The effects which student nurses have on the care of clients with mental health problems in the community is an area which has seldom been studied. With the closure of large psychiatric hospitals and the rise in community placements as part of Project 2000, an increasing number of students are being placed in the community. The study gathers data from clients attending five self-help/support groups in the North Derbyshire area of England. Analysis of the data challenges assumptions generally held by community psychiatric nurses (CPNs) that the presence of a third person, e.g. student/visitor, during a CPN home visit to the client is detrimental to the therapeutic interaction between the CPN and the client. The findings are inconclusive but suggest that some client groups (possibly those with long-term mental health problems) may find the presence of a student during a CPN visit facilitative. The study raises the issue of the student nurse/patient power relationship within a support group. Findings suggest that membership of a support group is empowering to the client and illustrates that clients in the community have greater control over the involvement of student nurses in their care than patients in hospital. Groups within the sample expressed a unanimous view that student nurse placements should be long enough to allow therapeutic nurse/client relationships to develop. H3K27M-mutant diffuse midline gliomas (DMGs) express high levels of the disialoganglioside GD2 (ref. 1 ). Chimeric antigen receptor-modified T cells targeting GD2 (GD2-CART) eradicated DMGs in preclinical models 1 . Arm A of Phase I trial no. NCT04196413 (ref.  2 ) administered one intravenous (IV) dose of autologous GD2-CART to patients with H3K27M-mutant pontine (DIPG) or spinal DMG (sDMG) at two dose levels (DL1, 1 × 10 6  kg - 1 ; DL2, 3 × 10 6  kg -1 ) following lymphodepleting chemotherapy. Patients with clinical or imaging benefit were eligible for subsequent intracerebroventricular (ICV) intracranial infusions (10-30 × 10 6 GD2-CART). Primary objectives were manufacturing feasibility, tolerability and the identification of maximally tolerated IV dose. Secondary objectives included preliminary assessments of benefit. Thirteen patients enroled, with 11 receiving IV GD2-CART on study (n = 3 DL1 (3 DIPG); n = 8 DL2 (6 DIPG, 2 sDMG)). GD2-CART manufacture was successful for all patients. No dose-limiting toxicities occurred on DL1, but three patients experienced dose-limiting cytokine release syndrome on DL2, establishing DL1 as the maximally tolerated IV dose. Nine patients received ICV infusions, with no dose-limiting toxicities. All patients exhibited tumour inflammation-associated neurotoxicity, safely managed with intensive monitoring and care. Four patients demonstrated major volumetric tumour reductions (52, 54, 91 and 100%), with a further three patients exhibiting smaller reductions. One patient exhibited a complete response ongoing for over 30 months since enrolment. Nine patients demonstrated neurological benefit, as measured by a protocol-directed clinical improvement score. Earth-abundant transition metal oxides are promising alternatives to precious metal oxides as electrocatalysts for the oxygen evolution reaction (OER) and are intensively investigated for alkaline water electrolysis. OER electrocatalysis, like most other catalytic reactions, is surface-initiated, and the catalyst performance is fundamentally determined by the surface properties. Most transition metal oxide catalysts show OER activities that depend on the predominantly exposed crystal facets/surface structure. Therefore, the design of synthetic strategies to obtain the most active crystal facets is of significant research interest. In this work, rock salt NiO OER catalysts with (111) predominantly exposed facets were synthesized by a solvothermal (ST) method either heated under supercritical or microwave-assisted (MW) conditions. Particular emphasis was placed on the influence of the post annealing temperature on the structural configuration and OER activity to compare their catalytic performances. The as-prepared electrocatalysts are pure α-Ni hydroxides which were converted to rock salt NiO (111) nanosheets with hexagonal pores after heat treatment at different temperatures. The OER activity of the electrodes has been evaluated in 0.1 M KOH using geometric and intrinsic current densities via normalization by the disk area and BET area, respectively. The lowest overpotential at a geometric current density of 10 mA/cm 2 is found for samples pretreated by heating between 400 and 500 °C with a catalyst loading of 115 μg/cm 2 . Despite the very similar nature of the catalysts obtained from the two methods, the ST electrodes show a higher geometric and intrinsic current density for 500 °C pretreatment. The MW electrodes, however, achieve an optimal geometric current density for 400 °C pretreatment, while their intrinsic current density requires pretreatment over 600 °C. Interestingly, pretreated electrodes show consistently higher OER activity as compared to the poorly crystalline/less ordered hydroxide as-prepared electrocatalysts. Thus, our study highlights the importance of the synthesis method and pretreatment at an optimal temperature.

As field-dispatchable power sources offer an alternative means of energy conversion, electrocatalyst development has become an area of intense focus. Emphasis has been placed on the transition from expensive electrocatalysts such as platinum and palladium toward earth abundant materials. Such a shift would result in lowered costs, enhanced durability, and an increased potential for implementation on a broader scale. High entropy oxides (HEOs) are an emerging class of materials that can offer both earth abundance and tunability of composition and morphology, making them excellent candidates for electrocatalysis. Several approaches have been taken to synthesize these materials and achieve balance between single-phase, highly crystalline products and high-surface area, nanostructured products. This work offers a survey of these methods, as well as our perspective on the most promising pathways forward. Emphasis is placed on clarifying the benefits, challenges, and overall suitability of each means of synthesis with electrocatalytic applications in mind.

Chemical screens across hundreds of cell lines have shown that the drug sensitivities of human cancers can vary by genotype or lineage. However, most drug discovery studies have relied on culture media that poorly reflect metabolite levels in human blood. Here, we perform drug screens in traditional and Human Plasma-Like Medium (HPLM). Sets of compounds that show conditional anticancer activity span different phases of global development and include non-oncology drugs. Comparisons of the synthetic and serum-derived components that comprise typical media trace sets of conditional phenotypes to nucleotide synthesis substrates. We also characterize a unique dual mechanism for brivudine, a compound approved for antiviral use. Brivudine selectively impairs cell growth in low folate conditions by targeting two enzymes involved in one-carbon metabolism. Cataloged gene essentiality data further suggest that conditional phenotypes for other compounds are linked to off-target effects. Our findings establish general strategies for identifying drug-nutrient interactions and mechanisms of action by exploiting conditional lethality in cancer cells.

H3K27M-mutant diffuse midline gliomas (DMGs) express high levels of the GD2 disialoganglioside and chimeric antigen receptor modified T-cells targeting GD2 (GD2-CART) eradicate DMGs in preclinical models. Arm A of the Phase I trial NCT04196413 administered one IV dose of autologous GD2-CART to patients with H3K27M-mutant pontine (DIPG) or spinal (sDMG) diffuse midline glioma at two dose levels (DL1=1e6/kg; DL2=3e6/kg) following lymphodepleting (LD) chemotherapy. Patients with clinical or imaging benefit were eligible for subsequent intracerebroventricular (ICV) GD2-CART infusions (10-30e6 GD2-CART). Primary objectives were manufacturing feasibility, tolerability, and identification of a maximally tolerated dose of IV GD2-CART. Secondary objectives included preliminary assessments of benefit. Thirteen patients enrolled and 11 received IV GD2-CART on study [n=3 DL1(3 DIPG); n=8 DL2(6 DIPG/2 sDMG). GD2-CART manufacturing was successful for all patients. No dose-limiting toxicities (DLTs) occurred on DL1, but three patients experienced DLT on DL2 due to grade 4 cytokine release syndrome (CRS). Nine patients received ICV infusions, which were not associated with DLTs. All patients exhibited tumor inflammation-associated neurotoxicity (TIAN). Four patients demonstrated major volumetric tumor reductions (52%, 54%, 91% and 100%). One patient exhibited a complete response ongoing for >30 months since enrollment. Eight patients demonstrated neurological benefit based upon a protocol-directed Clinical Improvement Score. Sequential IV followed by ICV GD2-CART induced tumor regressions and neurological improvements in patients with DIPG and sDMG. DL1 was established as the maximally tolerated IV GD2-CART dose. Neurotoxicity was safely managed with intensive monitoring and close adherence to a management algorithm.

