Introduction-Tongue injuries in children vary from slight lacerations to complete amputations. It could be due to fall, self-mutilation, epilepsy, child abuse, or sport injury. The anterior dorsum is the most common site followed by the mid dorsum and anterior ventral part. These injuries may cause dysfunction of tongue, hemorrhage, and disfigurement. Management of injuries of tongue in young children is challenging. There are no established guidelines for managing tongue lacerations. This is a case report of a three-year-old girl with a laceration on the anterolateral part of the tongue with non-approximating wound edges. The laceration extended from the dorsal to the ventral part. The parents were apprehensive about the surgical management of the injury due to the age of their daughter. Hence, healing of the injury with conservative management without suturing was achieved. Conclusion-The management of traumatic injuries of tongue in children is difficult due to their age and cooperation. The pediatrician has to decide whether to heal the tongue by primary or secondary intention. Our decision to allow the healing by secondary intention was successful. The tongue healed without any complications barring a small scar with satisfactory aesthetic and functional outcome. Both the parents and the child were satisfied with the management of the injury.

Traumatic injuries to the orofacial region occur frequently in children and can be challenging to pediatric dentists. The incidence of injury to the tongue is not frequently sustained. These injuries occur commonly when there's a fall or blow and the tongue is trapped between the teeth. They cause parents to panic and the child to cry uncontrollably with blood and soft tissue debris in the mouth. In the present case series, successful revascularization is achieved of a neartotal amputation of the tongue in a 2.5-year-old female, a through-and-through tongue bite injury in an 8-year-old male and a partial tongue laceration injury in a 6-year-old female. Flare gun injuries are rare, and patients often present with complex trauma that may require multiple operative interventions. Our objective is to explore a case of a 15-year-old male, who presented with second-degree flame burns to the face, left upper extremity and bilateral hands, as well as third-degree burns to the oropharynx and a tongue laceration after a flare gun was discharged into his mouth. The patient underwent multiple debridements of the oral cavity and oropharynx, and his hospital course was complicated by an intra-oral abscess. He eventually made a full recovery and was discharged. This case not only illustrates the need for early operative intervention, but it also underscores the need for awareness campaigns that highlight the dangers of flare guns and similar devices. Study Design. Prospective multicenter study. Objective. The aim of this study was to study the incidence of nonneurologic adverse events related to transcranial electric stimulation (TES) for intraoperative spinal cord monitoring (IOM) with motor-evoked potentials (MEPs) (Tc(E)- MEPs) and determine the need for safety precautions. Summary of Background Data. Tc(E)-MEPs monitoring requires high-voltage multipulse TES that causes widespread muscle contraction and movement. Improved awareness of TES-induced movement-related adverse events is needed. Methods. We analyzed data from 2643 patients who underwent high-risk spinal surgery with intraoperative Tc(E)-MEPs at 11 spinal centers from 2010 to 2016. Information about neurologic and non-neurologic postoperative complications was collected, including type of surgical procedure, operative time, estimated blood loss, and treatment for postoperative adverse events. Results. A 70% drop in Tc(E)-MEPs amplitude, which was the alarm criterion to interrupt surgery, predicted postoperative motor deficits with 93.5% sensitivity, 91.0% specificity, a false-positive rate of 8.2%, and a false-negative rate of 0.3%. Non-neurologic adverse events developed in 17 (0.64%) patients and were most commonly because of bite injuries (0.57%), including 11 cases of tongue laceration, two cases of lip laceration, and two cases of tooth breakage. Four (0.15%) tongue lacerations required surgical repair with sutures and two tooth breakages required dental treatment. One patient had hair loss corresponding to the TES site. One patient, who underwent additional IOM with transpharyngeal stimulation, had severe nasal hemorrhage following electrode placement by nasal route, which resolved spontaneously. Non-neurologic adverse events did not significantly affect the accuracy of IOM assessment. Neither operative times nor blood loss significantly influenced the occurrence of adverse events. Conclusion. During TES-IOM, both the surgeon and monitoring team must consider the possibility-although rare-of non-neurologic adverse events, particularly bite injuries. This case report describes the management of tongue and lip lacerations due to dystonia in a 1-year-old infant. A splint was given to raise the bite and prevent repeated trauma and aid in healing of the oral tissue. This paper highlights the importance of pediatric dentist's role in improving quality of patient care in an intensive care unit.

