In the current study, we investigated whether how the VA healthcare system for mental health and/or SUD treatment predicted program enrollment into vocational rehabilitation, simultaneous mental health and/or SUD treatment while enrolled in vocational rehabilitation predicted employment at discharge, and mental health and/or SUD treatment continues and employment remain 60-days-post-vocational-rehabilitation discharge.Methods: An outcome-based, summative program evaluation design to measure quality assurance of vocational rehabilitation services provided to 402 veteran patients enrolled in a VA healthcare located within the Great Lakes Health Care System – Veterans Integrated Services Network.Results: Multivariable logistic regression analyses showed psychological empowerment (confidence in one’s ability to work or find work) is a significant factor determining whether a veteran is enrolled in the vocational rehabilitation program, prior mental health treatment (yes/no) and frequency of mental health treatment did not predict program enrollment, and frequency of SUD VA system treatment 60 days prior did not predict program enrollment. Other findings showed that simultaneous mental health and/or SUD treatment while enrolled in vocational rehabilitation did not predict employment at discharge, and employment at discharge did not predict continued mental health and/or SUD treatment post-discharge from vocational rehabilitation. However, veterans with both SUD and mental health and continued mental health treatment were less likely to be employed.Conclusion: Utilization of real-world program evaluation data from an actual VHA vocational rehabilitation program enhances the study’s ecological validity, offering practical implications for policymakers and practitioners in the field. The findings support the importance of veterans enrolling in mental health and/or SUD treatment simultaneously while enrolled in vocational rehabilitation services, as integrating vocational rehabilitation with mental health and SUD treatment services can lead to improved vocational and health outcomes for veterans (eg, development of targeted interventions to support veterans’ successful reintegration into the workforce and society).Keywords: substance use disorders, mental health, veterans affairs, health administration, veterans, vocational rehabilitation, treatment access

Assessing personality traits may help clinicians address the complexity of patients’ experiences and design treatments that target these vulnerabilities. This study aimed to identify the distinguishing personality traits of a cohort of patients with disabling chronic low back pain and to determine associations between those traits and fear-avoidance beliefs, depressive, and anxious moods.Patients and Methods: This cross-sectional study included 102 chronic low back pain patients (57% male), who failed standard management and were referred to a multidisciplinary rehabilitation program. All patients completed the five domains of the NEO Personality Inventory (NEO PI)–Revised (Neuroticism, Extraversion, Openness to experience, Agreeableness and Conscientiousness), the Tampa Scale for Kinesiophobia (TSK), and the Hospital Anxiety and Depression Scale (HADS). One-sample t-test was used to compare sample personality means with average population norms. Association between the five personality domains with TSK and HADS was assessed using Pearson’s correlation, adjusted for patient demographics and pain-related variables. Linear regression was used to estimate associations adjusted for covariates.Results: Both men and women had significantly lower scores in the Openness to experience domain and significantly higher scores in the Conscientiousness domain than the general population norms. After adjusting for covariates, Neuroticism was associated with higher fear-avoidance, depression and anxiety scores. Conversely, Extraversion and Openness to experience negatively correlated with depression scores. Extraversion also inversely correlated with fear-avoidance. Conscientiousness negatively correlated with depression and anxiety after adjustment.Conclusion: Patients with disabling chronic low back pain show personality characteristics that deviate significantly from the normal population norms but do not reach maladaptive forms of personality disorders. Clinicians would benefit from assessing patients’ personality traits to address protective and risk factors for psychological distress, particularly in difficult-to-treat, highly disabled low back pain patients.Keywords: NEO PI, anxiety, depression, kinesiophobia, chronic low back pain, personality

8. Application of novel CAR technologies to improve treatment of autoimmune disease.

Abstract

Chimeric antigen receptor (CAR) T cell therapy has become an important treatment for hematological cancers, and its success has spurred research into CAR T cell therapies for other diseases, including solid tumor cancers and autoimmune diseases. Notably, the development of CAR-based treatments for autoimmune diseases has shown great progress recently. Clinical trials for anti-CD19 and anti-BCMA CAR T cells in treating severe B cell-mediated autoimmune diseases, like systemic lupus erythematosus (SLE), have shown lasting remission thus far. CAR T cells targeting autoreactive T cells are beginning clinical trials for treating T cell mediated autoimmune diseases. Chimeric autoantigen receptor (CAAR) T cells specifically target and eliminate only autoreactive B cells, and they have shown promise in treating mucosal pemphigus vulgaris and MuSK myasthenia gravis. Regulatory CAR T cells have also been developed, which show potential in altering autoimmune affected areas by creating a protective barrier as well as helping decrease inflammation. These new treatments are only the beginning of potential CAR T cell applications in treating autoimmune disease. Novel CAR technologies have been developed that increase the safety, potency, specificity, and efficacy of CAR T cell therapy. Applying these novel modifications to autoimmune CARs has the potential to enhance the efficacy and applicability of CAR therapies to autoimmune disease. This review will detail several recently developed CAR technologies and discuss how their application to autoimmune disease will improve this emerging field. These include logicgated CARs, soluble protein-secreting CARs, and modular CARs that enable CAR T cell therapies to be more specific, reach a wider span of target cells, be safer for patients, and give a more potent cytotoxic response. Applying these novel CAR technologies to the treatment of autoimmune diseases has the potential to revolutionize this growing application of CAR T cell therapies. [ABSTRACT FROM AUTHOR]

