Abstract Background and Aim This study aimed to clarify the efficacy and safety of 48‐week pemafibrate treatment in patients with metabolic dysfunction‐associated steatotic liver disease (MASLD) complicated by dyslipidemia. Methods A total of 110 patients diagnosed with MASLD complicated by dyslipidemia received pemafibrate at a dose of 0.1 mg twice daily for 48 weeks. Results The participants were 54 males and 37 females, with a median age of 63 (52–71) years. 3. Post-menopausal ovarian fibroma associated with steroid hormone synthesis: A case report

Objective: Ovarian fibromas are benign, sex cord-stromal tumors occurring in both peri- and post-menopausal women. Generally, these tumors are non-functional and do not produce hormones. However, this case report proves the first case of steroid hormone synthesis in an ovarian fibroma by immunohistochemistry. Case report: A 77-year-old post-menopausal woman presented with a left ovarian tumor, abnormal endometrial thickness, and high levels of estradiol (E2). The tumor was found to be a fibroma, which was positive for alpha-inhibin. We examined estrogen-producing enzymes using immunohistochemistry. The tumor was positive for estrogen receptor, progesterone receptor, 17β-hydroxysteroid dehydrogenase (HSD)-1, adrenal 4 binding protein/steroidogenic factor 1, 17β-HSD-5, steroid sulfatase, and P450c17. Conclusion: This case study shows that E2 can be locally produced from circulating inactive steroids, by estrogen-producing enzymes. This is the first report of steroid hormone synthesis in an ovarian fibroma.

4. A case of invasive mucinous adenocarcinoma presenting with massive bronchorrhea

An 80-year-old non-smoking woman was admitted to hospital due to persistent sputum production and dyspnea. She developed respiratory failure, and chest imaging revealed multifocal consolidation and cavities. Her respiratory status did not respond to antimicrobial treatment and progressively worsened, with massive sputum production of approximately 1 L per day, and she died 19 days after admission. The patient was diagnosed with invasive mucinous adenocarcinoma based on a postmortem needle biopsy of the lung. Clinicians should consider invasive mucinous adenocarcinoma in the differential diagnosis of patients who present with massive bronchorrhea and diffuse pulmonary cavity abnormalities.

5. Low vitamin D levels accelerates muscle mass loss in patients with chronic liver disease.

Sarcopenia frequently and progressively occurs in patients with chronic liver disease. This study aimed to clarify the relationship between vitamin D levels and muscle mass loss. A total of 166 patients with chronic liver disease were enrolled in this study. Skeletal muscle mass index (SMI) was measured by bioelectrical impedance analysis at baseline and after 1 year. The rate of change in SMI from baseline after 1 year was calculated: ΔSMI (%) = [(1-year SMI - baseline SMI) / baseline SMI] × 100. Muscle mass loss was defined as ΔSMI ≤ -2%. The median 25-hydroxyvitamin D was 15.2 (11.2-19.3) ng/mL. The median SMI were 6.8 (5.9-7.8) kg/m2 at baseline and 6.7 (5.9-7.6) kg/m2 after 1 year. The median ΔSMI was -1.23% (-2.21% to 1.61%). Multivariate analysis identified low 25-hydroxyvitamin D as an independent factor associated with muscle mass loss. The optimal cut-off value of 25-hydroxyvitamin D to predict muscle mass loss was 12.7 ng/mL. Muscle mass loss was found in 56.4% v.s. 18.0% of patients with 25-hydroxyvitamin D < 12.7 vs. ≥ 12.7 ng/mL, respectively (p = 9.01 × 10-7); with the highest incidence in patients with non-alcoholic fatty liver disease (NAFLD). Specifically, patients with NAFLD and 25-hydroxyvitamin D < 12.7 ng/mL had a significantly higher incidence of muscle mass loss than those with ≥ 12.7 ng/mL (p = 1.23 × 10-3). Low vitamin D levels are associated with muscle mass loss after 1 year in patients with chronic liver disease, especially NAFLD.

