women. Despite, 19% of them thought that some of the dental cares were only feasible in case of emergency and only 32,5% of them practiced all of the emergency care throughout pregnancy. Regarding routine care, results varied by acts and completion of pregnancy: 91% practiced descaling throughout pregnancy, 29% of them were realizing an endodontic treatment during the second trimester and 16% delayed radiography to postpartum. CONCLUSION: The practices of the dentists are not in line with the actual reports in science and their knowledge are still incomplete, despite their desire of getting involved in pregnant women ora l care. That’s why it seems necessary to improve initial training and to implement national recommendations.; OBJECTIF : Réaliser un état des lieux des pratiques et des connaissances des chirurgiens-dentistes concernant les soins bucco-dentaires et les thérapeutiques administrés aux femmes enceintes. POPULATION ET METHODE : Etude analytique, transversale, prospective, monocentrique, des dires d’experts, menée par un questionnaire semi-directif, anonyme, adressé aux dentistes et distribué par informatique du 6 février au 15 mars 2016. RESULTATS : Les 200 dentistes interrogés prenaient en charge les femmes enceintes. 19% pensaient que certains soins denta ires ne sont réalisables qu’en cas d’urgence, et 32,5% réalisaient tous les soins d’urgence quelque soit le terme. Concernant les soins de routine, les résultats variaient selon les actes et le terme de la grossesse : 91% réalisaient le détartrage quelque soit le terme, 29% ne réalisaient un traitement endodontique qu’au cours du 2nd trimestre et 16% reportaient la radiographie au post-partum. CONCLUSION : Les pratiques des chirurgiens-dentistes ne sont pas toujours concordantes avec les données de la science et leurs connaissances restent incomplètes, malgré leur volonté de s’impliquer dans la prise en charge des parturientes. Il semble donc nécessaire d’améliorer la formation initiale plus poussée et de mettre en place des recommandations nationales.

Access to cervical margins allows the practitioner to record the entire cervical margin in order to provide a true copy to the technician. This requires a gingival displacement obtainable by different techniques. This study aimed to assess the implementation of gingival displacement methods prior to impression taking in fixed prosthodontics.This is a descriptive and cross-sectional survey of sample of 71 dentists practising in Abidjan, Ivory Coast; which ran from October 2nd, 2010 to November 14th, 2010. A survey form was administered to dentists. The questionnaire was organised around the following headings: identification of dentists and practice of gingival displacement methods. The data processing done using software Epi Info 6 and Excel XP on Window XP, allowed calculation of frequencies, means and proportions and the establishment of connection between variables with the chi2 test. The significance level was set at p0.05.The results of the survey indicate that non-surgical methods of gingival displacement, including retraction cords and temporary crowns are those they use most frequently (76.4%) because the vast majority of practitioners (87.22%) believe the most traumatic to the periodontium are surgical methods.Our study showed that the gingival displacement methods are frequently carried out in daily practice, regardless of the topography of the abutment teeth and their number, but with a preference for non-surgical methods, particularly those using retraction cords and temporary crowns. The use of injectable gingival displacement paste is not harmful to the periodontal tissues, easy to use and have a very efficient haemostatic action. It should also be known and practiced.

Summary The aim of this study was to determine which smile asymmetries were less esthetic, dental or gingival. Material and methods Laypeople (297), generalists (223), prosthodontists (50) and orthodontists (49), evaluated the esthetics of digitally-modified images taken from the same frontal intra-oral photograph, using the same lips, simulating upper maxillary midline shift, occlusal plane inclination, asymmetric incisal edge and asymmetric gingival migration. The images were later paired into 3 groups. Results The only ones considered esthetic were the asymmetric incisal edge of the 0.5 mm shorter upper central incisor and the asymmetric gingival migration (2 mm) of the upper central incisor. In the paired images, upper maxillary midline shift vs. occlusal plane inclination, the former was rated less esthetic, while in the asymmetric incisal edge vs. asymmetric gingival migration pair, the latter was considered to be less esthetic. Conclusion Laypeople and generalists consider smiles more attractive. The only images considered esthetic were the asymmetric incisal edge of the central incisor shorter by 0.5 mm and the 2 mm asymmetric gingival migration of the upper central incisor. In the horizontal plane (maxillary midline shift vs. occlusal plane cant), the dental asymmetries were considered less esthetic than the gingival asymmetries. However, in the vertical plane (asymmetric incisal edge vs. asymmetric gingival migration) the opposite was recorded.

During its clinical practice, the dentist is often faced with issues concerning the determination of teeth shade. This step is considered sensitive by numerous clinicians and dental laboratories. The objective of this study was to test the ability of professional dentists to visually determine the teeth color.Eleven dentists classified 35 unmarked samples according to their hue, chroma and value from 2 standardized tooth shade guides Ivoclar-Vivadent (Chromascop) et Vita (3D Master). The ratio between the number of matching samples and the total amount of samples from each group was calculated and converted to a score between 0 and 10.Better scores were obtained for value and chroma with the 3D Master shade guide than with the Chromascop shade guide (p0.001). The opposite result was obtained for the hue (p0.05). Globally, better results were obtained for the value and chroma, the hue being the most difficult item to evaluate. A satisfying "color perception" was observed in 69% of the cases.The results of this study argue in favor of a specific training for tooth shade determination in Odontology.

Le lymphome de Burkitt (LB) est une forme de lymphome malin non-Hodgkinien qui provient de l'évolution maligne et de la prolifération de cellules lymphoïdes de type B. Le diagnostic positif repose sur la biopsie d'une masse tumorale ou la ponction de la moelle osseuse révélant la présence de cellules tumorales. Le cas rapporté est celui d'un jeune homme d'une vingtaine d'années qui a été adressé pour des tuméfactions gingivales évoluant depuis 1 mois suite à des extractions dentaires. L'examen anatomopathologique après biopsie complété par l'immunohistochimie conclut à un lymphome de Burkitt. La prise en charge a consisté en une chimiothérapie. Bien que rare, le lymphome de Burkitt est une tumeur agressive qui représente un véritable problème de santé publique d'où la place importante qu'occupe le médecin dentiste dans le diagnostic précoce afin de permettre une prise en charge rapide et appropriée seule garante de la guérison.

The treatments with composite prosthesis require the completion of milling. These precision preparations in the fixed prosthesis promote the integration of removable partial denture with metallic framework in mechanical (prosthetic balance), physiological, aesthetical and psychological point of view. Their conception and realization are always subordinated to the balance principles of the removable partial denture. The precision they require need the use of a dental milling machine working along the predetermined insertion axis of the partial denture casting and require an excellent communication and a close collaboration between dentist and experienced technician of laboratory. In this paper, after a recalling about the milling (definition, description, interests), we will detail the steps to achieve clinical and laboratory needs for a rehabilitation by composite prosthesis combining milling, and it will be illustrated through a clinical case with a bilateral tooth-supported edentulous in the maxilla.
