Background Digital health promotion programs tailored to the individual are a potential cost-effective and scalable solution to enable self-management and provide support to people with excess body weight. However, solutions that are widely accessible, personalized, and theory- and evidence-based are still limited. Objective This study aimed to develop a digital behavior change program, Choosing Health, that could identify modifiable predictors of weight loss and maintenance for each individual and use these to provide tailored support. Methods We applied an Intervention Mapping protocol to design the program. This systematic approach to develop theory- and evidence-based health promotion programs consisted of 6 steps: development of a logic model of the problem, a model of change, intervention design and intervention production, the implementation plan, and the evaluation plan. The decisions made during the Intervention Mapping process were guided by theory, existing evidence, and our own research—including 4 focus groups (n=40), expert consultations (n=12), and interviews (n=11). The stakeholders included researchers, public representatives (including individuals with overweight and obesity), and experts from a variety of relevant backgrounds (including nutrition, physical activity, and the health care sector). Results Following a structured process, we developed a tailored intervention that has the potential to reduce excess body weight and support behavior changes in people with overweight and obesity. The Choosing Health intervention consists of tailored, personalized text messages and email support that correspond with theoretical domains potentially predictive of weight outcomes for each participant. The intervention content includes behavior change techniques to support motivation maintenance, self-regulation, habit formation, environmental restructuring, social support, and addressing physical and psychological resources. Conclusions The use of an Intervention Mapping protocol enabled the systematic development of the Choosing Health intervention and guided the implementation and evaluation of the program. Through the involvement of different stakeholders, including representatives of the general public, we were able to map out program facilitators and barriers while increasing the ecological validity of the program to ensure that we build an intervention that is useful, user-friendly, and informative. We also summarized the lessons learned for the Choosing Health intervention development and for other health promotion programs. Background: Overweight and obesity are major problems worldwide, and they lead to an increased risk for several diseases. The use of technology in the treatment of obesity is promising, but in the existing literature, there is considerable uncertainty regarding its efficacy. In this review, we included web- and mobile-based weight loss interventions that were implemented remotely in rehabilitation settings. Objective: The aim of this systematic review is to study the effectiveness of physical activity-promoting web- and mobile-based distance weight loss interventions in rehabilitation settings on body composition in comparison with control groups that did not use technology. Methods: Studies were searched from 9 databases. The inclusion criteria were as follows: population: age 18-65 years; intervention: physical activity-promoting web- and mobile-based distance weight loss interventions; comparison: control groups without the use of technology; outcome: changes in BMI, waist circumference, or body fat percentage; study design: randomized controlled trial. The quality of the studies was assessed by 2 researchers. Meta-analysis was performed, and we also conducted a meta-regression analysis to evaluate the factors associated with the changes in body composition outcomes if statistical heterogeneity was observed. Results: The meta-analysis included 30 studies. The mean quality of the studies was 7 of 13 (SD 1.9; range 3-10). Prolonged periods of energy deficit leading to weight loss induce metabolic adaptations resulting in reduced energy expenditure, but the mechanisms for energy conservation are incompletely understood. We examined 42 healthy athletic females (age 27.5 +/- 4.0 years, body mass index 23.4 +/- 1.7 kg/m(2)) who volunteered into either a group dieting for physique competition (n = 25) or a control group (n = 17). The diet group substantially reduced their energy intake and moderately increased exercise levels to induce loss of fat mass that was regained during a voluntary weight regain period. The control group maintained their typical lifestyle habits and body mass as instructed. From the diet group, fasting blood samples were drawn at baseline (PRE), after 4- to 5-month weight loss (PRE-MID), and after 4- to 5-month weight regain (MID-POST) as well as from the control group at similar intervals. Blood was analyzed to determine leukocyte transcriptome by RNA-Sequencing and serum metabolome by nuclear magnetic resonance (NMR) platform. The intensive weight loss period induced several metabolic adaptations, including a prominent suppression of transcriptomic signature for mitochondrial OXPHOS and ribosome biogenesis. The upstream regulator analysis suggested that this reprogramming of cellular energy metabolism may be mediated via AMPK/PGC1-alpha signaling and mTOR/eIF2 signaling-dependent pathways. Our findings show for the first time that prolonged energy deprivation induced modulation of mitochondrial metabolism can be observed through minimally invasive measures of leukocyte transcriptome and serum metabolome at systemic level, suggesting that adaptation to energy deficit is broader in humans than previously thought.