Abstract The aim of this retrospective multicenter study was to clarify the antifibrotic effect and long‐term outcome of sodium glucose cotransporter 2 inhibitors (SGLT2‐Is) in patients with nonalcoholic fatty liver disease (NAFLD) complicated by type 2 diabetes mellitus (T2DM). Of the 1262 consecutive patients with T2DM who recently received SGLT2‐Is, 202 patients with NAFLD had been receiving SGLT2‐Is for more than 48 weeks and were subjected to this analysis. Furthermore, 109 patients who had been on SGLT2‐I therapy for more than 3 years at the time of analysis were assessed for the long‐term effects of SGLT2‐Is. Abstract Background and Aim This study aimed to clarify the efficacy and safety of oral semaglutide treatment in patients with non‐alcoholic fatty liver disease (NAFLD) complicated by type 2 diabetes mellitus (T2DM). Methods This was a single‐arm, open‐label pilot study. Sixteen patients with NAFLD who received oral semaglutide for T2DM were included in the analysis. Oral semaglutide was initiated at a dose of 3 mg once daily, and the dose was sequentially increased to 7 mg at 4 weeks and 14 mg at 8 weeks (maintenance dose) until the end of the 24‐week trial. Results Body weight and levels of liver‐related biochemistry, plasma glucose, and hemoglobin A1c decreased significantly from baseline to 12 weeks. These significant decreases were maintained until the end of the trial. Additionally, levels of the homeostasis model assessment‐insulin resistance and triglyceride significantly decreased at 24 weeks. Controlled attenuation parameter (CAP) values significantly decreased from baseline to 24 weeks. Changes in body weight were correlated with those in levels of alanine aminotransferase (r = 0.52) and CAP (r = 0.72). As for liver fibrosis markers, significant decreases from baseline to 24 weeks in levels of the fibrosis‐4 index, ferritin, and type IV collagen 7 s were found; however, the liver stiffness measurement did not significantly decrease. Most adverse events were grade 1–2 transient gastrointestinal disorders. Conclusions Oral semaglutide treatment in patients with NAFLD complicated by T2DM improved impaired liver function, hypertriglyceridemia, insulin resistance, and hepatic steatosis, as well as improving diabetic status and reducing body weight.

Physical symptoms such as fatigue and muscle weakness, and psychiatric symptoms like depression and anxiety are considered as complications and sequelae of COVID-19. This epidemiological study investigated the actual status of psychiatric symptoms and disorders caused by COVID-19, from four major university hospitals and five general hospitals in Fukuoka Prefecture, Japan, having a population of 5 million. We conducted a survey of psychiatric disorders associated with COVID-19 using Diagnosis Procedure Combination (DPC) data and the psychiatric records of the hospitals. In the study period from January 2019 to September 2021, 2743 COVID-19 admissions were determined from DPC data across the nine sites. These subjects had significantly more anxiety, depression, and insomnia, and were receiving higher rates of various psychotropic medications than controls influenza and respiratory infections. A review of psychiatric records revealed that the frequency of organic mental illness with insomnia and confusion was proportional to the severity of COVID-19 infection and that anxiety symptoms appeared independent of infection severity. These results indicate that COVID-19 is more likely to produce psychiatric symptoms such as anxiety and insomnia than conventional infections.

9. A novel formula used for predicting hepatocellular carcinoma after the achievement of sustained virologic response by direct-acting antivirals in patients with chronic hepatitis C.

Although eliminating HCV can prevent hepatocellular carcinoma (HCC), some patients develop HCC even after obtaining sustained virologic response (SVR). Previously, we developed a new formula to predict advanced liver fibrosis. This study aimed to clarify the usefulness of this formula for predicting HCC after achieving SVR. Among 351 consecutive patients who had been treated with direct-acting antivirals, 299 were included in this study. New formula scores were used as a marker for predicting liver fibrosis and as a predictive model for HCC incidence. The participants were 172 men and 127 women with a median age of 68 years. The median new formula score was -1.291. The cumulative HCC incidence rates were 4.3%, 9.7%, and 12.5% at 1, 3, and 5 years, respectively. The cumulative incidence of HCC was significantly higher in patients with a history of HCC than in those without treatment history of HCC (P = 2.52×10-26). Multivariate analysis revealed that male (HR = 6.584, 95% CI = 1.291-33.573, P = 0.023) and new formula score (HR = 1.741, 95% CI = 1.041-2.911, P = 0.035) were independent factors associated with the development of HCC in patients without a treatment history of HCC. The optimal cutoff value for predicting the development of HCC was -0.214. The cumulative incidence rates of HCC in patients with new formula scores ≥-0.214 were 5.4%, 15.3%, and 15.3% at 1, 3, and 5 years, respectively, whereas the incidence rates of HCC in patients with new formula scores

