1. The role of superior hemorrhoidal vein ectasia in the preoperative staging of rectal cancer.

Abstract

Objective: The prognosis of colorectal cancer has continuously improved in recent years thanks to continuous progress in both the therapeutic and diagnostic fields. The specific objective of this study is to contribute to the diagnostic field through the evaluation of the correlation between superior hemorrhoidal vein (SHV) ectasia detected on computed tomography (CT) and Tumor (T), Node (N), and distant metastasis (M) examination and mesorectal fascia (MRF) invasion in the preoperative staging of rectal cancer. Methods: Between January 2018 and April 2022, 46 patients with histopathological diagnosis of rectal cancer were retrospectively enrolled, and the diameter of the SHV was evaluated by CT examination. The cutoff value for SHV diameter used is 3.7 mm. The diameter was measured at the level of S2 during portal venous phase after 4× image zoom to reduce the interobserver variability. The parameters evaluated were tumor location, detection of MRF infiltration (defined as the distance < 1 mm between the tumor margins and the fascia), SHV diameter, detection of mesorectal perilesional lymph nodes, and detection of metastasis. Results: A total of 67.39% (31/46) of patients had SHV ectasia. All patients with MRF infiltration (4/46, 7.14%) presented SHV ectasia (average diameter of 4.4 mm), and SHV was significantly related with the development of liver metastases at the moment of primary staging and during follow-up. Conclusion: SHV ectasia may be related to metastasis and MRF involvement; therefore, it could become a tool for preoperative staging of rectal cancer. [ABSTRACT FROM AUTHOR]

Abstract

2013 - 2014 In this Thesis we report a general review of Extended Theories of Gravity and the fundamental aspects of General Relativity. We show the technicality of development of field equation with respect to Newtonian, Post-Newtonian approach and the post-Minkowskian limit. We analyse also the problem of how conformally transformed models behave in the weak field limit approximation. This issue could be extremely relevant in order to select conformally invariant physical quantities. The photon deflection is considered in the framework of the Newtonian Limit of a general class of f (R, Rαβ Rαβ, RαβγδRαβγδ) - Gravity where f is an unspecific function of the Ricci scalar R, Ricci tensor squareRαβ Rαβ and Riemann tensor square RαβγδRαβγδ. Studying in the weak-field approximation - Newtonian and Post-Newtonian limit - the geodesic and Lense-Thirring processions by using the recent experimental results of the Gravity Probe B and LARES satellite and using the damping of the orbital period of coalescing stellar binary systems, we impose constraints on the free parameters of such models of Extended Theories of Gravity. Abstract

A physical interpretation of the two-sheeted space, the most fundamental ingredient of noncommutative spectral geometry proposed by Connes as an approach to unification, is presented. It is shown that the doubling of the algebra is related to dissipation and to the gauge structure of the theory, the gauge field acting as a reservoir for the matter field. In a regime of completely deterministic dynamics, dissipation appears to play a key role in the quantization of the theory, according to the 't Hooft's conjecture. It is thus argued that the noncommutative spectral geometry classical construction carries the seeds of quantization, implicit in its feature of the doubling of the algebra. [ABSTRACT FROM AUTHOR]

Abstract

In Cosmology and in Fundamental Physics there is a crucial question like: where the elusive substance that we call Dark Matter is hidden in the Universe and what is it made of? that, even after 40 years from the Vera Rubin seminal discovery [1] does not have a proper answer. Actually, the more we have investigated, the more this issue has become strongly entangled with aspects that go beyond the established Quantum Physics, the Standard Model of Elementary particles and the General Relativity and related to processes like the Inflation, the accelerated expansion of the Universe and High Energy Phenomena around compact objects. Even Quantum Gravity and very exotic Dark Matter particle candidates may play a role in framing the Dark Matter mystery that seems to be accomplice of new unknown Physics. Observations and experiments have clearly indicated that the above phenomenon cannot be considered as already theoretically framed, as hoped for decades. The Special Topic to which this review belongs wants to penetrate this newly realized mystery from different angles, including that of a contamination of different fields of Physics apparently unrelated. We show with the works of this ST that this contamination is able to guide us into the required new Physics. This review wants to provide a good number of these “paths or contamination” beyond/among the three worlds above; in most of the cases, the results presented here open a direct link with the multi-scale dark matter phenomenon, enlightening some of its important aspects. Also in the remaining cases, possible interesting contacts emerges.