Inflammasomes are multiprotein signaling complexes that activate the innate immune system. Canonical inflammasomes recruit and activate caspase-1, which then cleaves and activates IL-1β and IL-18, as well as gasdermin D (GSDMD) to induce pyroptosis. In contrast, non-canonical inflammasomes, caspases-4/-5 (CASP4/5) in humans and caspase-11 (CASP11) in mice, are known to cleave GSDMD, but their role in direct processing of other substrates besides GSDMD has remained unknown. Here, we show that CASP4/5 but not CASP11 can directly cleave and activate IL-18. However, CASP4/5/11 can all cleave IL-1β to generate a 27-kDa fragment that deactivates IL-1β signaling. Mechanistically, we demonstrate that the sequence identity of the tetrapeptide sequence adjacent to the caspase cleavage site regulates IL-18 and IL-1β recruitment and activation. Background: Immunocompromised patients are at increased risk of SARS-CoV-2 infections. Patients undergoing chimeric antigen receptor (CAR) T-cell therapy for relapsed/refractory B-cell malignancies are uniquely immunosuppressed due to CAR T-mediated B-cell aplasia (BCA). While SARS-CoV-2 mortality rates of 33%-40% are reported in adult CAR T-cell recipients, outcomes in pediatric and young adult CAR T-cell recipients are limited., Methods: We created an international retrospective registry of CAR T recipients aged 0-30 years infected with SARS-CoV-2 within 2 months prior to or any time after CAR T infusion. SARS-CoV-2-associated illness was graded as asymptomatic, mild, moderate, or severe COVID-19, or multisystem inflammatory syndrome in children (MIS-C). To assess for risk factors associated with significant SARS-CoV-2 infections (infections requiring hospital admission for respiratory distress or supplemental oxygen), univariate and multivariable regression analyses were performed., Results: Nine centers contributed 78 infections in 75 patients. Of 70 SARS-CoV-2 infections occurring after CAR T infusion, 13 (18.6%) were classified as asymptomatic, 37 (52.9%) mild, 11 (15.7%) moderate, and 6 (8.6%) severe COVID-19. Three (4.3%) were classified as MIS-C. BCA was not significantly associated with infection severity. Prior to the emergence of the Omicron variant, of 47 infections, 19 (40.4%) resulted in hospital admission and 7 (14.9%) required intensive care, while after the emergence of the Omicron variant, of 23 infections, only 1 (4.3%) required admission and the remaining 22 (95.7%) had asymptomatic or mild COVID-19. Death occurred in 3 of 70 (4.3%); each death involved coinfection or life-threatening condition. In a multivariable model, factors associated with significant SARS-CoV-2 infection included having two or more comorbidities (OR 7.73, CI 1.05 to 74.8, p=0.048) and age ≥18 years (OR 9.51, CI 1.90 to 82.2, p=0.014). In the eight patients infected with SARS-CoV-2 before CAR T, half of these patients had their CAR T infusion delayed by 15-30 days., Conclusions: In a large international cohort of pediatric and young adult CAR-T recipients, SARS-CoV-2 infections resulted in frequent hospital and intensive care unit admissions and were associated with mortality in 4.3%. Patients with two or more comorbidities or aged ≥18 years were more likely to experience significant illness. Clinical pathways are evidence-based tools that have been integrated into many aspects of pediatric hospital medicine and have proven effective at reducing in-hospital complications from a variety of diseases. Adaptation of similar tools for specific, high-risk patient populations in pediatric oncology has been slower, in part due to patient complexities and variations in management strategies. There are few published studies of clinical pathways for pediatric oncology patients. Pediatric patients with a new diagnosis of leukemia or lymphoma often present with one or more "oncologic emergencies" that require urgent intervention and deliberate multidisciplinary care to prevent significant consequences. Here, we present two clinical pathways that have recently been developed using a multidisciplinary approach at a single institution, intended for the care of patients who present with hyperleukocytosis or an anterior mediastinal mass. These clinical care pathways have provided a critical framework for the immediate care of these patients who are often admitted to the pediatric intensive care unit for initial management. The goal of the pathways is to facilitate multidisciplinary collaborations, expedite diagnosis, and streamline timely treatment initiation. Diffuse intrinsic pontine glioma (DIPG) and other H3K27M-mutated diffuse midline gliomas (DMGs) are universally lethal paediatric tumours of the central nervous system 1 . We have previously shown that the disialoganglioside GD2 is highly expressed on H3K27M-mutated glioma cells and have demonstrated promising preclinical efficacy of GD2-directed chimeric antigen receptor (CAR) T cells 2 , providing the rationale for a first-in-human phase I clinical trial (NCT04196413). Because CAR T cell-induced brainstem inflammation can result in obstructive hydrocephalus, increased intracranial pressure and dangerous tissue shifts, neurocritical care precautions were incorporated. Here we present the clinical experience from the first four patients with H3K27M-mutated DIPG or spinal cord DMG treated with GD2-CAR T cells at dose level 1 (1 × 10 6 GD2-CAR T cells per kg administered intravenously). Patients who exhibited clinical benefit were eligible for subsequent GD2-CAR T cell infusions administered intracerebroventricularly 3 . Toxicity was largely related to the location of the tumour and was reversible with intensive supportive care. On-target, off-tumour toxicity was not observed. Three of four patients exhibited clinical and radiographic improvement. Pro-inflammatory cytokine levels were increased in the plasma and cerebrospinal fluid. Transcriptomic analyses of 65,598 single cells from CAR T cell products and cerebrospinal fluid elucidate heterogeneity in response between participants and administration routes. The surface of an electrocatalyst undergoes dynamic chemical and structural transformations under electrochemical operating conditions. There is a dynamic exchange of metal cations between the electrocatalyst and electrolyte. Understanding how iron in the electrolyte gets incorporated in the nickel hydroxide electrocatalyst is critical for pinpointing the roles of Fe during water oxidation. Here, we report that iron incorporation and oxygen evolution reaction (OER) are highly coupled, especially at high working potentials. The iron incorporation rate is much higher at OER potentials than that at the OER dormant state (low potentials). At OER potentials, iron incorporation favors electrochemically more reactive edge sites, as visualized by synchrotron X-ray fluorescence microscopy. Using X-ray absorption spectroscopy and density functional theory calculations, we show that Fe incorporation can suppress the oxidation of Ni and enhance the Ni reducibility, leading to improved OER catalytic activity. Our findings provide a holistic approach to understanding and tailoring Fe incorporation dynamics across the electrocatalyst-electrolyte interface, thus controlling catalytic processes.

