This research focuses on the needs analysis and perspectives in evaluating English for Specific Purposes (ESP) courses designed for medical students. The study intends to identify the medical students’ perceptions of the frequency and importance of using English language skills. Two different instruments, questionnaires and semi-structured interviews, were used to collect data. Respondents included 186 medical students, 4 ESP lecturers, and 10 Department of Medicine (DM) Subject Matter lecturers at Ibb University in Yemen. In this study, quantitative data were collected from three different subject groups (n=200) by means of three versions of the questionnaire. Correspondingly, some follow-up interviews, which were organised with a group of these three informants (n=29), complemented the data collection procedure. The collected data was analysed via the SPSS software package, followed by a qualitative data thematic analysis of semi-structured interviews. The findings showed some differences in which medical students believed the most frequent skills used are Listening and Reading, while their ESP and DM Subject Matter lecturers believed that medical students use Writing and Listening. The results revealed that although most of the English language skills and sub-skills in target needs were considered important or very important by all the participants, there were discrepancies in the preferences and priorities. The findings of this study are expected to be used as reference material for further research and give valuable considerations which may provide useful information for the ESP course designers to meet the target English needs of medical students in Yemen.

Salvia rosmarinus Spenn (synonym: Rosmarinus officinalis L.) is economically and medically important and grows naturally in Al-Marj (Libya). Macro, micro morphological and chemical characteristics were studied. Leaf venation, leaf anatomy and chemical compounds of essential oils were carried out according to traditional methods. The objective of the present study is to provide detailed description of the characteristics of this species. Morphological charactersistics of leaves, calyx, corolla, stamens are useful for sectional and specific delimitations in Salvia. The leaf architecture characters of primary, secondary, tertiary and quaternary veins are good diagnostic markers for the identification and classification of species. Anatomical characters such as glandular and non- glandular trichomes , mesophyll structure, presence of hypodermis and structure of vascular bundles have been found to have taxonomic value. Cineol (32.38%) is main component of essential oil, followed by Camphor, β-pinene, borneol, Caryophyllene, α-terpineol and α-pinene. Chemical data in essential oils carried taxonomic value of Salvia species.

Background: Over 15 million people died globally from acute coronary syndrome (ACS) in 2019, with individuals under 70 years old accounting for 40% of these untimely fatalities. Aim: The current study aimed to assess effectiveness of novel inflammatory index in prediction of coronary artery disease severity in Egyptian patients with acute coronary syndrome. Methods: This prospective cross-sectional study was conducted on a total of 100 patients with acute coronary syndrome. Systemic immune inflammation index (SII), systemic inflammation response index (SIRI) and systemic immuneinflammation response index (SIIRI) and systemic immune-inflammation response index were calculated. Results: SII ranged from 83.3 to 5310.8 with a mean value of 880.2 ± 778.62. SIRI ranged from 0.39 to 14.35 with a mean value of 3.6 ± 2.96. SIIRI ranged from 83.3 to 4632 with a mean value of 1007.3 ± 877.86. SII, SIRI and SIIRI were significantly lower in Syntax ≤ 20 group than Syntax >20 group (P value < 0.05). Conclusion: Patients with acute coronary syndrome (ACS), including STEMI, NSTEMI, and unstable angina (UA), exhibited elevated levels of the novel inflammatory marker SIRI. This index could serve as an effective inflammatory biomarker for prognostic assessment in ACS patients, demonstrating superior predictive ability for cardiac events and longterm outcomes. Consequently, measuring SIRI presents a new opportunity to implement more aggressive treatment strategies for high-risk populations. [ABSTRACT FROM AUTHOR]

Infantile systemic hyalinosis (ISH) is a rare multisystem fatal autosomal recessive disorder that involves widespread deposition of hyaline on connective tissues and certain internal organs. The major manifestations include painful articular contractures, hyperpigmentation, subcutaneous nodules, gingival hypertrophy, failure to thrive secondary to protein-losing enteropathy, and osteolytic bone lesions. In this paper, we report a 12-month-old girl with ISH presenting with recurrent diarrhea, failure to thrive, and refractory infections. A molecular study identified a homozygous missense mutation, c.134T > C; p.L45P, in exon 1 of the anthrax toxin receptor 2 (ANTRX2) gene. Our patient passed through an eventful course that included septic shock, central line infections, right atrial thrombosis, and pericardial effusion. She incurred acute bronchiolitis due to respiratory syncytial virus infection, which led to her death. In conclusion, this case report highlights that severe and life-threatening morbidities and complications can be encountered in ISH, to which some management options can be applied.

