5. Radiation therapy with phenotypic medicine: towards N-of-1 personalization

Abstract

Current radiotherapy (RT) treatment strategies are developed based on decades-old radiobiological modeling, preclinical work, and population-driven trials. The pre-determined total dose is divided into smaller dose fractions and delivered over several weeks. The treatment responses are typically assessed months after the treatment. However, due to the presence of heterogeneity, the conventional one-dose-fits-all strategy may pose a challenge to optimising outcome. A personalized regimen based on individual response will be favoured. As an alternative strategy, we are proposing a small-data AI-derived phenotypic medicine (PM) platform that does not to any model training and testing in guiding clinicians to dose patient based on individual treatment response in real-time with their own data. It potentially reduces unnecessary toxicity for good responders and prevents inadequate disease control for poor responders.

26. Comparison of First-Time Neurocognitive Baseline Cognigram Assessments between American and Brazilian Professional Bull Riding Athletes

Abstract

The purpose of this study was to compare the first-time neurocognitive concussion baseline outcomes, using the Cognigram assessment software, between American and Brazilian professional bull riders. The analysis was performed using the database provided by the Professional Bull Riding Association Sports Medicine staff. The Cognigram assessment measures and analyses four outcomes: Psychomotor Function, Attention, Learning, and Working Memory Speed. The study examined the test outcomes of 210 professional bull riders (150 American, 60 Brazilian). A one-way multivariate analysis of covariance (MANCOVA) was used to determine if there a significant difference in the four assessment outcomes when comparing the country of origin subject groups. The results of the analysis reported country of origin has a significant effect on the assessment outcomes, F(8, 406) = 6.407, p < 0.001, Wilks' [lambda] = 0.788, partial [eta][superscript 2] = 0.112. Post hoc analysis reported significant differences in three outcomes (Bonferroni correction, [alpha] = 0.012): Psychomotor Function, F(2, 206) = 21.25, p < 0.001, partial [eta][superscript 2] = 0.17, Attention, F(2, 206) = 18.90, p < 0.001, partial [eta][superscript 2] = 0.16, and Working Memory Speed, F(2, 206) = 7.70, p < 0.001, partial [eta][superscript 2] = 0.07. Country of Origin did not have a significant effect on Learning, F(2, 206) = 1.14, p = 0.321, partial [eta][superscript 2] = 0.01. Extrinsic factors (Testing environment and format and familiarity of testing content) and intrinsic factors (time orientation and physical and mental status at the time of testing) could have significant effects on the disparity between the two groups, affecting assessment outcomes. If cultural bias has a significant effect on Cognigram assessment outcomes, this poses a threat to the validity of the assessment toll; and this may contribute to an increase in the probability of under diagnosis of sport related concussions for Brazilian bull riders. Abstract

Even when successfully Induced, Immunological tolerance to solid organs remains vulnerable to Inflammatory Insults, which can trigger rejection. In a mouse model of cardiac allograft tolerance in which infection with Listeria monocytogenes (Lm) precipitates rejection of previously accepted grafts, we showed that recipient [CD4.sup.+] TCR75 cells reactive to a donor MHC class I-derived peptide become hypofunctional if the allograft is accepted for more than 3 weeks. Paradoxically, infection-induced transplant rejection was not associated with transcriptional or functional reinvigoration of TCR75 cells. We hypothesized that there is heterogeneity in the level of dysfunction of different allospecific T cells, depending on duration of their cognate antigen expression. Unlike [CD4.sup.+] TCR75 cells, [CD4.sup.+] TEa cells specific for a peptide derived from donor MHC class II, an alloantigen whose expression declines after transplantation but remains inducible in settings of inflammation, retained function in tolerant mice and expanded during Lm-induced rejection. Repeated injections of alloantigens drove hypofunction in TEa cells and rendered grafts resistant to Lm-dependent rejection. 45. Case report: Refractory focal motor seizure associated with cerebrospinal fluid neurochondrin antibody.

Abstract

Background: Focal onset seizures, characterized by localized neuronal hyperexcitability in the brain, can be related to various structural, immune, genetic, or metabolic abnormalities. Autoimmune epilepsies are increasingly recognized. Neurochondrin antibody has been reported in a variety of rare autoimmune neurological disorders. This article aims to highlight the relevance of anti-neurochondrin in autoimmune epilepsy. Methods: This is a case presentation and literature review of autoimmune epilepsy associated with anti-neurochondrin antibody. Case presentation: A 26-year-old African American right-handed man with a history of Sjogren’s syndrome presented with near constant, rhythmic left-sided facial twitching movements, and one episode of generalized tonic clonic seizure. Magnetic resonance imaging (MRI) of the brain revealed borderline low volume right hippocampus. Cerebrospinal fluid (CSF) studies yielded elevated protein and mild lymphocytic pleocytosis. Antibody Prevalence in Epilepsy 2 (APE2) score was 6, and autoimmune workup was initiated. Anti-neurochondrin antibody returned positive in the CSF autoimmune encephalitis panel with a titer of 1:512 (Mayo Clinic TEST ID: ENC2). Seizures remained refractory to anti-seizure medications including divalproex, lacosamide, and oxcarbazepine. Immunotherapy with methylprednisolone and immunoglobulin improved his epileptic seizures. Conclusion: This is the first reported case of refractory autoimmune epilepsy with positive CSF anti-neurochondrin antibody. This study contributes to the body of evidence supporting the role of neurochondrin antibody in epilepsy. Considering autoimmune testing in individuals with seizures having APE2 score > 4 can aid in timely diagnosis of immune-mediated epilepsy and initiation of immunotherapy, which can result in favorable clinical outcomes. Diagnosis of autoimmune epilepsy, in most cases, is based on clinical characteristics, MRI results, and CSF findings. In addition to the traditional antibody panel for autoimmune encephalitis, some novel antibodies, such as anti-neurochondrin, should also be considered. [ABSTRACT FROM AUTHOR]

46. The Asian Monetary Fund and De-Dollarization: The Reshaping of the Global Order.

Abstract

During the Malaysian prime minister's inaugural visit to China in April 2023, Anwar Ibrahim proposed the idea of establishing an Asian Monetary Fund (AMF), an idea that had previously been proposed at the height of the 1997 Asian financial crisis. The proposal in 1997 was due to frustration with the performance of the International Monetary Fund (IMF). Today, it is identified, together with de-dollarization, as part of efforts to reshape the global order. This paper explores the factors driving countries towards initiatives like the AMF and de-dollarization, and their prospects of successfully transforming the existing global order. This paper finds that initiatives such as the AMF and de-dollarization are driven by multiple factors, which include achieving monetary autonomy, and for a few, geostrategic objectives. This study argues that instead of replacing the liberal international order, the objective of the countries seeking change through the AMF or de-dollarization is to strengthen their voices in the international system by reducing those of the US. Nevertheless, these initiatives are hampered because they lack the qualities that give shape to and maintain the architecture of the international order: power and legitimacy. This study concludes that both initiatives are unlikely to significantly reshape the global order. De-dollarization still has a long way to go before it can displace the dollar in the global economy.
