Introduction and aim. Congenital epulis is a rare benign tumor that affects the oral cavity of newborns which typically presents as a solitary mass on the maxillary alveolar bridge at birth, with a predilection for the female gender. The rarity of congenital epulis is underscored by the limited number of reported cases in the literature, as evidenced by the infrequent occurrence of this condition in newborns. Description of the case. This case report presents a rare case of congenital epulis, a benign gingival tumor in a newborn. The report includes a brief literature review to provide insights into the clinical characteristics, diagnostic approaches, and management of congenital epulis. Conclusion. Background: Congenital epulis is a benign gingival tumor whose differential diagnosis includes other oral-facial masses such as teratoma, hemangioma, lymphatic malformation and dermoid cysts. This tumor can cause obstruction of the airway or feeding problems in the newborn. Surgical excision is the treatment of choice. Case Report: We present a case of congenital epulis, diagnosed prenatally with ultrasonography. Conclusion: Although difficult, a defined prenatal image of congenital epulis is possible by means of accurate high-resolution ultrasonography. Congenital granular cell tumor is an uncommon benign oral lesion of uncertain histogenesis that affects the alveolar ridge region of newborns. The present study aims to report an unusual congenital granular cell tumor case, emphasizing clinical, morphological, and immunohistochemical aspects. A 10-day-old newborn patient attended the stomatology service with a nodular pedunculated lesion of normal color located in the anterior region of the superior alveolar ridge. Thus, the diagnostic hypothesis was congenital epulis, and an excisional biopsy was performed. Histopathological analysis revealed features consistent with the diagnosis of congenital granular cell tumor. In addition, we performed immunohistochemical staining for vimentin, Ki-67, and S100. The patient has been under follow-up with no clinical signs of recurrence. Due to its rarity, the analysis of congenital granular cell tumor’s clinicopathological characteristics enables an adequate diagnosis and treatment.

Congenital epulis is a rare benign soft tissue lesion that occurs exclusively in neonates and usually develops from the alveolar mucosa at the frontal region of the maxilla. Lesions may be sessile or pedunculated, normal in color or reddish and vary in size from a few millimeters to a few centimeters. Although spontaneous regression has also been reported, surgical excision is indicated for large tumors that may obstruct the airway and make feeding the child difficult. In this case report, we present a rare case of congenital epulis involving the right side of the maxillary alveolar mucosa in a neonate delivered vaginally at Dhulikhel Hospital, Kavreplanchok, Nepal. The lesion was surgically removed by electrocautery, and histologic examination confirmed the clinical diagnosis of congenital epulis. Congenital epulis, also known as Neumann's tumor, is an uncommon benign tumor of the oral mucosa that occurs in newborns. It is a rare condition, with fewer than 250 reported cases worldwide. The exact cause or underlying mechanism of this tumor is still not well understood. We present a three-day-old male neonate who presented with a swelling on the gingiva that had been present since birth. The infant did not encounter any difficulties with feeding or breathing. The patient had a single, round, pink swelling measuring 2 × 2 × 1 cm on the right maxillary alveolar ridge. The swelling was surgically removed under general anesthesia. Microscopic examination revealed large polygonal cells with abundant granular cytoplasm, centrally located nuclei indicating a diagnosis of congenital epulis. Clinical manifestation could vary from no symptoms to feeding difficulty and rarely airway obstruction. It usually tends to grow on anterior alveolar ridge of the newborns, more on the maxilla than on the mandible. Confirmation of the diagnosis is by histopathology, which commonly shows proliferation of polygonal round cells with eosinophilic granular cytoplasm and round central nucleus. Congenital epulis can be approached using different management techniques depending on the size, site of the tumor, and presenting symptoms of the newborns. Abstract Background Congenital granular cell tumor (CGCT) is a rare benign soft tissue tumor of the new born. It is also called congenital epulis. Case presentation We report the case of a 3-day-old male neonate who was postnatally noted to have a 2-cm mass over the maxillary alveolar ridge in the midline. The mass was causing feeding difficulties, and hence, decision for surgical excision was taken. The postoperative recovery was uneventful. Conclusion Congenital granular cell tumor (CGCT) is commonly seen on the gingival margins of the maxilla or mandible. Large tumors may cause polyhydramnios in the antenatal period or feeding and respiratory difficulties in the postnatal period. Diagnosis is usually confirmed by histopathology. Malignant change or recurrence has not been reported following incomplete excision of this mass.

