Toen de Noordelijke en Zuidelijke Nederlanden in 1815 werden samengevoegd tot het Verenigd Koninkrijk der Nederlanden, probeerde koning Willem I nationale eenheid te creeren, ook in de kunst. De natievormende activiteiten in het Verenigd Koninkrijk der Nederlanden wierpen echter weinig vruchten af. Het Noorden en het Zuiden hadden verschillende culturele tradities en schilders opereerden veelal op lokaal niveau. In het proefschrift Moederstad en vaderland beschrijft Anna Rademakers de artistieke ontwikkelingen in de handelssteden Amsterdam en Antwerpen en de hofsteden Brussel en Den Haag. Zij onderzoekt de verschillen in kunstopvattingen en de pogingen tot integratie en samenwerking op nationaal niveau. Ze laat ook zien hoe schilders omgingen met de Belgische Revolutie en de uiteindelijke scheiding van de twee landsdelen. Zeker in het zuiden begon ‘nationale identiteit’ pas een rol te spelen na 1830. De schilders die zich met de nationale zaak bezig hielden deden dat eerder uit economische dan uit ideologische overwegingen. Toch was er ook binnen resp. de Noordelijke en Zuidelijke Nederlanden weinig sprake van een gemeenschappelijke kunstwereld. De lokale krachten waren veel sterker. Opvallend is dat er in grote lijnen zelfs meer overeenkomsten waren tussen de beide hofsteden (Brussel en Den Haag) en tussen de beide handelssteden (Antwerpen en Amsterdam), dan tussen de steden van dezelfde landsdelen. Aan de hand van contemporaine bronnen zoals brieven, redevoeringen, krantenartikelen en de kunstwerken zelf, geeft Moederstad en vaderland een kleurrijk beeld van het culturele leven in de vroege negentiende eeuw en de mate waarin noties als nationale identiteit en nationaal besef daarin een rol speelden.

4. Beta-HCG Secretion by a Pulmonary Choriocarcinoma in a Male Patient.

- Abstract
Background: Paraneoplastic secretion of beta-subunit of human chorionic gonadotropin ( β -HCG) in pulmonary carcinoma is rare. Case Presentation . A 65-year-old man presented with bilateral gynaecomastia with abnormally high levels of β -hCG and elevated oestradiol, progesterone, and testosterone levels on April 7, 2023. After excluding testicular malignancy, CT scan of the chest and abdomen revealed bilateral pulmonary lesions. Transthoracic biopsy confirmed malignancy with choriocarcinoma. MRI of the brain showed a solitary brain metastasis, while on a subsequent 18F-FDG PET/CT, no other metastatic lesions were seen. The patient was treated with chemoimmunotherapy carboplatin-etoposide-pembrolizumab with good partial response., Conclusion: Our case of a presumably stage IV dedifferentiated mNSCLC presenting as an extragonadal β -hCG secreting pulmonary choriocarcinoma is a very rare tumor with a poor prognosis. Its biology, origin, and treatment remain to be elucidated. 5. An observational study on the sexual, genital and fertility outcomes in bladder exstrophy and epispadias patients.

