The overconsumption of nonbiodegradable materials, particularly plastics, has had a significant and detrimental impact on the environment. Advancements in research sector have led to the development of biodegradable materials, namely starch‐based biodegradable films, which have the potential to reduce this environmental impact. Starch is a unique biopolymer with distinctive chemical, physical, mechanical, thermal, and optical properties that make it an attractive alternative to nonbiodegradable and harmful materials. This review paper comprehensively discusses the properties of starch and the techniques involved in transforming native starch into starch‐based films. Further a broad overview of recent research on combining starch with several composites to enhance the physicochemical properties has been discussed herein. In addition, this paper also discusses recent insights into the development of starch‐based composite films and their potential applications in food packaging systems. Future studies must focus on the development of starch composites that strike a balance between different versatile properties of the biopolymer. Additionally, a critical examination of the interactions at the molecular level will help to expand our understanding of this sustainable biopolymer. Ultimately, the findings of this review paper will provide valuable insights for researchers and industry professionals interested in the development and utilization of starch‐based biodegradable films. Probiotics are known as the live microorganisms that, upon adequate administration, elicit a health beneficial response inside the host. The probiotics are known as immunomodulators and exhibit anti-tumor properties. Advanced research has explored the potential use of probiotics as the oral vaccines without the latent risks of pathogenicity. Probiotic-based oral vaccines are known to induce mucosal immunity that prevents the host from several enteric infections. Probiotic bacteria have the ability to produce metabolites in the form of anti-inflammatory cytokines, which play an important role in the prevention of carcinogenesis and in the activation of the phagocytes that eliminate the preliminary stage cancer cells. This review discusses the advantages and disadvantages of using the oral probiotic vaccines as well as the mechanism of action of probiotics in colon cancer therapy. This review also employs the use of “PROBIO” database for selecting certain probiotics with immunomodulatory properties. Furthermore, the use of several probiotic bacteria as anti-colon cancer adjuvants has also been discussed in detail. Because the current studies and trials are more focused on using the attenuated pathogens instead of using the probiotic-based vaccines, future studies must involve the advanced research in exploiting the potential of several probiotic strains as adjuvants in cancer therapies. Introduction Temporal bone surgery is a unique and complicated surgical skill that requires extensive training. There is an educational requirement to maximize trainee experience and provide effective feedback. Objective We evaluate three temporal bone dissection scales for efficacy, reliability, and accuracy in identifying resident skill during temporal bone surgery. Methods Residents of various skill levels performed a mastoidectomy with posterior tympanotomy on identic 3D-printed temporal bone models. Four blinded otologic surgeons evaluated each specimen at two separate intervals using three separate dissection scales: the Welling Scale (WS), the Iowa Temporal Bone Assessment Tool (ITBAT), and the CanadaWest Scale (CWS). Scores from each scale were compared in their ability to accurately separate residents by skill level, inter- and intrarater reliability, and efficiency in application. Results Nineteen residents from 9 postgraduate programs participated. Assessment was clustered into junior (postgraduate year or PGY 1, 2), intermediate (PGY 3) and senior resident (PGY 4, 5) cohorts. Analysis of variance (ANOVA) found significant differences between cohort performance (p<0.05) for all 3 scales considering the PGY level and the subjective account of temporal bone surgical experience. The inter-rater reliability was consistent across each scale. The intrarater reliability was comparable between the CWS (0.711) and the WS (0.713), but not the ITBAT (0.289). Time (in seconds) to complete scoring for each scale was also comparable between the CWS (42.7±16.8), the WS (76.6±14.5), and the ITBAT (105.6±38.9). Regular use of certain drugs and other antibiotics has led to the development of many side effects and drug resistance in the humans. But researchers have already started to find out various other alternatives to combat the negative effects. Among these, probiotics are being used up as an alternate and are consumed to great extent by humans in day to day life. Lactobacillus is the most common bacterial strain that is commercially used in various probiotic products. These LABs must bind to the human epithelial gut in order to perform their activity. For binding, mucin binding protein plays a major role by providing the attachment sites to the bacteria. In this paper we have analyzed the mucin-binding proteins in Lactobacillus plantarum using computational tools. L plantarum was chosen due to its ability to show strong binding towards epithelial surface. The FASTA sequences were extracted from UniProtkb database and were used for further comparison based on different parameters. Physicochemical analysis was done by ProtPARAM software, followed by secondary structure prediction (CFSSP software) and tertiary structure prediction (CASTp software). The MUBs motifs were also compared with the motifs of gut pathogens using TOMTOM tool. Moreover, the particular motifs responsible during adhesion were being searched in MUBs using Motif Finder. These motifs showed some similarity with the binding motifs of pathogens present in human gut. Physicochemical analysis showed that these mucin binding proteins are thermostable. GRAVY value indicates that these are soluble proteins which are hydrophilic in nature. These proteins do not have any effect of cellular proteases and hence are able to survive in the small intestine. These chosen strains have conserved sequences determined by their secondary structure analysis using the CFSSP server. Moreover, it was found that these mucin binding proteins has some of the motifs having the same sequences as present in that of pathogens. Hence, these are able to show the competitive adherence against the pathogens by blocking the binding sites for pathogens and resulting in their elimination. The study conducted showed the presence of MUBs in Lactobacillus plantarum and also same motifs were found in pathogen which proved the competitive adherence of L. plantarum against pathogens hence providing some light to the host-microbes interactions for other future studies too. Background: Paediatric airway assessment remains the