1. Robot-assisted partial nephrectomy using the novel Hugo™ RAS system: Feasibility, setting and perioperative outcomes of the first off-clamp series.

Abstract

Introduction: Hugo Robot-Assisted Surgery (RAS) System has been conceived with enhanced modularity but its role for nephron-sparing surgery setting still remains poorly explored. We aimed to describe our experience in robot-assisted partial nephrectomy (RAPN) with a three-arms setting for the first off-clamp series using the new Hugo RAS System. Methods: Patients were placed on an extended flank position at the margin of the surgical bed with a slightly flexion (45°). The first 11 mm robotic trocar (camera port) was placed along the pararectal line 14 ± 2 cm far from the umbilicus. The pneumoperitoneum was then induced through the AirSeal system (SurgiQuest, Milford, Connecticut, USA©). Two more 8 mm operative robotic ports were placed under direct vision, either 8 ± 1 cm far from optic's port. Two 12 mm laparoscopic ports for bed-assistant were placed between robotic ports. Monopolar curved shears, fenestrated grasper, and large needle driver were used in a three-instruments configuration. Results: Off-clamp RAPN was successfully performed in seven patients with cT1 renal masses using a trans-peritoneal route. Median port placement and docking time was 6 min (IQR, 4–8 min). Hemostasis was achieved through renorraphy using a single transfix stitch with sliding clips technique. There was no need for additional ports placement. No intraoperative complications occurred, no clashing of robotic instruments or between the robotic arms was observed. No technical failures of the system occurred. Median console time was 83 min (IQR, 68–115 min). Median estimated blood loss were 200 ml (IQR, 50–400 ml). All patients were discharged between post-operative day 2 and 3, without the need of hospital readmission. No complications were recorded within the first 30 post-operative days. Conclusions: We performed the first series of off-clamp RAPN using the novel HUGO RAS System. This novel robotic platform showed an easy-friendly docking system, providing excellent perioperative outcomes with a simple three-arms configuration. [ABSTRACT FROM AUTHOR]

Abstract

Background In human immunodeficiency virus (HIV)–positive adults, low CD4 cell counts despite fully suppressed HIV-1 RNA on antiretroviral therapy (ART) have been associated with increased risk of morbidity and mortality. We assessed the prevalence and outcomes of poor immune response (PIR) in children receiving suppressive ART. Methods Sixteen cohorts from the European Pregnancy and Paediatric HIV Cohort Collaboration (EPPICC) contributed data. Children Results Of 2318 children included, median age was 6.4 years and 68% had advanced/severe immunosuppression at ART initiation. At 1 year of VS, 12% had PIR. In multivariable analysis, PIR was associated with older age and worse immunological stage at ART start, hepatitis B coinfection, and residing in Thailand (all P ≤ .03). Rates of AIDS/death (95% confidence interval) per 100 000 person-years were 1052 (547, 2022) among PIR versus 261 (166, 409) among immune responders; rate ratio of 4.04 (1.83, 8.92; P < .001). Conclusions One in eight children in our cohort experienced PIR despite sustained VS. While the overall rate of AIDS/death was low, children with PIR had a 4-fold increase in risk of event as compared with immune responders.

Abstract

Introduction : Limited data describe outcomes on second-line antiretroviral therapy (ART) among children globally. Our objective was to contribute data on outcomes among children living with HIV after initiation of second-line ART in the context of routine care within a large global cohort collaboration. Methods : Patient-level data from 1993 through 2015 from 11 paediatric HIV cohorts were pooled. Characteristics at switch and through two years of follow-up were summarized for children who switched to second-line ART after starting a standard first-line regimen in North America, Latin America, Europe, Asia, Southern Africa (South Africa & Botswana) and the rest of sub-Saharan Africa (SSA). Cumulative incidences of mortality and loss to follow-up (LTFU) were estimated using a competing risks framework. Results : Of the 85,389 children on first-line ART, 3,555 (4%) switched to second-line after a median of 2.8 years on ART (IQR: 1.6, 4.7); 69% were from Southern Africa or SSA and 86% of second-line regimens were protease inhibitor-based. At switch, median age was 8.4 years and 50% had a prior AIDS diagnosis. Median follow-up after switch to second-line ranged from 1.8 years in SSA to 5.3 years in North America. Median CD4 counts at switch to second-line ranged from 235 cells/mm(3) in SSA to 828 cells/mm(3) in North America. Improvements in CD4 counts were observed over two years of follow-up, particularly in regions with lower CD4 counts at second-line switch. Abstract