Congenital granular cell epulis is a very rare tumor first described by Neumann in 1871. It is of mesenchymal origin with the female predilection in the ratio of 8:1. The lesion appears as a large protuberant mass and is commonly associated with obstruction causing difficulty in feeding and deglutition. We report a case of a newborn female who had feeding difficulty because of a protuberant mass arising from the oral cavity. The mass was surgically excised under general anesthesia and was histopathologically proven as congenital granular cell epulis. The follow-up for 3 months showed no recurrence. There are reported complications prenatally and postnatally. Hence, the treatment should be initiated early. The treatment of choice is complete surgical excision and no recurrence has been reported.

Congenital epulis is one the rare neonatal oral pathologies. It is a soft tissue growth usually rising from alveolar ridge and rarely from the tongue as well. Prompt recognition leads to determining plan of management and appropriate parent's counselling owing to benign nature of the condition. Being a rare condition there are only few case reports in English literature. We report a retrospective observation of cases of congenital epulis managed at Department of Pediatric Surgery from March 2019 to Dec 2020. We have studied clinical presentation, management and histopathology where excision has been done. A total of 6 cases have been managed during study period. 3 of them underwent excision and rest were managed expectantly. Histopathology of all excised lesions was congenital granular cell lesion. The expectantly managed patients showed complete resolution over 12 months. Congenital epulis can be observed for 1 year or till eruption of dentition if it is not affecting mouth closure. Large tumors affecting feeding or breathing are to be surgically excised without any serious complications.

The objective of this case report is to describe a congenital epulis of the newborn. A 10-day-old female neonate was brought to a dental clinic for examination of an intraoral lesion that was present at birth. No systemic changes were noted during the extraoral examination. During the intraoral examination, a nodular exophytic lesion was found in the maxillary left alveolar ridge. The appearance suggested a congenital epulis. The nodular, pedunculated lesion had a diameter of 13 mm, firm consistency, smooth surface, and color varying from pink to red. Newborns with such lesions usually have difficulty breastfeeding and may experience respiratory difficulties. The patient underwent an excisional biopsy under local infiltrative anesthesia with 2% lidocaine. Histopathologic examination of the lesion confirmed the diagnosis of congenital epulis. There were no postoperative complications. At the follow-up examination 8 days after the biopsy, her weight and length had already increased, and the oral mucosa had a normal appearance. At 8 months of age, she demonstrated continued gains in weight and length. Oral mass in a newborn is rare but often quite daunting, leading to anxiety in parents. Very few hospitals have dental or maxillofacial facilities on site and this may lead to delayed diagnosis and intervention. A thorough clinical examination and knowledge of the oral mass is essential for precise diagnosis, management and parental reassurance. In this review article, we present a newborn baby with congenital epulis - an oral mass arising from the alveolar ridge. Aim: This case report aims to describe a rare congenital lesion of the incisive papilla with a high labial frenulum attachment, clinically mimicking congenital epulis but histopathologically diagnosed as an oral leiomyomatous hamartoma., Background: Oral leiomyomatous hamartoma is a very rare congenital lesion, mainly appearing on the median anterior maxilla/incisive papilla and tongue., Case Description: This clinical paper is about a rare lesion in a 6-year-old female child whose parents reported to the department with the complaint of slow-growing soft tissue overgrowth between the front teeth of the upper jaw, present since birth. The soft tissue growth is now causing difficulty in biting food and is visible during smiling and speaking, causing an esthetically unpleasing appearance. The clinical examination also revealed a high labial frenulum attached to the lesion. The lesion was provisionally diagnosed as congenital epulis based on the clinical picture. L'épulis gingivale congénitale est une lésion rare chez le nouveau-né de sexe féminin. Le diagnostic peut être fait en prénatal par une imagerie fœtale. Congenital epulis (CE) is a rare condition in newborns, whose histogenesis remains unclear. It mostly presents as a circumscribed mass in the maxilla alveolar ridge of female neonates and can interfere with their normal breathing and breastfeeding. This benign oral lesion usually appears as a pedunculated nodule covered with normal mucosa that extends from the anterior vestibular alveolar ridge up to the posterior region. There are some pathological conditions that show clinical and histopathological features similar to those of CE, such as granular cell tumor, gingival cyst of the newborn, vascular malformations and others. This case report aimed to describe the clinical and pathological features of a newborn patient with a clinical and histopathological diagnosis of CE on the right side of the maxillary alveolar ridge, treated with excision by laser surgery, with a follow-up of 5 months without any sign of recurrence. Congenital epulis, also known as granular cell tumor of the newborn or Neumann’s tumor, is a rare congenital tumor affecting the gingival mucosa of neonates. It occurs more often in female population with the ratio of 10:1. These lesions behave in a benign manner. No recurrence or metastasis have been reported. The present case had congenital epulis in the incisor region of maxillary alveolar ridge. The patient had no problem at prenatal follow-up, but applied to the clinic with a mass in the mouth at birth and diagnosed as with congenital epulis. Since this lesion interferes with nourishment, it was excised under general anaesthesia. There was no problem at intraoperative and postoperative course. No recurrence was seen in the following 3 months.

