Anterior cruciate ligament (ACL) tears are injuries with a high incidence in athletes, and ACL reconstruction (ACLR) is a surgical treatment orthopedists perform. This study aims to compare the 6 and 12 months post-op results of single leg hop tests (SLHT) performed in multidirectional conventional semitendinosus/gracilis (ST/G) and modified all inside (MAI) ACLR techniques on both sides and the limb symmetry indexes (LSI) of both techniques. This study evaluated a retrospective cohort of 50 male athletes who applied MAI (n = 23) and traditional ACLR ST/G (n = 27) techniques. Functional knee strength of the participants on both sides was measured with different SLHTs at 6 and 12 months postoperatively. The SLHT included medial side (MSTH), triple hop (TH), medial rotation (90°) (MRH), crossover triple (CH) ve the single hop (SH) hop for distance. Both ACLR techniques showed significant improvement in mean Tegner, International Knee Documentation Committee (IKDC) and Lysholm scores preoperatively and at 6 and 12 months postoperatively (p < 0.05). For MAI and ST/G techniques, there was no significant difference in the results of SLHTs at 6 and 12 months for both the operated and non-operated sides (p < 0.05). Significance was found only in the MRH test of the non-operated side (p < 0.05). There was no significant difference in SLHT scores between the operated and non-operated sides at 6 and 12 months postoperatively (p > 0.05). There was no difference in LSI scores between techniques (p < 0.05). Our study revealed similar LSI rates in ST/G and MAI techniques at 6 and 12 months, suggesting that MAI technique can be used as a functional ACLR technique for athletes.

Aromatic L-amino acid decarboxylase (AADC) deficiency is a disease in which neurological findings are dominant due to deficiencies in neurotransmitter synthesis. Hypoglycemia caused by autonomic dysfunction is one of the symptoms that may be encountered. Here we report a case of mild AADC deficiency presenting with hypoglycemia without any neurological signs. A 4-year-old girl presented with recurrent hypoglycemia. Her growth and development were normal. Plasma insulin and cortisol values were normal in the sample at the time of hypoglycemia. C8: 1-Carnitine elevation was detected in the acylcarnitine profile. A clinical exome panel was performed with the suggestion of a fatty acid oxidation defect. However, a homozygous variant in the DDC gene was detected. Furthermore, cerebrospinal fluid neurotransmitter analysis revealed low 5-hydroxyindolacetic acid and homovanillic acid and high 3-O-methyl-dopa and methyltetrahydrofolate (5 MTHF) consistent with AADC deficiency. Plasma AADC enzyme activity was low. The episodes of hypoglycemia were treated with uncooked cornstarch. This case suggests that AADC deficiency should be considered in some patients with hypoglycemia.

Objective: To reveal the profile and practice in patients with acute stroke who received intravenous tissue plasminogen activator (IV tPA) and/or neuro-interventional therapy in Türkiye. Materials and Methods: On World Stroke Awareness Day, May 10, 2018, 1,790 patients hospitalized in 87 neurology units spread over 30 health regions were evaluated retrospectively and prospectively. Results: Intravenous tPA was administered to 12% of 859 cases of acute ischemic stroke in 45 units participating in the study. In the same period, 8.3% of the cases received neurointerventional treatment. The rate of good prognosis [modified Rankin score (mRS) 0–2] at discharge was 46% in 83 patients who received only IV tPA [age: 67 +- 12 years; National Institutes of Health Stroke Scale (NIHSS): 12 +- 6; hospital stay, 24 +- 29 days]; 35% in 51 patients who underwent thrombectomy (MT) alone (age: 64 +- 13 years; NIHSS: 14.1 +- 6.5; length of hospital stay, 33 +- 31 days), 19% in those who received combined treatment (age: 66 +- 14 years; NIHSS: 15.6 +- 5.4; length of hospital stay, 26 +- 35 days), and 56% of 695 patients who did not receive treatment for revascularization (age: 70 +- 13 years; NIHSS: 7.6 +- 7.2; length of hospital stay, 21 +- 28 days). The symptom-to-door time was 87 +- 53 minutes in the IV treatment group and 200 +- 26 minutes in the neurointerventional group. The average door-to-needle time was 66 +- 49 minutes in the IV tPA group. In the neurothrombectomy group, the door-to-groin time was 103 +- 90 minutes, and the TICI 2b-3 rate was 70.3%. In 103 patients who received IV tPA, the discharge mRS 0–2 was 41%, while the rate of mRS 0–1 was 28%. In 71 patients who underwent neurothrombectomy, the mRS 0–2 was 31% and mRS 0–1 was 18%. The door-to-groin time was approximately 30 minutes longer if IV tPA was received (125 +- 107 and 95 +- 83 minutes, respectively). Symptomatic bleeding rates were 4.8% in IV recipients, 17.6% among those who received only MT, and 15% in combined therapy. Globally, the hemorrhage rate was 6.8% in patients receiving IV tPA and 16.9% in MT. Conclusion: IV thrombolytic and neurointerventional treatment applications in acute ischemic stroke in Türkiye can provide the anticipated results. Heterogeneity has begun to be reduced in our country with the dissemination of the system indicated by the 'Directive on Health Services to be Provided to Patients with Acute Stroke.'

