We used a two-phased mixed methods study with an explanatory sequential design to understand how frequently paramedics attend patients who, on paramedic assessment with the Gold Standards Framework Proactive Identification Guidance, are end-of-life and have advance care planning. We subsequently explored paramedic views on paramedic screening of patients to assess if they are end-of-life and onward referral to their General Practitioner for advance care planning. Paramedics screened and recorded 14.9% of patients as end-of-life and 44.3% of these patients were assessed to have no advance care plan in place. When paramedics screened patients and they did have an advance care plan in place, 36.8% had only a Do Not Attempt Cardiopulmonary Resuscitation. Paramedics found using the Gold Standards Framework Proactive Identification Guidance to screen patients for end-of-life status useful and straightforward and considered themselves well-placed to complete this task. Future research is required to address the practicalities of implementing a paramedic screening and referral tool for end-of-life care that results in the intended outcome of supporting effective advance care planning. Aims. We aim to reveal the sizes of the continuum and broad emission line (BEL) emitting regions in the gravitationally lensed quasar SDSS J1004+4112 by analyzing the unique signatures of microlensing in this system. Through a comprehensive analysis of 20 spectroscopic observations acquired between 2003 and 2018, we studied the striking deformations of various BEL profiles and determined the sizes of their respective emitting regions. Methods. Our approach involves a detailed analysis of the magnitude differences in the BEL wings and their adjacent continua, and the implementation of a statistical model to quantify the distribution and impact of microlensing magnifications. To ensure a reliable baseline for no microlensing, we used the emission line cores as a reference. We then applied a Bayesian estimate to derive the size lower limits of the Lyα, Si IV, C IV, C III], and Mg II emitting regions, as well as the sizes of the underlying continuum-emitting sources. Results. We analyzed the outstanding microlensing-induced distortions in the line profiles of various BELs in the quasar image A, characterized by a prominent magnification of the blue part and a strong demagnification of the red part. From the statistics of microlensing magnifications and using Bayesian methods, we estimate the lower limit to the overall size of the regions emitting the BELs to be a few light-days across, which is significantly smaller than in typically lensed quasars. The asymmetric deformations in the BELs indicate that the broad-line region is generally not spherically symmetric, and is likely confined to a plane and following the motions of the accretion disk. Additionally, the inferred continuum-emitting region sizes are larger than predictions based on standard thin-disk theory by a factor of ∼3.6 on average. Appropriate diagnosis, treatment and review of the efficacy of interventions can ameliorate these effects. Yet, the importance of AR is often overlooked, and appropriate therapy is neglected. The availability of effective medications and knowledge as to management are often lacking in both public and private health systems. Methods: This review is based on a comprehensive literature search and detailed discussions by the South African Allergic Rhinitis Working Group (SAARWG). Results: The working group provided up-to-date recommendations on the epidemiology, pathology, diagnosis and management of AR, appropriate to the South African setting. Conclusion: Allergic rhinitis causes significant, often unappreciated, morbidity. It is a complex disease related to an inflammatory response to environmental allergens. Therapy involves education, evaluation of allergen sensitisation, pharmacological treatment, allergen immunotherapy (AIT) and evaluation of the success of interventions. Regular use of saline; the important role of intranasal corticosteroids, including those combined with topical antihistamines and reduction in the use of systemic steroids are key. Practitioners should have a thorough knowledge of associated morbidities and the need for specialist referral. Contribution: This review summarises the latest developments in the diagnosis and management of AR such that it is a resource that allows easy access for family practitioners and specialists alike. Background. Despite a high burden of disease that requires critical care services, there are a limited number of intensivists in South Africa (SA). Medical practitioners at district and regional public sector hospitals frequently manage critically ill patients in the absence of intensivists, despite these medical practitioners having had minimal exposure to critical care during their undergraduate training. Objectives. To identify core competencies in critical care for medical practitioners who provide critical care services at public sector hospitals in SA where intensivists are not available to direct patient management. Methods. A preliminary list of core competencies in critical care was compiled. Thereafter, 13 national and international experts were requested to achieve consensus on a final list of core competencies that are required for critical care by medical practitioners, using a modified Delphi process. Results. A final list of 153 core competencies in critical care was identified. Conclusion. The core competencies identified by this study could assist in developing training programmes for medical practitioners to improve the quality of critical care services provided at district and regional hospitals in SA. - Abstract
Death is a medical occurrence that has social, legal, religious and cultural consequences requiring common clinical standards for its diagnosis and legal regulation. This document compiled by the Critical Care Society of Southern Africa outlines the core standards for determination of death in the hospital context. It aligns with the latest evidence-based research and international guidelines and is applicable to the South African context and legal system. The aim is to provide clear medical standards for healthcare providers to follow in the determination of death, thereby promoting safe practices and high-quality care through the use of uniform standards. Adherence to such guidelines will provide assurance to medical staff, patients, their families and the South African public that the determination of death is always undertaken with diligence, integrity, respect and compassion, and is in accordance with accepted medical standards and latest scientific evidence. The consensus guidelines were compiled using the AGREE II checklist with an 18-member expert panel participating in a three-round modified Delphi process. Checklists and advice sheets were created to assist with application of these guidelines in the clinical environment (https://criticalcare.org.za/resource/death-determination-checklists/). - Abstract
The CCSSA PBM Guidelines have been developed to improve patient blood management in critically ill patients in southern Africa. These consensus recommendations are based on a rigorous process by experts in the field of critical care who are also practicing in South Africa (SA). The process comprised a Delphi process, a round-table meeting (at the CCSSA National Congress, Durban, 2018), and a review of the best available evidence and international guidelines. The guidelines focus on the broader principles of patient blood management and incorporate transfusion medicine (transfusion guidelines), management of anaemia, optimisation of coagulopathy, and administrative and ethical considerations. There are a mix of low-middle and high-income healthcare structures within southern Africa. Blood products are, however, provided by the same notfor-profit non-governmental organisations to both private and public sectors. There are several challenges related to patient blood management in SA due most notably to a high incidence of anaemia, a frequent shortage of blood products, a small donor population, and a healthcare system under financial strain. The rational and equitable use of blood products is important to ensure best care for as many critically ill patients as possible. The summary of the recommendations provides key practice points for the day-to-day management of critically ill patients. 17. Risk factors and outcomes of sepsis-associated acute kidney injury in intensive care units in Johannesburg, South Africa

