A 35-year-old man with Fitzpatrick skin type IV, born in Brazil, presented to the dermatology clinic with a growth on his left hip. The physical examination revealed a pedunculated erythematous–violaceous nodule with a hyperkeratotic surface. Histopathological examination confirmed a diagnosis of Kaposi sarcoma (KS), a low-grade angioproliferative tumor associated with human herpesvirus 8 (HHV-8). The patient was found to be HIV-positive and underwent treatment with highly active antiretroviral therapy (HAART), which resulted in a positive response and no disease relapse after one year. This article discusses a case of allergic contact dermatitis (ACD) caused by a rotigotine transdermal therapeutic system (TTS) in a patient with Parkinson's disease. The allergic reaction was attributed to sodium metabisulphite, an excipient in the TTS. Patch tests confirmed the diagnosis of ACD to sodium metabisulphite. The article emphasizes the importance of patch testing in diagnosing contact dermatitis associated with TTS and highlights the need for awareness of potential allergens, including excipients, in transdermal patches. Sexually transmitted infections (STIs) are a significant public health concern across the globe. Vaccines have played a crucial role in mitigating the burden of infectious diseases and are the most effective candidates for preventing STIs. Currently, vaccines for hepatitis A, hepatitis B virus (HBV), and human papillomavirus (HPV) are available. More recently, the smallpox vaccine was approved for Mpox (MPX) prevention. Ongoing research efforts are focused on developing vaccines for other STIs. Https://doi.org/10.1093/ced/llad056 Dear Editor, Xanthogranuloma (XG) is the most common form of non-Langerhans cell histiocytosis.[[1], [3]] Multiple adult XG (MAXG) is unusual, with an occasional association with haematological malignancies.[4],[5] We herein report the case of a patient with MAXG as the first manifestation of chronic myelomonocytic leukaemia (CMML). Our differential diagnosis included xanthelasma and eruptive xanthomas or a histiocytosis. The development of the cutaneous lesions may take place before, during or after the haematological malignancy.[5] The haematological malignancies associated with XGs include follicular lymphoma, essential thrombocytosis, chronic lymphocytic leukaemia, B-cell acute lymphoblastic leukaemia, large B-cell lymphoma, adult T-cell lymphoma/leukaemia, monoclonal gammopathy and myelodysplastic syndrome.[2],[4],[5] The exact pathogenesis that would justify the association of haematological malignancy and XG remains to be elucidated. A retrospective study was conducted on pediatric patients examined via dermatology emergency consultation over 1 year. A total of 1352 diagnoses, including 100 distinct disorders, were recorded. Regardless of the diversity of skin conditions observed, a relatively small number of disorders were found repeatedly. We encourage education aimed at ED providers to focus on the diagnosis and management of the more commonly encountered disorders, including atopic dermatitis and infectious skin disorders. A 63-year-old woman presented at our emergency department with a disseminated dermatosis that developed after undergoing a cardiac catheterization procedure. Her past medical history included an end-stage renal disease undergoing hemodialysis. Clinical examination revealed erosions and hemorrhagic crusts located on her lips and along the arteriovenous fistula. Additionally, we observed five hyperpigmented macules on her left hand's dorsum and palm and multiple hypopigmented macules in the genital area. Upon medical record review, we discovered the occurrence of prior bullous eruptions following contrast administration. A fixed drug eruption (FDE) due to radiocontrast was diagnosed based on clinical history, clinical examination, eruption timeframe, and positive drug provocation test. Intravenous contrast media reactions can be immediate or delayed, with delayed hypersensitivity reactions (DHR) occurring one hour to seven days post-administration. DHRs often present as maculopapular rashes. FDEs are rare. Skin tests are used to identify culprit agents. Ideally, intradermal tests, with delayed readings, and patch tests are combined for optimal sensitivity. Despite lacking standardized protocols, premedication with corticosteroids may mitigate reaction severity., (© The Author(s) 2024.