Objective: Atrial fibrillation (AF) is the most common directly preventable cause of ischemic stroke. There is no dependable neurology-based data on the spectrum of stroke caused by AF in our country. Within the scope of NeuroTek-Turkey, hospital-based data on acute stroke patients with AF were collected to contribute to the creation of acute stroke algorithms. Materials and Methods: Atrial fibrillation (AF) is the most common directly preventable cause of ischemic stroke. There is no dependable neurology-based data on the spectrum of stroke caused by AF in Türkiye. Within the scope of NöroTek-Türkiye (TR), hospital-based data on acute stroke patients with AF were collected to contribute to the creation of acute-stroke algorithms. Results: The rate of AF in patients hospitalized for ischemic stroke/TIA was 29.8%, of which 65% were known before stroke, 5% were paroxysmal, and 30% were diagnosed after hospital admission. The proportion of patients with AF who received 'effective' treatment [international normalization ratio ≥2.0 warfarin or non-vitamin K antagonist oral anticoagulants (NOACs) at a guideline dose] was 25.3%, and, either no medication or only antiplatelet was used in 42.5% of the cases. The low dose rate was 50% in 42 patients who had a stroke while taking NOACs. Anticoagulant was prescribed to the patient at discharge at a rate of 94.6%; low molecular weight or unfractionated heparin was prescribed in 28.1%, warfarin in 32.5%, and NOACs in 31%. The dose was in the low category in 22% of the cases discharged with NOACs, and half of the cases, who received NOACs at admission, were discharged with the same drug. Conclusion: NöroTekTR revealed the high but expected frequency of AF in acute stroke in Türkiye, as well as the aspects that could be improved in the management of secondary prophylaxis. AF is found in approximately one-third of hospitalized acute stroke cases in Türkiye. Effective anticoagulant therapy was not used in three-quarters of acute stroke cases with known AF. In AF, heparin, warfarin, and NOACs are planned at a similar frequency (one-third) within the scope of stroke secondary prophylaxis, and the prescribed NOAC dose is subtherapeutic in a quarter of the cases. Non-medical and medical education appears necessary to prevent stroke caused by AF.

Yaşadığımız dünya, kişi için hem cennet hem de cehennem olabilir. Ölen kişinin ruhunun tekrardan dünyaya gelmek suretiyle önceki yaşamında yaptıklarının karşılığı olarak bir hayat süreceği inancına sahiptirler. Asıl olan, kişinin mümin bir Nusayrî olarak yaşamını sürdürmesi ve nihayetinde nurlar âleminde nûrânî bir varlığa dönüşmesidir. Bu mertebeye ulaşan Nusayrî naîm cennetine gidecek ve böylelikle mükâfatlandırılmış olacaktır. Mezhebin temel inanç esaslarından biri olan tenâsüh hakkında Nusayrîler, bu konunun Kur’an-ı Kerim’de geçtiğini, haliyle İslâm’a aykırı olmadığını iddia etmektedirler. Biz de bu çalışmada Nusayrîlerin tenâsüh inancı hakkındaki görüşlerini betimleyici bir bakış açısıyla ele alacağız. Herhangi bir değer yargısında bulunmadan ahiret inancı, cennet ve cehennem konusu ile reenkarnasyon hakkında fikirlerini tarafsız bir bakış açısıyla inceleyeceğiz.