Chimeric antigen receptor (CAR) T cells hold promise for the treatment of acute myeloid leukemia (AML), but optimal targets remain to be defined. We demonstrate that CD93 CAR T cells engineered from a novel humanized CD93-specific binder potently kill AML in vitro and in vivo but spare hematopoietic stem and progenitor cells (HSPC). No toxicity is seen in murine models, but CD93 is expressed on human endothelial cells, and CD93 CAR T cells recognize and kill endothelial cell lines. We identify other AML CAR T-cell targets with overlapping expression on endothelial cells, especially in the context of proinflammatory cytokines. To address the challenge of endothelial-specific cross-reactivity, we provide proof of concept for NOT-gated CD93 CAR T cells that circumvent endothelial cell toxicity in a relevant model system. We also identify candidates for combinatorial targeting by profiling the transcriptome of AML and endothelial cells at baseline and after exposure to proinflammatory cytokines., Significance: CD93 CAR T cells eliminate AML and spare HSPCs but exert on-target, off-tumor toxicity to endothelial cells. We show coexpression of other AML targets on endothelial cells, introduce a novel NOT-gated strategy to mitigate endothelial toxicity, and demonstrate use of high-dimensional transcriptomic profiling for rational design of combinatorial immunotherapies. Developing simple, inexpensive, and environmentally benign approaches to integrate morphologically well-defined nanoscale building blocks into larger high surface area materials is a key challenge in materials design and processing. In this work, we investigate the fundamental surface phenomena between MgO and water (both adsorption and desorption) with particles prepared via a vapor-phase process (MgO nanocubes) and a modified aerogel process (MgO(111) nanosheets). Through these studies, we unravel a strategy to assemble individual MgO nanoparticles into extended faceted single-crystalline MgO nanosheets and nanorods with well-defined exposed surfaces and edges. This reorganization can be triggered by the presence of H 2 O vapor or bulk liquid water. Water adsorption and the progressive conversion of vapor-phase grown oxide particles into hydroxides give rise to either one-dimensional or two-dimensional (1D or 2D) structures of high dispersion and surface area. The resulting Mg(OH) 2 lamella with a predominant (001) surface termination are well-suited precursor structures for their topotactic conversion into laterally extended and uniform MgO(111) grain surface configurations. To understand the potential of polar (111) surfaces for faceting and surface reconstruction effects associated with water desorption, we investigated the stability of MgO(111) nanosheets during vacuum annealing and electron beam exposure. The significant surface reconstruction of the MgO(111) surfaces observed shows that adsorbate-free (111)-terminated surfaces of unsupported MgO nanostructures reconstruct rather than remain as charged planes of either three-fold coordinated O 2- ion or Mg 2+ ions. In order to develop highly active non-precious metal catalysts for the selective oxidation of the platform compound 5-hydroxymethylfurfural (HMF) to the value-added bio-chemical 2,5-diformylfuran (DFF), we prepared high purity bivalent Mn 5 O 8 nanoplates by a microwave-assisted ionic liquid route. The precursor of bivalent Mn 5 O 8 nanoplates was formed through π-π stacking between imidazolium rings of the ionic liquid 1-butyl-3-methyl-imidazolium chloride and extending hydrogen bonds between Cl anions and hydrohausmannite. An oriented aggregation growth occurred on the basis of the Ostwald ripening under microwave heating. The high purity bivalent Mn 5 O 8 nanoplates obtained through calcination at 550 °C for 2 h exhibited high HMF conversion (51%) and DFF selectivity (94%) at 5 bar of oxygen pressure in 2 h. The high concentration of Mn 4+ on the exterior surfaces of Mn 5 O 8 nanoplates as active sites coupled with good crystallinity played key roles for desirable mass and heat transfer, and for fast desorption avoiding over-oxidation. The reaction process over the Mn 5 O 8 nanoplates was proposed based on the understanding of Mn 4+ active centers and lattice oxygen via a Mn 4+ /Mn 2+ two-electron cycle to enhance their catalytic performance. Furthermore, the Mn 5 O 8 nanoplates could be readily recovered and reused without loss of catalytic activity. Thus, the high purity Mn 5 O 8 nanoplates with good catalytic performance raises the prospect of using the type of sole metal oxide for practical applications.

Insufficient reactivity against cells with low antigen density has emerged as an important cause of chimeric antigen receptor (CAR) T-cell resistance. Little is known about factors that modulate the threshold for antigen recognition. We demonstrate that CD19 CAR activity is dependent upon antigen density and that the CAR construct in axicabtagene ciloleucel (CD19-CD28ζ) outperforms that in tisagenlecleucel (CD19-4-1BBζ) against antigen-low tumors. Enhancing signal strength by including additional immunoreceptor tyrosine-based activation motifs (ITAM) in the CAR enables recognition of low-antigen-density cells, whereas ITAM deletions blunt signal and increase the antigen density threshold. Furthermore, replacement of the CD8 hinge-transmembrane (H/T) region of a 4-1BBζ CAR with a CD28-H/T lowers the threshold for CAR reactivity despite identical signaling molecules. CARs incorporating a CD28-H/T demonstrate a more stable and efficient immunologic synapse. Precise design of CARs can tune the threshold for antigen recognition and endow 4-1BBζ-CARs with enhanced capacity to recognize antigen-low targets while retaining a superior capacity for persistence. SIGNIFICANCE: Optimal CAR T-cell activity is dependent on antigen density, which is variable in many cancers, including lymphoma and solid tumors. CD28ζ-CARs outperform 4-1BBζ-CARs when antigen density is low. However, 4-1BBζ-CARs can be reengineered to enhance activity against low-antigen-density tumors while maintaining their unique capacity for persistence. The catalytic activity of low-dimensional electrocatalysts is highly dependent on their local atomic structures, particularly those less-coordinated sites found at edges and corners; therefore, a direct probe of the electrocatalytic current at specified local sites with true nanoscopic resolution has become critically important. Despite the growing availability of operando imaging tools, to date it has not been possible to measure the electrocatalytic activities from individual material edges and directly correlate those with the local structural defects. Herein, we show the possibility of using feedback and generation/collection modes of operation of the scanning electrochemical microscope (SECM) to independently image the topography and local electrocatalytic activity with 15-nm spatial resolution. We employed this operando microscopy technique to map out the oxygen evolution activity of a semi-2D nickel oxide nanosheet. The improved resolution and sensitivity enables us to distinguish the higher activities of the materials' edges from that of the fully coordinated surfaces in operando The combination of spatially resolved electrochemical information with state-of-the-art electron tomography, that unravels the 3D complexity of the edges, and ab initio calculations allows us to reveal the intricate coordination dependent activity along individual edges of the semi-2D material that is not achievable by other methods. The comparison of the simulated line scans to the experimental data suggests that the catalytic current density at the nanosheet edge is ∼200 times higher than that at the NiO basal plane., Competing Interests: The authors declare no conflict of interest.