- Abstract
Background. Sepsis-associated acute kidney injury (SA-AKI) has been shown to be a significant contributor to morbidity and mortality in both children and adults with critical illness. In sub-Saharan Africa, there is a lack of information on factors associated with development of SA-AKI and outcomes after intensive care unit (ICU) admission.Objectives. To assess the rate of SA-AKI, factors associated with its development, and predictors of mortality at 90 days in critically ill patients admitted to the ICU with sepsis.Methods. This was a prospective observational study conducted at two of the biggest teaching hospitals in Johannesburg, South Africa, from 15 February 2016 to 15 February 2020. The study included consecutive patients with confirmed sepsis who were admitted to the ICU within 24 hours of admission to hospital. The primary outcome of the study was development of SA-AKI (defined according to Kidney Disease Improving Global Outcome (KDIGO) criteria), and secondary outcomes were risk factors for SA-AKI and predictors of mortality at 90 days. Multivariate logistic regression analysis was employed to determine the factors associated with SA-AKI and 90-day mortality.Results. In total, 327 critically ill patients with sepsis admitted to the ICUs were included in the study. The median (interquartile range) age was 39 (30 - 52) years, and 185 patients (56.6%) developed SA-AKI. Of these patients, blacks and whites comprised 91.0% and 6.1%, respectively, and the prevalent comorbidities were HIV/AIDS (19.3%), hypertension (14.2%) and diabetes mellitus (10.1%). Patients with SA-AKI were likely to be older and of male gender, and to have cardiovascular disease, malignancies, hypotension and a low serum albumin level. In multivariate analysis, the predictors of SA-AKI were age ≥55 years (odds ratio (OR) 2.43; 95% confidence interval (CI) 1.27 - 4.65), inotropic support (OR 3.61; 95% CI 2.18 - 5.96) and a low serum albumin level (OR 2.93; 95% CI 1.40 - 6.13). SA-AKI and need for inotropic support were respectively associated with 1.9-fold and 1.7-fold increased mortality at 90 days after ICU admission.Conclusion. SA-AKI was found to be frequent in this study in two tertiary hospital ICUs in Johannesburg, and the need for inotropic support predicted mortality after ICU admission.