The purpose of this study was to evaluate the relationship between the presence of pneumonia and blood parameters in cases of Coronavirus disease (COVID-19) and to examine their predictive characteristics in terms of pneumonia. We reviewed the file records of 151 pediatric patients with a diagnosis of COVID-19 confirmed by the real time-reverse transcription polymerase chain reaction test in nasopharyngeal swabs. The patients were divided into two groups based on direct chest X-ray and computed tomography results in [Group 1 (n:41), with pneumonia findings, and Group 2 (n:110), with no pneumonia findings]. The groups’ demographic data, clinical and laboratory findings were compared. Pulmonary involvement was determined in 41 (27.1%) of the 151 patients. The [body mass index (BMI) Z-score], red blood cell distribution width (RDW), mean platelet volume (MPV), neutrophil lymphocyte ratio, passive leg raise, and D-dimer levels were significantly higher in patients with pneumonia than those without pneumonia in our study. Based on multivariate logistic regression analysis, BMI Z-score, MPV, and RDW were found to be independent risk factors of pneumonia in patients. The current study showed higher levels of blood parameters in patients with coronavirus disease 2019 (COVID -19) presenting with pneumonia than those without pneumonia. We suggest that BMI-Z score and MPV value may assist in predicting pulmonary involvement in patients with COVID-19.

Aim:During the coronavirus disease-2019 (COVID-19) pandemic, visiting the hospital and getting regular infusions can be difficult for patients with chronic illnesses. Telemedicine may offer a good option for the management of chronic diseases such as lysosomal storage diseases (LSD).Materials and Methods:LSD patients at the Unit of Metabolic Diseases of Ege University were contacted by phone between April, 2020 and March, 2021 during the COVID-19 pandemic. Telemedicine appointments were performed at intervals every month or three months, depending on the patients’ compliance with their treatment.Results:Ninety-two LSD patients [Mucopolysaccharidosis (MPS) I, MPS II, MPS IVA, MPS VI, MPS VII, Gaucher, Fabry, and Pompe] were included in this study. The total skipped treatment rate within one year was 17.1%. Most of the months of interruption were consonant with the time of social isolation. The treatment interruption in patients under 18 years was lower than in patients over 18 years. A positive correlation was detected between the age of patients and the interruption of treatment.Conclusion:The curfew periods might be one of the causes of missed treatment sessions. Telemedicine is a good method to improve the continuity of treatment. This study showed that the number of interrupted enzyme replacement treatments could be decreased via ongoing telemedicine appointments.

Objective: Nutritional status assessment, dysphagia evaluation and enteral feeding decision are important determinants of prognosis in acute neurovascular diseases. Materials and Methods: NöroTek is a point prevalence study conducted with the participation of 87 hospitals spread across all health sub regions of Turkey conducted on 10-May-2018 (World Stroke Awareness Day). A total of 972 hospitalized neurovascular patients [female: 53%, age: 69+-14; acute ischemic stroke in 845; intracerebral hematoma (ICH) in 119 and post-resuscitation encephalopathy (PRE) in 8] with complete data were included in this sub-study. Results: Gastrostomy was inserted in 10.7% of the patients with ischemic stroke, 10.1% of the patients with ICH and in 50% of the patients with PRE. Independent predictors of percutaneous endoscopic gastrostomy (PEG) administration were The National Institutes of Health Stroke Scale score at admission [exp (β): 1.09 95% confidence interval (CI): 1.05-1.14, per point] in ischemic stroke; and mechanical ventilation in ischemic [exp (β): 6.18 (95% CI: 3.16-12.09)] and hemorrhagic strokes [exp (β): 26.48 (95% CI: 1.36-515.8)]. PEG was found to be a significant negative indicator of favorable (modified Rankin's scale score 0-2) functional outcome [exp (β): 0.032 (95% CI: 0.004-0.251)] but not of in-hospital mortality [exp (β): 1.731 (95% CI: 0.785-3.829)]. Nutritional and swallowing assessments were performed in approximately two-thirds of patients. Of the nutritional assessments 69% and 76% of dysphagia assessments were completed within the first 2 days. Tube feeding was performed in 39% of the patients. In 83.5% of them, tube was inserted in the first 2 days; 28% of the patients with feeding tube had PEG later. Conclusion: The NöroTek study provided the first reliable and large-scale data on key quality metrics of nutrition practice in acute stroke in Turkey. In terms of being economical and accurate it makes sense to use the point prevalence method.