Abstract Nonalcoholic fatty liver disease (NAFLD) is related to subclinical atherosclerosis. However, whether the severity of the disease (or which histopathological component) is associated with subclinical atherosclerosis remains controversial. This study aimed to investigate the association between the histopathological severity of NAFLD and carotid intima-media thickness (CIMT) in Japanese patients with liver biopsy-proven NAFLD. Maximum-CIMT (max-CIMT) was measured as an index of carotid atherosclerosis in 195 biopsy-proven NAFLD patients. A significant association was observed between the severity of fibrosis (but not steatosis, inflammation, and ballooning) and max-CIMT. Older age, male gender, hypertension, and advanced fibrosis were independently linked to max-CIMT ≥ 1.2 mm. Abstract Background Clinical course of pleuroparenchymal fibroelastosis (PPFE) shows considerable variation among patients, but there is no established prognostic prediction model for PPFE. Methods The prediction model was developed using retrospective data from two cohorts: our single-center cohort and a nationwide multicenter cohort involving 21 institutions. Cox regression analyses were used to identify prognostic factors. The total score was defined as the weighted sum of values for the selected variables. The performance of the prediction models was evaluated by Harrell’s concordance index (C-index). We also examined the usefulness of the gender-age-physiology (GAP) model for predicting the prognosis of PPFE patients. Results We examined 104 patients with PPFE (52 cases from each cohort). In a multivariate Cox analysis, a lower forced vital capacity (FVC [defined as FVC 550 U/mL, HR, 2.56) were significantly associated with a poor prognosis. The total score was calculated as 1 × (FVC, 550 U/mL). PPFE patients were divided into three groups based on the prognostic score: stage I (0–1 points), stage II (2 points), and stage III (3–4 points). The survival rates were significantly different in each stage. The GAP stage was significantly associated with the prognosis of PPFE, but no difference was found between moderate (stage II) and severe (stage III) disease. Our new model for PPFE patients (PPFE Prognosis Score) showed better performance in the prediction of mortality in comparison to the GAP model (C-index of 0.713 vs. 0.649). Conclusions Our new model for PPFE patients could be useful for predicting their prognosis.

Abstract Background and Aim Although tenofovir alafenamide (TAF), as well as entecavir (ETV), is widely used as first‐line treatment for patients with chronic hepatitis B, there are only a few studies comparing sequential therapy from ETV to TAF and continuous ETV monotherapy in patients with maintained virologic response to ETV. Methods In a retrospective multicenter study, we investigated the efficacy and safety of sequential therapy from ETV to TAF (ETV‐TAF group) and compared them with continuous ETV monotherapy (ETV group), using propensity score matching, in chronic hepatitis B patients. Results From 442 patients, we analyzed 142 patients from each group comprising 71 patients matched for several data, including age, HBV genotype, hepatitis B envelope antigen, cirrhosis, alanine aminotransferase, platelet count, prior ETV monotherapy period, and hepatitis B surface antigen (HBsAg) change during prior ETV monotherapy. In the ETV‐TAF group, HBsAg levels significantly decreased from baseline to 48 weeks after switching to TAF (−0.02 log IU/mL, P = 0.038). HBcrAg levels also significantly decreased after switching to TAF (−0.1 log IU/mL, P = 0.004). However, there were no significant differences in the reduction of HBsAg and HBcrAg levels between the ETV‐TAF and ETV groups. There was no significant difference in the change of estimated glomerular filtration rate levels from baseline to 48 weeks between the two groups. Conclusions The present study indicated that the efficacy, especially of the HBsAg‐reducing action, and safety of sequential therapy from ETV to TAF were similar to those of continuous ETV monotherapy among chronic hepatitis B patients with maintained virologic response to ETV.

A 32-year-old man who had worked as a welder for 13 years was hospitalized for a fever and hemosputum with dyspnea. He was diagnosed with welding fume-associated lung disease with alveolar hemorrhaging and acute respiratory failure. Despite surviving the acute phase with corticosteroid therapy, hypoxemia persisted after a month and a half, requiring home oxygen therapy. As a result of the introduction of nintedanib, his clinical findings gradually improved, and the patient was weaned from oxygen therapy after six months. Inhalation of a large amount of welding fumes in a short period can cause alveolar hemorrhaging and prolonged pulmonary dysfunction.

