Dabrafenib plus trametinib is the standard treatment for BRAF V600E‐mutated non‐small cell lung cancer. No treatment‐related cerebral infarction (CI) has been reported in previous clinical trials. Here, we described a 61‐year‐old Japanese man with BRAF V600E‐mutated lung adenocarcinoma treated with dabrafenib plus trametinib as a third‐line treatment. On the 10th day of dabrafenib plus trametinib treatment, the patient developed fever and was urgently hospitalized on the 18th day owing to impaired consciousness. The patient had disseminated intravascular coagulation because of infection, was treated with thrombomodulin and ceftriaxone, and subsequently improved. On the 44th day, dabrafenib plus trametinib was resumed with a one‐step dose reduction. Three hours after the first oral administration, the patient developed chills, fever, and hypotension. He received intravenous fluids. On the 64th day, 20 mg prednisolone was administered from the previous day, and dabrafenib plus trametinib was resumed with a further one‐step reduction in dose. Five hours after the first oral administration, the patient developed fever, hypotension, paralysis of the right upper and lower limbs, and dysarthria appeared. Head magnetic resonance imaging revealed multiple cerebral infarcts. Hemoconcentration because of intravascular dehydration may have caused CI. Authors tried to braze magnesium alloy in air using no flux with the aid of ultrasonic vibration, and investigated the effect of brazing conditions on the joint properties. The main results obtained in this study are as follows. Applying ultrasonic vibration made it possible to braze the magnesium alloy in air without flux and the joint strength was so high that the joint fractured partially in the base metal. The brazing temperature at which solid and liquid phases coexisted in filler metal could provide the brazed joint with the maximum tensile strength. This seemed to be because the liquid phase in the filler metal infiltrated into the cracks occurring in the oxide film on the faying surface during heating and the solid phase would rub against the oxide film to detach, resulted in removing the oxide film from the faying surface. The optimal time for applying ultrasonic vibration could effectually detach and remove the oxide film from the faying surface. The excessive applying time of ultrasonic vibration caused defects such as cavity formation in the brazed layer and led to decrease the joint strength. [ABSTRACT FROM PUBLISHER]

The authors ultrasonically welded A6061 aluminium alloy sheet using two types of weld tips with different contact face geometries, and investigated the effect of the weld tip on the performance and interface structure of the welds. One type of tip has a cylindrical contact face without knurl, which is called a C-tip in this study. The other type of tip has a flat contact face with knurl, which is a called K-tip in this study. The following main results were obtained. The strength of the joints welded using the C-tip was higher than that welded using the K-tip and the fluctuation in joint strength with the C-tip was smaller. The C-tip could stably produce the higher strength joint. Using the K-tip, the knurl indentations were made on the workpiece surface due to the pyramidal projections on the weld tip, and the indentation size expanded with welding time, resulting in the deterioration of the joint property. On the other hand, the indentation made on a workpiece surface by using the C-tip showed a distinctive shape like an ellipse, elongated perpendicular to the ultrasonic vibration, and the indentation grew with welding time. Using the K-tip, unbonded regions remained at the weld interface due to the concavity on the weld tip face. In the cross-sectional structure parallel to the workpiece width of the joint welded using the C-type tip, a distinctive feature was observed that the faying surface of the anvil workpiece was mixed with that of the sonotrode workpiece by intense plastic deformation and a horn-like protuberance intruding into the sonotrode workpiece was formed at the periphery of the welded area. In order to produce a high strength brazed joint of A5056 aluminium alloy containing magnesium of about 5 mass%, the authors applied a flux-free brazing method with the aid of ultrasonic vibration to the aluminium alloy by selecting pure Ag foil as brazing filler metal and examined the effect of brazing conditions on the joint properties. The main results obtained in this study are as follows. At a brazing temperature of 570°C, just above the eutectic point of Al-Ag binary system, application of ultrasonic vibration for 4.0 s provided the brazed joint with the maximum tensile strength and the strength decreased with the application time. When the brazing temperature was varied from 550 to 580°C and the application time of ultrasonic vibration was kept constant at 4.0 s, the joint brazed at 560°C attained the maximum tensile strength and fractured in the base metal. It was found that using a pure Ag foil as brazing filler metal successfully brazed A5056 aluminium alloy and the joint strength was equivalent to that of the base metal. Fracture of the joint was prone to occur along the (Al3Mg2 + Al solid solution) phase with high hardness formed at the grain boundary of the base metal. The amount of the hard (Al3Mg2 + Al solid solution) phase increased with the ultrasonic application time and the brazing temperature. It seemed that the increase of the hard (Al3Mg2 + Al solid solution) phase mainly caused the brazed joint strength to decrease. [ABSTRACT FROM AUTHOR]

