Background: Despite the recommendation of coronary physiology to guide revascularization in angiographically intermediate stenoses without established correlation to ischemia, its uptake in clinical practice is slow., Aims: This study aimed to analyze the use of coronary physiology in clinical practice., Methods: Based on a multicenter registry (Fractional Flow Reserve Fax Registry, F(FR) 2 , ClinicalTrials.gov identifier NCT03055910), clinical use, consequences, and complications of coronary physiology were systematically analyzed., Results: F(FR) 2 enrolled 2,000 patients with 3,378 intracoronary pressure measurements. Most measurements (96.8%) were performed in angiographically intermediate stenoses. Out of 3,238 lesions in which coronary physiology was used to guide revascularization, revascularization was deferred in 2,643 (78.2%) cases. Fractional flow reserve (FFR) was the most common pressure index used (87.6%), with hyperemia induced by an intracoronary bolus of adenosine in 2,556 lesions (86.4%) and intravenous adenosine used for 384 measurements (13.0%). The route of adenosine administration did not influence FFR results (change-in-estimate -3.1% for regression model predicting FFR from diameter stenosis). Agreement with the subsequent revascularization decision was 93.4% for intravenous and 95.0% for intracoronary adenosine (p = 0.261). Coronary artery occlusion caused by the pressure wire was reported in two cases (0.1%) and dissection in three cases (0.2%), which was fatal once (0.1%)., Conclusions: In clinical practice, intracoronary pressure measurements are mostly used to guide revascularization decisions in angiographically intermediate stenoses. Intracoronary and intravenous administration of adenosine seem equally suited. While the rate of serious complications of wire-based intracoronary pressure measurements in clinical practice seems to be low, it is not negligible., (© 2024. The Author(s).)

To evaluate diagnostic impact of clinical use of coronary computed tomography angiography (CCTA) in patients with suspected stable coronary artery disease (CAD) and its consequences in daily practice for patient management, depending on stress test results in daily practice. Between 2009 and 2014 of a total population of 1352 patients of the German Cardiac Computed Tomography (CT) Registry who had previously undergone stress tests, CCTA visualizations were carried out on the coronary arteries with suspected stable CAD. Patients were divided into three groups according to stress test results: Group 1 with inconclusive (n = 178, 13.2%), Group 2 with ischemia in stress test (n = 372, 27.5%) and Group 3 without ischemia in stress test (n = 802, 59.3%). The test of preference was the stress electrocardiogram (ECG), which was performed more frequently in patients without ischemia in stress test as compared to those with ischemia (96.3% vs. 93.0%, p = 0.015). The incidence of detected obstructive CAD was lower in patients with suggested ischemia in stress test as compared to patients with inconclusive results (14.1% vs. 21.1%, p = 0.037). There was no difference in the incidence of an obstructive CAD in patients with and without ischemia in stress test (14.1% vs. 15.8%, p = 0.440). CCTA is a reliable, non-invasive option for ruling-out obstructive CAD irrespective of the stress test result.

Cardiac computed tomography permits quantification of coronary calcification as well as detection of coronary artery stenoses after contrast enhancement. Moreover, cardiac CT offers high-resolution morphologic and functional imaging of cardiac structures which is valuable for various structural heart disease interventions and electrophysiology procedures. So far, only limited data exist regarding the spectrum of indications, image acquisition parameters as well as results and clinical consequences of cardiac CT examinations using state-of-the-art CT systems in experienced centers. Twelve cardiology centers with profound expertise in cardiovascular imaging participated in the German Cardiac CT Registry. Criteria for participation included adequate experience in cardiac CT as well of the availability of a 64-slice or newer CT system. Between 2009 and 2014, 7061 patients were prospectively enrolled. For all cardiac CT examinations, patient parameters, procedural data, indication and clinical consequences of the examination were documented. Mean patient age was 61 ± 12 years, 63% were males. The majority (63%) of all cardiac CT examinations were performed in an outpatient setting, 37% were performed during an inpatient stay. 91% were elective and 9% were scheduled in an acute setting. In most examinations (48%), reporting was performed by cardiologists, in 4% by radiologists and in 47% of the cases as a consensus reading. Cardiac CT was limited to native acquisitions for assessment of coronary artery calcification in 9% of patients, only contrast-enhanced coronary CT angiography was performed in 16.6% and combined native and contrast-enhanced coronary CT angiography was performed in 57.7% of patients. Non-coronary cardiac CT examinations constituted 16.6% of all cases. Coronary artery calcification assessment was performed using prospectively ECG-triggered acquisition in 76.9% of all cases. The median dose length product (DLP) was 42 mGy cm (estimated effective radiation dose of 0.6 mSv). Coronary CT angiography was performed using prospectively ECG-triggered acquisition in 77.3% of all cases. Tube voltage was 120 kV in 67.8% of patients and 100 kV in 30.7% of patients, with a resultant median DLP of 256 mGy cm (estimated effective dose of 3.6 mSv). Clinical consequences of cardiac CT were as follows: in 46.8% of the cases, invasive coronary angiography could be avoided; ischemia testing was recommended in 4.7% of the cases, invasive coronary angiography was recommended in 16.4% of the cases and change in medication in 21.6% of the examinations. Cardiac CT is performed in the majority of patients for non-invasive evaluation of the coronary arteries. CT frequently resulted in medication change, and otherwise planned downstream testing including invasive angiography could be avoided in a high percentage of patients. Radiation exposure in experienced centers is relatively low.

Background: Coronary computed tomography angiography (CTA) is increasingly used as a test to rule out coronary artery disease (CAD) in patients with a low to intermediate pre-test probability of the disease. We used the database of the German CT registry, collected between 2009 and 2014 in a broad patient population, to analyze contemporary radiation dose associated with coronary CTA in clinical practice., Patients and Methods: The prospective observational registry included a total of 7061 patients ≥18 years, referred to 12 participating centers for a clinically indicated cardiac CT examination. All centers were cardiology units well experienced in CTA and used multi-slice CT scanners with at least 64 rows. Coronary CTA was performed in a subset of 5001 patients, 59.6 ± 11.8 years, body mass index (BMI) 26.9 ± 4.5 kg/m 2 , 38% females. Three time periods with approximately equal numbers of patients were created (01/09-03/10, 04/10-03/11, 04/11-07/14). The dose-length product of all examinations and derived effective dose in mSv (conversion factor k = 0.014) as well as the influence of patient characteristics on dose were compared for the three time periods., Results: BMI and proportion of female patients remained stable over time, and mean heart rate decreased from 60.3 ± 9.0 to 58.5 ± 9.3 bpm from the first to the last time period (p < 0.001). Overall, the mean effective dose of coronary CTA was 3.6 mSv (Q1 1.8 mSv, Q3 7.4 mSv). Within the three time periods, it declined from 5.6 (2.7, 8.6) mSv during the first to 4.8 (2.1, 8.2) mSv during the second and 2.5 (1.3, 4.6) mSv during the last time period (p < 0.001). Paralleling the decline in radiation dose over time, the proportion of prospectively ECG-triggered examinations increased (68, 79, 83%; p < 0.001), and the proportion of examinations with retrospective gating and no tube current modulation decreased (5.3, 4.0, 1.6%; p < 0.001). Tube current (mAs) and voltage (kV) both decreased over time. In multivariable analysis, besides earlier time period, further independent predictors of an increased radiation dose were older age, higher heart rate, and higher BMI as well as the technical factors higher mAs, higher kV, and retrospective gating. At three sites, CT scanners with improved technology were installed during the last time period., Conclusions: In current clinical practice among German cardiology units with specific expertise in cross-sectional cardiovascular imaging, overall radiation dose of coronary CTA was comparably low. Over time, a decline in radiation dose was demonstrated, probably due to a combination of improvements in data acquisition protocols and patient preparation as well as installation of new CT scanners with advanced technology.