Patients with high risk neuroblastoma have a poor prognosis and survivors are often left with debilitating long term sequelae from treatment. Even after integration of anti-GD2 monoclonal antibody therapy into standard, upftont protocols, 5-year overall survival rates are only about 50%. The success of anti-GD2 therapy has proven that immunotherapy can be effective in neuroblastoma. Adoptive transfer of chimeric antigen receptor (CAR) T cells has the potential to build on this success. In early phase clinical trials, CAR T cell therapy for neuroblastoma has proven safe and feasible, but significant barriers to efficacy remain. These include lack of T cell persistence and potency, difficulty in target identification, and an immunosuppressive tumor microenvironment. With recent advances in CAR T cell engineering, many of these issues are being addressed in the laboratory. In this review, we summarize the clinical trials that have been completed or are underway for CAR T cell therapy in neuroblastoma, discuss the conclusions and open questions derived from these trials, and consider potential strategies to improve CAR T cell therapy for patients with neuroblastoma.

Nanostructured noble-metal catalysts traditionally suffer from sintering under high operating temperatures, leading to durability issues and process limitations. The encapsulation of nanostructured catalysts to prevent loss of activity through thermal sintering, while maintaining accessibility of active sites, remains a great challenge in the catalysis community. Here, we report a robust and regenerable palladium-based catalyst, wherein palladium particles are intercalated into the three-dimensional framework of SBA-15-type mesoporous silica. The encapsulated Pd active sites remain catalytically active as demonstrated in high-temperature/pressure phenol hydrodeoxygenation reactions. The confinement of Pd particles in the walls of SBA-15 prevents particle sintering at high temperatures. Moreover, a partially deactivated catalyst containing intercalated particles is regenerated almost completely even after several reaction cycles. In contrast, Pd particles, which are not encapsulated within the SBA-15 framework, sinter and do not recover prior activity after a regeneration procedure., Competing Interests: The authors declare no competing financial interest.

cis,cis-Muconic acid is a platform bio-based chemical that can be upgraded to drop-in commodity and novel monomers. Among the possible drop-in products, dimethyl terephthalate can be synthesized via esterification, isomerization, Diels-Alder cycloaddition, and dehydrogenation. The isomerization of cis,cis-dimethyl muconate (ccDMM) to the trans,trans-form (ttDMM) can be catalyzed by iodine; however, studies have yet to address (i) the mechanism and reaction barriers unique to DMM, and (ii) the influence of solvent, potential for catalyst recycle, and recovery of high-purity ttDMM. To address this gap, we apply a joint computational and experimental approach to investigate iodine-catalyzed isomerization of DMM. Density functional theory calculations identified unique regiochemical considerations owing to the large number of halogen-diene coordination schemes. Both transition state theory and experiments estimate significant barrier reductions with photodissociated iodine. Solvent selection was critical for rapid kinetics, likely because of solvent complexation with iodine. Under select conditions, ttDMM yields of 95 % were achieved in <1 h with methanol, followed by high purity recovery (>98 %) with crystallization. Lastly, post-reaction iodine can be recovered and recycled with minimal loss of activity. Solid metal oxides for carbon capture exhibit reduced adsorption capacity following high-temperature exposure, due to surface area reduction by sintering. Furthermore, only low-coordinate corner/edge sites on the thermodynamically stable (100) facet display favorable binding toward CO 2 , providing inherently low capacity. The (111) facet, however, exhibits a high concentration of low-coordinate sites. In this work, MgO(111) nanosheets displayed high capacity for CO 2 , as well as a ∼65% increase in capacity despite a ∼30% reduction in surface area following sintering (0.77 mmol g -1 @ 227 m 2 g -1 vs 1.28 mmol g -1 @ 154 m 2 g -1 ). These results, unique to MgO(111), suggest intrinsic differences in the effects of sintering on basic site retention. Spectroscopic and computational investigations provided a new structure-activity insight: the importance of high-temperature activation to unleash the capacity of the polar (111) facet of MgO. In summary, we present the first example of a faceted sorbent for carbon capture and challenge the assumption that sintering is necessarily a negative process; here we leverage high-temperature conditions for facet-dependent surface activation.

Tunable reaction selectivity on a single catalyst is a continual goal in chemical syntheses. Herein, we report an unexpected light-directed intermediate selectivity using well-known plasmonic photocatalysts. We observed distinct intermediate selectivity behaviors between using UV and visible light irradiations. Chemical computations and quenching experiments suggest that the radicals generated by the plasmonic excitation govern the light-directed selectivity. The broader impact of this work ranges from selective yield of desirable intermediates for subsequent syntheses without tedious separation procedures, to arousing interest in examining new opportunities for plasmonic photocatalysts.

Recent advances in theoretical structure prediction methods and high-throughput computational techniques are revolutionizing experimental discovery of the thermodynamically stable inorganic materials. Metastable materials represent a new frontier for these studies, since even simple binary non-ground state compounds of common elements may be awaiting discovery. However, there are significant research challenges related to non-equilibrium thin film synthesis and crystal structure predictions, such as small strained crystals in the experimental samples and energy minimization based theoretical algorithms. Here, we report on experimental synthesis and characterization, as well as theoretical first-principles calculations of a previously unreported mixed-valent binary tin nitride. Thin film experiments indicate that this novel material is N-deficient SnN with tin in the mixed ii/iv valence state and a small low-symmetry unit cell. Theoretical calculations suggest that the most likely crystal structure has the space group 2 (SG2) related to the distorted delafossite (SG166), which is nearly 0.1 eV/atom above the ground state SnN polymorph. This observation is rationalized by the structural similarity of the SnN distorted delafossite to the chemically related Sn3N4 spinel compound, which provides a fresh scientific insight into the reasons for growth of polymorphs of metastable materials. In addition to reporting on the discovery of the simple binary SnN compound, this paper illustrates a possible way of combining a wide range of advanced characterization techniques with the first-principle property calculation methods, to elucidate the most likely crystal structure of the previously unreported metastable materials.

The development of sodium ion batteries (NIBs) can provide an alternative to lithium ion batteries (LIBs) for sustainable, low-cost energy storage. However, due to the larger size and higher m/e ratio of the sodium ion compared to lithium, sodiation reactions of candidate electrodes are expected to differ in significant ways from the corresponding lithium ones. In this work, we investigated the sodiation mechanism of a typical transition metal-oxide, NiO, through a set of correlated techniques, including electrochemical and synchrotron studies, real-time electron microscopy observation, and ab initio molecular dynamics (MD) simulations. We found that a crystalline Na2O reaction layer that was formed at the beginning of sodiation plays an important role in blocking the further transport of sodium ions. In addition, sodiation in NiO exhibits a "shrinking-core" mode that results from a layer-by-layer reaction, as identified by ab initio MD simulations. For lithiation, however, the formation of Li antisite defects significantly distorts the local NiO lattice that facilitates Li insertion, thus enhancing the overall reaction rate. These observations delineate the mechanistic difference between sodiation and lithiation in metal-oxide conversion materials. More importantly, our findings identify the importance of understanding the role of reaction layers on the functioning of electrodes and thus provide critical insights into further optimizing NIB materials through surface engineering.

As a promising catalytically active nano reactor, gold nanoparticles intercalated in mesoporous silica (GMS) were successfully synthesized and properties of the materials were investigated. We used a one pot sol-gel approach to intercalate gold nano particles in the walls of mesoporous silica. To start with the synthesis, P123 was used as template to form micelles. Then TESPTS was used as a surface modification agent to intercalate gold nano particles. Following this process, TEOS was added in as a silica source which underwent a polymerization process in acid environment. After hydrothermal processing and calcination, the final product was acquired. Several techniques were utilized to characterize the porosity, morphology and structure of the gold intercalated mesoporous silica. The results showed a stable structure of mesoporous silica after gold intercalation. Through the oxidation of benzyl alcohol as a benchmark reaction, the GMS materials showed high selectivity and recyclability.

