Abstract

Cutting fluids are usually applied during milling to reduce the friction and to protect the tool and the material from corrosion. These fluids are associated with toxicity and environmental problems. Moreover, the waste management of cutting fluids entails large expenses. The need to reduce cutting fluids has fostered the use of alternative coolants such as supercritical (sc) CO2, alone or with minimum quantity lubrication (MQL). sc CO2 and sc CO2 + MQL coolants have been studied for face milling of a cold worked (CW) AISI 316L stainless steel (SS), evaluating their effect on the residual stresses generated in the surface, in the outermost microstructure of this material, and the corrosion performance. Furthermore, they are compared with those caused by traditional face milling and with a manually ground-generated surface. Ultrafine grain (UFG) layers of about 1 μm and passive layers (of similar chemical compositions) are identified for all the surfaces under study. The three milling processes under study generate a deformation layer under the UFG layer that does not appear below ground surfaces. Moreover, the preexistent compressive stresses created by the CW process change into tensile, being higher for the alternative green machining processes than for the traditional one. The probability of undergoing pitting (studied with cyclic polarization curves) appears to be linked to the nature and structure of the passive layer (characterized by Auger spectroscopy and Mott–Schottky analyses, respectively). Electrochemical impedance spectroscopy studies also confirm similar electrochemical performances for all analyzed surfaces. The active-to-passive transitions of the SS, which have been characterized by electrochemical potentiodynamic reactivation tests, appear to be related to the stresses and deformation state of the deformed layers. Passivation on the alloy in acid media appears to be favored after the sc CO2 and sc CO2 + MQL alternative milling processes than after traditional face milling and grinding. [ABSTRACT FROM AUTHOR]

Abstract

Using one-dimensional, long-timescale particle-in-cell simulations, we study the processes of ion acceleration from the interaction of ultraintense ( 10 20 W cm − 2 ), ultrashort (30 fs) laser pulses with near-critical, nonuniform gas targets. The considered initially neutral, nitrogen gas density profiles mimic those delivered by an already developed noncommercial supersonic gas shock nozzle: they have the generic shape of a narrow (20 μ m wide) peak superimposed on broad (∼1 mm, ∼ 180 μ m scale length), exponentially decreasing ramps. While keeping its shape constant, we vary its absolute density values to identify the interaction conditions leading to collisionless shock-induced ion acceleration in the gas density ramps. We find that collisionless electrostatic shocks (CES) form when the laser pulse is able to shine through the central density peak and deposit a few 10% of its energy into it. Under our conditions, this occurs for a peak electron density between 0.35 n c and 0.7 n c . Moreover, we show that the ability of the CES to reflect the upstream ions is highly sensitive to their charge state and that the laser-induced electron pressure gradients mainly account for shock generation, thus highlighting the benefit of using sharp gas profiles, such as those produced by shock nozzles. 18. Deep-learning for epicardial adipose tissue assessment with computed tomography: implications for cardiovascular risk prediction

Abstract

BACKGROUND: Epicardial adipose tissue volume (EAT) is a marker of visceral obesity that can be measured in coronary CT angiograms (CCTA). The clinical value of integrating this measurement in routine CCTA interpretation has not been documented. OBJECTIVE: To develop a deep-learning network (DLN) for automated quantification of EAT from CCTA, test it in technically challenging patients, and validate its prognostic value in routine clinical care. METHODS: The DLN was trained and validated to auto-segment EAT in 3720 CCTA scans from the Oxford Risk Factors And Non-invasive imaging cohort. The model was tested in patients with challenging anatomy and scan artefacts and applied to a longitudinal cohort of 253 post-cardiac surgery patients and 1558 patients from the SCOT-HEART trial, to investigate its prognostic value. RESULTS: External validation of the DLN yielded a concordance correlation coefficient of 0.970 for machine vs human. EAT was associated with coronary artery disease (OR[95%CI] per SD increase in EAT 1.13[1.04-1.30] p=0.01), and atrial fibrillation (AF) (1.25[1.08-1.40], p=0.03), after correction for risk factors (including BMI). EAT predicted all-cause mortality (HR[95%CI] per SD = 1.28[1.10-1.37], p=0.02), myocardial infarction (1.26[1.09-1.38] p=0.001) and stroke (1.20[1.09-1.38] p=0.02) independently of risk factors in SCOT-HEART (5y follow-up). It also predicted in-hospital (HR[95%CI] = 2.67[1.26-3.73], p=< 0.01) and long-term post-cardiac surgery AF (7y follow-up; HR[95%CI] = 2.14[1.19-2.97], p=< 0.01). CONCLUSIONS: Automated assessment of EAT is possible in CCTA, including in challenging patients; it forms a powerful marker of metabolically unhealthy visceral obesity, which could be used for cardiovascular risk stratification.

