5. Ten Years of Quality Monitoring of Abdominal Organ Procurement in the Netherlands and Its Impact on Transplant Outcome.

Abstract

In this study, 10 years of procurement quality monitoring data were analyzed to identify potential risk factors associated with procurement-related injury and their association with long-term graft survival. All deceased kidney, liver, and pancreas donors from 2012 to 2022 and their corresponding recipients in the Netherlands were retrospectively included. The incidence of procurement-related injuries and potential risk factors were analyzed. Of all abdominal organs procured, 23% exhibited procurement-related injuries, with a discard rate of 4.0%. In kidneys and livers, 23% of the grafts had procurement-related injury, with 2.5% and 4% of organs with procurement-related injury being discarded, respectively. In pancreas procurement, this was 27%, with a discard rate of 24%. Male donor gender and donor BMI >25 were significant risk factors for procurement-related injury in all three abdominal organs, whereas aberrant vascularization was significant only for the kidney and liver. In the multivariable Cox regression analyses, procurement-related injury was not a significant predictor for graft failure (kidney; HR 0.99, 95% CI 0.75-1.33, p = 0.99, liver; HR 0.92, 95% CI 0.66-1.28, p = 0.61, pancreas: HR 1.16; 95% CI 0.16-8.68, p = 0.88). The findings of this study suggest that transplant surgeons exhibited good decision-making skills in determining the acceptability and repairability of procurement-related injuries. Abstract

Objective: We evaluate nationwide perioperative outcomes of complex EVAR and assess the volume-outcome association of complex EVAR. Summary of Background Data: Endovascular treatment with fenestrated (FEVAR) or branched (BEVAR) endografts is progressively used for excluding complex aortic aneurysms (complex AAs). It is unclear if a volumeoutcome association exists in endovascular treatment of complex AAs (complex EVAR). Methods: All patients prospectively registered in the Dutch Surgical Aneurysm Audit who underwent complex EVAR (FEVAR or BEVAR) between January 2016 and January 2020 were included. The effect of annual hospital volume on perioperative mortality was examined using multivariable logistic regression analyses. Patients were stratified into quartiles based on annual hospital volume to determine hospital volume categories. Results: We included 694 patients (539 FEVAR patients, 155 BEVAR patients). Contains fulltext : 251573.pdf (Publisher’s version ) (Open Access) OBJECTIVE: The Dutch Surgical Aneurysm Audit (DSAA) initiative was established in 2013 to monitor and improve nationwide outcomes of aortic aneurysm surgery. The objective of this study was to examine whether outcomes of surgery for intact abdominal aortic aneurysms (iAAA) have improved over time. METHODS: Patients who underwent primary repair of an iAAA by standard endovascular (EVAR) or open surgical repair (OSR) between 2014 and 2019 were selected from the DSAA for inclusion. The primary outcome was peri-operative mortality trend per year, stratified by OSR and EVAR. Secondary outcomes were trends per year in major complications, textbook outcome (TbO), and characteristics of treated patients. The trends per year were evaluated and reported in odds ratios per year. RESULTS: In this study, 11 624 patients (74.8%) underwent EVAR and 3 908 patients (25.2%) underwent OSR. For EVAR, after adjustment for confounding factors, there was no improvement in peri-operative mortality (aOR [adjusted odds ratio] 1.06, 95% CI 0.94 - 1.20), while major complications decreased (2014: 10.1%, 2019: 7.0%; aOR 0.91, 95% CI 0.88 - 0.95) and the TbO rate increased (2014: 68.1%, 2019: 80.9%; aOR 1.13, 95% CI 1.10 - 1.16). For OSR, the peri-operative mortality decreased (2014: 6.1%, 2019: 4.6%; aOR 0.89, 95% CI 0.82 - 0.98), as well as major complications (2014: 28.6%, 2019: 23.3%; aOR 0.95, 95% CI 0.91 - 0.99). Furthermore, the proportion of TbO increased (2014: 49.1%, 2019: 58.3%; aOR 1.05, 95% CI 1.01 - 1.10). In both the EVAR and OSR group, the proportion of patients with cardiac comorbidity increased. CONCLUSION: Since the establishment of this nationwide quality improvement initiative (DSAA), all outcomes of iAAA repair following EVAR and OSR have improved, except for peri-operative mortality following EVAR which remained unchanged.

Abstract

We have set up the first multi-domain P4programmable network in the Netherlands, consisting of six different sites interconnected by SURF's optical network. The purpose of the network is to experiment with novel inter-domain network functions and emerging internet architectures, in particular to improve the transparency, accountability, and controllability of the Internet and future networks. We provide an overview of the testbed's set-up, examples of how we have used it, and our lessons learned.