Purpose: In the Bavarian Reperfusion Alternatives Evaluation (BRAVE)-3 study upstream administration of abciximab additional to 600 mg clopidogrel loading did not reduce the infarct size in patients with acute ST-segment elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary interventions. The aim of this study was to investigate 1-year clinical outcomes in the BRAVE-3 study patients., Methods: A total of 800 patients with acute STEMI within 24 h from symptom onset, all treated with 600 mg of clopidogrel were randomized in a double-blind fashion to receive either abciximab (n = 401) or placebo (n = 399) in the intensive care unit before being sent to the catheterization laboratory., Results: The main outcome of interest of the present study, the composite of death, recurrent myocardial infarction, stroke or revascularization of the infarct-related artery (IRA) at 1 year, was 23.0% (92 patients) in the abciximab versus 25.7% (102 patients) in the placebo group [relative risk (RR) = 0.90, 95% confidence interval (CI) 0.67-1.20; P = 0.46]. The combined incidence of death, recurrent myocardial infarction or stroke was 9.3% in the abciximab group versus 6.0% in the placebo group (RR = 1.55, 95% CI 0.93-2.58; P = 0.09). There was a significant reduction of the IRA revascularization with abciximab compared to placebo (16.3 vs. 22.3%, RR = 0.71, 95% CI 0.52-0.98; P = 0.04)., Conclusion: In patients with STEMI, all receiving 600 mg clopidogrel, abciximab did not improve overall clinical outcomes at 1 year after primary coronary stenting.

For selected indications, coronary computed tomographic (CT) angiography is an established clinical technology for evaluation in patients suspected of having or known to have coronary artery disease. In coronary CT angiography, image quality is highly dependent on heart rate, with heart rate reduction to less than 60 beats per minute being important for both image quality and radiation dose reduction, especially when single-source CT scanners are used. β-Blockers are the first-line option for short-term reduction of heart rate prior to coronary CT angiography. In recent years, multiple β-blocker administration protocols with oral and/or intravenous application have been proposed. This review article provides an overview of the indications, efficacy, and safety of β-blockade protocols prior to coronary CT angiography with respect to different scanner techniques. Background: The glycoprotein IIb/IIIa receptor inhibitor abciximab has improved the efficacy of primary percutaneous coronary interventions in patients with acute myocardial infarction. However, it is not known whether abciximab remains beneficial after adequate clopidogrel loading in patients with acute ST-segment-elevation myocardial infarction., Methods and Results: A total of 800 patients with acute ST-segment-elevation myocardial infarction within 24 hours from symptom onset, all treated with 600 mg clopidogrel, were randomly assigned in a double-blind fashion to receive either abciximab (n=401) or placebo (n=399) in the intensive care unit before being sent to the catheterization laboratory. The primary end point, infarct size measured by single-photon emission computed tomography with technetium-99m sestamibi before hospital discharge, was 15.7+/-17.2% (mean+/-SD) of the left ventricle in the abciximab group and 16.6+/-18.6% of the left ventricle in the placebo group (P=0.47). At 30 days, the composite of death, recurrent myocardial infarction, stroke, or urgent revascularization of the infarct-related artery was observed in 20 patients in the abciximab group (5.0%) and 15 patients in the placebo group (3.8%) (relative risk, 1.3; 95% CI, 0.7 to 2.6; P=0.40). Major bleeding complications were observed in 7 patients in each group (1.8%)., Conclusions: Upstream administration of abciximab is not associated with a reduction in infarct size in patients presenting with acute myocardial infarction within 24 hours of symptom onset and receiving 600 mg clopidogrel.

Background: Recent clinical trials have suggested that intensive versus standard lipid-lowering therapy provides for additional benefit. Electron-beam computed tomography provides the opportunity to quantify the progression of coronary artery calcification (CAC) in serial measurements., Methods and Results: In a multicenter, randomized, double-blind trial, 471 patients (age 61+/-8 years) who had no history of coronary artery disease and no evidence of high-grade coronary stenoses (>50% diameter reduction) were randomized if they had > or =2 cardiovascular risk factors and moderate calcified coronary atherosclerosis as evidenced by a CAC score > or =30. Patients were assigned to receive 80 mg or 10 mg of atorvastatin per day over 12 months. Progression of CAC volume scores could be analyzed in 366 patients. After pretreatment with 10 mg of atorvastatin for 4 weeks, 12 months of study medication reduced LDL cholesterol from 106+/-22 to 87+/-33 mg/dL in the group randomized to receive 80 mg of atorvastatin (P<0.001), whereas levels remained stable in the group randomized to receive 10 mg (108+/-23 at baseline, 109+/-28 mg/dL at the end of the study, P=NS). The mean progression of CAC volume scores, corrected for the baseline CAC volume score, was 27% (95% CI 20.8% to 33.1%) in the 80-mg atorvastatin group and 25% (95% CI 19.1% to 30.8%) in the 10-mg atorvastatin group (P=0.65). CAC progression showed no relationship with on-treatment LDL cholesterol levels., Conclusions: We did not observe a relationship between on-treatment LDL cholesterol levels and the progression of calcified coronary atherosclerosis. Over a period of 12 months, intensive atorvastatin therapy was unable to attenuate CAC progression compared with standard atorvastatin therapy. The possibility remains that the time window was too short to demonstrate an effect.