Aim: We aimed to reveal the similarities and differences between the rare and severe multisystem inflammatory syndrome (MIS-C) and active familial Mediterranean fever (FMF) disease in children. Our study may help in the early recognition of MIS-C syndrome in children and distinguish it from other diseases with similar symptoms. Methods: We evaluated the demographic and clinical characteristics, laboratory findings, treatments and outcomes of patients with MIS-C syndrome and active FMF. Results: The clinical and laboratory findings of a total of 66 patients hospitalized in our pediatric clinic with the diagnosis of active FMF (n: 42) and MIS-C syndrome (n: 24) were reviewed retrospectively. The reason for pediatric emergency admission was determined as resistant fever in all patients. When the clinical findings of the patients were compared, it was determined that joint and abdominal pain in the FMF group and vomiting, rash, cough, Lenfadenopati (LAP) and myalgia findings in the MIS-C group were statistically significantly higher (p

In this study, ground wastes of Nettle (Urtica dioica L.) fiber (GWN), which contains cellulose of 52.2 wt%, hemicellulose of 28.9 wt%, and lignin of 18.9 wt%, filled polypropylene (PP) composites were fabricated by using a high-speed kinetic mixer. The effect of filling GWN into PP at different weight fractions on density, crystallization behavior, coefficient of thermal expansion, and thermal stability was investigated. In addition, tensile and flexural tests were performed to determine how the mechanical properties of PP were affected. Morphological observations were conducted using scanning electron microscopy (SEM). The flexural strength values of PP increased by 10%, 15%, and 31% with the addition of 7 wt%, 14 wt%, and 21 wt% GWN, respectively. However tensile strength value of PP decreased slightly with the addition of GWN. 21 wt% GWN addition into PP led to a considerable decrease (about 29%) in the thermal expansion coefficient of PP. The degree of crystallinity of PP was not affected by the addition of GWN.

Objective: Coronavirus 2019 disease (COVID-19), the cause of the ongoing pandemic, is an acute respiratory tract infection, which has made it necessary for healthcare personnel to use protective equipment such as N95 masks, protective goggles and visors. External compression headaches caused by the compression of the pericranial soft tissues by wearing hats, helmets, or goggles (worn during swimming or diving) have been previously described. In our study, we aimed to evaluate the presence and characteristics of personal protective equipment–associated headaches in healthcare workers during the COVID-19 pandemic period and to determine the effects of such headaches. Materials and methods: A face-to-face questionnaire was delivered to 300 male and female healthcare personnel between the ages of 18–56 working in healthcare units where COVID-19 patients were evaluated and treated. The data from 296 completed questionnaires was evaluated. Results: We included 296 participants (166 females and 130 males) in our study; the mean age was 33.98 ± 8.52 years (range 18–56). One hundred ninety-six (66.22%) participants indicated that they had new onset personal protective equipment–associated headaches. Objective:To compare the demographic characteristics, vascular risk factors, stroke subtypes and early prognosis of patients with end-stage renal disease (ESRD)/normal renal function (NRF) with acute ischemic stroke (IS).Methods:Demographic data and vascular risk factors of patients with acute stroke between August 1 2013-April 1 2015 were determined. Patients diagnosed with ESRD who were receiving dialysis treatment with a glomerular filtration rate below 15, were included in the study. Stroke severity was determined by using the National Stroke Health Stroke scale. To determine the types of IS, “Trial of Org 10172 in Acute Stroke Treatment” TOAST criteria were used. Deaths during hospitalization were used to determine early mortality.Results:Of the 132 patients included in the study, 33.3% had ESRD and 66.6% had normal renal functions (NRF). Demographic characteristics and risk factors did not differ between ESRD and NRF groups. When stroke types were compared, it was determined that stroke with undetermined etiology with more than one etiology was higher in ESRD patients. Of all patients 10.6% died in hospital and this rate was 20.5% in ESRD patients. In the regression analysis, the presence of ESRD increased hospital mortality significantly (risk ratio: 11.6, 95% confidence interval 1.7-77, p=0.011).Conclusion:Our results showed that more than one etiology was seen in acute IS patients with ESRD and ESRD significantly increased the risk of hospital mortality.