AIMS OF THE STUDY Tongue lacerations are common in children, occurring mostly from falls or sports injuries. Optimal treatment of tongue lacerations is a challenge for paediatricians due to contradictory recommendations and a lack of current guidelines. It remains unclear which tongue lacerations should be sutured and which would benefit from spontaneous healing, which is a promising alternative. In recent years, the treatment of choice in our paediatric emergency department (ED) has shifted from generally suturing the wounds to more frequently advising secondary wound healing. The aim of this study was to analyse tongue lacerations treated at our ED in order to develop guidance for the optimal management of tongue lacerations in children. METHODS This retrospective study was conducted to assess tongue lacerations at the ED of a University Children’s Hospital Zurich from January 2010 to August 2015. All families were contacted for informed consent and photo documentation of the healed tongue. Clinical records of all the patients included were reviewed and different variables were defined and analysed. RESULTS A total of 73 children with tongue lacerations were included (75.3% boys, mean age ± standard deviation 4.0 ± 2.6 years). The mean size of the lacerations was 12.4 ± 8.3 mm, with affected tongue borders in 51 cases (69.9%) and a through-and-through laceration in 23 patients (31.5%). A primary wound closure was performed in 12 children (16.4%). These wounds were significantly larger than those of the secondary wound healing group (21 ± 10 mm compared to 10.8 ± 6.8 mm), presented gaping wound edges with the tongue at rest more frequently (91.7% compared to 32.8%), and showed through-and-through lacerations more often (91.7% compared to 19.7%). The group with wound suturing needed longer to recover (median 13 days compared to 6.2 days) and had a higher rate of complications (25 vs 3.3%). CONCLUSIONS Suturing is not required in gaping tongue lacerations less than 2 cm long that do not involve the tip of the tongue. The Zurich Tongue Scheme was developed as a guide for clinicians when deciding which tongue lacerations need suturing.

Abstract: Esophageal injuries may occur spontaneously because of iatrogenic instrumental injury, foreign body impaction, or external trauma. Traumatic esophageal laceration is rare and can lead to significant morbidity such as perforation, mediastinitis, retropharyngeal abscess, or deep neck infection. Early detection of esophageal injury improves patient outcome and survival compared with a diagnosis that is delayed by more than 24 hours after rupture. We describe the case of a 45-year-old man with esophageal laceration after facial contusion and tongue laceration. Upper airway compromise is the major concern for emergency physicians. In a nonsurgical approach, close observation is needed because there is a potential risk of progression to delayed esophageal rupture. If there is clinical deterioration, repeat endoscopy or surgical intervention should be considered. Early detection of esophageal rupture in patients with minor head injuries can reduce mortality and morbidity and avoid major surgery and, in most cases, allows the esophagus to heal normally. The tongue is an important structure for speech, mastication and deglutition. Avulsion of a portion of tongue can significantly limit the range of motion thereby impairing its functions. This is the case report of a traumatically amputated tongue which was reconstructed to fulfill its function effectively. Z-plasty is one of the most commonly used soft tissue corrective procedures in plastic surgery. Objective: The objective of this study was to determine whether suturing or conservative management of tongue lacerations results in differences in wound healing and functional outcome. The secondary aim was to identify whether antibiotics are required in the treatment of tongue lacerations., Methods: Studies published between December 1954 and August 2020 were extracted from MEDLINE via PubMed, Embase via OVID, CINAHL via EBSCO, Web of Science, and the Cochrane Library and evaluated for inclusion based on predetermined inclusion and exclusion criteria by two independent reviewers in accordance with PRISMA guidelines., Results: The search yielded a total of 16,111 articles, 124 of which were evaluated by full-text review, resulting in 11 articles included in this systematic review representing 142 unique cases of tongue lacerations. At least 26 lacerations (18.3%) included penetration of the muscle layer of the tongue, and 24 (16.9%) were classified as full-thickness lacerations. Thirty-five of the 142 tongue lacerations (24.6%) were sutured. The remaining lacerations underwent some form of conservative management. The majority of studies reported excellent healing of tongue lacerations regardless of the management method, with minimal scarring and excellent return to normal functional status. No cases of infection were reported., Conclusions: Current literature is inconsistent with regards to indications and guidelines for primary repair of tongue lacerations. The majority of tongue lacerations reported in the literature heal with excellent outcomes regardless of management method. Physician judgement along with patient and parental preference based on potential risks of the procedure should be used when deciding whether a tongue laceration requires primary repair. The purpose of this article is to highlight repair of a unique, severe injury pattern in a child., Presentation and Treatment: A 3-year-old boy fell on a cemented floor causing a midline full-thickness laceration through the median fibrous septum, resulting in a bifid tongue. A buffalo with history of fighting among fellow animals was presented with dysphagia, profuse salivation and bleeding from mouth. Clinical examination revealed extensive tongue lacerations. The oral cavity was cleaned thoroughly and lacerations were sutured. Another case of severe tongue laceration in a newborn Jersey calf due to faulty snare application to relieve dystocia was treated successfully by suturing. Both animals recovered uneventfully. A Osmanabadi cross female goat was presented with history of mandible fracture and tongue laceration following dog bite. Clinical examination revealed fracture of both horizontal mandible rami and tongue laceration. The fracture fragments were aligned ajid retained in position by interfragmentary wiring and lacerated tongue was sutured using simple interrupted manner, ape muzzle and modified aluminium splint were applied as external co-aptation for additional support.