Abstract Background Procalcitonin (PCT) is a useful marker for pneumonia. However, its clinical usefulness in elderly patients has not been studied extensively. This study aimed to assess the relationship between PCT and prognosis and pneumonia severity in elderly patients with pneumonia acquired outside the hospital. Methods Data considered relevant to pneumonia severity and prognosis were retrospectively obtained from clinical charts of all patients with pneumonia who were admitted to our hospital from 2010 to 2017. The primary outcome was 30-day mortality in elderly patients (aged ≥75 years), and the relationship between PCT levels and pneumonia severity, as determined by the pneumonia severity index (PSI) was also examined. Results Data were collected from 667 patients, of which 436 were elderly patients. Multivariate and receiver operating characteristic curve analysis revealed that albumin, body mass index, and PSI class rather than PCT are important factors related to 30-day mortality in elderly patients. Non-alcoholic fatty liver disease (NAFLD) is a progressive disorder that can develop into liver fibrosis and hepatocellular carcinoma. Natural killer (NK) cells have been shown to protect against liver fibrosis and tumorigenesis, suggesting that they may also play a role in the pathogenesis of NAFLD. Sialic acid-binding immunoglobulin-like lectins (Siglecs) are a family of inhibitory and activating receptors expressed by many cell types, including NK cells. Here, we investigated the phenotypic profiles of peripheral blood and intrahepatic NK cells, including expression of Siglecs and immune checkpoint molecules, and their association with NK cell function in patients with NAFLD. Immune cells in the peripheral blood of 42 patients with biopsy-proven NAFLD and 13 healthy volunteers (HVs) were identified by mass cytometry. The function of various NK cell subpopulations was assessed by flow cytometric detection of intracellular IFN-γ and CD107a/LAMP-1, a degranulation marker, after in vitro stimulation. We found that peripheral blood from NAFLD patients, regardless of fibrosis stage, contained significantly fewer total CD56+ NK cell and CD56dim NK cell populations compared with HVs, and the CD56dim cells from NAFLD patients were functionally impaired. Among the Siglecs examined, NK cells predominantly expressed Siglec-7 and Siglec-9, and both the expression levels of Siglec-7 and Siglec-9 on NK cells and the frequencies of Siglec-7+CD56dim NK cells were reduced in NAFLD patients. Notably, Siglec-7 levels on CD56dim NK cells were inversely correlated with PD-1, CD57, and ILT2 levels and positively correlated with NKp30 and NKp46 levels. Further subtyping of NK cells identified a highly dysfunctional Siglec-7−CD57+PD-1+CD56dim NK cell subset that was increased in patients with NAFLD, even those with mild liver fibrosis. Intrahepatic NK cells from NAFLD patients expressed elevated levels of NKG2D and CD69, suggesting a more activated phenotype than normal liver NK cells. These data identify a close association between NK cell function and expression of Siglec-7, CD57, and PD-1 that could potentially be therapeutically targeted in NAFLD.

The instantaneous photocarrier transport of perovskite solar cells was evaluated by assessing laser-induced terahertz (THz) emission to understand carrier dynamics in perovskite solar cells. The waveform of laser-induced THz radiation from an interface between the TiO2 electron transport layer and perovskite active layer of an n-i-p perovskite solar cell with varying external bias was measured using THz-time domain spectroscopy. The amplitude of the THz radiation decreased with increasing reverse bias voltage. The waveform of the THz radiation was inverted at a strong reverse bias. The measured bias voltage dependence suggests that the transient current generated at the interface between perovskite and TiO2 owing to the higher mobility of electrons than that of holes, namely the photo-Dember effect, is the dominant source of THz radiation and the destructive contribution of the interfacial electric field inverts the transient current when a reverse bias causes a strong interfacial electric field. The significant contribution of the interfacial electric field has not been previously reported in perovskite thin films and is unique to solar cells. We believe that band bending at interfaces in perovskite solar cells will be determined from the THz emission with proper modeling.

A thin film of unsubstituted polythiophene (PT), an insoluble conjugated polymer, with molecular chains uniaxially oriented in plane was prepared by the friction transfer method. The structure of highly oriented thin films of PT was investigated using grazing-incidence X-ray diffraction (GIXD), ultraviolet–visible (UV–vis) spectroscopy, and infrared (IR) spectroscopy. The polarized UV–vis and IR spectra and GIXD measurements showed the PT molecular chains were well aligned in parallel to the friction direction. The GIXD studies clarified that the polymer backbones were aligned with very narrow distribution, such that the half-width was about 4 degrees. The degree of orientation of the PT friction-transferred film was higher compared with those of regioregular poly(3-alkylthiophene)s. Moreover, the GIXD results show a preferred orientation where the a-axis is perpendicular to the substrate plane.