9. A Full Semester Flow Cytometry Course Improves Graduate and Undergraduate Student Confidence

Abstract

Flow cytometry is a versatile and high throughput technique for rapid and efficient biological testing. It requires a high level of conceptual, technical, and analytical skills to properly design experiments, effectively operate flow cytometry machines, and analyze the data. A lack of training and development of any of these three skills can result in underutilization and improper use of flow cytometric machines that can impede research progress. Often students develop these conceptual, technical, and analysis skills from trial and error, but many students either do not use this powerful flow cytometry technology, use it improperly or ineffectively, or give up using it without proper training and support. Here we report on a course which teaches flow cytometry skills to undergraduate and graduate students. The design of this course is unique in that it teaches conceptual, technical, and analytical skills related to flow cytometry in a full semester format. Undergraduate and graduate students reported significant increases in their confidence levels over the course of the semester. Here we provide our findings and resources for others who may want to implement a similar course.

15. Evaluating a Learning Progression on 'Transformation of Matter' on the Lower Secondary Level

Abstract

One of the most central tenets of chemistry education is developing an understanding of the processes that involve the Transformation of Matter. Current German curricula postulate content-related abilities ("Kompetenzen") that are expected to be achieved by secondary students when graduating from the lower secondary level at grade 10. These abilities can further be differentiated as relating to either structural aspects of matter or to aspects of chemical reaction. Little is known of how "Kompetenzen" in these two fields develop over time on the lower secondary level. This study aims at elucidating this development by suggesting a hypothetical learning progression for the lower secondary level. This learning progression is visualised as a Strand Map and is investigated using methods from three statistical approaches: Rasch-analyses, Classical Test Theory, and Bayesian Networks. Concurrent data from all three strands of analyses inform the evaluation of the learning progression and support the notion that an understanding of the Transformation of Matter relies on interrelated "Kompetenzen" to conceptualize Structure of Matter and Chemical Reaction. Moreover, Bayesian networks underline that there is more than one progression when learning about chemistry on the lower secondary level. Contains notes and references.

18. The Effects of Using Direct Instruction and a Re-Reading Contingency with a High School Student

Abstract

The purpose of this study was to determine the effectiveness of using re-reading as a consequence for failing to read passages rapidly with zero errors using the "Direct Instruction approach with Corrective Reading, Skills Applications: Decoding C" (Engelmann, Meyer, Johnson, & Carnine, 1988). The participant was a 16-year-old high school student who read at a 7.2 grade level at the beginning of the study. During reading, he read slowly, made few errors and had close to perfect comprehension at the 7th grade level. The number of words read correctly, the number of errors made during an oral reading, and the number of times the student had to re-read the passage in order to correctly read the materials in 1 minute and 20 seconds was measured. An AB single case design was implemented to examine the effectiveness of Direct Instruction and the re-reading contingency. The results indicated that Direct Instruction and the re-reading contingency were effective in improving the rate of correct words read. The combined use of the re-reading and Direct Instruction is discussed.

Abstract

Aim: At presentation, 15–20% of patients with rectal cancer already have synchronous liver metastases. The aim of this study was to determine the surgical and survival outcomes in patients with advanced rectal cancer who underwent combined pelvic exenteration and liver (oligometastatic) resection. Method: Data from 20 international institutions that performed simultaneous pelvic exenteration and liver resection between 2007 and 2017 were accumulated. Primarily, we examined perioperative outcomes, morbidity and mortality. We also assessed the impact that margin status had on survival. Results: Of 128 patients, 72 (56.2%) were men with a median age of 60 years [interquartile range (IQR) 15 years]. The median size of the liver oligometastatic deposits was 2 cm (IQR 1.8 cm). The median duration of surgery was 406 min (IQR 240 min), with a median blood loss of 1090 ml (IQR 2010 ml). A negative resection margin (R0 resection) was achieved in 73.5% of pelvic exenterations and 66.4% of liver resections. The 30-day mortality rate was 1.6%, and 32% of patients had a major postoperative complication. The 5-year overall survival for patients in whom an R0 resection of both primary and metastatic disease was achieved was 54.6% compared with 20% for those with an R1/R2 resection (P = 0.006). Conclusion: Simultaneous pelvic exenteration and liver resection is feasible, with acceptable morbidity and mortality. Simultaneous resection should only be performed where an R0 resection of both pelvic and hepatic disease is anticipated.