Objective: To compare the presence and extent of coronary calcifications in young patients with first, unheralded acute myocardial infarction with matched controls without a history of coronary artery disease., Methods: In 102 patients under 60 years of age (19-59 years, mean 41 years; 88% male), electron beam tomography was done 1-14 days after acute myocardial infarction, before any coronary intervention. Coronary calcifications were quantified using the Agatston score. Age related calcium centiles were determined based on the Mayo Clinic "epidemiology of coronary calcification" study, and results were compared with a group of 102 controls without coronary artery disease, matched for sex, age, and risk factors., Results: Calcifications were present in 95.1% of patients with acute myocardial infarction and in 59.1% of controls (p = 0.008). The mean (SD) Agatston score was 529 (901) in the infarct patients versus 119 (213) in the controls (p < 0.001). An Agatston score above the 50th centile was present in 87.2% of infarct patients and 47.0% of controls (p = 0.006), and above the 90th centile in 60.7% of infarct patients and only 5.8% of controls (p = 0.001)., Conclusions: In young patients with their first, unheralded acute myocardial infarction, the presence and extent of coronary calcium are significantly greater than in matched controls.

Rationale and Objectives: We compared maximum intensity projections (MIP) versus original source images (SI) in respect to detection of coronary artery stenoses by means of magnetic resonance (MR) coronary angiography., Methods: MR coronary angiography was performed on 61 patients. MIP and SI were independently evaluated as to presence of significant stenoses in the proximal and midcoronary segments and compared with x-ray angiography., Results: A total of 315 of 427 (74%) coronary artery segments could be evaluated in MIP and 328 of 427 (77%) in SI. In segments able to be evaluated, MIP images demonstrated 84% (54/64) sensitivity and 87% (219/251) specificity, whereas SI images showed 85% (58/68) sensitivity and 90% (235/260) specificity. Overall accuracy was 87% (273/310) for MIP and 89% (293/328) for SI. There was no statistically significant difference between both modalities., Conclusions: The MIP reconstructions showed comparable accuracy to unprocessed SI. However, MIP postprocessing is compromised by a higher number of images that were unable to be evaluated due to overlap of coronary arteries with adjacent cardiac structures.

Objective: A database with reference values of the durations of the various waveforms in a magnetocardiogram of fetuses in uncomplicated pregnancies is assessed. This database will be of help to discriminate between pathologic and healthy fetuses. A fetal magnetocardiogram is a recording of the magnetic field in a location near the maternal abdomen and reflects the electric activity within the fetal heart. It is a non-invasive method, which can be used with nearly 100% reliability from the 20th week of gestation onward., Design: Durations of the waveforms were assembled from averaged magnetocardiograms and statistically processed., Setting: Fetal magnetocardiograms were measured with different magnetocardiographs. All measurements were carried out in magnetically shielded rooms., Sample: Fetal magnetocardiograms were obtained for 582 healthy patients., Method: The durations of the waveforms were extracted from fetal magnetocardiograms measured at the cooperating centres. The variables collected included the duration of the P-wave, the PR interval, the PQ interval, the QRS complex, the QT interval and the T-wave and QTc value. The results were compared with values extracted from electrocardiograms of fetuses measured via electrodes attached to the maternal abdomen, from electrocardiograms measured during labour using a scalp electrode, and from electrocardiograms recorded in newborns, that were found in the literature., Main Outcome Measures: Values of the durations are given as a function of gestational age including the regression line as well as the bounds marking the 90%, 95% and 98% prediction interval., Results: The durations of the P-wave, the PR interval, the QRS complex, the QT interval and QTc value increase linearly with gestational age. The durations of the PQ interval and the T-wave are independent of fetal age., Conclusion: The values found agree with those found in the literature. The scatter of the data is wide due to the variation in normal physiology, the measuring system and signal processing and the subjectivity of the researcher. However, the system can define normal ranges and may be used in diagnosis.

Suppression of respiratory motion is one of the major challenges of magnetic resonance (MR) coronary angiography. Two approaches to compensate for respiratory motion have often been proposed: breath-hold (BH) and free-breathing respiratory-gated (FBRG) imaging. So far, however, these approaches have never been directly compared. MR coronary angiography was performed in 32 patients with suspected coronary artery disease. MR data were acquired using contrast-enhanced BH and FBRG 3-dimensional MR coronary angiographic techniques. MR images were compared with regard to image quality using quantitative parameters and with regard to accuracy for stenosis detection in the proximal and mid-coronary segments in comparison to x-ray angiography. With regard to image quality, BH was superior to FBRG. Background: Coronary calcification measured by fast computed tomography techniques is a surrogate marker of coronary atherosclerotic plaque burden. In a cohort study, we prospectively investigated whether lipid-lowering therapy with a cholesterol synthesis enzyme inhibitor reduces the progression of coronary calcification., Methods and Results: In 66 patients with coronary calcifications in electron beam tomography (EBT), LDL cholesterol >130 mg/dL, and no lipid-lowering treatment, the EBT scan was repeated after a mean interval of 14 months and treatment with cerivastatin was initiated (0.3 mg/d). After 12 months of treatment, a third EBT scan was performed. Coronary calcifications were quantified using a volumetric score. Cerivastatin therapy lowered the mean LDL cholesterol level from 164+/-30 to 107+/-21 mg/dL. The median calcified volume was 155 mm3 (range, 15 to 1849) at baseline, 201 mm3 (19 to 2486) after 14 months without treatment, and 203 mm3 (15 to 2569) after 12 months of cerivastatin treatment. The median annualized absolute increase in coronary calcium was 25 mm3 during the untreated versus 11 mm3 during the treatment period (P=0.01). The median annual relative increase in coronary calcium was 25% during the untreated versus 8.8% during the treatment period (P<0.0001). In 32 patients with an LDL cholesterol level <100 mg/dL under treatment, the median relative change was 27% during the untreated versus -3.4% during the treatment period (P=0.0001)., Conclusions: Treatment with the cholesterol synthesis enzyme inhibitor cerivastatin significantly reduces coronary calcium progression in patients with LDL cholesterol >130 mg/dL.

Rationale and Objectives: To compare electron beam tomography (EBT) with MR imaging (MRI) for detection of restenosis after coronary angioplasty (PTCA)., Methods: One hundred eighteen patients after PTCA were investigated. By EBT, 50 axial images were acquired (3-mm slice thickness, 120-160 mL radiographic contrast agent). MRI was performed using respiratory-gated sequences (24-48 cross-sections, 2-mm slice thickness, 20 mL Gd-DTPA). EBT and MRI images were evaluated concerning high-grade post-PTCA restenosis (> or = 70%) and validated against coronary angiography., Results: In EBT, 28 patients and in MRI, 31 patients were not evaluable. In the remaining patients, sensitivity for restenosis detection was 90% in EBT (17/19) and 73% in MRI (11/15; P = 0.370). In EBT, specificity was significantly higher (66% vs. 49%, P = 0.043). Overall accuracy was 71% for EBT and 53% for MRI (P = 0.014)., Conclusions: For the detection of high-grade restenosis after PTCA, EBT demonstrated significantly higher accuracy than MRI.