This study presents an interfacial modification strategy to improve the performance of electrochromic films that were fabricated by a magnetron sputtering technique. High-quality graphene sheets, synthesized by chemical vapor deposition, were used to modify fluorine-doped tin oxide substrates, followed by the deposition of high-performance nanocomposite nickel oxide electrochromic films. Electrochromic cycling results revealed that a near-complete monolayer graphene interfacial layer improves the electrochromic performance in terms of switching kinetics, activation period, coloration efficiency, and bleached-state transparency, while maintaining ∼100% charge reversibility. The present study offers an alternative route for improving the interfacial properties between electrochromic and transparent conducting oxide films without relying on conventional methods such as nanostructuring or thin film composition control.

Nanoparticle electrodes in lithium-ion batteries have both near-surface and interior contributions to their redox capacity, each with distinct rate capabilities. Using combined electron microscopy, synchrotron X-ray methods and ab initio calculations, we have investigated the lithiation pathways that occur in NiO electrodes. We find that the near-surface electroactive (Ni(2+) → Ni(0)) sites saturated very quickly, and then encounter unexpected difficulty in propagating the phase transition into the electrode (referred to as a "shrinking-core" mode). However, the interior capacity for Ni(2+) → Ni(0) can be accessed efficiently following the nucleation of lithiation "fingers" that propagate into the sample bulk, but only after a certain incubation time. Our microstructural observations of the transition from a slow shrinking-core mode to a faster lithiation finger mode corroborate with synchrotron characterization of large-format batteries and can be rationalized by stress effects on transport at high-rate discharge. The finite incubation time of the lithiation fingers sets the intrinsic limitation for the rate capability (and thus the power) of NiO for electrochemical energy storage devices. The present work unravels the link between the nanoscale reaction pathways and the C-rate-dependent capacity loss and provides guidance for the further design of battery materials that favors high C-rate charging.

The design and synthesis of shape-directed nanoscale noble metal particles have attracted much attention due to their enhanced catalytic properties and the opportunities to study fundamental aspects of nanoscale systems. As such, numerous methods have been developed to synthesize crystals with tunable shapes, sizes, and facets by adding foreign species that promote or restrict growth on specific sites. Many hypotheses regarding how and why certain species direct growth have been put forward, however there has been no consensus on a unifying mechanism of nanocrystal growth. Herein, we develop and demonstrate the capabilities of a mathematical growth model for predicting metal nanoparticle shapes by studying a well known procedure that employs AgNO3 to produce {111} faceted Pt nanocrystals. The insight gained about the role of auxiliary species is then utilized to predict the shape of Pd nanocrystals and to corroborate other shape-directing syntheses reported in literature. The fundamental understanding obtained herein by combining modeling with experimentation is a step toward computationally guided syntheses and, in principle, applicable to predictive design of the growth of crystalline solids at all length scales (nano to bulk).

The performance of battery materials is largely governed by structural and chemical evolutions during electrochemical reactions. Therefore, resolving spatially dependent reaction pathways could enlighten mechanistic understanding, and enable rational design for rechargeable battery materials. Here, we present a phase evolution panorama via spectroscopic and three-dimensional imaging at multiple states of charge for an anode material (that is, nickel oxide nanosheets) in lithium-ion batteries. We reconstruct the three-dimensional lithiation/delithiation fronts and find that, in a fully electrolyte immersion environment, phase conversion can nucleate from spatially distant locations on the same slab of material. In addition, the architecture of a lithiated nickel oxide is a bent porous metallic framework. Furthermore, anode-electrolyte interphase is found to be dynamically evolving upon charging and discharging. The present study has implications for resolving the inhomogeneity of the general electrochemically driven phase transition (for example, intercalation reactions) and for the origin of inhomogeneous charge distribution in large-format battery electrodes.

Electrochromic effects of transition metal oxides provide a great platform for studying lithium intercalation chemistry in solids. Herein, we report on an electronically modified nanocomposite nickel oxide (i.e., Li2.34NiZr0.28Ox) that exhibits significantly improved electrochromic performance relative to the state-of-the-art inorganic electrochromic metal oxides in terms of charge/discharge kinetics, bleached-state transparency, and optical modulation. The knowledge obtained from O K-edge X-ray absorption spectroscopy (XAS) and X-ray photoelectron spectroscopy (XPS) suggests that the internally grown lithium peroxide (i.e., Li2O2) species plays a major role in facilitating charge transfer thus enabling optimal electrochromic performance. This understanding is relevant to recent theoretical studies concerning conductivity in Li2O2 for lithium-air batteries (as cited in the main text). Furthermore, we elucidate the electrochromism in modified nickel oxide in lithium ion electrolyte with the aid of Ni K-edge XAS and Ni L-edge XAS studies. The electrochromism in the nickel oxide materials arises from the reversible formation of hole states on the NiO6 units, which then impacts the Ni oxidation state through the Ni3d-O2p hybridization states. This study sheds light on the lithium intercalation chemistry for general energy storage and semiconductor applications.

Polyoxometalates (POMs) have demonstrated the unusual ability to both stabilize nanoclusters and facilitate catalytic activity by allowing substrate access to sufficient metallic surface area. This paper presents a wet chemical synthetic method to prepare Pd, Ag, and Au metal nanoclusters stabilized by tetrabutylammonium salts of Phosphotungstates with the well-known Keggin [α-PW(12)O(40)](3-) (PW(12)), Wells-Dawson [P(2)W(18)O(62)](6-) (P(2)W(18)), and their lacunary derivatives [α-PW(11)O(39)](7-) (PW(11)) and [P(2)W(15)O(56)](12-) (P(2)W(15)) structures. The preparation of this series of nanoclusters has facilitated a comparison of the effect these anions have in nanocluster stabilization and catalysis. The nanocluster systems were found to be re-dispersible in acetonitrile and acetone. The effect of aging on stability was followed over 3 months and it was found that many systems were very stable for more than 70 days without precipitation, depending on type of polytungstate stabilizer and the presence of competing ions. The PW(12), P(2)W(18), PW(11), and P(2)W(15) stabilized Pd nanoclusters have been investigated for catalytic activity in the hydrogenation of 1-hexene and were found to be very active with the P(2)W(18) stabilized Pd nanoclusters demonstrating a best of 139,000 Total Turnovers, thus ranking this "soluble heterogeneous catalyst" among one of the longest lived reported in the literature. Electrochromic materials exhibit switchable optical properties that can find applications in various fields, including smart windows, nonemissive displays, and semiconductors. High-performing nickel oxide electrochromic materials have been realized by controlling the material composition and tuning the nanostructural morphology. Post-treatment techniques could represent efficient and cost-effective approaches for performance enhancement. Herein, we report on a post-processing ozone technique that improves the electrochromic performance of an aluminum-containing nickel oxide material in lithium-ion electrolytes. The resulting materials were studied using X-ray diffraction (XRD), scanning electron microscopy (SEM), X-ray photoelectron spectroscopy (XPS), ultraviolet-visible-near-infrared (UV-vis-NIR) spectroscopy, and X-ray absorption spectroscopy (XAS). It was observed that ozone exposure increased the Ni oxidation state by introducing hole states in the NiO(6) octahedral unit. In addition, ozone exposure gives rise to higher-performing aluminum-containing nickel oxide films, relative to nickel oxide containing both Al and Li, in terms of switching kinetics, bleached-state transparency, and optical modulation. The improved performance is attributed to the decreased crystallinity and increased nickel oxidation state in aluminum-containing nickel oxide electrochromic films. The present study provides an alternative route to improve electrochromic performance for nickel oxide materials.