Although the presence of nonalcoholic fatty liver disease (NAFLD) is known to be related to subclinical atherosclerosis, the relationship between the severity of NAFLD and subclinical atherosclerosis is not clear. This study aimed to clarify the factors related to subclinical arteriosclerosis, including the histopathological severity of the disease and PNPLA3 gene polymorphisms, in NAFLD patients. We measured brachial-ankle pulse wave velocity (baPWV) as an index of arterial stiffness in 153 biopsy-proven NAFLD patients. The baPWV values were significantly higher in the advanced fibrosis group than in the less advanced group (median, 1679 cm/s vs 1489 cm/s; p = 5.49×10-4). Multiple logistic regression analysis revealed that older age (≥55 years) (p = 8.57×10-3; OR = 3.03), hypertension (p = 1.05×10-3; OR = 3.46), and advanced fibrosis (p = 9.22×10-3; OR = 2.94) were independently linked to baPWV ≥1600 cm/s. NAFLD patients were categorized into low-risk group (number of risk factors = 0), intermediate-risk group (= 1), and high-risk group (≥2) based on their risk factors, including older age, hypertension, and biopsy-confirmed advanced fibrosis. The prevalence of baPWV ≥1600 cm/s was 7.1% (3/42) in the low-risk group, 30.8% (12/39) in the intermediate-risk group, and 63.9% (46/72) in the high-risk group. Non-invasive liver fibrosis markers and scores, including the FIB-4 index, NAFLD fibrosis score, hyaluronic acid, Wisteria floribunda agglutinin positive Mac-2-binding protein, and type IV collagen 7s, were feasible substitutes for invasive liver biopsy. Older age, hypertension, and advanced fibrosis are independently related to arterial stiffness, and a combination of these three factors may predict risk of arteriosclerosis in NAFLD patients.

Background: Sarcopenia worsens patient prognoses in chronic liver disease. This study aimed to elucidate the effects of vitamin D supplementation on skeletal muscle volume and strength in patients with decompensated cirrhosis. Methods: Thirty-three patients were entered into the study based on the criteria and then randomly assigned to two groups: Group A (n = 17), the control group, and Group B (n = 16), those who received oral native vitamin D3 at a dose of 2000 IU once a day for 12 months. Results: SMI values in Group B were significantly increased at 12 months (7.64 × 10−3). The extent of changes in the SMI and grip strength in Group B were significantly greater than that in Group A at 12 months (p = 2.57 × 10−3 and 9.07 × 10−3). The median change rates in the SMI were +5.8% and the prevalence of sarcopenia was significantly decreased from 80.0% (12/15) to 33.3% (5/15; p = 2.53 × 10−2) in Group B. Conclusions: Vitamin D supplementation might be an effective and safe treatment option for patients with decompensated cirrhosis to increase or restore the skeletal muscle volume and strength or prevent the muscle volume and strength losses.

Improvement of the critical current density (Jc) of superconducting wires/tapes is one of the key issues in the field of superconductivity applications. Here we report the fabrication of a silver-sheathed Ba1−xNaxFe2As2 (BaNa-122) superconducting tape by using a powder-in-tube technique and its superconducting properties, in particular transport Jc, as well as the tape-core texture. The optimally-doped BaNa-122 tape with Na concentration x = 0.4 exhibits the superconducting critical temperature (Tc) of 33.7 K and high transport Jc of 4 × 104 A/cm2 at 4.2 K in a magnetic field of 4 T. Patterns of x-ray diffraction for the superconducting core show that the degree of c-axis orientation is significantly enhanced through the tape fabrication process. The tendency of c-axis orientation is advantageous for achieving higher Jc, suggesting the high potential of BaNa-122 for superconducting wire/tape applications.

By perception-based extension for fuzzy random variables, a dynamic portfolio model for weighted average value-at-risks of fuzzy random variables is introduced. By dynamic programming and mathematical programming, this paper derives analytical solutions of the optimization problem for dynamic worst scenarios. A few numerical examples are given to discuss the results. Objective Semaphorin 4D (Sema4D)/CD100 has pleiotropic roles in immune activation, angiogenesis, bone metabolism, and neural development. We undertook this study to investigate the role of Sema4D in rheumatoid arthritis (RA). Methods Soluble Sema4D (sSema4D) levels in serum and synovial fluid were analyzed by enzyme-linked immunosorbent assay. Cell surface expression and transcripts of Sema4D were analyzed in peripheral blood cells from RA patients, and immunohistochemical staining of Sema4D was performed in RA synovium. Generation of sSema4D was evaluated in an ADAMTS-4–treated monocytic cell line (THP-1 cells). The efficacy of anti-Sema4D antibody was evaluated in mice with collagen-induced arthritis (CIA). Results Levels of sSema4D were elevated in both serum and synovial fluid from RA patients, and disease activity markers were correlated with serum sSema4D levels. Sema4D-expressing cells also accumulated in RA synovium. Cell surface levels of Sema4D on CD3+ and CD14+ cells from RA patients were reduced, although levels of Sema4D transcripts were unchanged. In addition, ADAMTS-4 cleaved cell surface Sema4D to generate sSema4D in THP-1 cells. Soluble Sema4D induced tumor necrosis factor α (TNFα) and interleukin-6 (IL-6) production from CD14+ monocytes. IL-6 and TNFα induced ADAMTS-4 expression in synovial cells. Treatment with an anti-Sema4D antibody suppressed arthritis and reduced proinflammatory cytokine production in CIA. Conclusion A positive feedback loop involving sSema4D/IL-6 and TNFα/ADAMTS-4 may contribute to the pathogenesis of RA. The inhibition of arthritis by anti-Sema4D antibody suggests that Sema4D represents a potential therapeutic target for RA.