Background: Recent studies demonstrated an influence of atherosclerotic risk factors on the progression of aortic valve stenosis. The extent of aortic valve calcification (AVC) was also found to be a strong predictor of stenosis progression. We investigated the influence of the LDL cholesterol level (LDL), other standard cardiovascular risk factors, and the extent of coronary calcification (CC) on the progression of AVC quantified by electron beam tomography (EBT)., Methods and Results: In 104 patients (64.7+/-8 years, 89 male) with an EBT scan positive for AVC, CC and AVC were quantified using a volumetric score. EBT was repeated at a mean interval of 15 months (10 to 36 months), and the progression of AVC and CC was determined. Patients were divided into 2 groups according to LDL: group 1, LDL3.36 mmol/L (130 mg/dL), 47 patients. Mean values for CC were 546+/-932 mm(3) in scan 1 and 665+/-1085 mm(3) in scan 2 for AVC 324+/-796 mm(3) and 404+/-1076 mm(3), respectively. In recent years, several techniques for noninvasive imaging of the coronary artery lumen (noninvasive coronary angiography) have been developed. These techniques include magnetic resonance imaging, electron-beam computed tomography, and, most recently, multislice computed tomography. Each of these techniques has specific advantages and disadvantages. Currently, EBCT seems to permit the most robust coronary artery imaging. In the future, imaging modalities will have to be further improved and validated in order to define specific areas for potential clinical applications.

Purpose: We conducted phantom studies to investigate whether overlapping cross-sections and volumetric scoring would significantly improve interscan reproducibility of electron beam tomography (EBT) for coronary artery calcium quantification., Method: Fifteen phantoms simulating various amounts of coronary calcification were scanned in five different positions with a slice thickness of 3.0 mm and a table feed of 3.0, 2.5, and 2.0 mm. For the conventional "Agatston score" and a "volume score" (total volume of calcified lesions), interscan variabilities were compared between the three image acquisition protocols., Results: Agatston score variability was significantly lower for the 2.0 mm table feed than for the 3.0 or 2.5 mm table feed (3.0 mm: 22.9 +/- 10.3%; 2.5 mm: 13.6 +/- 8.2%; 2.0 mm: 8.9 +/- 5.5%). Volume score variability was significantly lower for 2.5 and 2.0 mm table feed than for 3.0 mm table feed (3.0 mm: 21.7 +/- 11.0%; 2.5 mm: 10.9 +/- 5.9%; 2.0 mm: 9.8 +/- 5.9%)., Conclusion: Overlapping cross-sections, especially in combination with volumetric scoring, significantly improved interscan reproducibility of EBT calcium quantification in a phantom study.

Background: The detection and quantification of coronary calcifications by electron beam tomography (EBT) permits to diagnose coronary atherosclerosis in an early stage. Initial reports indicate that multislice spiral CT (MSCT) also permits the quantification of coronary calcium, while equivalency to EBT has not been definitely proven. Since image acquisition, reconstruction and evaluation parameters influence the results of calcium quantification with CT techniques, standardization of the investigation is mandatory to make results comparable., Aim: The article summarizes guidelines for image acquisition and evaluation by EBT and describes guidelines and consensus reports that were issued concerning the clinical use of the method.

Background: Multislice spiral computed tomography (MSCT) with retrospectively ECG-gated image reconstruction permits coronary artery visualization. We investigated the method's ability to identify high-grade coronary artery stenoses and occlusions., Methods and Results: A total of 64 consecutive patients were studied by MSCT (4x1 mm cross-sections, 500-ms rotation, table feed 1.5 mm/rotation, intravenous contrast agent, retrospectively ECG-gated image reconstruction). All coronary arteries and side branches with a luminal diameter >/=2.0 mm were assessed concerning evaluability and the presence of high-grade stenoses (>70% diameter stenosis) or occlusions. Results were compared with quantitative coronary angiography. Of 256 coronary arteries (left main, left anterior descending, left circumflex and right coronary artery, including their respective side branches), 174 could be evaluated (68%). In 19 patients (30%), all arteries were evaluable. Artifacts caused by coronary motion were the most frequent reason for unevaluable arteries. Overall, 32 of 58 high-grade stenoses and occlusions were detected by MSCT (58%). In evaluable arteries, 32 of 35 lesions were detected, and the absence of stenosis was correctly identified in 117 of 139 arteries (sensitivity, 91%; specificity, 84%). If analysis was extended to all stenoses with >50% diameter reduction, sensitivity was 85% (40 of 47) and specificity was 76% (96 of 127)., Conclusions: MSCT with retrospective ECG gating permits the detection of coronary artery stenoses with high accuracy if image quality is sufficient, but its clinical use may presently be limited due to degraded image quality in a substantial number of cases, mainly due to rapid coronary motion.

In 120 patients, the mean interscan variability of coronary calcium quantification by electron beam tomography was 19.9% (median 7.8%) for the traditional calcium score, and 16.2% (median 5.7%) for volumetric scoring. Although this difference was not significant, there was a significant influence of the total amount of calcium, number of acquired images, and image noise on interscan reproducibility.

Anomalous coronary arteries are rare conditions. However, they may cause myocardial ischemia and sudden death and their reliable identification is crucial for any imaging method that attempts coronary artery visualization. We studied the ability of contrast-enhanced electron beam tomography (EBT) to identify anomalous coronary arteries and their course. Thirty patients with previously identified coronary anomalies and 30 subjects with normal coronary anatomy were studied. By EBT, 40 to 50 axial images of the heart (3-mm slice thickness, 1 mm overlap, electrocardiographic trigger) were acquired in a single breathhold during continuous injection of contrast agent (160 ml, 4 ml/s). Based on the original images and 3-dimensional reconstructions, the EBT data were analyzed by 2 blinded observers as to the presence of coronary anomalies and their course. Results were compared with invasive angiography. EBT correctly identified all normal controls and all patients with coronary anomalies. The anatomic course of the coronary anomalies was correctly classified in 29 of 30 patients (97%), including right-sided origin of the left main coronary artery (n = 4) or of the left circumflex coronary artery (n = 15), left-sided origin of the right coronary artery (n = 9), and 1 coronary fistula from the left circumflex coronary artery to the right atrium. Only the distal anastomosis of a second fistula from the left circumflex coronary artery to a bronchial artery was not correctly identified. This study demonstrates that contrast-enhanced EBT is a reliable noninvasive technique to identify anomalous coronary arteries and their course.