Matrix-assisted laser desorption/ionization mass spectrometry (MALDI-MS) has been used for lipid analysis; however, one of the drawbacks of this technique is matrix interference peaks at low masses. Metal oxide surfaces are described here for direct, matrix-free analysis of small (MW < 1000 Da) lipid compounds, without interferences in the resulting spectra from traditional matrix background peaks. Spectra from lipid standards produced protonated and sodiated molecular ions. More complex mixtures including vegetable oil shortening and lipid extracts from bacterial and algal sources provided similar results. Mechanistic insight into the mode of ionization from surface spectroscopy, negative ion mass spectrometry, and stable isotope studies is also presented. The metal oxide system is compared to other reported matrix-free systems.

Stable gold/mesoporous silica nanocomposites (with Au nanoparticles intercalated in the walls of mesoporous silica) were successfully synthesized by the hydrothermal method and applied as catalysts. A challenging issue associated with intercalation and the use of coordinating agents is the effect of the coordinating agent on the mesoporous silica structure and periodicity. This investigation is targeted at elaborating the effect of the coordinating agent on the resulting mesoporous structure. The amount of Au coordinating agent bis[3-(triethoxysilyl)propyl]-tetrasulfide (TESPTS) was systematically altered to synthesize a range of materials with varying Au loadings and morphologies. These materials were characterized by N(2) adsorption-desorption, x-ray diffraction, transmission electron microscopy and UV-visible spectroscopy. The structures of the catalysts were found to range from mesoporous to vesical- and foam-like upon varying the TESPTS/polymer template (P123) ratio. Additionally, the sizes of Au nanoparticles increased by increasing the amount of TESPTS. The catalytic properties of the resulting materials were examined using oxidation of benzyl alcohol and reduction of 4-nitrophenol as probe reactions. The intercalated systems demonstrated high activity and more importantly were robust and readily reusable. This approach to imparting stability to nanoscale materials may be much more broadly applicable and expand the types of environments in which they can be utilized.

A low-temperature ozone exposure technique was employed for the post-treatment of WO(x) nanorod thin films fabricated from hot-wire chemical vapor deposition (HWCVD) and ultrasonic spray deposition (USD) techniques. The resulting films were characterized with x-ray diffraction (XRD), transmission electron microscopy (TEM), Raman spectroscopy, UV-vis-NIR spectroscopy and x-ray photoelectron spectroscopy (XPS). The stoichiometry and surface crystallinity of the WO(x) thin films were subsequently modulated upon ozone exposure and thermal annealing without particle growth. The electrochromic performance was studied in a LiClO(4)-propylene carbonate electrolyte, and the results suggest that the low-temperature ozone exposure technique is superior to the traditional high-temperature thermal annealing (employed to more fully oxidize the WO(x)). The optical modulation at 670 nm was improved from 35% for the as-deposited film to 57% for the film after ozone exposure at 150 °C. The coloration efficiency was improved and the switching speed to the darkened state was significantly accelerated from 18.0 s for the as-deposited film to 11.8 s for the film after the ozone exposure. The process opens an avenue for low-temperature and cost-effective manufacturing of electrochromic films, especially on flexible polymer substrates.

Background: The influence of cerebral perfusion pressure (CPP) on real-time focal cerebral blood flow (fCBF) is not fully understood, in either intact or injured brain. We wanted to evaluate that relationship, and by implication investigate the relative importance of perfusion pressure versus metabolism in the regulation and control of cerebral blood flow. Our hypothesis was that metabolic needs dominated over a physiologic range of blood pressure., Methods: This was an observational study of 23 patients, most of them with closed head injury, three with subarachnoid hemorrhage, one with a gunshot wound to the brain, and one monitored after craniotomy for unruptured aneurysm. Arterial lines, ventriculostomies, and fCBF monitors were placed. CPP (mean arterial pressure - intracranial pressure) and fCBF were measured and recorded to a computer database every minute. The relationship between CPP and fCBF was graphed and correlation coefficients were compared between survivors and non-survivors., Results: Graphs of CPP versus fCBF did not show any linearity over a range of 50-150 mm Hg in patients who survived. In those who died, four of seven showed some indication of linearity. The difference in the correlation coefficients between survivors and non-survivors was statistically significant (P < 0.05), with survivors having essentially no correlation, as expected with autoregulation intact, and non-survivors having a mean correlation of 0.311., Conclusions: In the functioning and viable brain, metabolic regulation of cerebral blood flow (CBF) predominates, leading to the lack of an obvious relationship between perfusion pressure and flow. This predominance of metabolic regulation is robust and preserved over a wide range of brain injury, with pressure autoregulation necessary but not clinically apparent in the metabolically active brain. This robust and constantly varying relationship of pressure and flow shown by our real-time measurements of fCBF has important implications for interpreting clinical measurements of autoregulation. Perhaps most importantly, the development of a correlation between pressure and flow may indicate and be an early warning of deterioration.

The selective reaction of one part of a bifunctional molecule is a fundamental challenge in heterogeneous catalysis and for many processes including the conversion of biomass-derived intermediates. Selective hydrogenation of unsaturated epoxides to saturated epoxides is particularly difficult given the reactivity of the strained epoxide ring, and traditional platinum group catalysts show low selectivities. We describe the preparation of highly selective Pd catalysts involving the deposition of n-alkanethiol self-assembled monolayer (SAM) coatings. These coatings improve the selectivity of 1-epoxybutane formation from 1-epoxy-3-butene on palladium catalysts from 11 to 94% at equivalent reaction conditions and conversions. Although sulphur species are generally considered to be indiscriminate catalyst poisons, the reaction rate to the desired product on a catalyst coated with a thiol was 40% of the rate on an uncoated catalyst. Interestingly the activity decreased for less-ordered SAMs with shorter chains. The behaviour of SAM-coated catalysts was compared with catalysts where surface sites were modified by carbon monoxide, hydrocarbons or sulphur atoms. The results suggest that the SAMs restrict sulphur coverage to enhance selectivity without significantly poisoning the activity of the desired reaction.