BACKGROUND: The congenital granular cell tumour of the newborn, also known as congenital epulis or Neumann's tumor, is rare. It occurs on the gingiva of the anterior alveolar ridge of the jaws. CASE REPORT: We are reporting a 2-day-old female neonate, who came to our unit with a well defined, solitary, firm mass arising from the maxillary anterior region measuring about 3.5 cms in diameter and causing difficulty in breast feeding but no hindrance to the airway. The mass was surgically excised under general anesthesia. Postoperative wound healing was uneventful. CONCLUSION: We have shared our experience in handling this rare type of tumor. A decisive diagnosis is made by histopathologic analysis as other newborn lesions can be incorrectly diagnosed as CE. This article presents a case report of a female infant who presented a fibrotic mass in the primary lateral incisor and canine region of the maxillary alveolar ridge. The lesion was not causing feeding or respiratory problems. After a watchful waiting procedure and no spontaneous regression, the lesion was excised under local anesthesia and confirmed by histopathologic analysis as CE.

Background: Congenital epulis of the newborn is a rare benign soft-tissue tumour of the gingival, which is also called gingival granular cell tumour of the newborn. These slow growing soft tissue tumors affect the gingivae of a new born child. It is essential to document the presentation and the management of this lesion because of its rare nature. Aim: To highlight the clinical presentation, diagnosis and management of the lesion. Methods: Two clinical cases of congenital epulis were used to illustrate the presentation and surgical management of the lesion. Findings: The first clinical case is a one month old female child with a protruding gum tumour on the anterior alveolar process of the mandible. The second case is a one week old male child with a gingival growth on the anterior alveolar process of the mandible. All the tumours were surgically excised under local anaesthesia. Conclusion: Congenital epulis could interfere with the functions of the oral cavity. Therefore, it is imperative to surgically excise it when diagnosed. Congenital epulis (CE) or "Granular cell epulis" also previously termed as "Neumann′s tumor" is a benign growth arising from the mucosa of the gingiva, typically seen as a mass protruding from the infant′s mouth, often interfering with respiration and feeding. These tumors generally present as a single mass arising from the upper alveolus. We report a rare case of two congenital epulides arising from the maxillary and mandibular alveolus in a day old female infant, which was surgically excised, allowing for early initiation of breast feeding. The tumor cells stained negative for S100 protein hence differentiating from other granular cell tumors. The clinical presentation, differential diagnosis in regard to the various neonatal oral swellings and the management of CE has been discussed.