Background: We investigated the applicability and image quality of contrast-enhanced coronary artery visualization by multislice spiral CT using retrospective ECG gating., Methods and Results: Twenty-five patients in sinus rhythm (significant coronary artery stenoses ruled out by invasive angiography) were studied with a multislice spiral CT (Siemens SOMATOM Volume Zoom). In inspiration (mean breath-hold, 37 seconds), a volume data set of the heart was acquired (intravenous contrast agent; 4 x 1-mm slice thickness; 500-ms rotation; table feed, 1.5 mm/360 degrees ). Simultaneous recording of the ECG permitted retrospective reconstruction of contiguous cross sections in intervals of 1 mm at any desired interval of the cardiac cycle. The mean duration of the image reconstruction window was 185 ms. Next to 3-dimensional reconstructions of the heart and coronary arteries, multiplanar reconstructions were rendered to determine the visualized length of the coronary arteries, the contrast-to-noise ratio, and the correlation of coronary artery diameters to quantitative coronary angiography., Conclusions: The coronary arteries could be visualized over long segments (left main, 9+/-4 mm; left anterior descending, 112+/-34 mm; left circumflex, 80+/-29 mm; right coronary artery, 116+/-33 mm). On average, 78+/-16% of these distances were visualized free of motion artifacts. The mean contrast-to-noise ratio was 9.3+/-3.3. Coronary artery diameters in multislice spiral CT showed close correlation to quantitative coronary angiography (CT, 3.3+/-1.0 mm; angiography, 3. 2+/-0.9 mm; mean difference, 0.38 mm; r=0.86). Contrast-enhanced multislice spiral CT permits visualization of the coronary artery lumen. Further studies are necessary to determine whether image quality is sufficient to reliably detect coronary artery stenoses.

Objective: To evaluate the accuracy of contrast enhanced electron beam computed tomography (EBCT) after acute myocardial infarction in determining patency of the infarct related artery and detecting high grade stenoses and occlusions in the coronary vessels., Design: Case study using blinded comparison with invasive coronary angiography., Patients: 36 patients (mean age 53 years) 4-70 days after acute myocardial infarction., Interventions: The patients were studied by EBCT and invasive coronary angiography. For EBCT, 50 axial images of the heart (3 mm slice thickness) were acquired. They were triggered by the ECG during breath holding, after intravenous injection of contrast agent. The original images, surface reconstructions, and maximum intensity projections were evaluated for the presence of high grade stenoses and occlusions of the coronary arteries., Main Outcome Measures: EBCT results were compared with invasive coronary angiography., Results: Of a total of 144 coronary arteries (left main, left anterior descending, left circumflex, and right coronary artery in 36 patients), 29 (20%) were unevaluable by EBCT. In the remaining arteries, 33 of 36 high grade lesions were correctly detected (92% sensitivity). Specificity was also 92% (73/79). Patency of the infarct related artery was correctly detected in 15 of 16 cases (94%). Five of the 14 occluded infarct related arteries (35%) were mistaken as stenotic but patent, and six could not be assessed., Conclusions: EBCT is very accurate in detecting significant coronary artery lesions in patients after acute myocardial infarction, but differentiation between occluded and patent infarct related arteries is currently unreliable.

Magnetocardiography constitutes a new tool for monitoring fetal cardiac activity. The fetal magnetocardiogram (FMCG) recorded noninvasively over the maternal abdomen is detectable with high temporal resolution and permits analysis of all parts of the PQRST waveform. In this way measurements of cardiac time intervals, including the QT interval, become possible. The following article constitutes the first report of antenatal detection of QT prolongation in two fetuses by FMCG.

Purpose: To determine the speed of and changes in the speed of coronary arterial movement during the cardiac cycle with electron-beam computed tomography (CT)., Materials and Methods: With electron-beam CT, 20 consecutive cross-sectional images were acquired at the mid right coronary artery (with 50-msec acquisition time, 8-msec intersection delay, 7-mm section thickness, and intravenous administration of 40 mL of contrast agent) in 25 patients. On the basis of the displacement of the left anterior descending, left circumflex, and right coronary arterial cross sections from image to image, movement velocity in the transverse imaging plane was calculated and was correlated with the simultaneously recorded electrocardiogram., Results: The velocity of in-plane coronary arterial motion varied considerably during the cardiac cycle. Peaks were caused by ventricular systole and diastole and by atrial contraction. The mean velocity was 46.6 mm/sec +/- 12. 5 (SD). The mean velocity of right coronary arterial movement (69.5 mm/sec +/- 22.5) was significantly faster than that of the left anterior descending (22.4 mm/sec +/- 4.1) or the left circumflex coronary artery (48.4 mm/sec +/- 15.0). The lowest mean velocity (27. 9 mm/sec) was at 48% of the cardiac cycle., Conclusion: The lowest velocity of coronary arterial movement, which displays considerable temporal variation, was at 48% of the cardiac cycle.

Objectives: The purpose of this study was to evaluate a contrast-enhanced three-dimensional (3D) breath-hold magnetic resonance (MR) technique for detection of coronary artery stenoses., Background: The accuracy of previously published MR coronary angiography protocols varies widely. Recently, coronary artery imaging using T1-shortening contrast agent has become possible, but so far there are no data concerning its clinical application., Methods: Magnetic resonance coronary angiography was performed in 50 patients with suspected coronary artery disease. Magnetic resonance data acquisition using an ultrafast 3D gradient-echo sequence lasted over 32 heartbeats within one single breath-hold. Twenty milliliters of gadopentetate dimeglumine was injected at a flow rate of 1 ml/s for two successive studies covering the main coronary arteries in single-oblique planes. Stenosis assessment by MR was compared with significant (diameter stenosis > 50%) stenoses on X-ray angiography. Evaluation was limited to the proximal and mid-coronary artery segments., Results: Two hundred sixty-eight of 350 artery segments (76.6%) could be evaluated. Left circumflex coronary artery was only evaluable in 50% of cases by MR. In the evaluable segments, 48 of 56 stenoses and 193 of 212 nonstenotic segments were correctly classified by MR. On a patient basis, MR correctly identified 34 of 36 patients with and 8 of 14 patients without significant coronary stenoses as demonstrated by X-ray angiography (sensitivity 94.4%, specificity 57.1%)., Conclusions: Oblique projection contrast-enhanced MR coronary angiograms obtained within one single breath-hold permit identification of patients with coronary stenoses in the proximal and mid segments of the major coronary arteries with satisfactory accuracy.

Magnetic resonance imaging of the coronary arteries is difficult due to the tortuous course of these vessels, their small diameter, and their rapid movement caused by respiration and cardiac contraction. Initial investigations could demonstrate the feasibility of non-invasive magnetic resonance coronary angiography using 2-dimensional turbo-FLASH gradient-echo sequences in repeated breathholds of approximately 16 heart beats duration. Further developments, especially the design of navigator-echo-based respiratory gated 3-dimensional imaging sequences, permitted the acquisition of contiguous volume data sets of the heart which eliminated many limitations of 2-dimensional repeated breathhold sequences. With a spatial resolution of approximately 1.2 x 1.2 x 2 mm and a temporal resolution of approximately 126 ms, several authors reported sensitivities of 70-80% and specificities of approximately 90% for the detection of coronary artery stenoses. Further improvements can be expected from new, intravascular contrast agents and from ultrafast sequences which permit acquisition of a sufficiently large imaging volume within one single breathold.