IntroductionDigital behavioural weight loss interventions have the potential to improve public health; however, these interventions are often not adequately tailored to the needs of the participants. This is the protocol for a trial that aims to determine the effectiveness and cost-effectiveness of the Choosing Health programme as a means to promote weight loss and weight loss maintenance among overweight/obese adults.Methods and analysisThe proposed study is a two-group randomised controlled trial with a nested interrupted time series (ITS) within-person design. Participants (n=285) will be randomly assigned to either the Choosing Health digital intervention or a control group. For intervention participants, ecological momentary assessment will be used to identify behavioural determinants for each individual in order to tailor evidence-based behaviour change techniques and intervention content.Control group participants will receive non-tailored weight loss advice via e-book and generic emails. The primary outcome is the mean difference in weight loss between groups at 6 months controlled for baseline. Secondary outcomes include blood pressure and percentage of body fat; self-reported measures of physical activity, sitting time, quality of life, cost and theory-derived correlates of weight loss. Secondary outcomes will be measured at baseline, 3, 6 and 12 months. The primary outcome for ITS will be daily weight loss plan adherence. Data will be analysed using regression and time series analyses.Ethics and disseminationEthics approval was granted by Faculty of Psychology, SWPS University of Social Sciences and Humanities, Wroclaw, Poland, approval number 03/P/12/2019. Purpose The purpose of this study was to analyze changes in the prevalence of weight reduction behaviors (WRBs) among European adolescents from 26 countries between 2001/2002 and 2017/2018. The impact of the perception of body weight on WLB was also analyzed, with particular attention being paid to overestimation. Methods The data of 639,194 European adolescents aged 11, 13, and 15 years who participated in the Health Behaviour in School-aged Children survey were analyzed. Age-standardized prevalence rates of WRB were estimated separately by survey round and gender for each country, using the overall 2017/2018 Health Behaviour in School-aged Children study population as the standard. Multivariate logistic regression analyses were used to assess WRB trends over time, adjusted for survey year, body mass index, body weight misperception, and family affluence and stratified by gender and age. Results In the 26 countries examined, the overall age-adjusted prevalence rates of WRB were 10.2% among boys and 18.0% among girls. The prevalence of WRB was higher for girls, but in the more recent surveys, gender differences in WRB decreased. There was a significant increase in the percentage of WRB among boys in most countries. Among girls, most countries did not experience significant changes. Increases in body mass index and overestimation of body weight were significant factors increasing the risk of WRB in both genders. Conclusions The change in the prevalence of WRB by gender warrants greater attention from researchers and practitioners alike. peerReviewed

PurposeThis study aims to understand how online weight loss services could help customers achieve a durable change. The particular focus is on exploring the roles of value co-creation and well-being outcomes in reinforcing the transformative value potential, which is argued to be realized as customers’ intentions to continue a healthier lifestyle after the service period has ended.Design/methodology/approachData was collected from the participants of an online weight loss service (n = 498), and a conceptual research model was tested using structural equation modelling.FindingsThe results imply that compliance with the guidelines and social support are two value co-creation activities that can influence the well-being outcomes of transformative services (i.e. perceived behavioral control and satisfaction with one’s achievements). These well-being outcomes help attain the transformative value potential of online weight loss services. However, the actual weight loss affected the transformative value potential only through customers’ satisfaction with their achievements as a subjective well-being outcome.Originality/valueThis study provides insight into the transformative value potential of services in the weight loss context. This study contributes to the transformative service research by focusing on the role of online services in reinforcing a durable change through the co-creation of value and improvements in customers’ well-being.