• Ni/TiO 2 catalysts were effective for the reduction of TFA levels. • TOF for Ni/TiO 2 was greater than those for other Ni catalysts. • The NiO monolayer was observed on the surface of TiO 2. • The NiO monolayer was reduced to Ni nanoparticles under a reductive condition. • The Ni nanoparticles contribute to increasing TOF and to reducing the TFA levels. The effects of the support on the catalytic performance of supported Ni catalysts for the hydrogenation of soybean oil were examined. The turnover frequency (TOF) for Ni/TiO 2 was greater than those for other Ni catalysts. Among the examined Ni catalysts, the Ni/TiO 2 catalysts were effective for the reduction of trans fatty acid (TFA) levels (minimum 10.5%) in hydrogenated oils at an iodine value (IV) of 70, independent of the difference in the crystalline structure of TiO 2. In addition, the oils hydrogenated by Ni/TiO 2 had suitable properties for feedstock of margarine and vegetable shortening. The highly dispersed Ni nanoparticles formed by reduction of the NiO monolayer on the surface of TiO 2 contribute to increasing the catalytic activity and to reducing the TFA levels. Among nontuberculous mycobacterial pulmonary diseases (NTM-PDs), Mycobacterium abscessus species pulmonary disease (MABS-PD) is one of the most severe and intractable infections. We herein report a 45-year-old woman with advanced lymphangioleiomyomatosis (LAM) who developed MABS-PD while undergoing sirolimus therapy. MABS-PD was immediately controlled using antibiotic therapy, although the patient's lung transplant registration was significantly delayed. To our knowledge, this is the first case report on the development of NTM-PD in a patient with LAM before lung transplantation. This case suggests that the early diagnosis and optimal treatment of NTM-PD are crucial in patients with advanced LAM.

A 39-year-old vegan man was admitted with diabetic ketoacidosis. He had also developed pneumonia that was unresponsive to antibiotics. Based on bronchoscopy findings, the diagnosis of Candida pneumonia was made, and the pulmonary shadow disappeared rapidly after antifungal therapy. Candida pneumonia has been mostly reported in severely immunocompromised patients. This is a rare case of Candida pneumonia that was found in a young vegan man with diabetes mellitus (DM). Although malnutrition caused by DM or an unbalanced diet is often underestimated as a cause of immunodeficiency, these conditions can be risk factors for serious opportunistic infections, including Candida pneumonia.

Autoimmune pulmonary alveolar proteinosis (APAP) is caused by macrophage dysfunction owing to the presence of anti-granulocyte-macrophage colony-stimulating factor (GM-CSF) autoantibodies. A 77-year-old man with APAP was referred to our hospital for whole-lung lavage (WLL) due to oxygenation exacerbation and pulmonary shadows. The patient had had coronavirus disease 2019 (COVID-19) during the APAP evaluation before WLL. About three months after COVID-19 resolved, his oxygenation and shadow reflecting APAP had obviously improved, thus avoiding the need for WLL. We suspected that the improvement in APAP was due to various immunological reactions induced by COVID-19.

A 50-year-old woman who had previously undergone right hemicolectomy and chemotherapy for colorectal cancer was hospitalized with respiratory failure. Chest computed tomography showed reticulonodular opacities and enlarged lymph nodes. A transbronchial biopsy revealed adenocarcinoma with epidermal growth factor receptor (EGFR) mutations T790M and L861Q. Treatment with the EGFR-tyrosine kinase inhibitor (TKI) osimertinib was started, and she achieved a partial response. We diagnosed her with metastatic lung tumors from colorectal cancer based on additional immunohistochemical staining and the EGFR mutation status (L861Q) of the specimens. Although cases with EGFR mutations have been rarely reported, an EGFR-TKI can be an effective treatment option for colorectal cancer.