Purpose: This study aimed to investigate the effect of glide path preparation before shaping with reciprocating single-file, rotary single-file and multiple-file systems on the debris extrusion from the apical, and on the root canal transportation. Materials and Methods: One hundred twenty curved mesial root canals of mandibular first molars were randomly distributed to six groups (n=20). The root canals were prepared with Reciproc in Group R, OneShape in Group OS, and ProTaper Next in Group PN. In the last three groups (RG, OSG and PNG), a glide path was created before the shaping instruments used in the first 3 groups. The pre- and post-preparation weight were measured by using a 10-5 microbalance. Pre- and post-preparation of the root canals were scanned by using cone-beam computed tomography (CBCT). Results: The debris extrusion in RG and OSG was significantly higher than R and OS, respectively. However, no significant differences were found between PN and PNG. Considering the root canal transportation, RG showed lower than R for 5 mm and 7 mm levels and the OSG group showed lower than OS for all levels. Creating a glide path significantly increased the shaping time for OS and PN groups. Conclusion: In advance of shaping with a single-file system in curved canals, creating a glide path preserves canal anatomy. However, it may lead to increase apical debris extrusion.

The study aimed to determine the nutritional composition of vegan recipes shared on the Instagram social media platform and to compare these ingredients with their non‐vegan/omnivorous versions. Turkish and English vegan recipes of meatballs (n = 53), burgers (n = 12), pizza (n = 15), pasta (n = 38), cake (n = 58), and cookies (n = 52) were obtained from Instagram. They all met the criteria of being shared as #vegan labeled, presented detailed information about ingredients, and only included plant‐based ingredients. Afterward, non‐vegan/omnivorous recipes (n = 228) were determined as equivalent to the vegan recipes in each food group, and a total of 456 recipes were evaluated. The amounts of macro‐ and micronutrients per 100‐gram serving were obtained by the Nutritional Data System (BeBiS). According to the outcomes, the most significant differences in nutritional composition were found between vegan and non‐vegan meatball recipes. The levels of cholesterol and B12 were significantly lower in English vegan recipes of meatballs, pasta, cake, and cookies compared to their non‐vegan versions (p < .05). Consuming different vegan foods throughout the day could provide complementary nutrient intake and sustainable optimal health. Nevertheless, recipe‐based updates could be an innovative approach in future vegan studies. In addition, analysis of vegan recipes could be considered to modulate vegan supplementation programs. With the aim of producing a 3D representation of tumors, imaging and molecular annotation of xenografts and tumors (IMAXT) uses a large variety of modalities in order to acquire tumor samples and produce a map of every cell in the tumor and its host environment. With the large volume and variety of data produced in the project, we developed automatic data workflows and analysis pipelines. We introduce a research methodology where scientists connect to a cloud environment to perform analysis close to where data are located, instead of bringing data to their local computers. Here, we present the data and analysis infrastructure, discuss the unique computational challenges and describe the analysis chains developed and deployed to generate molecularly annotated tumor models. Registration is achieved by use of a novel technique involving spherical fiducial marks that are visible in all imaging modalities used within IMAXT. The automatic pipelines are highly optimized and allow to obtain processed datasets several times quicker than current solutions narrowing the gap between data acquisition and scientific exploitation.