This paper contributes to the discussion on health behavior change support systems by examining one of the persuasive software features, social support. The paper studies the influence of structural social support on functional social support types in the online environment. Specifically, the paper examines the frequency of using two different kinds of online social platforms (online forums and Facebook groups) that are used to facilitate peer social support in weight loss and their influence on perceived functional social support types (emotional, informational, and instrumental). The results show that frequent use of Facebook based support peer group facilitates perceived emotional, informational, and instrumental support, whereas frequent use of online forum based peer support group facilitates emotional and informational support but much less than Facebook support group usage does. Thus, modifying the support platform makes it possible to influence perceived functional social support. peerReviewed

Objective To examine baseline prestroke weight loss and poststroke mortality among men. Design Longitudinal study of late-life prestroke body mass index (BMI), weight loss, and BMI change (midlife to late life) with up to 8-year incident stroke and mortality follow-up. Setting Community-based aging study data. Participants Japanese-American men (N=3581; age range, 71–93y) who were stroke free at baseline. Interventions Not applicable. Main Outcome Measure Poststroke mortality: 30 days poststroke, analyzed with stepwise multivariable logistic regression; and long-term poststroke (up to 8y), analyzed with stepwise multivariable Cox regression. Results Weight loss (4.5kg decrements) was associated with increased 30-day poststroke mortality (adjusted odds ratio=1.48; 95% confidence interval [CI], 1.14–1.92), long-term mortality after incident stroke (all types, n=225; adjusted hazards ratio (aHR)=1.25; 95% CI, 1.09–1.44), and long-term mortality after incident thromboembolic stroke (n=153; aHR=1.19; 95% CI, 1.01 to 1.40). Men with overweight/obese late-life BMI (≥25kg/m2, compared with healthy/underweight BMI) had increased long-term mortality after incident hemorrhagic stroke (n=54; aHR=2.27; 95% CI, 1.07–4.82). Neither desirable nor excessive BMI reductions (vs no change/increased BMI) were associated with poststroke mortality. In the overall sample (N=3581), nutrition factors associated with increased long-term mortality included the following: (1) weight loss (10lb decrements; aHR=1.15; 95% CI, 1.09–1.21), (2) underweight BMI (vs healthy BMI; aHR=1.76; 95% CI, 1.40–2.20), and (3) both desirable and excessive BMI reductions (vs no change or gain, separate model from weight loss and BMI; aHR range, 1.36–1.97; P

The prevalence of working-aged weight losers is high because of high prevalence of obesity. Unfortunately, the loss of weight is often temporary. The aim of the present study was to investigate the effect of intensive counselling on maintained weight loss and eating behaviour (cognitive restraint, emotional eating, uncontrolled eating and binge eating). Additionally, the associations of eating behaviour with maintained weight loss, discontinuation, dietary intake and anhedonia were studied. Eighty-two overweight or obese subjects (body mass index >27) participated in either intensive (n=35) or short-term (n=47) weight loss counselling (based on the Finnish Current Care guidelines) in a randomized weight loss follow-up study lasting 18 months. The subjects in the intensive counselling group lost more weight than those in the short-term counselling group at the 6 months follow-up (5.0±5.7kg, 2.4±2.5kg, respectively). The weight loss results were not maintained in either of the groups. The cognitive restraint increased and the emotional eating, uncontrolled eating and binge eating symptoms decreased in both groups. There were subjects in both counselling groups who succeeded in weight loss (weight loss result 9.3±6.3%) and those who failed (gained 3.3±1.7% of body weight). Those who succeeded had the highest score for cognitive restraint and lowest for uncontrolled eating, binge eating and emotional eating. In failure group, the scores for uncontrolled eating and binge eating were the highest already at the beginning of study. Those with the highest cognitive restraint at 18 months reported low intake of energy and fat while their intake of carbohydrates and fibre was high. Anhedonia existed in 24.4% of the participants at least once during the study period. They had higher binge eating scores continously, more uncontrolled eating and emotional eating at 6 months and they lost less weight than those without anhedonia. Thirty-two participants (39%) discontinued the study. Discontinuation was independently associated with a lack of free-time and a high weight loss goal. The association of eating behaviour with weight loss and dietary intake suggest that enhancing eating behaviour could be a target for improving the success of weight loss. The assessment and then the treatment of psychological factors (such as anhedonia), focus on setting realistic