The congenital granular cell epulis (CGCE) is a rare tumor, which is apparent at birth. The histogenesis is still uncertain, but several theories, including origin from epithelial, undifferentiated mesenchymal cells, pericytes, fibroblasts, smooth muscle cells, and nerve-related cells have been proposed. This case report describes management of a 2-day-old baby girl having a large, round, soft, single 3 × 4 cm, pedunculated swelling, on the lower anterior ridge, which was causing difficulty in feeding. Clinical diagnosis of congenital epulis (CE) was made and lesion was excised under conscious sedation. A vessel running over the surface of the lesion was continuous on the alveolar ridge. To reduce intra-operative hemorrhage transfixion suture was passed around the vessel on the alveolar ridge. Then the lesion was excised from the base of peduncle with a scalpel. Histopathologically, the diagnosis of a congenital granular cell lesion of the jaw was given. Follow up of 3 months shows no signs of recurrence. CGCE may interfere with feeding, requiring a conservative excision as soon as the child is fit to undergo surgery. Tendency for recurrence and malignant transformation has not been documented.

Dentistry for babies aims to contribute to the formation of a generation with fewer dental problems, higher quality of oral health and more aware of the importance of prevention. The congenital gingival granular cell tumour (CGCT) is a rare benign soft tissue tumour in the newborn that can lead to respiratory difficulties and feeding. The aim of this work was to report two cases of CGCT in babies and discuss the clinical and histological differential diagnosis and their treatment, as well as the importance of knowledge of this pathology for dentists. Clinical features and treatment approaches are presented and discussed. The surgical approach improved the child quality of life and restored the parent’s confidence and emotional stability. Knowledge of this pathology helps in better diagnosis and treatment, which lead to a better quality of life of children and return confidence and emotional stability to parents.

Children may be born with birth defects, the most common being oro-facial clefts and fissural cysts. A well circumscribed pedunculated soft tissue growth that occurs congenitally is known as congenital epulis of the newborn or ‘Neuman’s Tumour’ as described in the literature. It is a rare lesion and the diagnosis has to be confirmed histologically. We present a rare case of a 7-year-old child with a congenital growth in the pre-maxillary region of the anterior hard palate clinically diagnosed as congenital epulis however, histologically confirmed as a mucous retention cyst. An elaborate clinical differential diagnosis is discussed. The anterior hard palate is devoid of salivary glands and the presence of a mucous retention cyst in the area is suggestive of ectopic salivary gland tissue and in a child manifesting at birth is probably the first case to be reported in the English literature.

The congenital epulis is a benign congenital granular cell tumor arising most often of the alveolar ridge of the jawbone. When giant, it is source of digestive discomfort disabling feeding. We report the case of a newborn female, vaginal delivery, presented with a giant intraoral tumor. Tumor obstructing the mouth of the newborn and prevent the attachment and feeding. The treatment consisted of excision of the tumor under general anesthesia. The histology of the tumor was revealed that it was an epulis.

Congenital epulis of the newborn is a very rare and unique tumor first described in 1871 by Neuman. It has a female predilection. It is a tumor with no tendency to recur after excision. The histogenesis of the lesion is unknown, but it is believed to be of mesenchymal origin. We report a 2-day-old female with tumor mass on the anterior mandibular alveolar ridge, which demonstrated immunoreactivity for vimentin, S-100 and neuron-specific enolase; thus, suggesting a similar histogenesis with granular cell tumor.

