Importance: Evidence regarding corticosteroid use for severe coronavirus disease 2019 (COVID-19) is limited.Objective: To determine whether hydrocortisone improves outcome for patients with severe COVID-19.Design, Setting, and Participants: An ongoing adaptive platform trial testing multiple interventions within multiple therapeutic domains, for example, antiviral agents, corticosteroids, or immunoglobulin. Between March 9 and June 17, 2020, 614 adult patients with suspected or confirmed COVID-19 were enrolled and randomized within at least 1 domain following admission to an intensive care unit (ICU) for respiratory or cardiovascular organ support at 121 sites in 8 countries. Of these, 403 were randomized to open-label interventions within the corticosteroid domain. The domain was halted after results from another trial were released. Follow-up ended August 12, 2020.Interventions: The corticosteroid domain randomized participants to a fixed 7-day course of intravenous hydrocortisone (50 mg or 100 mg every 6 hours) (n = 143), a shock-dependent course (50 mg every 6 hours when shock was clinically evident) (n = 152), or no hydrocortisone (n = 108).Main Outcomes and Measures: The primary end point was organ support-free days (days alive and free of ICU-based respiratory or cardiovascular support) within 21 days, where patients who died were assigned -1 day. The primary analysis was a bayesian cumulative logistic model that included all patients enrolled with severe COVID-19, adjusting for age, sex, site, region, time, assignment to interventions within other domains, and domain and intervention eligibility. Superiority was defined as the posterior probability of an odds ratio greater than 1 (threshold for trial conclusion of superiority >99%).Results: After excluding 19 participants who withdrew consent, there were 384 patients (mean age, 60 years; 29% female) randomized to the fixed-dose (n = 137), shock-dependent (n = 146), and no (n = 101) hydrocortisone groups; 379 (99%) completed the study and were included in the analysis. The mean age for the 3 groups ranged between 59.5 and 60.4 years; most patients were male (range, 70.6%-71.5%); mean body mass index ranged between 29.7 and 30.9; and patients receiving mechanical ventilation ranged between 50.0% and 63.5%. For the fixed-dose, shock-dependent, and no hydrocortisone groups, respectively, the median organ support-free days were 0 (IQR, -1 to 15), 0 (IQR, -1 to 13), and 0 (-1 to 11) days (composed of 30%, 26%, and 33% mortality rates and 11.5, 9.5, and 6 median organ support-free days among survivors). The median adjusted odds ratio and bayesian probability of superiority were 1.43 (95% credible interval, 0.91-2.27) and 93% for fixed-dose hydrocortisone, respectively, and were 1.22 (95% credible interval, 0.76-1.94) and 80% for shock-dependent hydrocortisone compared with no hydrocortisone. Serious adverse events were reported in 4 (3%), 5 (3%), and 1 (1%) patients in the fixed-dose, shock-dependent, and no hydrocortisone groups, respectively.Conclusions and Relevance: Among patients with severe COVID-19, treatment with a 7-day fixed-dose course of hydrocortisone or shock-dependent dosing of hydrocortisone, compared with no hydrocortisone, resulted in 93% and 80% probabilities of superiority with regard to the odds of improvement in organ support-free days within 21 days. However, the trial was stopped early and no treatment strategy met prespecified criteria for statistical superiority, precluding definitive conclusions.Trial Registration: ClinicalTrials.gov Identifier: NCT02735707.