Objectives The aim of the study was to explore factors associated with CD4 percentage (CD4%) reconstitution following treatment interruptions (TIs) of antiretroviral therapy (ART). Methods Data from paediatric HIV-infected cohorts across 17 countries in Europe and Thailand were pooled. Children on combination ART (cART; at least three drugs from at least two classes) for > 6 months before TI of >= 30 days while aged CD4% at restart of ART (r-ART) and in the long term (up to 24 months after r-ART) following the first TI was modelled using asymptotic regression. Results In 779 children with at least one TI, the median age at first TI was 10.1 [interquartile range (IQR) 6.4, 13.6] years and the mean CD4% was 27.3% [standard deviation (SD) 11.0%]; the median TI duration was 9.0 (IQR 3.5, 22.5) months. In regression analysis, the mean CD4% was 19.2% [95% confidence interval (CI) 18.3, 20.1%] at r-ART, and 27.1% (26.2, 27.9%) in the long term, with half this increase in the first 6 months. r-ART and long-term CD4% values were highest in female patients and in children aged TI. Long-term CD4% was highest in those with a TI lasting 1 to = 25% (all P < 0.001). The effect of CD4% nadir during the TI differed significantly (P = 0.038) by viral suppression at the start of the TI; in children with CD4% nadir TI, recovery was better in those virally suppressed prior to the TI; viral suppression was not associated with recovery in children with CD4% nadir >= 25%. Conclusions After restart of ART following TI, most children reconstituted well immunologically. Nevertheless, several factors predicted better immunological reconstitution, including younger age and higher nadir CD4% during TI.

Abstract

Background : Estimates of incidence of switching to second-line antiretroviral therapy (ART) among children with HIV are necessary to inform the need for paediatric second-line formulations. We aimed to quantify the cumulative incidence of switching to second-line ART among children in an international cohort collaboration. Methods : In this international cohort collaboration study, we pooled individual patient-level data for children younger than 18 years who initiated ART (two or more nucleoside reverse-transcriptase inhibitors [NRTI] plus a non-NRTI [NNRTI] or boosted protease inhibitor) between 1993 and 2015 from 12 observational cohort networks in the Collaborative Initiative for Paediatric HIV Education and Research (CIPHER) Global Cohort Collaboration. Patients who were reported to be horizontally infected with HIV and those who were enrolled in trials of treatment monitoring, switching, or interruption strategies were excluded. Switch to second-line ART was defined as change of one or more NRTI plus either change in drug class (NNRTI to protease inhibitor or vice versa) or protease inhibitor change, change from single to dual protease inhibitor, or addition of a new drug class. We used cumulative incidence curves to assess time to switching, and multivariable proportional hazards models to explore patient-level and cohort-level factors associated with switching, with death and loss to follow-up as competing risks. Findings : At the data cutoff of Sept 16, 2015, 182 747 children with HIV were included in the CIPHER dataset, of whom 93 351 were eligible, with 83 984 (90.0%) from sub-Saharan Africa. At ART initiation, the median patient age was 3.9 years (IQR 1.6-6.9) and 82 885 (88.8%) patients initiated NNRTI-based and 10 466 (11.2%) initiated protease inhibitor-based regimens. Median duration of follow-up after ART initiation was 26 months (IQR 9-52). 3883 (4.2%) patients switched to second-line ART after a median of 35 months (IQR 20-57) of ART. Abstract