Gadolinium-enhanced, three-dimensional, breath-hold magnetic resonance (MR) coronary angiography was performed in two healthy volunteers and 11 patients suspected or known to have coronary artery disease. MR angiograms were compared with those obtained with retrospective respiratory gating. Of 52 main coronary arteries, 47 could be visualized with the breath-hold technique and 49 with the gating technique. Signal-to-noise and contrast-to-noise ratios were significantly higher with the breath-hold technique. Overall image quality was slightly lower with breath-hold imaging. With either technique, three of five, significant coronary stenoses were correctly identified.

Background: Reliable noninvasive assessment of coronary-artery stenoses and occlusions would constitute an advantage in the care of patients with known or suspected coronary artery disease. We investigated the accuracy of contrast-enhanced electron-beam computed tomography (CT) for the detection of high-grade coronary-artery stenoses and occlusions., Methods: Electron-beam CT was performed in 125 patients. After intravenous injection of a contrast agent, 40 cross-sectional images of the heart were acquired during inspiration, triggered by the electrocardiogram in diastole. Three-dimensional reconstructions of the heart and coronary arteries were rendered to facilitate evaluation of the images. The proximal and middle segments of the major coronary arteries were evaluated for the presence or absence of high-grade stenoses and occlusions. The results were compared with those of invasive coronary angiography in a blinded fashion., Results: Because of technical problems that impaired the quality of the images, 124 (25 percent) of the 500 coronary arteries studied (left main, left anterior descending, left circumflex, and right coronary) in a total of 125 patients were excluded from evaluation. No vessels could be evaluated in 19 patients (15 percent), and another 28 patients (22 percent) had one, two, or three vessels that could not be evaluated. In the remaining coronary arteries with adequate image quality, electron-beam CT permitted visualization of 69 of 75 high-grade stenoses and occlusions (sensitivity, 92 percent), whereas in 282 of 301 arteries, the absence of high-grade stenoses and occlusions was correctly detected (specificity, 94 percent)., Conclusions: When image quality is adequate, electron-beam CT may be useful to detect or rule out high-grade coronary-artery stenoses and occlusions.

The aim of this study was to evaluate the association between carotid and coronary atherosclerosis and their diagnostic value for predicting angiographically significant coronary artery disease (CAD). We investigated 80 subjects (mean age 55 +/- 8 years) by electron beam computed tomography (EBT), the most sensitive technology for noninvasive detection of coronary calcification (a marker of coronary atherosclerosis); by carotid sonography; and by coronary angiography. Carotid ultrasound was performed with a 7.5 MHz Duplex probe, EBT was done with an EVOLUTION(R) scanner. In 47 subjects, coronary calcification as well as carotid atherosclerosis was present. Thirteen subjects showed isolated coronary calcification, 10 had isolated carotid atherosclerosis, and 10 showed neither coronary calcification nor carotid atheroscleroosis. The association between carotid atherosclerosis and CAD as well as the relationship between coronary calcification and CAD were statistically significant (p < 0.05), but the correlation between coronary calcification and CAD was higher than between carotid atherosclerosis and CAD (Pearson contingency coefficient: 0.34 vs 0.86). In subjects without carotid atherosclerosis, the mean area of coronary calcificiation was significantly (p < 0.01) lower (41 mm2) than in subjects with carotid atherosclerosis (113 mm2). For noninvasive detection of CAD, electron beam CT was superior to carotid sonography on sensitivity (95% vs 78%) and specificity (81% vs 48%). Former studies suggested that sonography of extracoronary vessels could aid in screening for CAD. In our study, the sonographic status of carotid arteries was associated with cardiovascular atherosclerosis but it did not allow us to make a judgment about the presence of CAD with sufficient reliability and it was inferior to electron beam CT as a noninvasive marker of CAD.

This report describes a large congenital coronary artery fistula of the left circumflex artery draining into the coronary sinus in a 46 year old woman. The fistula was initially diagnosed by cross-sectional and transesopha-geal echocardiography using multiplane probe. Additionally, the entire course of the fistula was visualised in electron beam tomography. Angiography confirmed echocardiographic and tomographic findings.

Coronary blood flow quantification provides essential information on the hemodynamic significance of coronary artery stenoses. Recently, magnetic resonance (MR) flow mapping has emerged as a new promising method to noninvasively determine flow velocity and flow volume within the coronary arteries. The aim of this study was to compare phase difference (PD) MR flow quantification with intracoronary Doppler flow measurements in 15 patients with suspected or known coronary artery disease. Flow quantification was attempted before and after systemic application of 5mg Isosorbiddinitrate (ISDN) in order to determine possible alterations in coronary flow volume. PD MR flow mapping was performed successfully in 13 of the 15 patients. For flow velocities and flow volume values, a close correlation between PD MR and Doppler flow measurements was found (r = 0.79 and r = 0.90, respectively). However, average flow measured by PD MR was significantly lower than the invasively obtained values (9.0 +/- 4.4 cm/sec vs. 11.7 +/- 4.9 cm/sec; p < 0.001 and 46.3 +/- 28.7 ml/min vs. 53.4 +/- 32.8 ml/min; p < 0.05). Although the mean flow volume remained constant in the entire patient group after ISDN application, individual changes (increase in 6, decrease in 4 cases) could be documented with PD MR imaging and showed a good correlation to the Doppler method (r = 0.82). In conclusion, PD MR flow mapping is a promising method for the noninvasive quantification of coronary blood flow and therefore offers the potential of assessing coronary artery stenoses. However, technical improvements are mandatory in order to increase accuracy of the method.

Electron beam computed tomography (EBCT) is a cross-sectional imaging method with high temporal and spatial resolution. So far, it has mainly been applied for the detection of coronary artery calcifications which permit the very sensitive detection of coronary atherosclerosis even in the very early stages. However, after intravenous injection of a contrast agent, EBCT also permits the direct visualization of the coronary artery lumen. For these investigations, a volume data set is acquired that consists of 40 axial cross-sections of the heart (3 mm slice thickness). To evaluate the coronary arteries as to the presence of stenoses and occlusions, various forms of post-processing, including shaded surface display, maximum intensity projection, and multiplanar reconstruction, are applied. The sensitivities and specificities for the detection of coronary artery stenoses and occlusions are about 90%. Best results are obtained for coronary artery bypass grafts, the left main coronary artery, and the left anterior descending coronary artery, while reduced image quality impairs the results for the right coronary artery and the left circumflex coronary artery.