The attenuation coefficient of a rectangular dielectric line enclosed by a rectangular shield is obtained by the use of a rigorous mode-matching method to calculate the required mode field intensity. The cross section of the waveguide is overlaid by a grid, and numerical integration is used to determine the power flow, dielectric, and conductor losses and respective attenuation coefficients. To obtain experimental verification, a length of waveguide was made into a resonator, and measured and calculated Q factors were compared. The results for the [E.sup.y.sub.11] mode show how the influence of the shield decreases with distance. This is relevant to the design of dielectric waveguide structures and in filter applications where dielectric resonators are used. Rectangular cross-sectional dielectric waveguides are widely used at millimeter wavelengths. In addition, shielded dielectric resonators having a square cross section are often used as filter elements; however, there is almost no information available on the effect of the shield. Rectangular or square dielectric waveguide is notoriously difficult to analyze because of the singular behavior of the fields at the corners. Most published analyses are for materials with a low dielectric constant, and do not include the effects of a shield. This paper describes a numerically efficient mode-matching method for the analysis of shielded dielectric-rod waveguide, which is applicable to both low and high dielectric-constant materials. The effect of the shield on the propagation behavior is studied. The shield dimensions may be selected such that the shield has a negligible effect so that results can be compared with free-space data. The results are verified by comparison with several sets of published data, and have been confirmed by measurement for a nominal [[epsilon].sub.r] of 37.4. Plant macrofossil analyses of 5 peat cores obtained from undisturbed (i.e. not cut for peat) flood-plain mires situated in the Ant Valley in the Norfolk Broads have indicated the successional development of the vegetation over approximately the last 2000 years. These have been supplemented by analysis of diatom and foraminiferal content of some of the deposits together with the application of radiocarbon dating to give an approximate chronological framework within which to place macrofossil zones. Macrofossil assemblages have been grouped into 5 major zones. These are interpreted as representing: A: Carr woodland communities (pre-Roman) B: Salt marsh communities formed during a marine transgressive phase (Romano-British). C: Fen tussock/carr communities indicative of drier conditions (Early Medieval). D: Aquatic communities indicative of wetter conditions (late/post Medieval). E: Communities suggestive of present day vegetation influenced by human management (post Medieval-present day). The zones have been interpreted largely in terms of the response of the vegetation to changes in sea-level, climate and management over the last two millenia. Macrofossil analyses were also carried out on samples collected from a variety of former peat cuttings in the Catfield and Irstead Fens. Successional changes were deduced and compared with previous investigations. Historical studies of archival documentary material has suggested that the post-Medieval use of peat as a fuel in Norfolk was largely a feature of the eighteenth and nineteenth centuries and that it was a commodity of major importance amongst the poor during this time. Study of archive material specifically relevant to the Catfield and Irstead Fens has suggested that at least some of the former turf cuttings may have been dug in the first half of the nineteenth century but many may also date from the second half.

To characterize patients' willingness to donate a biospecimen for future research as part of a breast cancer-related biobank involving a general screening population. We performed a prospective cross-sectional study of 4,217 women aged 21-89 years presenting to our facilities for screening mammogram between December 2010 and October 2011. This HIPAA-compliant study was approved by our institutional review board. We collected data on patients' interest in and actual donation of a biospecimen, motivators and barriers to donating, demographic information, and personal breast cancer risk factors. A multivariate logistic regression analysis was performed to identify patient-level characteristics associated with an increased likelihood to donate. Mean patient age was 57.8 years (SD 11.1 years). While 66.0 % (2,785/4,217) of patients were willing to donate blood or saliva during their visit, only 56.4 % (2,378/4,217) actually donated. Women with a college education (OR = 1.27, p = 0.003), older age (OR = 1.02, p < 0.001), previous breast biopsy (OR = 1.23, p = 0.012), family history of breast cancer (OR = 1.23, p = 0.004), or a comorbidity (OR = 1.22, p = 0.014) were more likely to donate. Asian-American women were significantly less likely to donate (OR = 0.74, p = 0.005). The major reason for donating was to help all future patients (42.3 %) and the major reason for declining donation was privacy concerns (22.3 %). A large proportion of women participating in a breast cancer screening registry are willing to donate blood or saliva to a biobank. Among minority participants, Asian-American women are less likely to donate and further qualitative research is required to identify novel active recruitment strategies to insure their involvement. © 2012 Springer Science+Business Media New York.