40. Analysis of backgrounds for a radon-emanation system.

Abstract

Expected backgrounds for electrostatic PIN-diode radon-emanation systems consist of three basic terms: a grow-in term, due to radon emanating from the detection chamber itself; a decaying term, due to radon that is transferred into the chamber along with radon from the sample; and a term constant in time due to environmental backgrounds such as cosmic rays. The first two backgrounds should produce energies corresponding to the radon-daughter peaks, while the third is expected to produce a continuous energy spectrum. To determine what is the dominant background for the South Dakota Mines emanation system, many background runs were co-added. Results indicate that environmental backgrounds are small, producing < 0.28 counts/day at 95% confidence in the 214Po energy region of interest (ROI). The grow-in and decay terms are 2.01±0.39 counts/day and 0.95±0.15 counts/day respectively in the same ROI (for the best-fit value for environmental backgrounds). The decay term is larger than expected, so a cold radon trap is now being used to attempt to reduce it. [ABSTRACT FROM AUTHOR]

Abstract

Neuroinflammation is associated with several neurodegenerative disorders, including Alzheimer's disease (AD), Parkinson's disease (PD), and multiple sclerosis (MS). Neuroinflammation provides protection in acute situations but results in significant damage to the nervous system if chronic. Overexpression of chemokines within the brain results in the recruitment and activation of glial and peripheral immune cells which can propagate a cascading inflammatory response, resulting in neurodegeneration and the onset of neurodegenerative disorders. Recent work has identified the role of atypical chemokine receptors (ACKRs) in neurodegenerative conditions. ACKRs are seven-transmembrane domain receptors that do not follow canonical G protein signaling, but regulate inflammatory responses by modulating chemokine abundance, location, and availability. This review summarizes what is known about the four ACKRs and three putative ACKRs within the brain, highlighting their known expression and discussing the current understanding of each ACKR in the context of neurodegeneration. The ability of ACKRs to alter levels of chemokines makes them an appealing therapeutic target for neurodegenerative conditions. However, further work is necessary to understand the expression of several ACKRs within the neuroimmune system and the effectiveness of targeted drug therapies in the prevention and treatment of neurodegenerative conditions. Abstract

This article, published in the journal Neuropediatrics, explores the connection between sleep and epilepsy in children. The researchers aimed to compare the sleep patterns of children with epilepsy to a healthy control group. They found that children with epilepsy had higher rates of sleep slow waves (SW) and increased coupling between SW and sleep spindles during non-rapid eye movement (NREM) sleep. These findings suggest that children with epilepsy may experience altered sleep macro- and microstructure, which could potentially contribute to cognitive impairment. The compound orders antiferromagnetically at around 36 K accompanied by a simultaneous structural transition from a tetragonal to an orthorhombic phase. The hydrostatic pressure dependence of the magnetic and structural transitions is investigated using electrical-transport, ac magnetic-susceptibility, ac calorimetry, M$\ddot{\rm o}$ssbauer, muon-spin relaxation ($\mu$SR), and x-ray diffraction measurements. External pressure suppresses the first-order transition to the antiferromagnetic phase (AFM1) around 1.8 GPa. The structural transition is largely unaffected by pressure and remains between 30 to 35 K for pressures up to 2 GPa. A second antiferromagnetic phase (AFM2) is observed at higher pressures. The transition from the paramagnetic to the AFM2 phase is of second-order nature and appears to be connected to the structural transition. The magnetic volume fraction obtained from $\mu$SR and M$\ddot{\rm o}$ssbauer measurements reveal that the entire sample undergoes magnetic ordering in both magnetic phases. In addition, similar low-temperature muon-precession frequencies in AFM1 and AFM2 phases point at similar ordered moments and magnetic structures in both phases. Our results further indicate enhanced magnetic fluctuations in the pressure induced AFM2 phase. The experimental observations together with density functional theory calculations suggest that the magnetic and structural order parameters in LuFe$_4$Ge$_2$ are linked by magnetic frustration, causing the simultaneous magneto-structural transition., Comment: 9 pages, 8 figures