Abstract

Background: While [18F]-fluordeoxyglucose ([18F]FDG) uptake is associated with arterial inflammation, [18F]-sodium fluoride ([18F]NaF) is a marker for arterial micro-calcification. We aimed to investigate the prospective correlation between both PET markers over time and whether they are prospectively ([18F]FDG) and retrospectively ([18F]NaF) related to progression of systemic arterial disease in a longitudinal study in patients with type 2 diabetes mellitus (T2DM). Methods: Baseline [18F]FDG PET/Low Dose (LD) Computed Tomography (CT) scans of ten patients with early T2DM without cardiovascular history (70% men, median age 63 years) were compared with five-year follow-up [18F]NaF/LDCT scans. Systemic activity was expressed as mean target-to-background ratio (meanTBR) by dividing the maximal standardized uptake value (SUVmax) of ten arteries by SUVmean of the caval vein. CT-assessed macro-calcifications were scored visually and expressed as calcified plaque (CP) score. Arterial stiffness was assessed with carotid-femoral pulse wave velocity (PWV). Five-year changes were expressed absolutely with delta (Δ) and relatively with %change. Results: Baseline meanTBR[18F]FDG was strongly correlated with five-year follow-up meanTBR[18F]NaF (r = 0.709, P =.022). meanTBR[18F]NaF correlated positively with ΔCPscore, CPscore at baseline, and follow-up (r = 0.845, P =.002 and r = 0.855, P =.002, respectively), but not with %change in CPscore and PWV. Conclusion: This proof-of-concept study demonstrated that systemic arterial inflammation is an important pathogenetic factor in systemic arterial micro-calcification development. Abstract

Organ transplantation is performed worldwide, but policies regarding donor imaging are not uniform. An overview of the policies in different regions is missing. This study aims to investigate the various protocols worldwide on imaging in deceased organ donation. An online survey was created to determine the current policies. Competent authorities were approached to fill out the survey based on their current protocols. In total 32 of the 48 countries approached filled out the questionnaire (response rate 67%). In 16% of the countries no abdominal imaging is required prior to procurement. In 50%, abdominal ultrasound (US) is performed to screen the abdomen and in 19% an enhanced abdominal Computed Tomography (CT). In 15% of the countries both an unenhanced abdominal CT scan and abdominal US are performed. In 38% of the countries a chest radiographic (CXR) is performed to screen the thorax, in 28% only a chest CT, and in 34% both are performed. Policies regarding radiologic screening in deceased organ donors show a great variation between different countries. Consensus on which imaging method should be applied is missing. Abstract

The methods and results of using Landsat Thematic Mapper (TM) data to classify and estimate the acreage of forest covertypes in northeastern Minnesota are described. Portions of six TM scenes covering five counties with a total area of 14,679 square miles were classified into six forest and five nonforest classes. The approach involved the integration of cluster sampling, image processing, and estimation. Using cluster sampling, 343 plots, each 88 acres in size, were photo interpreted and field mapped as a source of reference data for classifier training and calibration of the TM data classifications. Classification accuracies of up to 75 percent were achieved; most misclassification was between similar or related classes. An inverse method of calibration, based on the error rates obtained from the classifications of the cluster plots, was used to adjust the classification class proportions for classification errors. The resulting area estimates for total forest land in the five-county area were within 3 percent of the estimate made independently by the USDA Forest Service. Area estimates for conifer and hardwood forest types were within 0.8 and 6.0 percent respectively, of the Forest Service estimates. A trial of a second method of estimating the same classes as the Forest Service resulted in standard errors of 0.002 to 0.015. A study of the use of multidate TM data for change detection showed that forest canopy depletion, canopy increment, and no change could be identified with greater than 90 percent accuracy. The project results have been the basis for the Minnesota Department of Natural Resources and the Forest Service to define and begin to implement an annual system of forest inventory which utilizes Landsat TM data to detect changes in forest cover.

40. Optimization of the information content of multitemporal Landsat TM data sets for monitoring forest cover disturbance

Abstract

Procedures that were developed to optimize the information content of multitemporal thematic mapper (TM) data sets for forest cover disturbance monitoring in Minnesota are described. TM imagery from three different years was calibrated to exoatmospheric reflectance. An atmospheric correction routine was applied combining two major components, atmospheric normalization over time and transformation to ground reflectance. Atmospheric conditions were modeled over time using regression functions derived from five ground features known to be unchanged over the time interval of interest and spanning the entire image reflectance range. The correlation between digital data and the forest cover was subsequently maximized and irrelevant information content was reduced by converting the band-specific reflectances into seven vegetation indices that were assumed to carry unique information. The application of two change detection algorithms to these seven indices ultimately resulted in 14 change features for each time interval of interest. Results show that the preprocessing sequence is vital to forest cover monitoring methodology.

Abstract

Oral mucositis (OM) may occur in up to 100% of patients undergoing condition regimen to hematopoietic stem cell transplant (HSCT). From the patient’s perspective, OM is one of the most debilitating side effects of transplantation. It is commonly thought that oral hygiene can modify the incidence and severity of oral mucositis, therefore professional oral health care (POHC) is recommended prior to conditioning regimen for HSCT. A new strategy for the treatment of OM is sodium hyaluronate (SH) combined with amino acid precursors of collagen (Aas) (Mucosamin®). SH is a mucoaderent polymer acting as a mechanical barrier and pain reliever. Furthermore, it allows prolonged contact of the product with the mucous membrane. In this study, a total of 68 adult patients due to undergo HSCT for allogenic and autologous transplant were enrolled at the Stem Cell Transplant Unit. The patients were divided into two groups. One group was treated with POHC before HSCT and applications of Mucosamin® during the recovery after transplantation. The second group served as controls, with the usual treatment of Clorexidine 0.20% adopted by the department. After HSCT the same clinician, an expert in oral medicine trained for the clinical trial, evaluated symptoms of the patients’ mucositis of both groups every day. The treated patients developed less severe OM, therefore Mucosamin® seems to have a protective role against the more severe phases of mucositis. The maximum OM pain, measured with the VAS scale, was higher in patients who did not use Mucosamin®. In the treated group OM resolved sooner than in the control group.