- Abstract
Introduction: Bladder exstrophy and epispadias complex (BEEC) is a spectrum of congenital malformations ranging from an isolated epispadias to a full exstrophy. It is an uncommon disease and little is known on how patients cope with its implications later in life., Objective: The goal of this study is to assess the sexual, continence and fertility outcomes of BEEC patients, who had reconstructive bladder surgery during childhood. Considering the sensitive nature of these topics, they are not easily spoken about in the doctor's office. Our aim is to shed some light on possible points of improvement in follow-up., Study Design: 63 patients between 18 and 45 years old were sent an electronic questionnaire based on previous existing standardized questionnaires. They were asked about sexual and psychosexual wellbeing, urinary incontinence and fertility. Data from their medical files (medical history on previous surgeries). and questionnaire answers are linked through an anonymous subject number and put into an Excel file for descriptive representation., Results: 22 men and 8 women filled in the questionnaire. All but 2 are sexually active. Reasons to avoid sexual activity are equally divided as BEEC-related and non-BEEC-related. Sexual satisfaction is lower in the male group due to problems with erection, ejaculation, condom usage and embarrassment about physical appearance. In females problems concerning pain and reaching orgasm are mentioned. 30% report depressive feelings. There is a clear correlation between number of reconstructive surgeries and sexual satisfaction. 90% of patients urinate via catheterization, mostly through a Mitrofanoff connection. This leads to complications such as foul odors, infection, embarrassment and sexual dysfunction. 8 out of 13 men conceived a child (with the use of their own sperm), 2 out of 4 women did., Discussion: A strength of this study is the use of standardized questionnaires which allow comparison to a control patient group. Our study is one of the first to show how patients cope with the challenges of BEEC by the use of open questions. We see an overall high quality of life yet an important impact on mental health., Conclusion: BEEC is associated with many challenges in the adult life of patients. 6. Glucose control using fast-acting insulin aspart in a real-world setting: A 1-year, two-centre study in people with type 1 diabetes using continuous glucose monitoring.

- Abstract
Aim: To evaluate the efficacy and safety of switching from traditional mealtime insulins to fast-acting insulin aspart (Fiasp) in a "real-world" clinical practice setting in adult people with type 1 diabetes (PWD1) who were using intermittently scanned or real-time continuous glucose monitoring (isCGM or rtCGM, respectively)., Materials and Methods: Data from 438 adult PWD1 (60% men, age 44.6 ± 16.2 years, diabetes duration 21.5 ± 14.0 years, isCGM/rtCGM: 391/47, multiple daily injections/continuous subcutaneous insulin infusion: 409/29), who initiated Fiasp from January 2018 to May 2020, were analysed. The primary objective was the evolution of time in range (TIR; 70-180 mg/dL) at 6 and 12 months. Secondary objectives included change in HbA1c, body mass index (BMI), insulin doses, time below range (<70 and <54 mg/dL), and time above range (>180 and >250 mg/dL)., Results: TIR improved from 50.3% ± 15.6% to 54.3% ± 15.1% at 6 months (n = 425) and to 55.5% ± 15.2% at 12 months (n = 385) (P < .001), corresponding to 57 min/d at 6 months and 75 min/d at 12 months. Time spent below 54 mg/dL evolved from 3.1% ± 3.3% to 3.1% ± 3.7% and 2.5% ± 3.0% at 6 and 12 months, respectively (P = .011). Also, time spent above 180 mg/dL decreased from 42.3% ± 16.7% at start by 4.2% at 6 months and by 4.6% at 12 months (P < .001). The proportion of people reaching TIR more than 70% increased from 11.0% to 14.8% (P = .002), and those spending less than 4% at time less than 70 mg/dL increased from 36.1% to 42.1% (P = .002). After 12 months, HbA1c, insulin doses, and BMI did not change significantly., Conclusions: In a Belgian real-world setting of adult PWD1, switching to Fiasp was associated with a 5% increased TIR after 12 months, corresponding to 75 min/d, in combination with less time spent below and above range., (© 2021 John Wiley & Sons Ltd.)

- Abstract
We report the case of a woman with a metastatic breast cancer, who started a third-line treatment with dasatinib, a new oral tyrosine kinase inhibitor, and who developed, one week later, a progressive breathless sensation. Workup demonstrated pleuropericardial effusion that turned out to be a side effect of this new investigational drug. Although this dasatinib-induced side effect is well known, this case clearly illustrates the importance of an accurate diagnosis and adequate treatment of complications of new agents which are easy to use since most of them are orally taken, and the difficulty to clearly separate drug origin and cancer morbidities. The patient recovered completely one month after discontinuation of dasatinib. In this report, we will review the differential diagnosis and management of pleuropericardial effusion.