Objective: We investigated the applicability of curved multiplanar reconstructions for the evaluation of contrast-enhanced electron beam CT scans of the coronary arteries., Subjects and Methods: Thirty-two patients (43-72 years old; mean age, 58 years old) underwent electron beam CT. After injection of i.v. contrast medium, 40 axial cross sections of the heart were acquired, triggered to the ECG during breath-hold (3-mm slice thickness, 1-mm overlap). Curved multiplanar reconstructions were obtained separately for each coronary artery. The reconstructions were independently evaluated by two investigators for the presence of high-grade stenoses and occlusions. The results were then compared with coronary angiography results, of which the two investigators had been unaware., Results: Because of degraded image quality, 15 (12%) of the 128 vessels (left main, left anterior descending, left circumflex, and right coronary arteries in 32 patients) were excluded from evaluation. In the remaining 113 vessels, 16 (89%) of 18 high-grade stenoses and occlusions were correctly detected (89% sensitivity). Absence of significant stenosis was correctly detected in 87 (92%) of 95 vessels (92% specificity). The negative and positive predictive values were 98% and 67%, respectively., Conclusion: Curved multiplanar reconstructions are useful in the evaluation of contrast-enhanced electron beam CT scans of coronary arteries.

Electron beam tomography (EBT) has been shown to permit non-invasive imaging of the coronary arteries after intravenous injection of contrast agent and 3-dimensional reconstruction. We compared the vessel diameters in EBT reconstructions to quantitative coronary angiography (QCA). 10 patients were investigated by EBT and QCA. 3-dimensional EBT reconstructions (shaded surface display) were performed after acquisition of 40 axial cross-sections of the heart with 3 mm slice thickness (1 mm overlap) which were obtained triggered to the ECG in breathhold following intravenous injection of 120-160 ml of contrast agent. A fixed lower reconstruction threshold of 80 HU was used to selectively visualize the contrast-enhanced coronary artery lumen. At 60 sites in the coronary artery system, the vessel diameters measured in the EBT reconstructions were compared to the diameters found in quantitative analysis of the patients' coronary angiograms. The correlation coefficient of the vessel diameters in EBT and QCA was 0.83. Mean vessel diameters were not significantly different in EBT and QCA (3.06 +/- 0.93 vs. 2.97 +/- 0.94 mm). However, very small vessel diameters tended to be underestimated in the EBT reconstructions, which was due to the partial volume effect. It only vessel diameters measured in the left main or left anterior descending coronary artery were compared to QCA, the correlation increased to 0.87, since these vessel segments are less prone to artifacts in the EBT investigation. Vessel diameters in EBT and QCA correlated reasonably well. Due to partial volume effects, the diameter of very small vessels and stenotic segments tends to be underestimated by EBT.

Background: Contrast-enhanced electron beam computed tomography (EBCT) has been shown to permit noninvasive visualization of the coronary arteries. We determined the value of EBCT to noninvasively detect high-grade restenosis after percutaneous transluminal coronary angioplasty (PTCA)., Methods and Results: Fifty patients (37 to 68 years of age), were investigated by EBCT at a mean interval of 9 months after PTCA of coronary artery stenoses. Forty axial cross-sections of the heart (3-mm slice thickness, 1-mm overlap) were acquired triggered to the ECG after intravenous injection of contrast agent. Three-dimensional reconstructions of the coronary arteries were rendered with a lower threshold of 80 HU to selectively visualize the contrast-enhanced vessel lumen. EBCT results were compared with conventional quantitative coronary angiography (QCA) performed within 1 week. In 6 patients, the PTCA segment could not be evaluated because of impaired image quality. Sixteen of the remaining 44 patients had high-grade restenoses in QCA (>70% diameter reduction), which was correctly detected by EBCT in 15 cases (94% sensitivity). There were 5 false-positive EBCT results of high-grade restenosis (82% specificity)., Conclusions: EBCT with intravenous injection of contrast agent permits the noninvasive diagnosis of restenosis after PTCA, with high sensitivity and sufficient specificity.

Magnetic resonance coronary angiography (MRCA) is a promising method for the assessment of proximal coronary artery stenosis. Conventional 2-dimensional techniques require repetitive breath holds to image multiple sections. This may lead to misregistrations if the respiratory level is not exactly reproduced. In the present study, MRCA was performed using a 3-dimensional approach with navigator echo-based respiratory gating. In 73 patients (55 men and 18 women) who were referred for cardiac catheterization, the assessment of significant stenoses (> or = 50%) was performed in the proximal and midsegments of the coronary arteries after multiplanar reconstruction of the visualized coronary arteries. In addition, in 8 patients with coronary artery bypass grafts the patency of the transplants was evaluated. After withdrawing 8 patients from analysis because of poor image quality, stenosis evaluation was possible in 236 of 455 reviewed coronary segments (52%). In the other 219 cases, either the visualization of the vessel segment was indistinct (30%) or the segment was located outside the imaging volume (18%). In total, 28 of 43 significant coronary stenoses could be correctly identified (65%). Evaluation of bypass graft patency was possible in 7 patients. All 4 occluded and 13 of 15 patent grafts were correctly classified. Thus, respiratory gated MRCA is a feasable method for the assessment of hemodynamically significant coronary stenoses and bypass graft patency. However, technical improvements are mandatory to improve accuracy of the method.

The persistence of enteroviral ribonucleic acid (RNA) in the myocardium has been implicated as a pathogenetic factor in idiopathic dilated cardiomyopathy. Enteroviral persistence may lead to myocardial cell membrane damage, resulting in increased uptake of antimyosin antibodies. To further evaluate this hypothesis, a direct comparison of myocardial antimyosin uptake with the presence of enteroviral RNA was performed in ten patients (one female, nine male; 53+/-8 years) with chronic dilated cardiomyopathy. Planar antimyosin images were obtained 48 h after the injection of indium-111-labelled antimyosin Fab. Using a region of interest technique, the heart to lung uptake ratio (HLR) was calculated as a semiquantitative parameter of myocardial tracer uptake. Cardiac catheterization was performed to assess left ventricular function and to obtain myocardial biopsy samples. In the biopsy samples, gene amplification by polymerase chain reaction (PCR) was used to specifically detect enteroviral RNA. In the ten patients, the left ventricular ejection fraction was 39%+/-11% and the end-diastolic volume 131+/-46 ml/m2. The HLR was 1.72+/-0.21 and showed no correlation with functional parameters. In two patients with a positive PCR consistent with persisting enteroviral RNA, the HLR was not higher than that in eight patients with a negative PCR (1.46+/-0. 18 vs 1.78+/-0.18, respectively). These results suggest that increased uptake of 111In-antimyosin in chronic idiopathic dilated cardiomyopathy cannot be explained by pure persistence of enteroviral RNA. Other pathogenetic factors such as myocardial autoantibodies or microvascular spasm may be responsible for myocyte membrane damage detected by antimyosin.