Objectives The aim of the study was to explore factors associated with CD4 percentage (CD4%) reconstitution following treatment interruptions (TIs) of antiretroviral therapy (ART). Methods Data from paediatric HIV-infected cohorts across 17 countries in Europe and Thailand were pooled. Children on combination ART (cART; at least three drugs from at least two classes) for > 6 months before TI of >= 30 days while aged CD4% at restart of ART (r-ART) and in the long term (up to 24 months after r-ART) following the first TI was modelled using asymptotic regression. Results In 779 children with at least one TI, the median age at first TI was 10.1 [interquartile range (IQR) 6.4, 13.6] years and the mean CD4% was 27.3% [standard deviation (SD) 11.0%]; the median TI duration was 9.0 (IQR 3.5, 22.5) months. In regression analysis, the mean CD4% was 19.2% [95% confidence interval (CI) 18.3, 20.1%] at r-ART, and 27.1% (26.2, 27.9%) in the long term, with half this increase in the first 6 months. r-ART and long-term CD4% values were highest in female patients and in children aged TI. Long-term CD4% was highest in those with a TI lasting 1 to = 25% (all P < 0.001). The effect of CD4% nadir during the TI differed significantly (P = 0.038) by viral suppression at the start of the TI; in children with CD4% nadir TI, recovery was better in those virally suppressed prior to the TI; viral suppression was not associated with recovery in children with CD4% nadir >= 25%. Conclusions After restart of ART following TI, most children reconstituted well immunologically. Nevertheless, several factors predicted better immunological reconstitution, including younger age and higher nadir CD4% during TI.

Abstract

Objective: To identify predictors of faster time to virological suppression among infants starting combination antiretroviral therapy (cART) early in infancy. Design: Cohort study of infants from Europe and Thailand included in studies participating in the European Pregnancy and Paediatric HIV Cohort Collaboration. Methods: Infants with perinatal HIV starting cART aged less than 6 months with at least 1 viral load measurement within 15 months of cART initiation were included. Multi-variable interval-censored flexible parametric proportional hazards models were used to assess predictors of faster virological suppression, with timing of suppression assumed to lie in the interval between last viral load at least 400 and first viral load less than 400 copies/ml. Results: Of 420 infants, 59% were female and 56% from Central/Western Europe, 26% United Kingdom/Ireland, 15% Eastern Europe and 3% Thailand; 46 and 54% started a boosted protease inhibitor-based or nonnucleoside reverse transcriptase inhibitor-based regimen, respectively. At cART initiation, the median age, CD4(+) % and viral load were 2.9 [interquartile range (IQR): 1.4-4.1] months, 34 (IQR: 24-45)% and 5.5 (IQR: 4.5-6.0) log(10) copies/ml, respectively. Overall, an estimated 89% (95% confidence interval: 86-92%) achieved virological suppression within 12 months of cART start. In multivariable analysis, younger age [adjusted hazard ratio (aHR): 0.84 per month older; P < 0.001], higher CD4(+) % (aHR: 1.11 per 10% higher; P=0.010) and lower log(10) viral load (aHR: 0.85 per log(10) higher; P < 0.001) at cART initiation independently predicted faster virological suppression. Conclusion: We observed a significant independent effect of age at cART initiation, even within a narrow 6 months window from birth. These findings support the earliest feasible cART initiation in infants and suggest that early therapy influences key virological and immunological parameters that could have important consequences for long-term health.

Abstract

“The Scream” is a well-known painting by Edvard Munch (1863–1944). The Norwegian word used by Munch was “Skrik,” which can be translated as “shriek” or “scream”. “The Scream” may be of interest to meteorologists because of the quite striking representation of the sky. It has been suggested that the dramatic red-colored sky was inspired by a volcanic sunset seen by Munch, after the Krakatau eruption in 1883, that it was inspired by a sighting of stratospheric nacreous clouds and also that it is part of the artist’s expression of a scream from nature. The evidence for the volcanic sunset theory and Munch’s psyche are briefly reviewed. We provide support that Munch’s inspiration may have been from a sighting of nacreous clouds, observable from southern Norway during the winter months. We show that the colors and patterns of the sky in Munch’s painting match the sunset colors better if nacreous clouds are present. Their sudden appearance around and after sunset creates an impressive and dramatic effect. By comparing the color content of photographs and paintings of regular sunsets, volcanic sunsets, and nacreous clouds after sunset, with the color content of the sky in “The Scream”, the match is better with nacreous clouds present. If this conjecture is correct then Munch’s sky in “The Scream” represents one of the earliest visual documentations of a nacreous cloud display.

