International audience; Various studies in western countries found Akt amplification to be a frequent event in human cancers, including bladder, but the correlation with clinicopathological features is controversial. Such studies have not been reported in African populations, including Tunisians. The purpose of this study was to assess expression of the phosphorylated/activated forms of Akt in tumors from Tunisian patients with bladder cancer and to correlate its expression with pathological and clinical parameters of the disease. The study included 72 patients of whom 34 were diagnosed as low- to medium-grade and 35 as high-grade; 30 were muscle stage and 39 non-muscle stage. Primary tumors from these patients, normal adjacent tissues, or bladder cancer cell-lines were analyzed for Ser473 phosphorylated Akt expression by Western blot. Seventy-two percent of primary tumors from patients with bladder cancer had increased levels of p-Akt. The p-Akt levels in patients with high-grade bladder cancer were significantly elevated compared to patients with low- or medium-grade bladder cancer. In invasive carcinoma, the p-Akt level was significantly higher than in superficial non-invasive bladder tumors. Concerning the influence of tobacco on Akt activation, no significant differences of p-Akt expression were found between non-smoker and smoker patients. Altogether, our results suggest that Akt activation can provide useful prognostic information and that tobacco represents a serious risk factor for recurrence in a cohort of Tunisian patients.

Keratoacanthoma centrifugum marginatum (KCM) is a rare variant of keratoacanthoma characterized by a progressive peripheral growth with concomitant central healing. We report here a case of multiple KCM of the lower legs in a 48-year-old man. The lesions had progressively evolved over 3 years. They were multiple asymptomatic and confluent annular plaques of 5 to 20 cm, having papulo-nodular with hyperkeratotic and crusted borders and cicatricial center. Within the centers were numerous firm and pigmented minipapules of 1 to 2 mm. The typical clinical aspect, together with characteristic histological features confirmed the diagnosis of KCM. Herein we will highlight the clinical and histological features of KCM, as well as the different effective treatments. We will also briefly discuss KCM among the other types of keratoacanthomas.

Adenomas of the stomach are histologically classified into tubular, villous and mixed (tubulovillous) adenoma. In the literature, in two previous studies, only six cases of serrated adenoma of the stomach have been reported. In this communication, we report a newly identified case. The patient presented with gastric complaints. The endoscopic examination showed a pediculated protruding tumour localised in the gastric corpus. Biopsies were reported as tubular adenoma with high-grade dysplasia. A surgical local excision of the tumour was performed. The histological examination revealed elongated fronds with lateral crenated, saw-tooth-like notches as a result of scalloped epithelial indentations. Areas with high grade dysplasia but without invasive carcinoma were demonstrated. This seems to be the first case of serrated adenoma localised in the gastric corpus in the literature. Introduction Nevus lipomatosus cutaneous superficialis (NLCS) is a rare benign hamartomatous skin tumor characterized by dermal deposition of mature adipose tissue. It’s classified in two types: the classical form with multiple soft, pedunculated, cerebriform papules and nodules that coalesce into plaques, and the solitary form that consists of a solitary papule or nodule. In this study, eight cases of NLCS are reported. Methods The study was a retrospective case series including all patients with histopathologically documented NLCS who attended the Dermatology Department of Charles Nicolle hospital between January 1997 and December 2010. The objective of our study was to determine the epidemioclinical characteristics, the histopathologic features, and the treatment of this hamartoma. Patients included three males and five females aged between 7 and 41 years. Results In four cases hamartoma was present since childhood, and in the other four cases it appeared in the third and fourth decades. Classical form was noted in seven cases and the solitary form in one case. Lesions involved limbs in four patients and trunk in four patients. Seven patients underwent surgical excision, and for one case no treatment was proposed. Conclusion The multiple or classical form is largely predominant in our study. Habitually, NLCS has an asymptomatic course. Treatment is usually not necessary unless for cosmetic reasons; surgical excision is curative and recurrence after is rare.

Background Erosive lichen planus (LP) of the soles is a rare variant of LP, characterized by chronic, painful, and disabling plantar ulcerations. Herein, we report a case with complete healing following treatment with systemic steroids. Case report A 38-year-old woman was referred with painful and disabling erosive bilateral plantar LP, which she had experienced for 6 weeks. A 1 mg/kg/day, oral prednisone therapy led to rapid improvement and complete healing within 3 weeks, with a sustained result under a low dose maintenance therapy. Discussion Ulcerative plantar LP is significantly known to be unresponsive to several topical and systemic therapies. Surgical excision and grafting is the treatment of choice. Systemic steroids are reported to have inefficient or partial results; both on cutaneous healing and on maintenance of the result. Our patient achieved complete cicatrisation with a sustained result of 3 months under a low dose of prednisone (5 mg/day).

Background The bullous variant of Darier’s disease (DD) is a rare type characterized by histological and clinical overlap with Hailey-Hailey disease (HDD). Case reports The following case report describes two cases of familial DD; a 51-year-old woman who presented with erythematous plaques, covered by small blisters in axillary and inguinal areas, and the first patient’s daughter, who presented with keratotic papules localized on the axillary and inframammary folds. Conclusion These two cases are original by the predominant flexural distribution, and by a bullous form in the first case, clinically and histologically mimicking HHD.
