Abstract

A rare case of an anomalous location of the orifice of the coronary artery was found in a 99-year-old male cadaver undergoing routine dissection. The presence of the right coronary artery (RCA), left coronary artery (LCA), and conus artery (conus branch) originating from the right Valsalva sinus are the characteristic findings of this case. Then, the LCA passed through the aorta and the pulmonary artery. The LCA and RCA branches were normal. These findings are useful for future surgical procedures, including cardiac catheterization. 22. Current Status of Infant Mental Health in Day-Care Centres in Japan: An Investigation of Okinawa and Aomori Prefectures

Abstract

Grasping both the extent and the actual situation of psychosomatic disorders, neurotic habits and developmental disorders of infancy and childhood is vital for their prevention and for taking appropriate measures to deal with the current situation. The purpose of this study is to explore the current situation of infant mental health in Japanese day-care centres. The investigation regions for this study were Okinawa prefecture and Aomori prefecture in Japan. Our investigation method was a mailed survey, which was answered by the director or a teacher of the day-care centre. The response rates were 40.9% (Okinawa) and 41.2% (Aomori). This included the answered surveys from 78 day-care centres (6250 children) in Okinawa and 134 day-care centres (9613 children) in Aomori. We found that the approximate prevalences of mutism in infants and children in the above-mentioned day-care centres were 0.34% (Okinawa) and 0.33% (Aomori). In the same way, the rates of tic disorder were 0.51% (Okinawa) and 0.54% (Aomori), and the rates of elimination disorder were 0.30% (Okinawa) and 0.28% (Aomori). These rates did not show any significant difference. When we compared the rates with those obtained from conventional studies, only slight differences were observed. The addition of fentanyl to the anesthetic solution became popular for TEB because of the stronger effects; however, there have been few comparable trials about the addition of fentanyl to PVB. The purpose of this study was thus to compare postoperative analgesia, side effects, and complications between ultrasound-guided PVB (USG-PVB) and TEB with the addition of fentanyl to ropivacaine after lung surgery. Methods We examined 90 consecutive patients (age 18-75 years) scheduled for video-assisted thoracic surgery (VATS). In both groups, all blocks (four blocks in USG-PVB and one block in TEB) and one catheter insertion were performed preoperatively. Continuous postoperative infusion (0.1 % ropivacaine plus fentanyl at 0.4 mg/day) was undertaken for 36 h in both groups. The recorded data included the verbal rating scale (VRS) for pain, blood pressure, side effects, complications for 2 days, and overall satisfaction score. Results There was no difference in the frequency of taking supplemental analgesics (twice or more frequently), or in VRS. Hypotension occurred significantly more frequently in TEB (n = 7/33) than in PVB (n = 1/36) (P = 0.02); on the other hand, the incidences of PONV and pruritus, as well as overall satisfaction score, were similar. There were no complications in both groups; however, the catheters migrated intrathoracically in four patients in PVB. Conclusion USG-PVB achieved similar pain relief and lowered the incidence of hypotension compared with TEB. We conclude that both blocks with the same concentration of ropivacaine and fentanyl can provide adequate postoperative analgesia for VATS.

24. Hemostatic Radiotherapy for Gastric Cancer: MRI as an Alternative to Endoscopy for Post-Treatment Evaluation.

Abstract

Introduction: Pretreatment diagnosis by diffusion-weighted magnetic resonance imaging (DW-MRI) is useful to determine the effect of chemotherapy for gastric cancer. Here, we investigated the relationship among DW-MRI, endoscopy, and tumor markers. Patients: Eight patients underwent hemostatic radiotherapy (RT) for gastric cancer in this prospective study from 2019 to 2021. The patients completed MRI, endoscopy, and blood tests before RT; MRI, endoscopy, and blood tests 1 month after RT; and MRI and blood tests 3 months after RT. Correlations between changes in apparent diffusion coefficient (ADC) derived from DW-MRI and the tumor marker carcinoembryonic antigen (CEA) were investigated. Results: Univariate analysis of overall survival showed that sex and chemotherapy treatment were statistically significant factors. The CEA values before and 1 month after RT decreased significantly. There was no statistical difference between the CEA value 1 and 3 months after RT. The ADC value before and 1 month after RT increased significantly but not between 1 and 3 months after RT. Comparing the ratio of ADC before RT to 1 (or 3) month(s) after RT with that of CEA before RT to 1 (or 3) month(s) after RT, we found an inverse relationship between the two ratios. Conclusions: Therefore, changes in ADC and CEA are correlated. Additionally, 3 months after RT, the decrease in ADC appeared earlier than the decrease in CEA. ADC may indicate a biological change earlier than CEA, and the ratios of ADC and CEA may be important factors. These aspects warrant further confirmation in a larger sample population. 29. Vascular events from carotid artery atherosclerosis after radiation therapy for laryngeal and hypopharyngeal cancer: the incidence and risk factors