The reaction of [Ru(arene)Cl(2)](2) (arene = benzene, p-cymene) with [X(2)W(22)O(74)(OH)(2)](12-) (X = Sb(III), Bi(III)) in buffer medium resulted in four organo-ruthenium supported heteropolytungstates, [Sb(2)W(20)O(70)(RuC(6)H(6))(2)](10-) (1), [Bi(2)W(20)O(70)(RuC(6)H(6))(2)](10-) (2), [Sb(2)W(20)O(70)(RuC(10)H(14))(2)](10-) (3), and [Bi(2)W(20)O(70)(RuC(10)H(14))(2)](10-) (4), which have been characterized in solution by multinuclear ((183)W, (13)C, (1)H) NMR, UV-vis spectroscopy, electrochemistry, and in the solid state by single-crystal X-ray diffraction, IR spectroscopy, thermogravimetric analysis, and elemental analysis. Polyanions 1-4 consist of two (L)Ru(2+) (L = benzene or p-cymene) units linked to a [X(2)W(20)O(70)](14-) (X = Sb(III), Bi(III)) fragment via Ru-O(W) bonds resulting in an assembly with idealized C(2h) symmetry. Polyanions 1-4 are stable in solution as indicated by the expected (183)W, (13)C, and (1)H NMR spectra. The electrochemistry of 1-4 is described by considering the reduction and the oxidation processes. The nature of the arene in Ru(arene) has practically no influence on the formal potentials of the W-centers, which are more sensitive to the Sb or Bi hetero atoms. The results suggest that the respective Sb- and Bi derivatives have very different pK(a) values, with the reduced form of 1 being the most basic, thus permitting the observation of two well-developed voltammetric waves at pH 6. In contrast, the identity of the arene influences the oxidation processes, thus permitting to distinguish them. A strong electrocatalytic water oxidation peak is observed that is more positive than the one corresponding to the Ru(arene) oxidation process. Also a stepwise oxidation of the Ru(benzene) group could be observed at pH 3. The catalytic efficiency, on the other hand, of 1-4 toward the oxidation of n-hexadecane and p-xylene illustrated the effect of ruthenium substitution on the polyanion catalytic performance.

The selective oxidation of alkanes as a green process remains a challenging task because partial oxidation is easier to achieve with sacrificial oxidants, such as hydrogen peroxide, alkyl hydroperoxides or iodosylbenzene, than with molecular oxygen or air. Here, we report on a heterogeneous catalyst for n-hexadecane oxidation comprised of the wheel shaped Cu20-polyoxotungstate [Cu20Cl(OH)24(H2O)12(P8W48O184)]25- anchored on 3-aminopropyltriethoxysilane (apts)-modified SBA-15. The catalysts were characterized by powder X-ray diffraction (XRD), N2-adsorption measurements and Fourier transform infrared reflectance (FT-IR) spectroscopy. The heterogeneous Cu20-polyanion system catalyzed the solvent-free aerobic oxidation of n-hexadecane to alcohols and ketones by using air as the oxidant under ambient conditions. The catalyst exhibits an exceptionally high turn over frequency (TOF) of 20,000 h(-1) at 150 degrees C and is resistant to poisoning by CS2. Moreover, it can be easily recovered and reused by filtration without loss of its catalytic activity. Possible homogeneous contributions also have been examined and eliminated. Thus, this system can use air as oxidant, which, in combination with its good overall performance and poison tolerance, raises the prospect of this type of heterogeneous catalyst for practical applications.

Nanoscale iron-doped zirconia solid-solution aerogels are prepared via a simple ethanol thermal route using zirconyl nitrate and iron nitrate as starting materials, followed by a supercritical fluid drying process. Structural characteristics are investigated by means of powder X-ray diffraction (XRD), thermal analyses (TG/DTA), N(2) adsorption measurements and diffuse reflectance infrared Fourier transform spectroscopy (DRIFTS). The results show that the resulting iron-doped solid solutions are metastable tetragonal zirconia which exhibit excellent dispersibility and high solubility of iron oxide. Further, when the Fe:(Fe+Zr) ratio x is lower than 0.10, all of the Fe(3+) ions can be incorporated into ZrO(2) by substituting Zr(4+) to form Zr(1-) (x)Fe(x)O(y) solid solutions. Moreover, for the first time, an additional hydroxyl group band that is not present in pure ZrO(2) is observed by DRIFTS for the Zr(Fe)O(2) solid solution. This is direct evidence of Fe(3+) ions incorporated into ZrO(2). These Zr(1-) (x)Fe(x)O(y) solid solutions are excellent catalysts for the solvent-free aerobic oxidation of n-hexadecane using air as the oxidant under ambient conditions. The Zr(0.8)Fe(0.2)O(y) solid-solution catalyst demonstrates the best catalytic properties, with the conversion of n-hexadecane reaching 36.2 % with 48 % selectivity for ketones and 24 % selectivity for alcohols and it can be recycled five times without significant loss of activity.

Two experiments are reported in which participants made familiarity decisions (Is this face familiar or not?) or gender decisions (Is this face male or female?) to the same sets of faces presented as whole faces or as internal features only. The experimental items on which the analysis was performed were famous and unfamiliar male faces that differed on rated masculinity. The famous faces also differed on age of acquisition (AoA). The experimental male faces were combined with an equal number of famous and unfamiliar female faces for presentation to participants. In experiment 1 we found faster and more accurate familiarity decisions to whole male faces than to internal features; also an interaction between AoA and masculinity such that familiarity decisions were both faster and more accurate to high- than to low-masculinity faces when those faces were late-acquired, but not when they were early-acquired. In experiment 2 we found the same benefit for whole faces over internal features and the same interaction between AoA and masculinity in gender decisions. The similarity between the effects of AoA and masculinity in familiarity and gender decisions is more readily accounted for by models of face processing which posit a close relationship between gender and identity processing than by models which maintain that those aspects of face perception are dealt with by quite separate processing streams. We propose that gender decisions, like familiarity decisions, are semantic judgments (rather than judgments based on the analysis of the surface features of faces), and that the shared basis of the two forms of decision explains why they show similar influences of AoA and masculinity.

The tetrairon(III)-substituted polytungstates [Fe(4)(H(2)O)(10)(beta-XW(9)O(33))(2)](n-) (n = 6, X = As(III), Sb(III); n = 4, X = Se(IV), Te(IV)) were immobilized on (3-aminopropyl)triethoxysilane-modified SBA-15 and showed an excellent catalytic performance for solvent-free aerobic oxidation of long-chain n-alkanes using air as the oxidant under ambient conditions through a classical free-radical chain autoxidation mechanism.

This report describes a woman who presented with a breast lump, which was initially misdiagnosed as lymphoma, but later turned out to be granulocytic sarcoma of the breast.

Papillomaviruses (PV) comprise a large family of nonenveloped DNA viruses that include the oncogenic PV types that are the causative agents of human cervical cancer. As is true of many animal DNA viruses, PV are taken into the cell by endocytosis and must escape from the endosomal compartment to the cytoplasm to initiate infection. Here we show that this step depends on the site-specific enzymatic cleavage of the PV minor virion protein L2 at a consensus furin recognition site. Cleavage by furin, a cell-encoded proprotein convertase, is known to be required for endosome escape by many bacterial toxins. However, to our knowledge, furin has not been previously implicated in the viral entry process. This step is potentially a target for PV inhibition.