Lipoma is a benign soft tissue tumor that is mostly found in the trunk, however, lipomas of the thumb are rarely reported, around 1% of cases. We report a case of a Saudi male aged 32-year-old who presented with a progressive left thumb swelling for a long time. Wadi El-Assiuti Protected Area (WAPA) in Assiut Governorate is considered as the first and foremost zone for wildlife breeding and endangered species. This study investigates the effects of different environmental issues of both anthropogenic and natural factors on the wildlife of WAPA. The examined factors are air quality, water quality, noise levels, local weather, and human activity in WAPA. Additional to environmental agents, biodiversity is monitored by recording the number of each fauna type from 2008 to 2016. The results showed that several kinds of wildlife in WAPA have been endangered while numbers of other increased. The examined air, water, and local weather proved that they have no negative effects on the endangered species. The main reason of the disappeared or increases some species are the human activities especially cultivation and construction of several quarries in the WAPA zone. The quarries are companion with noise, dust, contaminations from their vehicles, and huge movement of human and trucks that have direct impact on the wildlife in WAPA. The main conclusion of this study is that removing the encroachment from the WAPA is the only method to protect its biodiversity. [ABSTRACT FROM AUTHOR]

In Islam, all legitimate motives for asylum are valid. From its beginning, Islam has provided for the granting of refuge as well as the protection of refugees. The Islamic concept, practices and principles on the matter are clear and unambiguous. Islamic Shari'ah embraces a comprehensive set of rules concerning this topic on the one hand; on the other hand, the greatest number of refugees are actually Muslims and those hosting them are nearly all Muslim States. The refugees currently fleeing to Europe from Muslim States may be divided into two groups, namely: those pushed to flee their countries under threats of war, torture or persecution; and those motivated to improve their status quo. A hybrid energy system (HPS) simulation model is developed to maximize the renewable energy extraction at Farafra, an oasis in Egypt. In Farafra a microgrid composed of 2*1.6 MW+1*2MW+2*2.5MW diesel generating units (DG) is already installed and serve an electric demand consisting of 52365 KWh/day. The system may need to be upgraded. In the model, part of the load is recommended to be supplied as deferrable load. Also battery/convertor modules are added in the HPS so that when renewable power generation exceeds local demand, energy can be stored and used during times when renewable power is not available or the electrical load increases. The model is based on two types of renewable power generation: wind turbine generator and photovoltaic generator in conjunction with existing diesel generators supply sources. The HOMER software was used to select an optimum hybrid energy system sized according to the electric demand, the availability of renewable energy resources and to maximize the renewable energy to total energy ratio. The optimization process simulates every system configuration in the search space and the feasible ones are sorted by total net present cost (NPC). Sensitivity analysis shows that the most least cost and optimized hybrid system is combination of 231 Enercon (330 kW) WTG; 155 Surrette 4KS25P batteries; and a 283 kW Convertor requiring only 3916 hours annual operation of one of the 1.6 MW diesel generators and 2306 hours annual operation of the 2.0 MW diesel generator. The two 2.5 MW diesel generators are saved. The photovoltaic array was not chosen because its costs were not justified with this particular solar regime and diesel cost. [ABSTRACT FROM AUTHOR]

Web space reconstruction is the key point and an important component of syndactyly treatment. A design of the dorsal metacarpal artery flap was used as an island flap based on the direct cutaneous branch arising from the dorsal metacarpal artery at the distal end of the flap to reconstruct congenital web syndactyly. The flap rotated from 0° to 180° and passed under a skin bridge at the distal end of the flap to reach the web defect. Such design enables a better use of the skin territory from the intermetacarpal space proximal to the pedicle, with a short donor site scar and a more supple reconstruction of the web space. Polydactyly is defined as an extra finger or toe that is present at birth. It is a congenital anomaly that can manifest as a single disorder or as a component of a syndrome. Treatment can be conservative or operative depending on the anticipated function of the extra digit. In this report, we describe a case of a bifid 5th digit on the right hand. The aim of this report was to report a new variant of type IIIB described by Duran (2015). Background: A variety of surgical methods have been described to repair wide cleft palate; they are all challenging to perform and yield consistently good results. The islandized mucoperiosteal flap, the technique described in the present article, is very versatile because it can close palatal defects of any size without undue tension. Moreover, it provides adequate length and mobility of the soft palate with improved speech and feeding functions without fistula formation., Methods: Between 2005 and 2011, 36 patients with wide cleft palate were operated on using islandized mucoperiosteal flaps. This technique involves dissection of the neurovascular bundle from the mucoperiosteal flaps for approximately 1 cm and dissecting the muscle from the posterior edge of the hard palate with intravelar veloplasty. The flaps subsequently become freely mobile in all directions. It can move medially to close palatal defects of any size without tension. In addition, posterior or backward mobilization lengthens the soft palate and renders it freely mobile., Results: All repairs were successful, with no complications and no patients requiring secondary procedures. All patients regained normal feeding function three weeks postoperatively. All patients showed normal nasal resonance of speech except for two (three and five years of age) who experienced abnormal resonance in the form of open nasality that required regular speech therapy for six months. There was significant improvement and no secondary procedures were required for either., Conclusions: A technical modification for closure of wide palatal clefts is introduced. The islandized mucoperiosteal flap, which is a very versatile technique, can close cleft palates of any width without tension, lengthens the soft palate and renders it freely mobile for proper speech functions. Using this technique, good speech and feeding function with no complications were achieved.
