21. The one-model technique: a new method for immediate loading with fixed prostheses in edentulous or potentially edentulous jaws.

Abstract

This article examines a new prosthetic procedure for the immediate loading of implants with a fixed prosthesis in edentulous or potentially edentulous arches. In these situations, one of the main problems associated with immediate loading is the transfer of diagnostic information to the master cast. This technique takes advantage of an acrylic resin transfer plate that enables transfer of the study cast information to the master cast. Without any intrasurgical impression or any recording of the maxillomandibular relationship during or after surgery, construction of an adequate esthetic and functional fixed prosthesis is possible on the basis of diagnostic information acquired in the presurgical phase. The methodology is always applicable when there is an indication for immediate loading of implants. The rationale and guidelines for the successful use of this technique in edentulous or potentially edentulous arches are discussed and illustrated with a clinical case. [ABSTRACT FROM AUTHOR]

Abstract

In a randomized, phase 3 study, superior complete/near-complete response (CR/nCR) rates and extended progression-free survival were demonstrated with bortezomib-thalidomide-dexamethasone (VTD) versus thalidomide-dexamethasone (TD) as induction therapy before, and consolidation after, double autologous stem cell transplantation for newly diagnosed myeloma patients (intention-to-treat analysis; VTD, n = 236; TD, n = 238). This per-protocol analysis (VTD, n = 160; TD, n = 161) specifically assessed the efficacy and safety of consolidation with VTD or TD. Before starting consolidation, CR/nCR rates were not significantly different in the VTD (63.1%) and TD arms (54.7%). After consolidation, CR (60.6% vs 46.6%) and CR/nCR (73.1% vs 60.9%) rates were significantly higher for VTD-treated versus TD-treated patients. VTD consolidation significantly increased CR and CR/nCR rates, but TD did not (McNemar test). With a median follow-up of 30.4 months from start of consolidation, 3-year progression-free survival was significantly longer for the VTD group (60% vs 48% for TD). Grade 2 or 3 peripheral neuropathy (8.1% vs 2.4%) was more frequent with VTD (grade 3, 0.6%) versus TD consolidation. The superior efficacy of VTD versus TD as induction was retained despite readministration as consolidation therapy after double autologous transplantation. VTD consolidation therapy significantly contributed to improved clinical outcomes observed for patients randomly assigned to the VTD arm of the study. Abstract

Patients with celiac disease have an increased rate of enteropathy-associated T-cell lymphoma, but conflicting data are available about the protective role of a gluten-free diet with regard to the development of this malignancy. We followed 1,757 celiac patients for a total period of 31,801 person-years, collecting data about the frequency of gluten intake and the incidence of the enteropathy-associated T-cell lymphoma. Out of the nine celiac patients who developed an intestinal lymphoma [standard morbidity ratio of 6.42 (95% CI = 2.9-12.2; P < 0.001)], only two kept a strict gluten-free diet after the diagnosis of celiac disease and developed the malignancy after the peridiagnosis period of 3 years, dropping therefore the standard morbidity ratio to 0.22 (95%CI = 0.02-0.88; P < 0.001). The risk of developing an intestinal lymphoma for the celiac patients that used to have dietary gluten was significant (X(2 )= 4.8 P = 0.01). These results show that a strict gluten-free diet is protective towards the development of enteropathy-associated T-cell lymphoma.

Abstract

Background The association between coeliac disease (CD) and neoplasms has been long established, but few data are available about the risk factors. The aim of this paper is to estimate the risk of developing a neoplasm among non diagnosed coeliac patients and to evaluate if this risk correlates with the age of patients at diagnosis of coeliac disease. Methods The study population consists of patients (n = 1968) diagnosed with CD at 20 Italian gastroenterology referral Centers between 1st January 1982 and 31st March 2005. Results The SIR for all cancers resulted to be 1.3; 95% CI = 1.0–1.7 p < 0.001. The specific SIRs for non Hodgkin lymphoma was 4.7; 95% CI = 2.9–7.3 p < 0.001, for the small bowel carcinoma 25; 95% CI = 8.5–51.4 p < 0.001, for non Hodgkin lymphoma 10; 95% CI = 2.7–25 p = 0.01, finally for the stomach carcinoma 3; 95% CI = 1.3–4.9 p < 0.08. The mean age at diagnosis of CD of patients that developed sooner or later a neoplasm was 47,6 ± 10.2 years versus 28.6 ± 18.2 years of patients who did not. Conclusion Coeliac patients have an increased risk of developing cancer in relation to the age of diagnosis of CD. This risk results higher for malignancies of the gastro-intestinal sites. An accurate screening for tumors should be performed in patients diagnosed with CD in adulthood and in advancing age.

