Background: Home environment is a core domain in the care of community-dwelling older adults with dementia, but there is no suitable instrument to measure it in China.Objective: To develop and psychometrically test the home environment assessment checklist for community-dwelling older adults with dementia.Methods: A three-step process was performed to develop and test this instrument: 1) based on the evidence-based theory, the checklist was summarized as the main points of evidence from living environment settings among older adults with dementia, 2) the draft tool was assigned to an iterative process of evaluation by a panel of examiners consisting of experts from treatment, nursing and caring, people with dementia and their caregivers, 3) inter-rater reliability and internal consistency were calculated with a sample of 348 caregivers of the older adults with dementia.Results: The HEAC consisted of 71 items in domains addressing safety, stability and familiarity, visual cues, and sensory stimulation. Psychometric evaluation showed that this tool demonstrated sound reliability and validity. Content validity was 0.969 which was established by a panel of experts (n = 10). Inter-rater reliability of two researchers was 0.978, and 0.848 for researchers and caregivers. Test-retest reliability was excellent (ICC = 0.757-0.877) in community-dwelling older adults with dementia 2 week apart.Conclusion: The HEAC is a new tool to help collect the reliable information on the barriers and facilitators of home environment for community-dwelling older adults with dementia and to precipitate the home modification process to improve the quality of care for people with dementia and their caregivers in daily life. Aims: To investigate the career success and work environment among nurses with a master's or doctoral degree and analyse the effect of work environment on their career success. Methods: This was a national descriptive, cross‐sectional study. A total of 1223 nurses with a master's or doctoral degree from 115 tertiary hospitals across mainland China participated in the study. Subscales of the practice environment scale of the nursing work index and career success scale were utilized for data collection via email in the summer of 2017. Descriptive and inferential statistics were used to analyse the collected data. Results: The research showed that the career success and work environment ratings of nurses with a master's or doctoral degree were at a moderate level. Hierarchical regression analysis showed that work environment was positively correlated with career success for three of the four subscales. Conclusion: The career success and work environment is moderate among nurses with a master's or doctoral degree, and improving the work environment for nurses may lead to higher career success. Nurses, especially those with a master's or doctoral degree, play an indispensable role in health care system.Nursing shortage is a global problem that is even severer in China than that of developed countries.Although nursing work environment has been reported to have a significant bearing on nurses' job satisfaction, studies on the relationship between work environment and career success are still limited. What this paper adds? The career success and work environment ratings of nurses with a master's or doctoral degree were at a moderate level.Work environment including nurse participation in hospital affairs, collegial nurse‐physician relations, and staffing and resources adequacy were positively correlated with career success. The implications of this paper: for policy/practice/research/education The findings of this study contribute more evidence in support of relationship between work environment and career success among nurses with a master's or doctoral degree.This study provides practical suggestions for nurse managers or policymakers regarding how to create a motivated work environment and enhance nurses' career success, which can reduce nurses' turnover intention and ensure nursing stability and quality.This finding can be applied to medical institutions in other developing countries with similar cultural beliefs and educational background and may help relieve the pressure on nurses' recruitment and retention. Glycosyltransferases are frequently dysregulated in lung cancer. Core 1 β 1, 3-galactosyltransferase 1 (C1GALT1), an enzyme highly expressed in various cancers, is correlated with tumor initiation and development. However, the role of C1GALT1 in lung cancer remains poorly understood. In this study, through bioinformatic analysis and clinical validation, we first discovered that C1GALT1 expression was upregulated in lung adenocarcinoma (LUAD) tissues and was closely related to poor prognosis in patients with LUAD. Gain- and loss-of-function experiments showed that C1GALT1 promoted LUAD cell proliferation, migration, and invasion in vitro , as well as tumor formation in vivo . Further investigation demonstrated that RAC1 expression was positively regulated by C1GALT1 in LUAD, whereas silencing Rac1 could reverse C1GALT1-induced tumor growth and metastasis. Moreover, miR-181d-5p was identified as a negative regulator for C1GALT1 in LUAD. As expected, the inhibitory effects of miR-181d-5p on LUAD cell proliferation, migration, and invasion were counteracted by restoration of C1GALT1. In summary, our results highlight the importance of the miR-181d-5p/C1GALT1/RAC1 regulatory axis during LUAD progression.