The effect of keto defects in fluorene units on the performance of organic field-effect transistors (OFETs) was examined based on fluorene end-capped fused bithiophenes (BFTT) and biphenyl end-capped fused bithiophene oligomers (BPTT). The formation of keto defects after various periods of UV illumination in air on BFTT films was confirmed by the increase of the long-wavelength emission at 2.1-2.3 eV in the photoluminescent sPLd spectrum and the generation of a Fourier transfer infrared (FTIR) peak at 1721 cm
-1 , corresponding to the carbonyl stretching mode of the fluorenone moiety. For both BPTT films irradiated in air and BFTT in nitrogen, i.e., a keto-free system, no increase in long-wavelength emission in the PL spectrum, was found and the peak corresponding to the carbonyl stretching mode of the fluorenone moiety was absent in the FTIR spectrum. The threshold voltage, i.e., switch-on voltage, of the OFETs was increased and the field-effect mobility and on-state drain current were rapidly decreased after the formation of ketonic defects, since these defects induce the formation of numerous trap sites in the band gap of the semiconducting conjugated oligomer. The density of trap sites (Nt ) generated after the formation of keto defects was determined using space-charge-limited current spectroscopy. A Nt of around 2.7 ×1015 cm-3 was found for the BFTT film due to the formation of keto defects after 6 h of UV irradiation. [ABSTRACT FROM AUTHOR]- Published

B-cell receptor (BCR) signaling plays a critical role in B-cell activation and humoral immunity. In this study, we discovered a critical function of leucine-rich repeat kinase 1 (LRRK1) in BCR-mediated immune responses. Lrrk1−/− mice exhibited altered B1a-cell development and basal immunoglobulin production. In addition, these mice failed to produce IgG3 antibody in response to T cell–independent type 2 antigen due to defects in IgG3 class-switch recombination. Concomitantly, B cells lacking LRRK1 exhibited a profound defect in proliferation and survival upon BCR stimulation, which correlated with impaired BCR-mediated NF-κB activation and reduced expression of NF-κB target genes including Bcl-xL, cyclin D2, and NFATc1/αA. Furthermore, LRRK1 physically interacted and potently synergized with CARMA1 to enhance NF-κB activation. Our results reveal a critical role of LRRK1 in NF-κB signaling in B cells and the humoral immune response.

We calculated the maximum power conversion efficiency as a function of the optical band gap for organic photovoltaic (PV) cells by assuming that charge separation is accompanied by the energy loss required to dissociate strongly bound charge pairs. The dissociation energy can be estimated from the relationship between the open circuit voltage (VOC) and the optical band gap (Eg). By analyzing the published data on VOC and Eg, the dissociation energy can be estimated. The result could be used as a guide for selecting donor and acceptor materials. The concept of fuzziness, inspired by Zadeh (1965), brings us fruitful results when it is applied to problems in decision making. Recently, problems in fuzzy decision making are getting more complex, and one of the most complex fac tors is dynamics in systems. Dynamical approach to fuzzy decision making has been proposed by Bellman and Zadeh's celebrated paper'Decision-making in a fuzzy environment'(1970). The idea has developed into fuzzy mathemati cal programming and has been applied in many fields including management science, operations research, control theory, engineering, systems analysis, computer science, mathematical finance etc. Dynamic programming, advo cated in Bellmans book'Dynamic programming'(1957), is one of the most powerful tools to deal with dynamics in systems, and Bellman and Zadeh has proposed the optimality principle in fuzzy decision making by (1970) introducing fuzzy dynamic programming. Fuzzy dynamic programming and fuzzy mathematical programming has been making remakable progress after they were given life by Bellman and Zadeh's paper (1970). In this volume, various kinds of dynamics, not only time but also structure of systems, are considered. This volume contains ten reviewed papers, which deal with dynamics in theory and applications and whose topics are poten tially related to dynamics and are expected to develope dynamical study in near future. first, fuzzy dynamic programming is reviewed from a viewpoint of its origin and consider its developement in theory and applications.