Abstract

Summary Background Since 1960, oral melphalan and prednisone (MP) has been regarded as the standard of care in elderly multiple myeloma patients. This multicentre randomised trial compared oral MP plus thalidomide (MPT) with MP alone in patients aged 60–85 years. Methods Patients with newly diagnosed multiple myeloma were randomly assigned to receive oral MP for six 4-week cycles plus thalidomide (n=129; 100 mg per day continuously until any sign of relapse or progressive disease) or MP alone (n=126). Analysis was intention-to-treat. This study is registered at ClinicalTrial.gov, number NCT00232934. Results Patients treated with thalidomide had higher response rates and longer event-free survival (primary endpoints) than patients who were not. Combined complete or partial response rates were 76·0% for MPT and 47·6% for MP alone (absolute difference 28·3%, 95% CI 16·5–39·1), and the near-complete or complete response rates were 27·9% and 7·2%, respectively. 2-year event-free survival rates were 54% for MPT and 27% for MP (hazard ratio [HR] for MPT 0·51, 95% CI 0·35–0·75, p=0·0006). 3-year survival rates were 80% for MPT and 64% for MP (HR for MPT 0·68, 95% CI 0·38–1·22, p=0·19). Rates of grade 3 or 4 adverse events were 48% in MPT patients and 25% in MP patients (p=0·0002). Introduction of enoxaparin prophylaxis reduced rate of thromboembolism from 20% to 3% (p=0·005). Conclusion Oral MPT is an effective first-line treatment for elderly patients with multiple myeloma. Anticoagulant prophylaxis reduces frequency of thrombosis. Longer follow-up is needed to assess effect on overall survival.

Sweet sorghum (Sorghum bicolor (L.) Moench var. saccharatum) is arousing interest as bioenergy crop because of its versatility, relative drought tolerance, and ability to grow in environmental conditions not suitable for many other crops. The aim of this study was to evaluate sweet sorghum genotypes potential as biofuel feedstock. The experiment was conducted in two growing areas in Europe also to determine the possibility of using the sweet sorghum in cropping systems at high northern latitudes which could be considered marginal areas for C4 plants. The research was performed in 2009 at Berlin, Germany, and in 2010 atViterbo, Italy. The biomass production, fermentable sugar content and ethanol yield potential of 6 sweet sorghum genotypes (Keller, M81E, Dale, Della, Bovital, Goliath) were compared. The experimental design was a randomized block with four replicates.The results suggest that sweet sorghum has good potential as alternative energy cropalso in Central European climates and that the further studies are required. Abstract

The inclusion of energy crops in farming systems is an opportunity for growers to increase the efficiency of agricultural inputs and reduce emissions of potential greenhouse gases, while maintaining profitability and soil ferility. Among the few species eligible for this purpose, H. tuberosus L. (Ht) may become an alternative to dismissed maize or sugar beet or grape cropping systems. The efficiency of Ht stalk and tuber biomass production depends, among other factors, on plant density and water resources supply. Ht clones expressing new structure for plant architecture and phenological phases were selected to explore a range of farm production environments in Central Italy, differing mainly for the air temperature and rainfall patterns. A pilot-study was developed for preparing a Ht-cropping system for carbohydrate-rich biomass production in rainfed conditions or in farms equipped with emergence irrigation facilities. We have identified two Ht clones which are expected that, under a larger experimental field test, will be suitable for high stalk-biomass and high tuber-biomass production.