weight loss targets as well as emphasizing the fact that counselling visits and lifestyle changes are time consuming processes should be included in weight loss counselling. Tiivistelmä Lihavuuden lisääntyessä myös työikäisten laihduttajien määrä kasvaa. Laihdutustulos jää valitettavan usein väliaikaiseksi. Tämän työn tavoitteena oli tutkia tehostetun ohjauksen vaikutusta laihtumistulokseen ja syömiskäyttäytymiseen (tietoinen syömisen hillintä, tunnesyöminen, impulsiivinen syöminen ja ahmimistaipumus). Lisäksi tutkittiin syömiskäyttäymisen yhteyksiä laihtumistulokseen, tutkimuksen keskeyttämiseen, energiaravintoaineiden saantiin sekä anhedoniaan. Tutkimukseen osallistui 82 ylipainoista tai lihavaa henkilöä (painoindeksi >27kg/m²). Heidät satunnaistettiin kahteen tutkimusryhmään: tehostetun ohjauksen (n=35) ja lyhytohjauksen ryhmään (n=47). Ohjaus perustui aikuisten lihavuuden Käypä hoito -suositukseen. Tutkimuksen kesto ohjaus- ja seurantajakson kanssa oli 18 kuukautta. Tehostetun ohjauksen ryhmässä henkilöt laihtuivat ensimmäisen kuuden kuukauden aikana enemmän kuin lyhytohjausryhmässä (5.0±5.7kg ja 2.4±2.5kg). Laihtumistulos ei kuitenkaan ollut pysyvä. Syömiskäyttäytymisessä tapahtui pysyvä muutos kummassakin tutkimusryhmässä; tietoinen hillintä lisääntyi, ja samanaikaisesti tunnesyöminen, impulsiivinen syöminen ja ahmiminen vähenivät. Kummassakin ryhmässä oli laihtumisessa onnistuneita (laihtumistulos 9.3±6.3 %) sekä epäonnistuneita (paino nousi 3.3±1.7 %). Onnistujilla esiintyi enemmän tietoista syömisen hillintää ja samanaikaisesti vähiten tunnesyömistä, impulsiivistä syömistä ja ahmimistaipumusta. Epäonnistujat söivät jo alkutilanteessa impulsiivivisemmin, ja heillä oli enemmän ahmimistaipumusta kuin onnistujilla. Henkilöt, joilla oli korkein tietoinen syömisen hillintä, saivat vähiten energiaa ja rasvaa mutta eniten hiilihydraatteja ja kuitua. Anhedoniaa esiintyi 24.4 %:lla tutkituista ainakin kerran tutkimuksen kuluessa. Anhedoniaan yhdistyi myös muita korkeammat ahmimistaipumuspisteet koko tutkimuksen ajan, eniten impulsiivista syömistä ja tunnesyömistä kuuden kuukauden seurannassa ja vähäisempi laihtuminen kuin henkilöillä, joilla ei esiintynyt anhedoniaa. Tutkimuksen keskeytti 39 % mukaan lähteneistä. Keskeyttämisen itsenäisiä riskitekjöitä olivat vapaa-ajan puute ja suuri laihtumistavoite. Syömiskäyttäytymisen yhteys sekä laihtumiseen että energiaravintoaineiden saantiin osoittaa, että syömiskäyttäytymisen ohjaus tulisi olla keskeinen osa laihdutusohjausta. Myös ahedonian arviointi ja hoito sekä keskustelu realistisesta painotavoiteesta ja elämäntilanteen kuorimittavuudesta voisivat tehostaa laihdutuksen onnistumista.

Painonpudotukseen johtavien pysyvien käyttäytymismuutosten tekeminen on osoittautunut hyvin vaikeaksi, minkä vuoksi paino useimmiten nousee painonpudotuksen jälkeen. Tämän ilmiön paremmaksi ymmärtämiseksi tutkimuksemme tarkoituksena oli selvittää ylipainoisten painonhallitsijoiden käsityksiä painoonsa vaikuttavista tekijöistä sekä painonhallintaan ja elämäntapamuutoksiin motivoivia tekijöitä painonpudotuksen jälkeisessä seurannassa. Lisäksi tutkittiin painonpudotusohjelman aikaisten ja sen jälkeisten painomuutosten sekä psykologisen joustavuuden yhteyksiä näihin painonhallitsijoiden käsityksiin ja motiiveihin. 49 ylipainoista painonpudotusohjelmassa mukana ollutta henkilöä osallistui noin 8 kuukautta ohjelman päättymisen jälkeen pidettyyn seurantaan. Painoon vaikuttavia tekijöitä kysyttiin avoimella lomakkeella, jossa kysyttiin lisäksi osallistujien halukkuutta tehdä muutosta kuhunkin mainitsemaansa tekijään. Motiiveja painonhallintaan ja elämäntapamuutoksiin kysyttiin elämänalueittain. Painoon vaikuttavina tekijöinä ylipainoiset painonhallitsijat ilmoittivat liikunnan ja syömistottumusten lisäksi yleisesti sosiaalisia tekijöitä, kuten perheen tai ystävät. Tärkeitä motiiveja painonhallintaan ja elämäntapamuutoksiin oli terveys, parisuhde, vapaa-ajan harrastukset, työ sekä muut sosiaaliset suhteet. Painonpudotuksessa ja -hallinnassa eritavoin onnistuneet erosivat painoon vaikuttavien tekijöiden maininnan sekä painonhallinnan motiivien suhteen. Myös psykologinen joustavuus oli yhteydessä painonhallitsijoiden erilaisiin näkemyksiin ja motiiveihin. Sosiaalisten tekijöiden vaikutus painoon oli moninainen. Tulokset ehdottavat, että sosiaaliset tekijät motivoivat enemmän henkilöitä, joilla on alhaisempi psykologinen joustavuus. Lisäksi henkilöt, joita parisuhde motivoi painonhallintaan, pudottivat vähemmän painoa painonpudotusohjelman aikana. Psykologinen joustavuus oli yhteydessä myös halukkuuteen tehdä muutoksia elämäntilanteeseen ja terveyteen, sekä itsekontrollin korostamiseen painoon vaikuttavana tekijänä. Siten tutkimuksen tulokset ehdottavat, että psykologisen joustavuuden lisäämisellä voisi olla vaikutusta ylipainoisten käsityksiin painoon vaikuttavista tekijöistä sekä ylipainoisten motivaatioon tehdä elämäntapamuutoksia.