In phantom studies we investigated the effects of magnetic resonance imaging (MRI) on pacemakers and electrodes. Twenty-five electrodes were exposed to MRI in a 1.5T scanner with continuous registration of the temperature at the electrode tip. Eleven pacemakers (five single chamber and six dual chamber) were exposed to MRI. Pacemaker output was monitored to detect malfunction in VOO/DOO and VVI/DDD modes. A temperature increase at the electrode tip of up to 63.1 degrees C was observed during 90 seconds of scanning. In seven electrodes the temperature increase exceeded 15 degrees C. Although no pacemaker malfunctions were observed in asynchronous pacing mode (VOO/DOO), inhibition and rapid pacing were observed during spin-echo imaging if the pacemakers were set to VVI or DDD mode. Pacemaker function was not impaired during scanning with gradient-echo sequences. Next to pacemaker dysfunction, electrode heating has to be considered a possible adverse effect when exposing patients with pacemakers to MRI.

Electron beam tomography (EBT, ultrafast computed tomography [CT], cine CT) combines unique temporal and high spatial resolution and is especially well suited for cardiac imaging. We established and evaluated a protocol for the noninvasive visualization and assessment of aortocoronary artery bypass grafts. Twenty-five patients with 56 bypass grafts were studied by EBT. Forty contiguous cross-sectional images were acquired triggered to the electrocardiogram during breathhold and intravenous injection of contrast agent. Three-dimensional reconstructions of the heart and bypass grafts were performed and compared with selective angiography of the bypass grafts. In 1 patient with 2 bypass grafts, a technically inadequate EBT examination was obtained. In the remaining patients (54 grafts), all 13 bypass occlusions were diagnosed with a sensitivity and specificity of 100%. Evaluation for hemodynamically relevant stenosis was possible in 84% of cases (36 of 43 patent grafts) and yielded a sensitivity of 100% (5 of 5 high-grade stenoses correctly detected) and specificity of 97% (1 false-positive diagnosis of high-grade graft stenosis). The main reasons for impaired ability to evaluate the scans were breathing artifacts and misplacement of the imaging volume, causing parts of the bypass grafts to be cut off. EBT permits noninvasive determination of bypass graft occlusion and relevant stenosis with high accuracy.

The presented computer model reconstructs the ECG with 500 myocardium cells based on seven ionic channels. The low hardware-requirement and the exact demonstration of physiological and pathophysiological coherence make this simulation model very useful for medical scientific education.

Electron Beam Tomography (EBT), a non-invasive imaging method with very high spatial and temporal resolution, is well suited for cardiac imaging. We established a protocol for the visualization of the coronary arteries by EBT and have so far evaluated the method in 88 patients. EBT imaging was performed using an Evolution scanner with a matrix of 512 x 512, a field of view of 15 cm and a slice thickness of 3 mm. After venous injection of contrast agent, 40 axial cross-sections of the heart were obtained triggered to the ECG at 80% of the R-R interval in inspiratory breathhold. With a lower threshold of 80 HU, 3D reconstructions of the heart and coronary arteries were rendered using shaded-surface display and maximum-intensity projection techniques. In all patients, the coronary arteries and, if present, aorto-coronary bypass grafts, were evaluated as to their visibility and presence or absence of haemodynamically relevant stenoses. All results were compared to the X-ray angiograms obtained by selective catheterization in a blinded manner. In 90% of cases, image quality was sufficient for evaluation. In 10% of the investigations, the EBT images could not be evaluated, mainly due to respiration artifacts. While the left anterior descending coronary artery was represented in very good image quality, the right coronary artery and left circumflex coronary artery showed decreased image quality due to motion artifacts and close correlation to venous structures such as the coronary sinus and atrial appendages. In a subgroup of 30 patients in which EBT results were compared to quantitative coronary angiography, the correlation of the vessel diameter in EBT and quantitative angiography was 0.82. Twelve out of 14 high-grade stenoses and 5/5 occlusions of the left anterior descending coronary artery as well as 3/4 highgrade stenoses and 1/2 occlusions of the right coronary artery and 2/4 high-grade stenoses of the left circumflex coronary artery were correctly identified. Nine patients were reinvestigated by EBT after successful angioplasty of high-grade coronary artery stenoses. The increase in vessel diameter could be documented in all cases. Twelve patients had a total of 28 aorto-coronary bypass grafts. Except for one mammarian artery graft which could not be evaluated due to small vessel lumen and clip artifacts, graft patency (21 cases) or occlusion (6 cases) was correctly diagnosed by EBT. EBT is a non-invasive method which permits the visualization of the coronary arteries. Stenoses of coronary artery bypass grafts and of the left anterior descending coronary artery can be reliably diagnosed, while reduced image quality, mainly due to faster vessel motion, impairs the results obtained for the right and left circumflex coronary artery.

The purpose of this study was to assess the value of electron beam computed tomography in the detection of cardiac calcifications in coronaries and valves of dialysis patients and to determine the rate at which calcification progresses. Forty-nine chronic hemodialysis patients aged 28 to 74 years were compared with 102 non-dialysis patients aged 32 to 73 years with documented or suspected coronary artery disease, all of whom underwent coronary angiography. We used high-resolution electron beam computed tomography scanning to make 30 axial slices with a distance of 3 mm between each slice. The number of calcifications, the surface area, and the average and highest density values were measured. We calculated a quantitative coronary artery calcium score and assessed calcification of mitral and aortic valves. In dialysis patients, the measurements were repeated after 12 months. The coronary artery calcium score was from 2.5-fold to fivefold higher in the dialysis patients than in the non-dialysis patients. Hypertensive dialysis patients had higher calcium scores than non-hypertensive dialysis patients (P < 0.05). A stepwise, multiple regression analysis confirmed the importance of age and hypertension. No correlation between calcium, phosphate, or parathyroid hormone values and the coronary calcium score was identified; however, the calcium score was inversely correlated with bone mass in the dialysis patients (r = 0.47, P < 0.05). The mitral valve was calcified in 59% of dialysis patients, while the aortic valve was calcified in 55%. The coronary artery calcium score was correlated with aortic valvular, but not mitral valvular calcification. A repeat examination of the dialysis patients at an interval of 1 year showed a disturbing tendency for progression. Our data under-score the frequency and severity of coronary and valvular calcifications in dialysis patients, and illustrate the rapid progression of this calcification. Finally, they draw attention to hypertension as an important risk factor in this process.