Abstract

In this retrospective cohort study, we evaluated the incidence of vascular events from carotid artery atherosclerosis after radiotherapy indication for laryngeal and hypopharyngeal cancer. From January 2007 to December 2016, we investigated 111 laryngeal/hypopharyngeal cancer patients who underwent curative radiotherapy and were followed up for ≥1 year (median follow-up duration, 60 months). We evaluated the incidence of vascular events from carotid artery atherosclerosis, defined as a transient ischemic attack or an atherothrombotic cerebral infarction, or from undergoing treatment such as carotid artery stenting for carotid artery stenosis. The median radiation dose was 66 Gy (range, 60–74); 48 patients (43.2%) received concurrent chemotherapy. The 5-year overall survival was 86.2%. Six patients required treatment for carotid artery disease. Carotid stenting was performed in three patients with carotid artery stenosis; three patients developed atherosclerotic cerebral infarction and received medical treatment, with a median of 51.7 months (range, 0.3–78.3) after radiotherapy initiation. The vascular event occurrence rate was 5.4% within 5 years and 10.7% within 8 years. In the univariate analysis, dyslipidemia, diabetes mellitus, and carotid calcification were significant factors for event occurrence. Because three out of six cases occurred out of the irradiated field, no carotid artery or carotid bulb dosimetric parameters showed significant cor-relation. As laryngeal/hypopharyngeal cancer patients, particularly with complications including dyslipidemia and diabetes mellitus, are at a high risk of carotid artery stenosis after radiotherapy, long-term carotid artery evaluation is necessary. Early intervention by stroke specialists can reduce the risk of fatal cerebral infarction.

Abstract

Aim: In the present study, we aimed to investigate the practical usefulness of a single gatekeeper-training lecture for suicide prevention and examine the potential factors that modulate gatekeeper capability for suicide prevention in the general population. Methods: A 12-item questionnaire, comprising questions about recognition/attitudes regarding suicidality, was administered to 493 members of the general population before and after they attended a gatekeeper-training lecture. Each item was assessed using a 4-point scale (1＝very negative, and 4＝very positive). For subgrouping of the 12 items, the baseline scores were analyzed using an exploratory factor analysis. With regard to the subscale scores, gender effects were examined by Studentʼs t-test, while the effects of generation and employment/occupation status were tested using ANOVA followed by the Tukey test. Finally, multiple regression analysis was performed to test for the possible determinants of post-lecture recognition, attitude, and approaching skills as gatekeepers for suicide prevention. Results: An exploratory factor analysis of the baseline scores of the 12 items was performed using the following three distinct subscales: the expressed attitudes, cognitive understanding, and approaching skills. Scores of the 12 items improved significantly after the lecture. The older participants (aged≧60 years) had lower post-lecture scores in all the subscales than the younger participants. Moreover, at both baseline and post-lecture, medical care professionals scored higher than the unemployed participants and/or employed non-medical workers in all the subscales, while the employed non-medical workers scored higher than the unemployed participants in the expressed attitudes subscale. Multiple regression analyses revealed that the score for each subscale post-lecture was strongly dependent on its baseline score, and the baseline score of expressed attitudes predicted the post-lecture scores of the other two subscales. Conclusion: The present study suggests that active workers aged＜60 years with positive expressed attitudes for suicide prevention gain more educational effects from the gatekeeper-training lecture than the older group (aged≧60 years).

Abstract

A tumoral presentation of amyloidosis is uncommon in soft tissue. We here report a case of bulky amyloidomas on bilateral buttocks in a patient with osteoarthropathy of the hips. The 66-year-old patient’s history included 26 years of hemodialysis along with osteoarthropathy of the hips and a pathological fracture of the femoral neck.CT showed ill-defined bulky masses with a slightly higher density compared with that of muscles in the subcutaneous regions of the bilateral buttocks. Moderate contrast enhancement of the lesions was revealed. The masses contained diffuse tiny fat depositions and psammomatous calcifications, suggestive of amyloid deposition. On MRI, the masses demonstrated markedly hypointense signals on T2-weighted image, consistent with amyloidosis. Characteristic scattered spots of high intensity, suggesting fat or microcalcifications, were noted within the masses on T1-weighted image.Mass-like synovial thickening was observed in the bilateral hip joints with discrete marginal erosions of the femoral heads. The CT and MRI features of the hips were compatible with typical amyloid osteoarthropathy.