Interleukin-6 (IL-6) is a representative cytokine featuring pleiotropic activity and redundancy. A transient synthesis of IL-6 contributes to host defense against infectious agents and tissue injuries by inducing acute phase reactions and immunological and hematopoietic responses. However, uncontrolled persistent production of IL-6 may lead to the development of several immune-mediated diseases. Rheumatoid arthritis (RA) is a chronic disease with joint and systemic inflammation resulting from immunological abnormalities and it has been found that IL-6 plays a key role in the development of this disease. Clinical trials in various parts of the world of tocilizumab, a humanized anti-IL-6 receptor antibody, have proved its efficacy and tolerable safety either as monotherapy or in combination with disease-modifying antirheumatic drugs. As a result, it is currently used as a first-line biologic for the treatment of moderate-to-severe RA in more than 100 countries. Clarification of the mechanism(s) through which tocilizumab exerts its effect on RA and of the reason(s) why IL-6 is continuously produced in RA can be expected to lead to the best use of this agent for RA patients and aid in investigations into the pathogenesis of RA.

Owl monkeys are the only one species possessing the nocturnal lifestyles among the simian monkeys. Their eyes and retinas have been interested associating with the nocturnal adaptation. We examined the cellular specificity and electroretinogram (ERG) reactivity in the retina of the owl monkeys by comparison with the squirrel monkeys, taxonomically close-species and expressing diurnal behavior. Owl monkeys did not have clear structure of the foveal pit by the funduscope, whereas the retinal wholemount specimens indicated a small-condensed spot of the ganglion cells. There were abundant numbers of the rod photoreceptor cells in owl monkeys than those of the squirrel monkeys. However, the owl monkey's retina did not possess superiority for rod cell-reactivity in the scotopic ERG responses. Scanning electron microscopic observation revealed that the rod cells in owl monkey's retina had very small-sized inner and outer segments as compared with squirrel monkeys. Owl monkeys showed typical nocturnal traits such as rod-cell dominance. However, the individual photoreceptor cells seemed to be functionally weak for visual capacity, caused from the morphological immaturity at the inner and outer segments. [ABSTRACT FROM AUTHOR]

We demonstrated the polymer field-effect transistors (FETs) utilizing regioregular poly(3-alkylthiophene)s (P3AT) films prepared by a drawing method. The P3AT film exhibited large optical dichroic ratio, which originated in the polymer backbones aligned to the drawing direction. In-plane anisotropy and enhancement of FET characteristics have been observed that are caused by molecular alignment. In the case of poly(3-dodecylthiophene), the hole mobility along the drawing direction was enhanced by a factor of 25 compared with that of spin-coated film.

A dynamic financial portfolio model with uncertainty is discussed. A dynamic value-at-risk portfolio method is proposed, and the risk criterion is composed by unexpected short-term risks which occur suddenly in each period. Analytical solutions for the VaR portfolio problem are obtained at each period. By dynamic programming approach, we derive an optimality condition for the optimal value-at-risk portfolio in a decision process. It is shown that a dynamic optimal value-atrisk is a solution of the optimality equation under a reasonable assumption, and an optimal trading strategy is obtained from the equation. Zinc phthalocyanine (ZnPc) thin films are vacuum-evaporated on bare indium-tin-oxide (ITO) coated glass by varying substrate temperature and growth rate. The samples are characterized by atomic force microscopy, x-ray diffraction, and infrared spectroscopy. The temperature does not play a clear role in the crystalline growth of ZnPc possibly due to the significant structural defects on ITO surface, while it strongly influences the surface morphology and molecular alignment. The relationships between growth characteristics and performances of photovoltaics with planar heterojunction are discussed in detail. Increasing temperature or growth rate leads to a rougher surface morphology, which enables more donor/accepter interface area for photocurrent generation. Moreover, at elevated temperature, more molecules adopt standing-up geometry, resulting in a reduction in overall efficiency. The results imply that low-temperature process in order to control the molecular alignment is preferred for efficient organic photovoltaics. By simply increasing the growth rate of ZnPc up to 0.40 Å/s at room temperature, ZnPc/C60 planar heterojunction shows an efficiency of 1.66, compared to 1.24 for the cell when ZnPc is prepared at 0.10 Å/s. © 2012 American Institute of Physics.