The study was carried out to the effects of different plant densities and nitrogen levels on okra yield and plant development. Field trials were conducted in two locations, the training fields of the Ege University, Ödemiş Research and Training area, and Ege University Faculty of Agriculture, Menemen Research and Training Farm. The field layout was a split-plot design with 3 replicates, where main plots consisted of nitrogen applications and sub-plots of plant densities. Fertilizer was applied in the form of Triple Superphosphate, Potassium Sulfate, Urea and Ammonium Nitrate. Five different nitrogen levels (F1: 0, F2: 40, F3: 80; F4: 120 and F5: 160 kg N ha-1) and two different rows spacing (PD1: 15 cm×70 cm and PD2: 25 cm×70 cm) have been tested. Yield and yield components, plant height at flowering, height of the first fruits, the number of leaves, the number of branches, the number of days from sowing to first flowering, the number of days from sowing to first harvest, plant height, foliation status, total yield, yield of plant, average fruit weight, fruit length, fruit diameter have been noted in the field experiments. Total yield ranged from 5923.8 to 12888.9 kg ha-1 at the Ödemiş location, while it was 3363.3-5009.2 kg ha-1 at the Menemen location. In conclusion, the highest yield is taken from F2 in both plant densities in Ödemiş. In Menemen, the highest yield is taken from the dose of F3 (80 kg Nha-1) and F4 (120 kg Nha-1).

Lacosamide (LCM) has both frequent side effects, such as headache, diplopia, and nausea, and rare side effects such as depression, mood changes, and confusion. This article presents a case of possible LCM-induced psychosis and reviews the literature. A 22-year-old woman presented to the Emergency Department in Status Epilepticus. The seizure was treated with intravenous midazolam and valproic acid. However, she developed cardiopulmonary arrest and was resuscitated successfully. She was intubated and admitted to the intensive care unit. She had first been diagnosed with epilepsy 15 years earlier and had been seizure-free for 3 years. She had undergone a sleeve gastrectomy 1 year earlier. She was extubated the day after admission. LCM 200 mg/day was added to the treatment because of generalized seizures beginning in the right arm. Her seizures were controlled on the 4th hospital day. However, after starting LCM, she developed agitation, visual hallucinations with a sexual content, and intellectual delusions. Therefore, the LCM was discontinued and her psychosis resolved completely 4 days later. On the 12th hospital day, she was discharged. LCM blocks sodium channels and can act as a mood stabilizer and sedative. However, our patient developed psychosis with LCM treatment. This is the first reported case of LCM-induced psychosis in Turkey. Visual hallucinations and psychosis may develop immediately after starting LCM therapy. Clinical recovery can be achieved by discontinuing the drug.

Purpose: The aim of this study was to determine the levels of irisin in the heart, which is a vital organ, in rats of different ages and to reveal whether Ferula elaeochytris (FE) has an effect on the expression of the irisin. Materials and Methods: Our study was designed in six groups, young (4 months), middle age (12 months), old (24 months) rats which were given FE extract for 8 weeks and their control groups. After that their heart tissues were taken and irisin levels were measured by immunohistochemical staining method. Results: The obtained results were analyzed by performing histoscoring. It was observed that the level of irisin in old rats was statistically significantly lower in young and middle age rats. In addition, irisin level of in all groups administered with FE extract was found significantly increased. Conclusion: FE extract increases the level of irisin through other pathways besides its protective effect against tissue damage.

Purpose: To discern whether reduced infection rates were attributed to antiseptic solutions or mechanical rectal irrigation. Patients and Methods: After receiving ethical approval, the study included patients who underwent transrectal prostate biopsy due to elevated PSA or abnormal digital rectal examination findings, and prostate cancer under active surveillance, at Tepecik Training and Research Hospital between April 2022 and June 2023. Standard antibiotic prophylaxis was administered. Patients were randomized into three equal groups according to the rectal irrigation strategy. Results: Overall complications occurred in 4%. Despite distinct cleaning agents, there was no significant difference in infection rates (p = 0.780) or fever incidence (p = 0.776). Pathological analyses revealed comparable outcomes (p = 0.764). Conclusion: The study challenges the prevailing belief that antiseptic solutions are indispensable for infection prevention, as saline demonstrated similar efficacy. Limitations include data gaps from potential external hospital visits and absent rectal microorganism swab culture. While TRUS-PB remains the gold standard, this study suggests that mechanically cleansing the rectal mucosa with saline—a cost-effective, side-effect-free alternative—may be a viable infection prevention method, particularly beneficial for patients with antiseptic allergies. The findings prompt a reconsideration of the necessity of antiseptic solutions in TRUS-PB, offering an alternative approach to mitigate infectious complications. [ABSTRACT FROM AUTHOR]