18. David Sackett Fellowship 2022: CEBM Report to the McCall MacBain Foundation

- Abstract
We are delighted to report on the success of the inaugural David Sackett Fellowship, building on the success of the 2019 Doug Altman Scholarship, with generous funding and support from the McCall MacBain Foundation (MMF). We used the funding for nine forward-thinking future leaders in Evidence-Based Medicine (EBM) to attend the 2022 EBMLive Conference in Oxford. The nine Fellows brought provocative, bold, and constructive contributions on the theme of making research relevant, replicable, and accessible to end-users. From a highly competitive field, we selected the scholars because of their forward-thinking solutions to address the issue of making research evidence relevant, replicable, and accessible to end-users, as set out in the EBM manifesto, building on the legacy of David Sackett. After an unanticipated delay in hosting EBMLive in 2020 due to the COVID-19 pandemic, the return to an in-person meeting in Oxford, where David Sackett first founded the Centre for Evidence-Based Medicine, re-invigorated the EBM movement with their ideas, enthusiasm, and perspectives. This report highlights the contributions of the awardees, their outputs and the work of the CEBM that supports early-mid career researchers and builds capacity in EBM globally.

Training is often organized on a grassroots level, offered by early career researchers, for early career researchers. Buffet style courses that cover many topics can inspire participants to try new things; however, they can also be overwhelming. Participants who want to implement new practices may not know where to start once they return to their research team. We describe ten simple rules to guide participants of relevant training courses in implementing robust research practices in their own projects, once they return to their research group. This includes (1) prioritizing and planning which practices to implement, which involves obtaining support and convincing others involved in the research project of the added value of implementing new practices; (2) managing problems that arise during implementation; and (3) making reproducible research and open science practices an integral part of a future research career. We also outline strategies that course organizers can use to prepare participants for implementation and support them during this process.

- Abstract
Death is a medical occurrence that has social, legal, religious and cultural consequences requiring common clinical standards for its diagnosis and legal regulation. This document compiled by the Critical Care Society of Southern Africa outlines the core standards for determination of death in the hospital context. It aligns with the latest evidence-based research and international guidelines and is applicable to the South African context and legal system. The aim is to provide clear medical standards for healthcare providers to follow in the determination of death, thereby promoting safe practices and high-quality care through the use of uniform standards. Adherence to such guidelines will provide assurance to medical staff, patients, their families and the South African public that the determination of death is always undertaken with diligence, integrity, respect and compassion, and is in accordance with accepted medical standards and latest scientific evidence. The consensus guidelines were compiled using the AGREE II checklist with an 18-member expert panel participating in a three-round modified Delphi process. Background. There are limited data on the clinical characteristics and outcomes of patients with myasthenia gravis (MG) admitted to the intensive care unit (ICU) at Charlotte Maxeke Johannesburg Academic Hospital (CMJAH). Objectives. The aim was to study the clinical characteristics and outcomes of patients with MG admitted to the CMJAH over two decades. Methods. A retrospective study was undertaken of patients with MG admitted to the multidisciplinary ICU of CMJAH over a 20-year period, from 1998 to 2017. Demographic data, clinical features, management and outcomes of patients were assessed and reviewed from the case records. Results. Thirty-four patients with MG were admitted to the ICU during this period: 24 female and 10 male. The mean age ± SD was 37.4 ± 13.0 years, with a range of 16 - 66 years. Four patients were human immunodeficiency virus (HIV)-positive. The mean length of stay (LOS) in ICU was 10.6 ± 20.1 days, ranging from 1 to 115 days. Two patients were diagnosed with MG in the ICU after failure to wean from the ventilator. Overall, 22 patients were intubated and ventilated on admission. Morbidities included self-extubation, aspiration pneumonia and iatrogenic pneumothorax. History of thymectomy was present in 12 patients. The treatments received for MG included pyridostigmine (73.5%), corticosteroids (55.9%), azathioprine (35.3%), plasmapheresis (26.5%) and intravenous immunoglobulin (8.8%). The overall mortality in the ICU was 5.9%. Conclusion. MG is a serious disorder with considerable morbidity and mortality. It is, however, a potentially manageable disease, provided that appropriate ICU resources are available. [ABSTRACT FROM AUTHOR]