Introduction Congenital epulis or congenital granular cell tumour is a rare benign lesion arising from alveolar mucosa of neonates which can cause respiratory and feeding problems. It is also known as Neumann’s tumour. Females are affected 10 times more frequent. Method A review of medical record was performed. Result A newborn presented with lesion over left lower gum which was seen during antenatal abdominal ultrasonography scan by Obstetrics team at 35-week of gestation. The case was referred for ex utero intrapartum treatment procedure in view of anticipating upper airway obstruction. Upon delivery via Caesarean section, patient had no obstructive symptoms however was immediately intubated few hours of life to secure airway. Computed tomography scan confirmed a soft tissue mass arising from either lower alveolus or tip of tongue. The lesion was excised and histopathological examination reported as congenital epulis. Conclusion Even though it is a rare case, congenital epulis should be considered as one of differential diagnosis of an oral mass. The objective of the study is to determine the clinicopathological features and immunohistochemical findings of congenital epulis. A retrospective study was carried out to determine the clinicopathological features of congenital epulis, diagnosed histologically in the main oral histopathology laboratory in Malaysia from 1967 to 2014. Immunostaining using vimentin, muscle specific actin, smooth muscle antigen, desmin, S100, CD34, CD68 and CD1a was carried out. Twelve cases of congenital epulis were reviewed. All of the patients were females and the presentation age ranged from 2 to 90 days. The patients comprised of 6 Malays, 3 Chinese, 2 Indians and 1 Orang Asli. Most of the cases (n=7) involved the maxillary ridge and presented as pedunculated well-defined lumps (n=8). Excisional biopsy was performed in all cases. Via immunohistochemistry, vimentin expression was observed in all cases; but negative for CD34, muscle specific actin, smooth muscle antigen, and desmin. CD1a and S100 positivity was seen in five cases. The interstitial cells were highlighted by CD68. Congenital epulis (CE) is an extremely rare benign tumor of the gingiva that is found on the alveolar ridge of newborns, and the main treatment option is simple excision. Postoperative prognosis is very good, and spontaneous regression may occur despite incomplete excision. This report presented a rare case of CE and its healing process after surgery under local anesthesia. The treatment plan was decided upon through consultation between a medical team and the patient's family, with surgical excision for the main lesion, which benefitted from surgery, and follow-up for a very small-sized lesion, which was thought to be appropriate for a newborn. No recurrence was found after its removal, and favorable healing was observed.