Abstract

BACKGROUND: Published estimates of mortality and progression to AIDS as children with HIV approach adulthood are limited. We describe rates and risk factors for death and AIDS-defining events in children and adolescents after initiation of combination antiretroviral therapy (cART) in 17 middle- and high-income countries, including some in Western and Central Europe (W&CE), Eastern Europe (Russia and Ukraine), and Thailand. METHODS AND FINDINGS: Children with perinatal HIV aged 6 months of cART) death and progression to AIDS were assessed. Of 3,526 children included, 32% were from the United Kingdom or Ireland, 30% from elsewhere in W&CE, 18% from Russia or Ukraine, and 20% from Thailand. At cART initiation, median age was 5.2 (IQR 1.4-9.3) years; 35% of children aged 400 c/mL predicted late death. Predictors of early and late progression to AIDS were similar. Study limitations include incomplete recording of US Centers for Disease Control (CDC) disease stage B events and serious adverse events in some countries; events that were distributed over a long time period, and that we lacked power to analyse trends in patterns and causes of death over time. CONCLUSIONS: In our study, 3,526 children and adolescents with perinatal HIV infection initiated antiretroviral therapy (ART) in countries in Europe and Thailand. We observed that over 40% of deaths occurred =6 months after cART initiation. Greater early mortality risk in infants, as compared to older children, and in Russia, Ukraine, or Thailand as compared to W&CE, raises concern. Current severe immune suppression, being underweight, and unsuppressed viral load were associated with a higher risk of death at >6 months after initiation of cART.

A sample of 31 HIV-infected children and adolescents was assessed based on anthropometry (weight, height, body mass index (BMI), triceps skinfolds and upper arm circumference), body composition obtained by electrical bioimpedance and expressed as fat mass index (FMI) and lean body mass (LBM); dietary intake (recall 24 hours) and HRQoL (Kidscreen 27). Statistical analysis IBMu00ae SPSSu00ae v.25. Results: Z-scores of BMI: 25% overweight and 7,1% obese. Dietary intake of vitamin A, D, E, folate, iron, calcium, potassium, magnesium, fiber, zinc, sodium, sugar, protein, energy sodium and sugar do not correspond to DRIu2019s. Differences by gender were found in physical well-being (p=0,003) and psychological well-being (p=0,046), with males being associated with better well-being. Higher BMI z-score values were associated with lower values of resistance, reactance and fiber intake.Conclusions: Childrenu2019s and adolescents are now living with HIV as a chronic disease, which is related with an increased risk of cardiovascular disease (CVD) that has been shown to be associated with unhealthy dietary patterns. Nutritional intervention is necessary to optimize health status, including the dietary intake of macronutrients and micronutrients, BMI z-score and ensure a good quality of life.

Abstract

Methods for the determination of ship fuel sulphur content and NOx emission factors from remote measurements have been compared in the harbour of Rotterdam and compared to direct stack emission measurements on the ferry Stena Hollandica. The methods 5 were selected based on a review of the available literature on ship emission measurements. They were either optical (LIDAR, DOAS, UV camera), combined with model based estimates of fuel consumption, or based on the so called "sniffer" principle, where SO2 or NOx emission factors are determined from simultaneous measurement of the increase of CO2 and SO2 or NOx concentrations in the plume of the ship compared to the background. The measurements were performed from stations at land, from a boat, and from a helicopter. Mobile measurement platforms were found to have important advantages compared to the landbased ones because they allow to optimize the sampling conditions and to sample from ships on the open sea. Although optical methods can provide reliable results, it was found that at the state of the art, the "sniffer" approach is the most convenient technique for determining both SO2 and NOx emission factors remotely. The average random error on the determination of SO2 emission factors comparing two identical instrumental set-ups was 6 %. However, it was found that apparently minor differences in the instrumental characteristics, such as response time, could cause significant differences between the emission factors determined. Direct stack measurements showed that about 14% of the fuel sulphur content was not emitted as SO2. We compare these with traditional MODIS AOD measurements. The detection of the last three types is unprecedented in the infrared in nadir mode, but is very promising, especially for sulfuric acid droplets which are detected in the lower troposphere and up to 6 months after injection in the upper troposphere/lower stratosphere. Abstract