Background A humanized anti-interleukin-6 receptor, tocilizumab, has been approved as a biological drug for the treatment of rheumatoid arthritis, systemic juvenile idiopathic arthritis and Castleman's disease. Since dysregulation of IL-6 production also plays a pathologic role in other various autoimmune and allergic diseases, we tested whether tocilizumab might have beneficial effect on refractory autoimmune or allergic diseases to conventional treatment regimens. Methods After informed consent by patients and approval by the Ethics Committee of Osaka University Hospital were obtained, patients were treated with tocilizumab at 8 mg/kg every 4 weeks. Results The diseases for which off-label use of tocililizumab was performed included amyloid A amyloidosis, relapsing polychondritis, systemic sclerosis, HLA-B27 positive spondyloarthritis such as reactive arthritis and psoriatic arthritis, polymyalgia rheumatica and polymyositis. After 3 injections of tocilizumab amyloid fibril deposits in the colon disappeared in a patient with gastrointestinal AA amyloidosis, who was resistant to anti-TNF drugs and disease-modifying antirheumatic drugs. In 2 patients with refractory relapsing polychondritis, the continuous tocilizumab treatment for more than 3 years could ameliorate clinical symptoms related to upper and lower airways and stabilize the disease activity. The skin sclerosis of 2 patients with systemic sclerosis became softened with reductions of 52 and 23% in the modified Rodnan total skin score by the tocilizumab treatment. Two administrations of tocilizumab led to the disappearance of joint swelling, pain and complete resolution of symptoms in a patient with refractory reactive arthritis to several therapeutic regimens for 4 years, whereas 2 patients with severe psoriatic arthritis did hardly respond to tocilizumab. In a patient with polymyalgia rheumatica, the tocilizumab treatment caused a reduction of the disease activity score (PMR-AS) from 22.14 to 0.74, indicating remission. Creatine phosphokinase normalized by 2 patients with polymyositis who had been resistant to corticosteroids and immunosuppressive drugs, in association with the disappearance of the high intensity zones in the thigh muscles on MR images. Conclusions These clinical effects of tocilizumab suggest that it may be an optional treatment for refractory autoimmune or allergic diseases although further clinical trails will be essential.

We report the fabrication of high quality thin films for semiconducting organic donor-acceptor charge-transfer (CT) compounds, (diC
8 BTBT)(Fn TCNQ) (diC8 BTBT = 2,7-dioctyl[1]benzothieno[3,2-b][1]benzothiophene and Fn TCNQ [n = 0,2,4] = fluorinated derivatives of 7,7,8,8,-tetracyanoquinodimethane), which have a high degree of layered crystallinity. Single-phase and uniaxially oriented polycrystalline thin films of the compounds were obtained by co-evaporation of the component donor and acceptor molecules. Organic thin-film transistors (OTFTs) fabricated with the compound films exhibited n-type field-effect characteristics, showing a mobility of 6.9 × 10-2 cm²/V s, an on/off ratio of 106 , a sub-threshold swing of 0.8 V/dec, and an excellent stability in air. Uniaxially oriented films of beta-phase poly(9,9-dioctylfluorene) (PFO) were realized by a friction-transfer technique followed by thermal annealing and vapor treatments. Absorption and photoluminescence (PL) spectra show the characteristics of beta-phase: an additional absorption peak at 433 nm and redshifted PL peaks compared with those of the usual nematic (N) phase. We fabricated polarized polymer light-emitting diodes utilizing oriented films of beta-phase PFO. Highly polarized beta-phase emission with an integrated polarization ratio of 51 was observed from the devices. The efficiency of the devices based on beta-phase reached 2.0 cd/A, which is two times higher than that based on N-phase.

Polarized polymer light-emitting diodes (PLEDs) have been constructed utilizing friction-transferred poly(9,9-dioctylfluorene) (PFO) thin films. The friction transfer technique allows oriented PFO to be deposited directly onto an indium tin oxide anode without an alignment layer such as polyimide. Polarized absorption and photoluminescence spectra revealed that the polymer backbones are highly aligned in the friction direction. We fabricated PLEDs consisting of friction-transferred PFO as an emissive layer, vacuum-deposited bathocuproine as an electron transport and hole-blocking layer, and a vacuum-deposited LiF/Al cathode. Highly polarized blue emission with an integrated polarization ratio of 31 and a luminance of up to 300 cd/m2 was observed from the PLEDs.

Crystallization of amorphous poly(lactic acid) (PLA) was investigated in various organic solvents, such as acetone, ethylacetate, diethylether, tetrahydrofurane, methanol, hexane, toluene, xylene, and o‐dichlorobenene. Most of the solvents, except hexane, induced crystallization of amorphous PLA. Acetone was the most effective solvent to accelerate the crystallization among the solvents used. The crystallization was induced by permeation of acetone into the amorphous phase of PLA, and the permeation obeyed Fick type diffusion. The crystallization rate increased with increasing of conducting temperature. Crystallized PLA formed α crystalline structure. The permeated acetone in the crystallized PLA gradually evaporated as time passes, and the elimination of acetone affected thermal and mechanical properties of the crystallized PLA. © 2010 Wiley Periodicals, Inc.