Abstract

Many important societal activities are global in scope, and as these activities continually expand world-wide, they are increasingly based on a foundation of advanced communication services and underlying innovative network architecture, technology, and core infrastructure. To continue progress in these areas, research activities cannot be limited to campus labs and small local testbeds or even to national testbeds. Researchers must be able to explore concepts at scale—to conduct experiments on world-wide testbeds that approximate the attributes of the real world. Today, it is possible to take advantage of several macro information technology trends, especially virtualization and capabilities for programming technology resources at a highly granulated level, to design, implement and operate network research environments at a global scale. GENI is developing such an environment, as are research communities in a number of other countries. Recently, these communities have not only been investigating techniques for federating these research environments across multiple domains, but they have also been demonstration prototypes of such federations. This chapter provides an overview of key topics and experimental activities related to GENI international networking and to related projects throughout the world.

Esophageal multiple intraluminal impedance baseline is an additional impedance parameter that was recently related to esophageal integrity. The aim of this study was to assess the relationship between mean esophageal impedance value and endoscopic findings in a large group of children. Children with symptoms of gastroesophageal reflux submitted to both endoscopy and impedance were included. Esophagitis was graded according to the Los Angeles classification. Mean impedance value was automatically calculated over 24-hour tracings. Data were adjusted for age through z-score transformation using percentiles normalized by the LMS (Lambda for the skew, Mu for the median, and Sigma for the generalized coefficient of variation) method. Abstract

Background: Data on chronic pain after kidney donation are sparse. The aim of this study was to assess the incidence of chronic pain after hand‐assisted laparoscopic nephrectomy. Methods: Living kidney donors who donated between 2011 and 2017 at the University Medical Centre Groningen were included. All patients underwent hand‐assisted laparoscopic donor nephrectomy. Postdonation pain and movement disabilities were assessed using the Carolinas Comfort Scale (CCS) and a visual analogue scale (VAS). The prevalence, severity of pain and the need for analgesics were reported. Results: Some 333 living kidney donors with a mean age of 56 years were included. At a median of 19 (i.q.r. 10–33) months after donation, 82 donors (24·6 per cent) had a CCS score above 0, of which 58 (71 per cent) had a CCS score of at least 2 and 57 (70 per cent) reported movement limitations. Some 110 donors (33·0 per cent) had a VAS score of more than 0. Complaints mainly occurred during bending over (12·3 per cent) and exercising (12·4 per cent). Thirty‐two donors (9·7 per cent) required analgesics during follow‐up between donation and the time of measurement, and six of 82 (7 per cent) reported chronic inguinal pain. In multivariable analysis, donor age (odds ratio (OR) 0·97, 95 per cent c.i. 0·95 to 0·99; P = 0·020) and length of hospital stay (OR 1·21, 1·01 to 1·51; P = 0·041) were independently associated with chronic pain. Conclusion: One‐quarter of donors experienced chronic postdonation pain or discomfort, most of which was bothersome. Younger donors and those with a longer postoperative hospital stay had more symptoms. Living donor nephrectomy is associated with chronic pain in 25 per cent of donors. The main objects in this paper are s-ideals Ipdimq, J0pμq, Jf pμq of Borel sets in X that can be covered by countably many compacta which are finite-dimensional, or of μ-measure null, or of finite μ-measure, respectively. Answering a question of J. Zapletal, we shall show that for the Hilbert cube, the s-ideal Ipdimq is not homogeneous in a strong way. We shall also show that in some natural instances of measures μ with nonhomogeneous s-ideals J0pμq or Jf pμq, the completions of the quotient Boolean algebras BorelpXq{J0pμq or BorelpXq{Jf pμq may be homogeneous. 49. Experiences with MPTCP in an intercontinental OpenFlow network

Abstract

The size and amount of e-science data sets is growing rapidly. Keeping up with the network demand in order to transfer these data sets over the Internet is a challenge. Single links do not have enough capacity anymore. Therefore we need to install more interfaces in the servers and use all available paths in the network. In this paper we describe two new technologies that help to optimally use the capacity of all multiple paths simultaneously. OpenFlow is used to discover the topology of the network,calculate multiple paths and configure those paths on the OpenFlow network. Multipath TCP (MPTCP) is used on the servers to distribute the load across the paths. We describe the end-to-end good put measurements we did in our OpenFlow test bed. We show that we reach a much higher throughput with multiple paths compared to a single path.
