3. Canadian Orthopaedic Residents Perception of Their Needs in Elbow Surgery Teaching

Abstract

Introduction: The aim of this paper is to guide training program and review course curriculum planning in elbow disorders. To this end, a nationwide email survey was administered to residents' in orthopaedic surgery training programs. Material and Methods: The survey had 12 items that examined learning needs in several domains: assessment of acute and chronic elbow disorders, treatment of elbow disorders and the perceived effectiveness of various practical skills simulation sessions. A rank order list of learning needs was created. Results: Eighty-eight of 351 residents completed the survey (25%). Ninety percent of respondents thought that a one-day course would be helpful. The majority of residents felt comfortable evaluating acute traumatic elbow disorders. Their level of comfort was lower in treatment of elective disorders, with only 4% of residents comfortable managing posterior interosseous nerve and 5% comfortable managing chronic elbow instability. Only 24% of residents were comfortable treating terrible triad injuries. Conclusions: Residents reported a need for additional education in elbow surgery; especially for elective disorders. Educational needs were clustered in several areas including surgical approaches, ligament repair, and surgical management of fracture dislocations.

10. Evaluating the potential of continuous processes for monoclonal antibodies : economic, environmental and operational feasibility

Abstract

The next generation of monoclonal antibody (mAb) therapies are under increasing pressure from healthcare providers to offer cost effective treatments in the face of intensified competition from rival manufacturers and the looming loss of patent exclusivity for a number of blockbusters. To remain completive in such a challenging environment companies are looking to reduce R&D and manufacturing costs by improving their manufacturing platform processes whilst maintaining flexibility and product quality. As a result companies are now exploring whether they should choose conventional batch technologies or invest in novel continuous technologies, which may lead to lower production costs. This thesis explores the creation of a dynamic tool as part of a decision-support framework that is capable of simulating and optimising continuous monoclonal antibody manufacturing strategies to assist decision-making in this challenging environment. The decision-support framework is able to tackle the complex problem domain found in biopharmaceutical manufacturing, through holistic technology evaluations employing deterministic discrete-event simulation, Monte Carlo simulation and multi-attribute decision-making techniques. The hierarchal nature of the framework (including a unique sixth hierarchal layer; sub-batches) made it possible to simulate multiple continuous manufacturing scenarios on a number of levels of detail, ranging from high-level process performance metrics to low-level ancillary task estimates. The framework is therefore capable of capturing the impact of future titres, multiple scales of operation and key decisional drivers on manufacturing strategies linking multiple continuous unit operations (perfusion cell culture & semi-continuous chromatography). The work in this thesis demonstrates that the framework is a powerful test bed for assessing the potential of novel continuous technologies and manufacturing strategies, via integrated techno-economic evaluations that take proof-of-concept experimental evaluations to complete life-cycle performance evaluations.

A 25% risk of acute esophagitis is experienced by patients as a result of the volume of esophagus encompassed within a conformal radiotherapy technique. We reviewed our institutional experience administering the radioprotectant amifostine prior to daily radiotherapy to determine its effects on the onset of esophagitis. Materials and methods: From 2005 to 2016, 49 patients diagnosed with stage 3 small cell lung carcinoma received concurrent chemoradiotherapy. Chemotherapy (CT) consisted of cisplatin and etoposide with radiotherapy (RT) encompassing CT-identified gross tumor volume. In 32 patients (group 1), amifostine was delivered (500 mg subcutaneously divided in two injections) prior to the second daily RT fraction. The remaining 17 patients (group 2) did not receive amifostine due to choice or drug intolerance. Results: Metrics of esophagitis included weight loss and opiate requirement during treatment. About 31% of group 1 required opiates at a median RT dose of 3300 cGy, and 41% of group 2 required opiates at a median dose of 2250 cGy. The dose of radiotherapy delivered to 50% of the esophageal volume for group 1 was significantly greater than that in group 2 (3000 cGy vs 576 cGy). Conclusion: In this modern retrospective series of thoracic chemoradiotherapy in the treatment of stage 3 small cell lung cancer, amifostine that was delivered subcutaneously postponed the onset of esophagitis. Abstract

X-ray imaging is a fast, precise and non-invasive method of imaging which, combined with computed tomography, provides detailed 3D rendering of samples. Incorporating propagation-based phase contrast can vastly improve data quality for weakly attenuating samples via material-specific phase retrieval filters, allowing radiation exposure to be reduced. However, applying phase retrieval to multi-material phantoms complicates analysis by requiring a choice of which material boundary to tune the phase retrieval. Filtering for the boundary with strongest phase contrast increases noise suppression, but with the detriment of over-blurring other interfaces, potentially obscuring small or neighbouring features and removing quantitative sample information. Additionally, regions bounded by more than one material type inherently cannot be conventionally filtered to reconstruct the whole boundary. As remedy, we present a computationally-efficient, non-iterative nor AI-mediated method for applying strong phase retrieval, whilst preserving sharp boundaries for all materials within the sample. This technique was tested on phase contrast images of a rabbit kitten brain encased by the surrounding dense skull. Using 24 keV synchrotron radiation with a 5 m propagation distance, our technique provided a 6.9-fold improvement in the signal-to-noise ratio (SNR) of brain tissue compared to the standard phase retrieval procedure, without over-smoothing the images. Simultaneous quantification of edge resolution and SNR gain was performed with an aluminium-water phantom imaged using a microfocus X-ray tube at mean energy 19.58 keV and 0.576 m effective propagation distance. Our method provided a 4.2-fold SNR boost whilst preserving the boundary resolution at 54 $\pm$ 1 $\mu$m, compared to 108 $\pm$ 2 $\mu$m in conventional phase retrieval.

Abstract

Background: Despite recent advances in arthroscopic rotator cuff repair, the retear rate remains high. New methods to optimize healing rates must be sought. Bone channeling may create a quicker and more vigorous healing response by attracting autologous mesenchymal stem cells, cytokines, and growth factors to the repair site. Hypothesis: Arthroscopic rotator cuff repair with bone channeling would result in a higher healing rate compared with arthroscopic rotator cuff repair without adjuvant channeling. Study Design: Randomized controlled trial; Level of evidence, 1. Methods: Our primary objective was to compare healing rates in patients undergoing arthroscopic rotator cuff repair for degenerative tears, with and without bone channeling. Secondary objectives included comparisons of the Western Ontario Rotator Cuff Index (WORC) score, American Shoulder and Elbow Surgeons (ASES) score, Constant score, Constant strength subscore, and visual analog scale (VAS) for pain score between groups. Patients undergoing arthroscopic rotator cuff repair were recruited at 3 sites and were randomized to receive either bone channeling augmentation or standard repair. Healing was determined via ultrasound at 24 months postoperatively. WORC, ASES, and Constant scores were compared between groups at baseline and at 3, 6, 12, and 24 months postoperatively. Results: A total of 168 patients were enrolled between 2013 and 2018. Intention-to-treat analysis revealed no statistical differences in healing rates between the 2 interventions at 24 months postoperatively. Statistically significant improvements occurred in both groups from preoperatively to all time points for the WORC, the ASES score, the Constant score or Constant strength subscore, and the VAS for pain (P <.0001). No differences were observed between the bone channeling and control groups in WORC, ASES, Constant, and VAS pain scores at any time point. Conclusion: This trial did not demonstrate the superiority of intraoperative bone channeling in rotator cuff repair over standard rotator cuff repair at 24 months postoperatively.