Abstract

Strong electromagnetic pulses (EMPs) are generated from intense laser interactions with solid-density targets and can be guided by the target geometry, specifically through conductive connections to the ground. We present an experimental characterization by time- and spatial-resolved proton deflectometry of guided electromagnetic discharge pulses along wires including a coil, driven by 0.5 ps, 50 J, 1019 W/cm2 laser pulses. Proton-deflectometry allows us to time-resolve first the EMP due to the laser-driven target charging and then the return EMP from the ground through the conductive target stalk. Both EMPs have a typical duration of tens of ps and correspond to currents in the kA-range with electric-field amplitudes of multiple GV/m. The sub-mm coil in the target rod creates lensing effects on probing protons due to both magnetic- and electric-field contributions. This way, protons of the 10 MeV-energy range are focused over cm-scale distances. Experimental results are supported by analytical modeling and high-resolution numerical particle-in-cell simulations, unraveling the likely presence of a surface plasma, in which parameters define the discharge pulse dispersion in the non-linear propagation regime. Abstract

OBJECTIVE: To assess whether eculizumab, a terminal complement inhibitor, improves patient- and physician-reported outcomes (evaluated using the myasthenia gravis activities of daily living profile and the quantitative myasthenia gravis scale, respectively) in patients with refractory anti-acetylcholine receptor antibody-positive generalized myasthenia gravis across four domains, representing ocular, bulbar, respiratory, and limb/gross motor muscle groups. METHODS: Patients with refractory anti-acetylcholine receptor antibody-positive generalized myasthenia gravis were randomized 1:1 to receive either placebo or eculizumab during the REGAIN study (NCT01997229). Patients who completed REGAIN were eligible to continue into the open-label extension trial (NCT02301624) for up to 4 years. The four domain scores of each of the myasthenia gravis activities of daily living profile and the quantitative myasthenia gravis scale recorded throughout REGAIN and through 130 weeks of the open-label extension were analyzed. RESULTS: Of the 125 patients who participated in REGAIN, 117 enrolled in the open-label extension; 61 had received placebo and 56 had received eculizumab during REGAIN. Patients experienced rapid improvements in total scores and all four domain scores of both the myasthenia gravis activities of daily living profile and the quantitative myasthenia gravis scale with eculizumab treatment. These improvements were sustained through 130 weeks of the open-label extension. Abstract

Warm-forged Type 304L stainless steel specimens have been tested in high temperature hydrogenated water under slow strain rate tensile test conditions to investigate the effect of machining on stress corrosion crack initiation. Roughness, residual stress and cross-section microstructures of the as-machined surfaces were characterized prior to the tests, and both plan-view and cross-section examinations were performed post-test. The results indicated that machining produced a deformation layer characterized by an ultrafine-grained outer layer and a highly deformed inner layer. The ultrafine-grained layer promoted a more uniform oxidation and enhanced SCC initiation resistance of the material compared to the highly polished deformation-free surfaces, which initiated significantly more intergranular cracks. The mechanisms for SCC initiation in the material have been discussed.

Abstract

Introduction/Aims: Individuals with refractory generalized myasthenia gravis (gMG) who have a history of rituximab use and experience persistent symptoms represent a population with unmet treatment needs. The aim of this analysis was to evaluate the efficacy and safety of eculizumab in patients with refractory anti-acetylcholine receptor antibody-positive (AChR+) gMG previously treated with rituximab. Methods: This post hoc subgroup analysis of the phase 3 REGAIN study (NCT01997229) and its open-label extension (OLE; NCT02301624) compared baseline characteristics, safety, and response to eculizumab in participants who had previously received rituximab with those who had not. Rituximab use was not permitted within the 6 months before screening or during REGAIN/OLE. Results: Of 125 REGAIN participants, 14 had received rituximab previously (7 received placebo and 7 received eculizumab). In the previous-rituximab group, 57% had used at least four other immunosuppressants compared with 16% in the no-previous-rituximab group. Myasthenia Gravis Activities of Daily Living total scores from eculizumab baseline to week 130 of eculizumab treatment improved in both the previous-rituximab and no-previous-rituximab groups (least-squares mean −4.4, standard error of the mean [SEM] 1.0 [n = 9] and least-squares mean −4.6, SEM 0.3 [n = 67], respectively; difference = 0.2, 95% confidence interval −1.88 to 2.22). In addition, in both groups, most patients who were treated with eculizumab for 130 weeks achieved a Myasthenia Gravis Foundation of America post-intervention status of minimal manifestations (66.7% and 65.0%, respectively). The eculizumab safety profile was similar between groups and consistent with its established profile. Discussion: Eculizumab is an effective therapy for patients with refractory AChR+ gMG, irrespective of whether they had received rituximab treatment previously.

Abstract

We demonstrate laser-driven Helium ion acceleration with cut-off energies above 25 MeV and peaked ion number above $10^8$ /MeV for 22(2) MeV projectiles from near-critical density gas jet targets. We employed shock gas jet nozzles at the high-repetition-rate (HRR) VEGA-2 laser system with 3 J in pulses of 30 fs focused down to intensities in the range between $9\times10^{19}$ W/cm$^2$ and $1.2\times10^{20}$ W/cm$^2$. We demonstrate acceleration spectra with minor shot-to-shot changes for small variations in the target gas density profile. Difference in gas profiles arise due to nozzles being exposed to a experimental environment, partially ablating and melting., 22 pages, 28 figures

Abstract

Fluid targets are attractive for laser-driven ion acceleration at high-repetition-rate laser facilities. In particular, liquid targets have several interesting features since they can regenerate in situ and are debris-free (or little) and operate at a high repetition rate. Here, we report high-repetition-rate MeV-range proton acceleration by the interaction of the VEGA-2 laser system with a micrometer-thick water sheet target in a high vacuum (10âˆ'4 mbar) environment. The water target was running continuously at this pressure for 4 h and more than 1000 shots were performed. Two different water thicknesses were studied, and we measured a continuous proton distribution with energies up to 3.5 MeV.