Ash, tephra, and gas ejected during explosive eruptions pose a major far-reaching threat to population, health, and air traffic. Lava flows, lahars, and floods from ice-capped volcanos, as well as landslides that have a potential for tsunami generation if they reach into sea or lakes, can also have a major influence. Remote sensing contributes to the mitigation of these hazards through the use of synthetic aperture radar interferometry (InSAR) and spectroradiometry. In the case of InSAR, displacements of a volcano's surface can be interpreted in terms of magma movement beneath the ground. Thus, the technique can be used to identify precursors to eruptions and to track the evolution of eruptions. Recent advances in algorithm development enable relative displacements over many kilometers to be measured with an accuracy of only a few millimeters. Spectroradiometry on the other hand allows monitoring of a volcanic eruption through the detection of hot spots, and monitoring and quantification of the ash and SO2 emitted by volcanos into the atmosphere. Abstract

Lidar and sun-photometer measurements were performed intensively over the Iberian Peninsula (IP) during the eruption of the Eyjafjallajökull volcano (Iceland) in April-May 2010. The volcanic plume reached all the IP stations for the first time on 5 May 2010. A thorough study of the event was conducted for the period 5-8 May. Firstly, the spatial and temporal evolution of the plume was described by means of lidar and sun-photometer measurements supported with backtrajectories. The volcanic aerosol layers observed over the IP were rather thin (<1000 m) with a top height up to 11-12 km. However, in some cases at the beginning of the period the thickness of those layers reached several kilometers in Évora and Madrid. The optical thicknesses associated to those layers were rather low (between 0.013 and 0.020 in average over the whole period), with peak values near 0.10 detected on 7 May. Secondly, the volcanic aerosols were characterized in terms of extinction and backscatter coefficients, lidar ratios, Ångström exponents and linear particle depolarization ratio. Lidar ratios at different sites varied between 30 and 50 sr without a marked spectral dependency. Similar extinction-related Ångström exponents varying between 0.6 and 0.8 were observed at different sites. The temporal evolution of the backscatter-related Ångström exponents points out a possible decrease of the volcanic particle size as the plume moved from west to east. Particle depolarization ratios on the order of 0.06-0.08 confirmed the coexistence of both ash and non-ash particles. Additionally, profiles of mass concentration were obtained with a method using the opposite depolarizing effects of ash particles (strongly depolarizing), non-ash particles (very weakly depolarizing), and sun-photometer observations. In Granada the ash mass concentration was found to be approximately 1.5 times higher than that of non-ash particles, and probably did not exceed the value of 200 µgm-3 during the whole event. Abstract

During the eruption of Eyjafjallajökull in April-May 2010 multi-wavelength Raman lidar measurements were performed at the CNR-IMAA Atmospheric Observatory (CIAO), whenever weather conditions permitted observations. A methodology both for volcanic layer identification and accurate aerosol typing has been developed. This methodology relies on the multi-wavelength Raman lidar measurements and the support of long-term lidar measurements performed at CIAO since 2000. The aerosol mask for lidar measurements performed at CIAO during the 2010 Eyjafjallajökull eruption has been obtained. Volcanic aerosol layers were observed in different periods: 19-22 April, 27-29 April, 8-9 May, 13-14 May and 18-19 May. A maximum aerosol optical depth of about 0.12-0.13 was observed on 20 April, 22:00 UTC and 13 May, 20:30 UTC. Volcanic particles were detected at low altitudes, in the free troposphere and in the upper troposphere. Occurrences of volcanic particles within the PBL were detected on 21-22 April and 13 May. A Saharan dust event was observed on 13-14 May: dust and volcanic particles were simultaneously detected at CIAO at separated different altitudes as well as mixed within the same layer. Lidar ratios at 355 and 532 nm, the Angström exponent at 355/532 nm, the backscatter-related Angström exponent at 532/1064 nm and the particle linear depolarization ratio at 532 nm measured inside the detected volcanic layers are discussed. The dependence of these quantities on relative humidity has been investigated by using co-located microwave profiler measurements. The measured values of these intensive parameters indicate the presence of volcanic sulfates/ continental mixed aerosol in the volcanic aerosol layers observed at CIAO. In correspondence of the maxima observed in the volcanic aerosol load on 19-20 April and 13 May, different values of intensive parameters were observed. Apart from the occurrence of sulfate aerosol, these values indicate also the presence of some ash which is affected by the aging during transport over Europe. [ABSTRACT FROM AUTHOR]