Risk factors for mortality were assessed by logistic regression analysis.ResultsThe cohort included 2621 patients. Setting of infection acquisition was community-acquired in 31.6%, early onset hospital-acquired in 25%, and late-onset hospital-acquired in 43.4% of patients. Overall prevalence of antimicrobial resistance was 26.3% and difficult-to-treat resistant Gram-negative bacteria 4.3%, with great variation according to geographic region. No difference in prevalence of antimicrobial resistance was observed according to setting of infection acquisition. Overall mortality was 29.1%. Independent risk factors for mortality included late-onset hospital-acquired infection, diffuse peritonitis, sepsis, septic shock, older age, malnutrition, liver failure, congestive heart failure, antimicrobial resistance (either methicillin-resistant Staphylococcus aureus, vancomycin-resistant enterococci, extended-spectrum beta-lactamase-producing Gram-negative bacteria, or carbapenem-resistant Gram-negative bacteria) and source control failure evidenced by either the need for surgical revision or persistent inflammation.ConclusionThis multinational, heterogeneous cohort of ICU patients with intra-abdominal infection revealed that setting of infection acquisition, anatomical disruption, and severity of disease expression are disease-specific phenotypic characteristics associated with outcome, irrespective of the type of infection. Antimicrobial resistance is equally common in community-acquired as in hospital-acquired infection. Any further distribution of this work must maintain attribution to the author(s) and the title of the work, journal citation and DOI., We provide an updated assessment of the power of the Cherenkov Telescope Array (CTA) to search for thermally produced dark matter at the TeV scale, via the associated gamma-ray signal from pair-annihilating dark matter particles in the region around the Galactic centre. We find that CTA will open a new window of discovery potential, significantly extending the range of robustly testable models given a standard cuspy profile of the dark matter density distribution. Importantly, even for a cored profile, the projected sensitivity of CTA will be sufficient to probe various well-motivated models of thermally produced dark matter at the TeV scale. This is due to CTA's unprecedented sensitivity, angular and energy resolutions, and the planned observational strategy. The survey of the inner Galaxy will cover a much larger region than corresponding previous observational campaigns with imaging atmospheric Cherenkov telescopes. CTA will map with unprecedented precision the large-scale diffuse emission in high-energy gamma rays, constituting a background for dark matter searches for which we adopt state-of-the-art models based on current data. We aim to uncover the structure of the continuum and broad emission line (BEL) emitting regions in the gravitationally lensed quasar SDSS J1004+4112 through unique microlensing signatures. Analyzing 20 spectroscopic observations from 2003 to 2018, we study the striking deformations of various BEL profiles and determine the sizes of their respective emitting regions. We use the emission line cores as a baseline for no microlensing and then apply Bayesian methods to derive the sizes of the Ly α , Si IV, C IV, C III], and Mg II emitting regions, as well as of the underlying continuum-emitting sources. We find that the sizes of the emitting regions for the BELs are a few light-days across, notably smaller than in typical lensed quasars. The asymmetric distortions observed in the BELs suggest that the broad-line region lacks spherical symmetry and is likely confined to a plane. The inferred continuum emitting region sizes are larger than predictions based on standard thin-disk theory by a factor of ∼ 4. We find that the size-wavelength relation is in agreement with that of a geometrically thin and optically thick accretion disk. [ABSTRACT FROM AUTHOR]

Background: Since the 1990s, the prescribing of opioids has increased in England and most high-income countries. However, insufficient datasets used in previous research may have underrepresented the extent of the problem. Aims: To assess global and national use of opioids, drivers of suboptimal use, and consider strategies to prevent opioid-related deaths. Methods: I conducted two cross-sectional analyses and two retrospective observational studies, using several datasets, including controlled opioid consumption statistics from the International Narcotic Control Board, the Global Essential Medicines Database of 137 national essential medicines lists, sales of over-the-counter products containing codeine, and the Preventable Deaths Database. I performed a synthesis of pharmacological resources and a systematic review of observational studies to explore factors associated with high-dose opioids. Results: 687 million people (10% of the global population) consumed 89% of the world’s controlled opioids in 2015-17. Countries included a median of 6 opioids (IQR: 5-9; range: 0-19) in national essential medicines lists; morphine (95%), fentanyl (83%), and codeine (69%) were the most common. 31.5 billion units of codeine, costing £2.55 billion, were sold over-the-counter across 31 countries in 2013-19, with 3% and 54% increases respectively. In the UK, 4.75 billion dosage units of codeine in combination with 1711 tonnes of paracetamol and 96 tonnes of ibuprofen, costing £638 million, were purchased over-the-counter in 2013-19. I identified 233 opioid drugs and created the Oxford Catalogue of Opioids (https://www.catalogueofopioids.net/). Five factors were associated with high-dose opioids, including co-prescription of benzodiazepines, depression, Emergency Department visits, unemployment, and being male. There were 176 opioid-related Prevention of Future Deaths reports in 2013-19, with a 262% increase. I created the Preventable Deaths Tracker (https://preventabledeathstracker.net/) and launched the Coroners’ Concerns to Prevent Harms series in BMJ Evidence-Based Medicine. Conclusions: There are persistent disparities and variations in the use of and access to prescribed and non-prescribed opioids. While opioids can be overused or underused, and can cause fatal and non-fatal harms, they still have a place in modern medicine in the right patient, at the right dosage regimen, and for the right reasons. I have outlined future research to serve patient safety, reduce avoidable harms, and improve evidence-based decision-making in clinical practice and policy.