The congenital epulis is a benign congenital granular cell tumor arising most often of the alveolar ridge of the jawbone. When giant, it is source of digestive discomfort disabling feeding. We report the case of a newborn female, vaginal delivery, presented with a giant intraoral tumor. Tumor obstructing the mouth of the newborn and prevent the attachment and feeding. Abstract: Purpose: To present our experience in the perinatal management of fetuses with large oropharyngeal tumors by ex utero intrapartum treatment (EXIT). Methods: We performed a retrospective chart review of all patients with congenital oropharyngeal tumor who underwent an EXIT procedure between May 2006 and June 2012. Results: Four patients were included in the series, three females and one male. The diagnoses were epignathus (n =2) and congenital epulis (n =2). Three EXITs were done at term and one at late preterm due to premature rupture of membranes. Median maternal time under anesthesia was 185min (range: 166–281) and median maternal operative time was 99min (range: 85–153). Median maternal blood loss was 550ml (range: 350–2000); one mother required a blood transfusion. Mean maternal hospital stay was 4days. Median hysterotomy-to-cord clamp time was 24min (range: 18–66). Mean fetal birth weight was 2.7kg (range: 2.4–3). The airway was successfully accessed and secured under placental circulation in all cases. In the two patients with congenital epulis the tumors were resected at the base of their pedicles and the airway accessed via direct laryngoscopy before the umbilical cord was clamped. One patient with epignathus underwent a retrograde tracheal intubation under placental circulation and had the tumor resected thereafter. The second patient with epignathus had a tracheostomy done under placental circulation and then had tumor debulking immediately after the EXIT. The maternal morbidity was minimal and there were no mortalities. Conclusions: We conclude that the EXIT procedure is the ideal delivery strategy for fetuses with prenatally diagnosed oropharyngeal tumors and potential airway obstruction at birth. Patients with prenatally diagnosed oropharyngeal tumors should be promptly referred to a fetal treatment center with a dedicated multidisciplinary team and EXIT capabilities. Background The glucose transporter type 1 ( GLUT-1) protein is a useful marker for perineurial cells. Because of the possible neuroectodermal histogenesis of the granular cell tumour and congenital granular cell epulis, the aim of this study was to assess the immunoexpression of GLUT-1 protein in granular cell tumour and congenital granular cell epulis to aid in clarifying their histogenesis. Methods The protocol of this study was approved by the Committee of Bioethics in Research at Universidade Federal Minas Gerais. Six cases of granular cell tumour and three cases of congenital granular cell epulis were submitted to immunohistochemistry for GLUT-1 and S-100 using the streptavidin-biotin standard protocol. Results Five cases of granular cell tumour were located on the tongue and one case on the upper lip. All cases of congenital granular cell epulis were observed in the alveolar ridge of newborns. All lesions evaluated proved to be immunonegative for GLUT-1. S-100 was found to be positive in all granular cell tumours and negative in congenital granular cell epulis. Palatal swelling is a rare condition presenting with a difficult airway or airway obstruction. These swellings can range from epulis, encephalocoele, dermoid cysts or teratoma to benign and malignant neoplasms. Airway management of such patients, especially in children is difficult and challenging. Inhalational induction with maintenance of spontaneous respiration is useful. Congenital epulis is a benign gingival tumor that has rarely been diagnosed prenatally. We report a fetus with congenital epulis diagnosed by ultrasound at 37 weeks' gestation. The mass was diagnosed as originating from the inferior alveolar ridge prenatally. After birth, it was completely removed by simple excision under general anesthesia. Summary: Introduction: Congenital epulis, known as a congenital gingival granular cell tumor, is a benign tumor and very rare in newborns. Voluminous or multiple tumors can cause mechanical obstruction of the oral cavity and may result in postnatal feeding and respiratory problems. Discussion: We report the clinical case of a female full-term newborn who presented a tumor on the upper gum obtruding into the oral cavity discovered at birth. The pregnancy was followed normally with three prenatal ultrasounds, which did not show abnormalities. The mass was excised under local anesthesia on the second day of life. The outcome was good after surgery and regular feedings were started on the second postoperative day. Histological examination confirmed the diagnosis of gingival tumor with granular cells and absence of signs of malignancy. Conclusion: Prenatal diagnosis is fundamental in the therapeutic approach to this rare lesion but remains difficult because the findings are non specific and the generally late development of the tumor. Neumann's tumor is a rare benign soft tissue lesion presents during birth. It arises from the gingival mucosa, at the anterior part of the maxillary alveolar ridge and is typically seen as protruding mass out of the newborn child's mouth that may interfere with respiration or feeding or lip closure. Female predilection is more compared to male population with ratio of 8:1. Excision of the lesion is the only possible treatment, but its spontaneous regression has been reported. Post-operative recurrence rate of this tumor is less and no damage to any dentition has not been reported, this suggests that radical excision is not required. This case report presents the case of a female infant with a solid ovoid mass protruding from the oral cavity. Treatment of this lesion was surgically done and subjecting to histopathological examination confirmed the diagnosis of congenital epulis. This article describes five cases of congenital epulis, a rare and benign swelling in the mouth of a newborn, which is not widely known. We present five cases: four cases presented as single pedunculated nodules of the gingiva and in one case two nodules were present. Of all, 50% were located at the maxilla. Excision was performed in four of the five cases and in one case, spontaneous regression was awaited. No recurrence was reported. The characteristic features of congenital epulis are a pedunculated, flesh-pink coloured tumour with a predominant occurrence on the anterior maxillary alveolar ridge in a female newborn. Although the aetiology is unknown, most authors suggest a mesenchymal, rather than an odontogenic, origin. Endogenous hormonal factors might influence growth prenatally. Histological findings include granular cells with eosinophilic cytoplasm and small, eccentric nuclei. Despite the fact that the lesion can be a striking sight, spontaneous regression is possible and can be awaited. Indications for non-radical excision under local anaesthesia are severe upper airway obstruction and interference with feeding technique. In conclusion, we provide clinical and histological information about congenital epulis, so that this entity will be more easily recognised and relevant information given to parents. Leiomyomatous hamartoma (LH) is congenital lesion rarely seen in oral cavity. In English literature, all reported cases appeared as solitary lesion in alveolar ridge or the tongue, and there have never been a report showing a case of multiple occurrence of this lesion. A quite rare case of multiple LH occurred in a 2-year-old Japanese boy is presented. A polypoid lesion was presented at incisive papilla and two isolated lesions in the tongue dorsum, one appeared as a polypoid mass in the posterior dorsum and other as a small spindle-shaped agger in the anterior dorsum.