Background: The practice of pharmaceutical care in primary care settings in Thailand is currently not generally accepted., Objective: To evaluate the effect of pharmacist involvement in treatment with hypertensive patients in primary care settings., Methods: The treatment objective was to stabilize the blood pressure (BP) of hypertensive patients in accordance with the Joint National Committee on Prevention, Detection, Evaluation and Treatment of High Blood Pressure guidelines. Patients were randomly assigned to a pharmacist-involved group (treatment) or a group with no pharmacist involvement (control). Pre- and post-test BPs, tablet counts, lifestyle modifications, and pharmacists' recommendations were recorded. The 6-month study was carried out in Mahasarakham University pharmacy and 2 primary care units. Patients were monitored monthly by reviewing their medications and supported by providing pharmaceutical care and counseling., Results: From a total of 235 patients, the treatment group (n = 118) had a significant reduction in both systolic (S) and diastolic (D) BP compared with the 117 patients of the control group (p = 0.037, 0.027, respectively). The 158 patients (76 treatment, 82 control) with BPs >or=140/90 mm Hg at the beginning of the study showed significant BP reductions (p = 0.002 SBP, 0.008 DBP). The proportion of 158 patients whose BP became stabilized was higher in the treatment group (p = 0.017). The treatment group showed significantly better adherence (p = 0.014) and exercise control (p = 0.012) at the end of the study. Physicians accepted 42.72% of medication modifications and 5.34% of the suggestions for additional investigations., Conclusions: Hypertensive patients who received pharmacist input achieved a significantly greater benefit in BP reduction, BP control, and improvement in adherence rate and lifestyle modification.

A technique based on the back scattering phenomenon of dynamic light scattering has been employed to monitor the kinetics of gold and platinum metal nanoparticle growth and silver nanoparticle oxidation as well as in the determination of particle sizes ranging from 1 to 200 nm in diameter. The systems were chosen to examine the applicability of dynamic light scattering to nanoresearch over a broad range of sizes as well as both metallic and nonmetallic systems. The advantages of this instrumentation over traditional instruments such as X-ray diffraction (XRD), transmission electron microscopy (TEM), and scanning electron microscopy (SEM) have been highlighted.

Research on metacognition often uses various kinds of paired-associate items (e.g., foreign language translation equivalents) to investigate judgments of learning (JOLs) under the assumption that the JOLs are made independently of each other. We tested this assumption by exploring the effect of manipulating the difficulty of prior Swahili-English translation equivalents to determine whether there is a contrasting change in the magnitude of the JOLs assigned to subsequent Swahili-English translation equivalents (e.g., intermediate-difficulty items might receive lower JOLs when following easy items than when following difficult items). The magnitude of JOLs for the subsequent items did not vary as a function of the difficulty of the prior items, even when there were 10 prior items of homogeneous difficulty that differed from the difficulty of the subsequent to-be-judged items.

Background: A computerized monitoring system was developed and utilized to evaluate the relationship between mean arterial pressure and intracranial pressure and its effect on cerebral perfusion pressure to determine how best to maximize cerebral perfusion pressure., Methods: The monitoring system recorded mean arterial pressure and intracranial pressure, the values of which were used to calculate cerebral perfusion pressure. We developed the program for and utilized a "moving window" regression analysis to divide the total monitoring period into 2-hour overlapping epochs. The relationship between mean arterial pressure and intracranial pressure was then analyzed in each 2-hour epoch and classified into Types A, B, C, and D epochs, each with clinical significance. Type A epochs involved intracranial pressure increasing more than mean arterial pressure and resulted in deterioration of cerebral perfusion pressure. Type B epochs involved mean arterial pressure increasing more than intracranial pressure and resulted in an increase of perfusion pressure. Type C epochs involved mean arterial pressure increasing and intracranial pressure decreasing and also resulted in an increase in perfusion pressure. Type D epochs had no statistically significant relationship between mean arterial pressure and intracranial pressure., Results: Forty-five patients with various pathologies were studied. The majority (n = 35) were patients with severe brain injury. Of 5,694 total hours monitored and 22,776 total epochs analyzed, 1.5% epochs were Type A, 5.9% were Type B, 2.1% were Type C, and 90.5% were Type D., Conclusion: In most patients, most of the time, increasing mean arterial pressure did not worsen cerebral perfusion pressure.

Background: Patients' perceptions of adverse effects caused by the medicines they are prescribed may influence how they use these medicines. Little is known about patients' perceptions of the adverse effects of specific drugs in everyday use and whether these differ from those identified by clinical trials and standard post-marketing surveillance methods., Aim: To compare reports of perceived adverse drug reactions (ADRs) obtained directly from patients taking tramadol to those found in clinical trials and two methods of post-marketing surveillance., Method: Postal questionnaire distributed to 1048 patients who had a prescription for tramadol dispensed over a 3-month period., Results: Most (84%) of the 344 respondents reported at least one symptom perceived as an ADR to tramadol. Dry mouth, light-headedness and constipation were most commonly reported. Almost half (48%) rated their most bothersome symptom as at least moderate and 43% claimed to have reported symptoms to their doctor. Perceived problems had led 38 respondents to stop taking tramadol. The 10 most frequently reported symptoms were all previously reported ADRs to tramadol. Although relatively minor, all 10 also appeared in reports to the UK Committee on the Safety of Medicines (CSM) and in prescription event monitoring. For many symptoms, the estimated range of frequency was in line with published reports, but considerably higher than that of post-marketing surveillance methods., Conclusions: Symptoms were reported by the majority of respondents and for many symptoms the frequency was high. Many patients did not report symptoms they perceived to be adverse effects to their doctor. The results indicate that patient perceptions of potential ADRs are relevant and should be an integral part of a pain management strategy.

Aims: To develop a systematic generic method of enabling patients to report symptoms which they believe to be due to a particular prescribed drug., Methods: A piloted body system-based questionnaire was distributed to patients registered with 79 medical practices in Grampian prescribed one of nine recently marketed 'black triangle' drugs. These comprised four antidepressants, three antiepileptics and two analgesics. This requested respondents to identify any symptoms experienced over the previous year which they thought could be due to the 'black triangle' drug they had used. A sample of medical records was examined to compare symptoms recorded with those reported by patients. A classification system was developed for the study to enable the assessment of symptoms reported for their potential relationship to patients' drug therapy. All symptoms reported were classified, taking into account information provided by patients on their concomitant drugs and diseases. A specialist pharmacist independently re-classified a sample of the symptoms to validate the process., Results: A 36.3% response rate was obtained (837/2307) with 742 respondents (88.6%) reporting at least one symptom. The median per patient was 6.0 (range 0--71), with almost half (406, 48.5%) reporting fewer than five symptoms. Most symptoms (71.0%) were classified as being probably or possibly related to the drugs studied. Agreement between researcher and specialist on the classification of 75.3% of 716 symptoms was obtained (Kappa=0.563). Responses from patients prescribed antidepressant drugs were more likely to include symptoms potentially caused by these drugs (74.5% of all symptoms reported) than those from patients prescribed analgesics (67.4%) or antiepileptics (65.1%, chi2 = 23.858, d.f. = 2, P < 0.001). Patients reporting large numbers of symptoms were more likely to report some which were classed as unlikely to be an ADR or unattributable (chi2 = 80.587, d.f. = 3, P < 0.001). Of the 742 reporting symptoms in questionnaires, 402 (54.2%) claimed to have reported some or all of these to their doctor. Only 162 (22.6%) of 716 patient-reported symptoms were documented in the primary care medical records of 103 patients prescribed tramadol or venlafaxine., Conclusions: Respondents were clearly willing to report symptoms, the majority of which were classed as possibly/probably related to the drugs studied. The results suggest that patients do not report all symptoms they suspect to be ADRs to their GP and that GPs do not record all symptoms which may be reported to them. The method could help to identify problems which patients perceive as being related to their drug therapy and contribute to increased ADR reporting.