most challenging task before the anaesthesiologists. Recent advancement in ultrasonography techniques should now allow for accurate and descriptive evaluation of paediatric airway. To compare calculated external diameters of the endotracheal tube from physical indices of traditional formulas and predetermined by ultrasound. Materials and Methods: 100 subjects of either sex between 12-60 months of age, undergoing various elective surgeries under general anaesthesia requiring endotracheal intubation were enrolled in the study. The transverse diameter was measured at the level of cricoids cartilage by ultrasonography. The tracheal tube was considered best fit if air leak was satisfactory at 15-20 cm H2O of airway pressure. The obtained values were compared with the values of endotracheal tube size calculated by various age, height, weight based formulas and diameter of right and left little finger. The correlation of size of Endotracheal tube by different modalities was done and Pearson's correlation coefficient was obtained. Results: According to Pearson's correlation there was a moderate correlation of best fit Endotracheal tube with endotracheal tube size by age based formula (r = 0.743), body length based formula (r = 0.683), right little finger based formula (r = 0.587), left little finger based formula (r = 0.587) and multivariate formula (r = 0.741). There was a strong correlation with ultrasonography (r = 0.943). Conclusion: Ultrasonography is a reliable method of estimation of subglottic diameter and for prediction of endotracheal tube size in children. [ABSTRACT FROM AUTHOR]

Background Numerous studies have examined the effect of geographic place of residence on access to cardiovascular care, but few have examined their effect on outcomes after cardiac operations. This study examined the effect of geographic place of residence on in-hospital and 30-day outcomes after cardiac operations. Methods We performed a retrospective analysis of all patients undergoing nonemergency cardiac operations at a single institution between April 2004 and March 2011. Geographic place of residence was defined as the driving distance from the patient’s home to the tertiary cardiac care center divided into the following categories: 0 to 50 km, 50 to 100 km, 100 to 150 km, 150 to 200 km, 200 to 250 km, and more than 250 km. Multivariable logistic regression was used to determine the independent effect of driving distance on in-hospital and 30-day outcomes. Results The final study population included 4,493 patients, of whom 3,897 (86.7%) had 30-day follow-up. After adjusting for differences among patient groups, no consistent relationship existed between distance and in-hospital outcomes. However, increased distance beyond 100 km was significantly associated with a greater risk of adverse outcomes at 30 days (0 to 50 km: referent; 50 to 100 km: odds ratio, 1.16 [95% confidence interval, 0.83 to 1.62]; 100 to 150 km: 1.32 [1.05 to 1.65], 150 to 200 km: 1.68 [1.33 to 2.11], 200 to 250 km: 1.41 [1.06 to 1.88], and >250 km: 1.30 [1.04 to 1.63]). Conclusions Patients who live at an increased distance from the tertiary cardiac care center are more likely to have worse 30-day outcomes after cardiac operations. Antimicrobial resistance (AMR) is a growing public health threat caused by the widespread overuse of antibiotics. Bacteria with antibiotic resistance may acquire resistance genes from soil or water. Endogenous hydrogen sulfide (H 2 S) production in bacteria confers antibiotic tolerance in many, suggesting a universal defense mechanism against antibiotics. In this study, we isolated and identified soil-based antibiotic-resistant bacteria collected from contaminated areas. An antibiotic-resistant bacterium was identified as non-endogenous-H 2 S-producing, allowing us to examine the effect of exogenous H 2 S on its resistance mechanism. Therefore, we demonstrated that different classes of antibiotic resistance can be reverted by employing H 2 S with antibiotics like ampicillin and gentamicin. Methods like Kirby-Bauer Disk-Diffusion, Scanning Electron Microscopy, and Flow Cytometer analysis were performed to assess the antibacterial activity of H 2 S with ampicillin and gentamicin. The antioxidative efficiency of H 2 S was evaluated using the DCFH-DA (ROS) test, as well as lipid peroxidation, and LDH activity. These were further confirmed with enzymatic and non-enzymatic (SOD, CAT, GST, and GSH) antioxidant studies. These findings support H 2 S as an antibiotic-potentiator, causing bacterial membrane damage, oxidative stress, and disrupting DNA and proteins. Probiotics are known as the live microorganisms which upon adequate administration elicit a health beneficial response inside the host by decreasing the luminal pH, eliminating the pathogenic bacteria in the gut as well as producing short chain fatty acids (SCFA). With advancements in research; probiotics have been explored as potential ingredients in foods. However, their use and applications in food industry have been limited due to restrictions of maintaining the viability of probiotic cells and targeting the successful delivery to gut. Encapsulation techniques have significant influence on increasing the viability rates of probiotic cells with the successful delivery of cells to the target site. Moreover, encapsulating techniques also prevent the live cells from harsh physiological conditions of gut. This review discusses several encapsulating techniques as well as materials derived from natural sources and nutraceutical compounds. In addition to this, this paper also comprehensively discusses the factors affecting the probiotics viability and evaluation of successful release and survival of probiotics under simulated gastric, intestinal conditions as well as bile, acid tolerant conditions. Lastly applications and challenges of using encapsulated bacteria in food industry for the development of novel functional foods have also been discussed in detail too. Introduction: This study aims to understand patient factors associated with refusal of surgery for nonmetastatic colorectal cancer and the associated cancer-specific mortality., Methods: Patients diagnosed with nonmetastatic colorectal cancer between 2004 and 2015 from the Surveillance, Epidemiology, and End Results Program were included., Results: A total of 152,731 (99.4%) patients underwent surgery, and 983 (0.6%) refused surgery. Independent predictors of refusal included male sex, older age, minority race, single relationship status, being uninsured, more recent date of diagnosis, having an earlier stage of diagnosis, and rectal versus colon cancer. Refusing surgery for nonmetastatic colorectal cancer increased cancer-specific mortality (adjusted hazard ratio 5.10, 95% confidence interval 4.62-5.62)., Conclusion: Most patients diagnosed with nonmetastatic colorectal cancer undergo surgery in the United States. However, refusal of surgery is increasing and associated with higher cancer-specific mortality.
